    Human dietary exposure to dioxins and dioxin-like PCBs through the consumption of Atlantic herring from fishing areas in the Norwegian Sea and Baltic Sea

    The concentrations of dioxins [polychlorinated dibenzo-p-dioxins (PCDDs) and polychlorinated dibenzofurans (PCDFs)], and dioxin-like polychlorinated biphenyls (DL-PCBs) in Atlantic herring depend on the fishing area. These substances originate from various anthropogenic sources and accumulate in the environment and in food. The influence of country-specific contaminant concentrations on human dietary exposure was studied exemplary for herring to show the influence of fish origin. PCDD/F and DL-PCB concentrations in herring from the Norwegian Sea and the Baltic Sea were combined with country-specific herring consumption. Herring concentrations showed geographical variation. For herring consumers, the 50th percentile dietary exposure to the total sum of PCDD/Fs and DL-PCBs amounted to 1.2 and 8.9 pg WHO-2005-TEQ/kg BW/week for Norway and Germany, respectively. The different exposure was mainly related to higher concentrations in herring from the Baltic Sea, rather than in herring from the Norwegian Sea. If contaminant concentrations are influenced by geographical origin, this should be integrated into the dietary exposure assessments. For herring, relevant fishing areas should be integrated into the sampling strategy to generate concentration data.     Quantify the monthly to decadal variability of climate effects on the lower trophic levelse of shelf sea ecosystems

    ECOOP WP10, Deliverable no: D10.1.2.1This report describes three studies using multi-decadal simulations of regional coupled hydrodynamics ecosystem models. These models are used to investigate the relationship between lower trophic level marine ecosystems and biogeochemistry, and the physical environment. The models considered here: POLCOMS-ERSEM Atlantic Margin Model run from 1960 to 2003 (NERC and PML) NORWECOM North Sea Model run from 1985-2006 (IMR) ECOSMO (UiB-GFI) North sea and Baltic Sea run 1980-2004 (UiB-GFI) The POLCOMS-ERSEM model is validated using in-situ data from the world ocean data centre and analysed to investigate the potential long term changes in primary production across the period 1960-2004, in the context of model open boundary conditions and drift. The model experiments demonstrate a strong sensitivity of the on-shelf primary production to the oceanic nutrient boundary conditions, suggesting cross-shelf edge nutrient fluxes provide a significant source of variability. The relationship between the model results and the North Atlantic Oscillation are also considered, demonstrating a r~0.65 correlation with on-shelf nutrients and the NAO The NORWECOM model is validated here using time series data from the Dutch coast. Correlations between model variables in a selection of ICES boxes are compared with a number of driving factors. River loads are shown to dominate coastal boxes. The relationships in open-shelf boxes are more ambiguous, although the southerly inflow is demonstrated to have an important role. The validation of the POLCOMS-ERSEM and NORWECOM models both conclude that the simulations have better skill for nutrients than chlorophyll and in open-shelf seas away from the coast. The validation of ECOSMO presented here focuses on zooplankton and comparison with data from the continuous plankton recorder, investigating six different approaches to matching CPR records with model data. Across the North Sea the mean annual cycle shows good agreement between model and CPR. There is also good correlation with along-track variability. EOF and correlation analysis is used to relate the primary production in the North Sea to atmospheric forcing parameters. The EOF patterns tend to match the distribution of summer time stratification, while the wind speed is shows the highest correlation, particularly during the onset and breakdown of stratification. This indicates the strength of cross-thermocline mixing is an important control on primary production variability. The ECOSMO model has been further developed for use in the Baltic by inclusion of nitrogen fixing cyanobacteria. These studies each demonstrate significant control of the inter-annual variability of shelf sea ecosystems through a range of external forcing vectors: oceanic through cross-shelf edge nutrient flux, terrestrial through variations in river nutrient loading, and atmospheric via the wind control of vertical mixing.     Hospital treatment -is it affordable? A structured cost analysis of vaginal deliveries and planned caesarean sections

    <p>Abstract</p> <p>Introduction</p> <p>The analysis of cost effectiveness in hospitals is as difficult as treating the patients properly. We are yet not able to answer the simple question of what costs are caused by a certain diagnosis and its treatment during an average hospital stay.</p> <p>Methods</p> <p>To answer some issues of the global problem of cost effectiveness during hospitalisation, we analysed the costs and the cost structure of a normal obstetrical hospital stay during an uncomplicated vaginal delivery and a planned caesarean section. Cost data was collected and summarized from the patients file, the hospital's computer system gathering all cost centres, known material expenses and expenses of non obstetrical medical services.</p> <p>Results</p> <p>For vaginal deliveries/planned caesareans we can calculate with a surplus of about 83 €/1432 €. About 45% of the summarized costs are calculated on a reliable database.</p> <p>Discussion</p> <p>The introduction of the DRG based clearing system in Germany has aggravated the discussion on cost effectiveness. Our meticulous work-up of expenses excluded personal precautionary costs and personnel costs of documentation because no tools are described to depict such costs. If we would add these costs to the known expenses of our study, we strongly suspect that hospital treatment of vaginal deliveries or planned caesarean sections is not cost effective.</p

    Postgraduate Students’ Experience of Using a Learning Management System to Support Their Learning: A Qualitative Descriptive Study

    Introduction Educational institutions worldwide have implemented learning management systems (LMSs) to centralise and manage learning resources, educational services, learning activities and institutional information. LMS has mainly been used by teachers as storage and transfer of course material. To effectively utilise digital technologies in education, there is a need for more knowledge of student experiences with digital technology, such as LMSs and especially regarding how LMSs can contribute to student engagement and learning. Objective This study aimed to gain knowledge about postgraduate nursing students’ experiences with the use of LMS in a subject in an advanced practice nursing master's programme. Methods A qualitative method with a descriptive design was employed. Two focus group interviews were performed with eight postgraduate nursing students from an advanced practice nursing programme at a university college in Norway. Data were analysed using qualitative content analysis. Results Three themes emerged from the data material: 1) A course structure that supports learning; 2) LMS tools facilitate preparation, repetition and flexibility; and 3) own responsibility for using the LMS for preparation before on-campus activities. Conclusion The course structure within the LMS seemed to be important to enhance postgraduate students’ ability to prepare before on-campus activities. Implementation and use of LMS tools can facilitate preparation, repetition and flexibility, especially when postgraduate students study difficult topics. Postgraduate students seem to have different views regarding their own responsibility for using the LMS to prepare before on-campus activities.publishedVersio

    Dynamical models for sand ripples beneath surface waves

    We introduce order parameter models for describing the dynamics of sand ripple patterns under oscillatory flow. A crucial ingredient of these models is the mass transport between adjacent ripples, which we obtain from detailed numerical simulations for a range of ripple sizes. Using this mass transport function, our models predict the existence of a stable band of wavenumbers limited by secondary instabilities. Small ripples coarsen in our models and this process leads to a sharply selected final wavenumber, in agreement with experimental observations.Comment: 9 pages.     Workplace Psychosocial Resources and Risk of Sleep Disturbances Among Employees

    IMPORTANCE: Workplace psychosocial resources naturally tend to cluster in some work teams. To inform work-related sleep health promotion interventions, it is important to determine the associations between clustering of workplace resources and sleep disturbances when some resources are high while others are low and to mimic an actual intervention using observational data. OBJECTIVE: To examine whether clustering of and changes in workplace psychosocial resources are associated with sleep disturbances among workers. DESIGN, SETTING, AND PARTICIPANTS: This population-based cohort study used data from the Swedish Longitudinal Occupational Survey of Health (2012-2018), the Work Environment and Health in Denmark study (2012-2018), and the Finnish Public Sector Study (2008-2014), collected biennially. Statistical analysis was conducted from November 2020 to June 2022. EXPOSURE: Questionnaires were distributed measuring leadership quality and procedural justice (ie, vertical resources) as well as collaboration culture and coworker support (ie, horizontal resources). Resources were divided into clusters of general low, intermediate vertical and low horizontal, low vertical and high horizontal, intermediate vertical and high horizontal, and general high. MAIN OUTCOMES AND MEASURES: Odds ratios (ORs) and 95% CIs were reported from logistic regression models for the associations between the clustering of resources and concurrent and long-term sleep disturbances. Sleep disturbances were measured by self-administered questionnaires. RESULTS: The study identified 114 971 participants with 219 982 participant-observations (151 021 [69%] women; mean [SD] age, 48 [10] years). Compared with participants with general low resources, other groups showed a lower prevalence of sleep disturbances, with the lowest observed in the general high group concurrently (OR, 0.38; 95% CI, 0.37-0.40) and longitudinally after 6 years (OR, 0.52; 95% CI, 0.48-0.57). Approximately half of the participants (27 167 participants [53%]) experienced changes in resource clusters within 2 years. Improvements in vertical or horizontal dimensions were associated with reduced odds of persistent sleep disturbances, and the lowest odds of sleep disturbances was found in the group with improvements in both vertical and horizontal dimensions (OR, 0.53; 95% CI, 0.46-0.62). A corresponding dose-response association with sleep disturbances was observed for decline in resources (eg, decline in both dimensions: OR, 1.74; 95% CI, 1.54-1.97). CONCLUSIONS AND RELEVANCE: In this cohort study of workplace psychosocial resources and sleep disturbances, clustering of favorable resources was associated with a lower risk of sleep disturbances

    Workplace psychosocial resources and risk of cardiovascular disease among employees : a multi-cohort study of 135 669 participants

    Objective In terms of prevention, it is important to determine effects on cardiovascular disease (CVD) when some workplace psychosocial resources are high while others are low. The aim of the study was to assess the prospective relationship between clustering of workplace psychosocial resources and risk of CVD among employees.Methods We pooled data from three cohort studies of 135 669 employees (65% women, age 18-65 years and free of CVD) from Denmark, Finland and Sweden. Baseline horizontal resources (culture of collaboration and support from colleagues) and vertical resources (leadership quality and procedural justice) were measured using standard questionnaire items. Incident CVD, including coronary heart and cerebrovascular disease, was ascertained using linked electronic health records. We used latent class analysis to assess clustering (latent classes) of workplace psychosocial resources. Cox proportional hazard models were used to examine the association between these clusters and risk of CVD, adjusting for demographic and employment-related factors and pre-existing physical and mental disorders.Results We identified five clusters of workplace psychosocial resources from low on both vertical and horizontal resources (13%) to generally high resources (28%). High horizontal resources were combined with either intermediate [hazard ratio (HR) 0.84, 95% confidence interval (CI) 0.74-0.95] or high (HR 0.88, 95% CI 0.78-1.00) vertical resources were associated with lower risks of CVD compared to those with generally low resources. The association was most prominent for cerebrovascular disease (eg, general high resources: HR 0.80, 95% CI 0.67-0.96).Conclusions Individuals with high levels of workplace psychosocial resources across horizontal and vertical dimensions have a lower risk of CVD, particularly cerebrovascular disease.Peer reviewe
