    Pedometers to enhance physical activity in COPD: a randomised controlled trial

    Physical inactivity is a cardinal feature of chronic obstructive pulmonary disease (COPD), and is associated with increased morbidity and mortality. Pedometers, which have been used in healthy populations, might also increase physical activity in patients with COPD. COPD patients taking part in a 3-month individualised programme to promote an increase in their daily physical activity were randomised to either a standard programme of physical activity encouragement alone, or a pedometer-based programme. Assessments were performed by investigators blinded to treatment allocation. Change in average 1-week daily step count, 6-min walking distance (6MWD), modified Medical Research Council scale, St George’s respiratory questionnaire (SGRQ) and COPD assessment test (CAT) were compared between groups. 102 patients were recruited, of whom 97 completed the programme (pedometer group: n=50; control group: n=47); 60.8% were male with a mean±sd age of 68.7±8.5 years, and forced expiratory volume in 1 s (FEV1) 66.1±19.4% and FEV1/forced vital capacity 55.2±9.5%. Both groups had comparable characteristics at baseline. The pedometer group had significantly greater improvements in: physical activity 3080±3254 steps·day−1 versus 138.3±1950 steps·day−1 (p<0.001); SGRQ −8.8±12.2 versus −3.8±10.9 (p=0.01); CAT score −3.5±5.5 versus −0.6±6.6 (p=0.001); and 6MWD 12.4±34.6 versus −0.7±24.4 m (p=0.02) than patients receiving activity encouragement only. A simple physical activity enhancement programme using pedometers can effectively improve physical activity level and quality of life in COPD patients

    The Cerro Munro sub-volcanic intrusion is emplaced in the back-arc (400 km from the trench) as small sub-circular tonalite-granodiorite plutons with abundant radial porphyritic dikes. U-Pb zircon SHRIMP data give an age of crystallization of 57 Ma ± 1.4 Ma. It is located to the east of the North Patagonian Batholith (NPB) that shows a protracted and episodic magmatic history from Cretaceous to Miocene time. The NPB Palaeogene episode is characterized by the lack of magmatic activity at the arc axis, as small plutonic emplacements move to the fore-arc and back-arc. This Palaeogene tectono-magmatic episode is ruled by the detachment of the Aluk plate during the Aluk-Farallon-SAM triple junction, active at that time along northern Patagonia active margin, changing the Cretaceous ?NPB orogenic? setting to a Palaeogene ?Munro transitional? tectono-magmatic setting. Near the contacts, the tonalite contains abundant enclaves of igneous appearance and variable size from several cm to dm, described as autoliths. The study of autoliths and host tonalite reveals interesting results on the processes of fractionation in a thermally zoned magma chamber. Autoliths, and in a large extent the host tonalite, represent disguised cumulates from which a hydrous silicic liquid was extracted. Barometry calculations from mineral chemistry in both autoliths and tonalites record a shallow pressure of emplacement of 0.5 kbar. Rhyolite-dacite flows and ignimbrites, surrounding the northern contact of the Cerro Munro tonalite, may represent the exsolved liquid from the plutonic cumulates.     Current Approaches for Glioma Gene Therapy and Virotherapy

    Glioblastoma (GBM) is the most common and aggressive primary brain tumor in the adult population and it carries a dismal prognosis. Inefficient drug delivery across the blood brain barrier (BBB), an immunosuppressive tumor microenvironment (TME) and development of drug resistance are key barriers to successful glioma treatment. Since gliomas occur through sequential acquisition of genetic alterations, gene therapy, which enables to modification of the genetic make-up of target cells, appears to be a promising approach to overcome the obstacles encountered by current therapeutic strategies. Gene therapy is a rapidly evolving field with the ultimate goal of achieving specific delivery of therapeutic molecules using either viral or non-viral delivery vehicles. Gene therapy can also be used to enhance immune responses to tumor antigens, reprogram the TME aiming at blocking glioma-mediated immunosuppression and normalize angiogenesis. Nano-particles-mediated gene therapy is currently being developed to overcome the BBB for glioma treatment. Another approach to enhance the anti-glioma efficacy is the implementation of viro-immunotherapy using oncolytic viruses, which are immunogenic. Oncolytic viruses kill tumor cells due to cancer cell-specific viral replication, and can also initiate an anti-tumor immunity. However, concerns still remain related to off target effects, and therapeutic and transduction efficiency. In this review, we describe the rationale and strategies as well as advantages and disadvantages of current gene therapy approaches against gliomas in clinical and preclinical studies. This includes different delivery systems comprising of viral, and non-viral delivery platforms along with suicide/prodrug, oncolytic, cytokine, and tumor suppressor-mediated gene therapy approaches. In addition, advances in glioma treatment through BBB-disruptive gene therapy and anti-EGFRvIII/VEGFR gene therapy are also discussed. Finally, we discuss the results of gene therapy-mediated human clinical trials for gliomas. In summary, we highlight the progress, prospects and remaining challenges of gene therapies aiming at broadening our understanding and highlighting the therapeutic arsenal for GBM.Fil: Banerjee, Kaushik.     Adaptation of the Landau-Migdal Quasiparticle Pattern to Strongly Correlated Fermi Systems

    A quasiparticle pattern advanced in Landau's first article on Fermi liquid theory is adapted to elucidate the properties of a class of strongly correlated Fermi systems characterized by a Lifshitz phase diagram featuring a quantum critical point (QCP) where the density of states diverges. The necessary condition for stability of the Landau Fermi Liquid state is shown to break down in such systems, triggering a cascade of topological phase transitions that lead, without symmetry violation, to states with multi-connected Fermi surfaces. The end point of this evolution is found to be an exceptional state whose spectrum of single-particle excitations exhibits a completely flat portion at zero temperature. Analysis of the evolution of the temperature dependence of the single-particle spectrum yields results that provide a natural explanation of classical behavior of this class of Fermi systems in the QCP region.Comment: 26 pages, 14 figures.     Association of Mediterranean diet with survival after breast cancer diagnosis in women from nine European countries: results from the EPIC cohort study

    BACKGROUND: The Mediterranean diet has been associated with lower risk of breast cancer (BC) but evidence from prospective studies on the role of Mediterranean diet on BC survival remains sparse and conflicting. We aimed to investigate whether adherence to Mediterranean diet prior to diagnosis is associated with overall and BC-specific mortality. METHODS: A total of 13,270 incident breast cancer cases were identified from an initial sample of 318,686 women in 9 countries from the European Prospective Investigation into Cancer and Nutrition (EPIC) study. Adherence to Mediterranean diet was estimated through the adapted relative Mediterranean diet (arMED), a 16-point score that includes 8 key components of the Mediterranean diet and excludes alcohol. The degree of adherence to arMED was classified as low (score 0-5), medium (score 6-8), and high (score 9-16). Multivariable Cox proportional hazards models were used to analyze the association between the arMED score and overall mortality, and Fine-Gray competing risks models were applied for BC-specific mortality. RESULTS: After a mean follow-up of 8.6 years from diagnosis, 2340 women died, including 1475 from breast cancer. Among all BC survivors, low compared to medium adherence to arMED score was associated with a 13% higher risk of all-cause mortality (HR 1.13, 95%CI 1.01-1.26). High compared to medium adherence to arMED showed a non-statistically significant association (HR 0.94; 95% CI 0.84-1.05). With no statistically significant departures from linearity, on a continuous scale, a 3-unit increase in the arMED score was associated with an 8% reduced risk of overall mortality (HR3-unit 0.92, 95% CI: 0.87-0.97). This result sustained when restricted to postmenopausal women and was stronger among metastatic BC cases (HR3-unit 0.81, 95% CI: 0.72-0.91). CONCLUSIONS: Consuming a Mediterranean diet before BC diagnosis may improve long-term prognosis, particularly after menopause and in cases of metastatic breast cancer. Well-designed dietary interventions are needed to confirm these findings and define specific dietary recommendations

    Evaluation of classification algorithms in the Google Earth Engine platform for the identification and change detection of rural and periurban buildings from very high-resolution images

    [EN] Building change detection based on remote sensing imagery is a key task for land management and planning e.g., detection of illegal settlements, updating land records and disaster response. Under the post- classification comparison approach, this research aimed to evaluate the feasibility of several classification algorithms to identify and capture buildings and their change between two time steps using very-high resolution images (&lt;1 m/pixel) across rural areas and urban/rural perimeter boundaries. Through an App implemented on the Google Earth Engine (GEE) platform, we selected two study areas in Colombia with different images and input data. In total, eight traditional classification algorithms, three unsupervised (K-means, X-Means y Cascade K-Means) and five supervised (Random Forest, Support Vector Machine, Naive Bayes, GMO maximum Entropy and Minimum distance) available at GEE were trained. Additionally, a deep neural network named Feature Pyramid Networks (FPN) was added and trained using a pre-trained model, EfficientNetB3 model. Three evaluation zones per study area were proposed to quantify the performance of the algorithms through the Intersection over Union (IoU) metric. This metric, with a range between 0 and 1, represents the degree of overlapping between two regions, where the higher agreement the higher IoU values. The results indicate that the models configured with the FPN network have the best performance followed by the traditional supervised algorithms. The performance differences were specific to the study area. For the rural area, the best FPN configuration obtained an IoU averaged for both time steps of 0.4, being this four times higher than the best supervised model, Support Vector Machines using a linear kernel with an average IoU of 0.1. Regarding the setting of urban/rural perimeter boundaries, this difference was less marked, having an average IoU of 0.53 in comparison to 0.38 obtained by the best supervised classification model, in this case Random Forest. The results are relevant for institutions tracking the dynamics of building areas from cloud computing platfo future assessments of classifiers in likewise platforms in other contexts.[ES] La detección de cambios de áreas construidas basada en datos de teledetección es una importante herramienta para el ordenamiento y la administración del territorio p.e.: la identificación de construcciones ilegales, la actualización de registros catastrales y la atención de desastres. Bajo el enfoque de comparación post-clasificación, la presente investigación tuvo como objetivo evaluar la funcionalidad de varios algoritmos de clasificación para identificar y capturar las construcciones y su cambio entre dos fechas de análisis usando imágenes de alta resolución (&lt;1 m/píxel) en ámbitos rurales y límites del perímetro urbano municipal. La anterior evaluación fue llevada a cabo a través de una aplicación desarrollada mediante la plataforma Google Earth Engine (GEE), donde se alojaron y analizaron diferentes imágenes y datos de entrada sobre dos áreas de estudio en Colombia. En total, ocho algoritmos de clasificación tradicional, tres no supervisados (K-means, X-Means y Cascade K-Means) y cinco supervisados (Random Forest, Support Vector Machine, Naive Bayes, GMO maximum Entropy y Minimum distance) fueron entrenados empleando GEE. Adicionalmente, se entrenó una red neuronal profunda denominada Feature Pyramid Networks (FPN) sobre la cual se aplicó la estrategia de modelos preentrenados, usando pesos del modelo EfficientNetB3. Por cada una de las dos áreas de estudio, tres zonas de evaluación fueron propuestas para cuantificar la funcionalidad de los algoritmos mediante la métrica Intersection over Union (IoU). Esta métrica representa la evaluación de la superposición de dos regiones y tiene un rango de valores de 0 a 1, donde a mayor coincidencia de las imágenes mayor es el valor de IoU. Los resultados indican que los modelos configurados con la red FPN tienen la mejor funcionalidad, seguido de los algoritmos tradicionales supervisados. Las diferencias de la funcionalidad fueron específicas por área de estudio. Para el ámbito rural, la mejor configuración de FPN obtuvo un IoU promedio entre ambas fechas de 0,4, es decir, cuatro veces el mejor modelo supervisado, correspondiente al Support Vector Machine de kernel Lineal con un IoU de 0,1. Respecto al área de límites del perímetro urbano municipal, esta diferencia fue menos marcada, con un IoU promedio de 0,53 en comparación con el 0,38 derivado del mejor modelo de clasificación supervisada, que en este caso fue Random Forest.     Overview of recent TJ-II stellarator results

    The main results obtained in the TJ-II stellarator in the last two years are reported. The most important topics investigated have been modelling and validation of impurity transport, validation of gyrokinetic simulations, turbulence characterisation, effect of magnetic configuration on transport, fuelling with pellet injection, fast particles and liquid metal plasma facing components. As regards impurity transport research, a number of working lines exploring several recently discovered effects have been developed: the effect of tangential drifts on stellarator neoclassical transport, the impurity flux driven by electric fields tangent to magnetic surfaces and attempts of experimental validation with Doppler reflectometry of the variation of the radial electric field on the flux surface. Concerning gyrokinetic simulations, two validation activities have been performed, the comparison with measurements of zonal flow relaxation in pellet-induced fast transients and the comparison with experimental poloidal variation of fluctuations amplitude. The impact of radial electric fields on turbulence spreading in the edge and scrape-off layer has been also experimentally characterized using a 2D Langmuir probe array. Another remarkable piece of work has been the investigation of the radial propagation of small temperature perturbations using transfer entropy. Research on the physics and modelling of plasma core fuelling with pellet and tracer-encapsulated solid-pellet injection has produced also relevant results. Neutral beam injection driven Alfvénic activity and its possible control by electron cyclotron current drive has been examined as well in TJ-II.
