    Acquired periungual fibrokeratoma: surgery treatment

    Acquired periungual fibrokeratoma is a benign fibroepitelial tumor, relatively common, but with minor incidence at the foot. It is characterized by presenting a fleshy mass covered with hyperpigmented skin and with hyperqueratosis in its distal portion, whose consistence is strong and "indurada" with a size that varies from 5 to 10 mm in length. Sometimes it can be painful after minor trauma or it can even bleed. In that case, surgical treatment can be suggested, as in the case reported. We present the case of a 31 years old patient, who comes to the office due to a painful isolated periungeal fibroma at the level of the third right toe, being recommended the surgical excision of the mass. Blood analysis and dorsolateral and dorsoplantar x-rays are required, to make sure that there´s no evidence of underlying bony pathology. The surgical technique has been carried out, under local anaesthesia making an elliptical incision following the lines of tension of the skin, and removing the fibroma. Sutures were necessary to heal the wound. After removing the mass, a biopsy was carried out and sent to the pathological anatomy department to ensure the diagnosis. The postoperative period was painless and the appearance of the toe was excellent. After one year follow-up, nor recurrences or associated problems appeared. We conclude that acquired periungual fibrokeratoma represents a well defined clinically and histologically benign tumor, easily diagnosed. In this way, television series are creators of narratives that alter the perception of one's own identity and of society. In this article, we analyse the representation of gender in three selected episodes from one of the most popular series on Netflix that promotes a feminist narrative, Sex Education. As a methodological approach to describe and analyse the representations of womanhood and feminism, we use critical discourse analysis (CDA) from a gender perspective. We have found that the female protagonists are represented as being subject to adversity as a result of misogyny that they, at times, also perform alongside their male counterparts. Nonetheless, they act in solidarity with each other when this adversity arises given their shared experiences of misogyny which helps them to come to some sort of resolution that brings them closer together, and in some cases, they have access to resources to give them moments of catharsis or closure. We consider that media literacy is the ideal strategy to develop the capacity for critical analysis of audio-visual content and to introduce the analysis of gender representations in the media.Resumen: Hoy en día los medios de comunicación se han convertido en una de las principales fuentes de educación, complementando el proceso educativo de las escuelas y las familias. De esta forma, las series de televisión son creadoras de narrativas que alteran la percepción de la propia identidad y de la sociedad. En este artículo, analizamos la representación de género en tres episodios de una de las series más populares en Netflix que promueve una narrativa feminista, Sex Education. Como aproximación metodológica para describir y analizar las representaciones de la feminidad y la feminidad, utilizamos el análisis crítico del discurso (ACD) desde una perspectiva de género. Hemos encontrado que mientras que las protagonistas femeninas son representadas como sujetas a la adversidad como resultado de la misoginia y a veces también realizan junto a sus contrapartes masculinas dicha acción. Sin embargo, se observa que se presenta una solidaridad grupal cuando entienden que comparten experiencias de misoginia que los ayuda a llegar a algún tipo de resolución que les une. Consideramos que la alfabetización mediática es la estrategia idónea para desarrollar la capacidad de análisis crítico de los contenidos audiovisuales e introducir el análisis de las representaciones de género en los medios.JUT

    Impact of quality of life related to foot problems : a case-control study

    [Abstract] Foot problems are highly prevalent conditions, being a frequent reason for medical and podiatric consultation. The aim of this study was to compare the differences of quality of life (QoL) related to foot health in people with and without the presence of foot problems. A case-control study was carried out in an outpatient centre, where a clinician recorded data related to sociodemographic and clinical characteristics. In addition, self-reported data on foot health-related quality of life were recorded using the Spanish version of the Foot Health Status Questionnaire. The sample consisted of 498 participants (249 cases and 249 controls), with a median age of 30 years and an interquartile range of 23 years. The differences between the groups were statistically significant for gender, age, footwear, general health, foot health, and physical activity. Cases showed lower scores for the domain of footwear, physical activity and vitality compared to controls. Foot pathologies have a negative impact on quality of life related to foot health, and the domains of footwear, general health and physical activity seem to be the factors that are associated with the presence of alterations and foot deformities

    Educar en la escuela infantil del siglo XXI: diálogo, inclusión y tecnología

    The objectives of education for the 21st century seek to guarantee an inclusive, quality and equitable process that fosters educational opportunities for all infants. Increasingly, minors are exposed for a longer time of digital consumption. Because of the increase in the number of screens in the home, as well as digital resources, we ask ourselves: What are the characte ristics of today's childhood? What place do technologies occupy in children and preschool teachers' lives? This article makes a bibliographic review that tries to answer these questions. The main objective is to elaborate the state of the art in relation to the development and implementation of digital technology in childhood education. Every day we observe how children manipulate the tablet before learning to speak or write and that they hardly know what it is to play in the real world. We are facing a complex issue, so it is necessary to guarantee the full development of the child and continue investigating the advances in research on childhood, inclusion and technology. The results show that technologies are an opportunity to achieve profound changes that contribute to improving the teaching-learning processes from new pedagogical and curricular approaches. Los objetivos de la educación del siglo XXI tratan de garantizar una educación inclusiva, de calidad y equitativa, que propicie oportunidades educativas para todos los infantes. Cada vez más, los menores son expuestos a un mayor tiempo de consumo digital. Ante el aumento del número de pantallas en el hogar, así como de recursos digitales nos planteamos: ¿cuáles son las características de la infancia actual?, ¿qué lugar ocupan las tecnologías en la vida del niño?, ¿y en la del docente de infantil? En este artículo se realiza una revisión bibliográfica que trata de responder a los interrogantes planteados. El objetivo es ela- borar el estado del arte en relación al desarrollo e implementación de lo digital en Educación Infantil. A diario, observamos cómo los niños manipulan la tablet antes de aprender a hablar o escribir y que apenas saben lo que es jugar en el mundo real. Estamos ante un tema complejo por lo que es preciso garantizar el pleno desarrollo del niño y seguir indagando en los avances de la investigación sobre infancia, inclusión y tecnología. Los resultados evidencian que las tecnologías son una oportunidad para lograr cambios profundos que contribuyan a mejorar los procesos de enseñanza aprendizaje desde nue - vos planteamientos pedagógicos y curriculares

    Diverse Large HIV-1 Non-subtype B Clusters Are Spreading Among Men Who Have Sex With Men in Spain

    In Western Europe, the HIV-1 epidemic among men who have sex with men (MSM) is dominated by subtype B. However, recently, other genetic forms have been reported to circulate in this population, as evidenced by their grouping in clusters predominantly comprising European individuals. Here we describe four large HIV-1 non-subtype B clusters spreading among MSM in Spain. Samples were collected in 9 regions. A pol fragment was amplified from plasma RNA or blood-extracted DNA. Phylogenetic analyses were performed via maximum likelihood, including database sequences of the same genetic forms as the identified clusters. Times and locations of the most recent common ancestors (MRCA) of clusters were estimated with a Bayesian method. Five large non-subtype B clusters associated with MSM were identified. The largest one, of F1 subtype, was reported previously. The other four were of CRF02_AG (CRF02_1; n = 115) and subtypes A1 (A1_1; n = 66), F1 (F1_3; n = 36), and C (C_7; n = 17). Most individuals belonging to them had been diagnosed of HIV-1 infection in the last 10 years. Each cluster comprised viruses from 3 to 8 Spanish regions and also comprised or was related to viruses from other countries: CRF02_1 comprised a Japanese subcluster and viruses from 8 other countries from Western Europe, Asia, and South America; A1_1 comprised viruses from Portugal, United Kingom, and United States, and was related to the A1 strain circulating in Greece, Albania and Cyprus; F1_3 was related to viruses from Romania; and C_7 comprised viruses from Portugal and was related to a virus from Mozambique. A subcluster within CRF02_1 was associated with heterosexual transmission. Near full-length genomes of each cluster were of uniform genetic form. Times of MRCAs of CRF02_1, A1_1, F1_3, and C_7 were estimated around 1986, 1989, 2013, and 1983, respectively. MRCA locations for CRF02_1 and A1_1 were uncertain (however initial expansions in Spain in Madrid and Vigo, respectively, were estimated) and were most probable in Bilbao, Spain, for F1_3 and Portugal for C_7.     Impact of the COVID-19 pandemic in Spain in the successive pandemic waves on hemodialysis patients and healthcare personnel

    (1) Background: The impact of SARS-CoV-2 has been variable over the time course of the pandemic and in different populations. The aim was to analyze the impact of COVID-19 infection in a known population of hemodialysis (HD) patients and professionals in Spain at different times of the pandemic. (2) Methods: We conducted an observational, descriptive study with a follow-up from 3 March 2020 to 23 April 2022 (776 days), using in average of 414 professionals and 1381 patients from 18 HD units in Spain. The data from the positive PCR or the rapid antigen detection test (RADT) subject were analyzed and segmented into six periods (waves). (3) Results: Of 703 positive COVID-19 tests, 524 were HD patients (74.5%), and 179 were HD professionals (25.5%). Overall, 38% of staff and 43% of patients were affected. Differences were observed in regard to incidence (21% vs. 13%), mortality (3.5% vs. 0%), and symptomatology between the patients and professionals and throughout the pandemic. (4) Conclusions: COVID-19 severity varied during different pandemic waves, with a greater impact seen in the first wave. HD professionals and patients had similar infection rates, but patients had higher mortality rates.     PKD phosphorylation and COP9/Signalosome modulate intracellular Spry2 protein stability

    Spry2 is a molecular modulator of tyrosine kinase receptor signaling pathways that has cancer-type-specific effects. Mammalian Spry2 protein undergoes tyrosine and serine phosphorylation in response to growth factor stimulation. Spry2 expression is distinctly altered in various cancer types. Inhibition of the proteasome functionality results in reduced intracellular Spry2 degradation. Using in vitro and in vivo assays, we show that protein kinase D (PKD) phosphorylates Spry2 at serine 112 and interacts in vivo with the C-terminal half of this protein. Importantly, missense mutation of Ser112 decreases the rate of Spry2 intracellular protein degradation. Either knocking down the expression of all three mammalian PKD isoforms or blocking their kinase activity with a specific inhibitor contributes to the stabilization of Spry2 wild-type protein. Downregulation of CSN3, a component of the COP9/Signalosome that binds PKD, significantly increases the half-life of Spry2 wild-type protein but does not affect the stability of a Spry2 after mutating Ser112 to the non-phosphorylatable residue alanine.
