Liver X receptor modulators: Effects on lipid metabolism and potential use in the treatment of atherosclerosis
Erään psykiatrisen potilaan kokemus sairaudestaan ja hoidostaan
Opinnäytetyön tarkoituksena oli selvittää erään potilaan kokemuksia sairaudestaan ja saamastaan psykiatrisesta hoidosta. Aineistona oli potilaan vuosien varrella kirjoittamat kirjeet. Tilaajana oli eräs Etelä-Suomessa sijaitseva psykiatrinen sairaala. Opinnäytetyön tavoitteena oli saada potilaan ääni kuuluville ja tuoda esille psykiatrisen hoidon kehittämistarpeita.
Teoriaosuuden taustana käytettiin alan kirjallisuutta, psykiatrisen hoitotyön laatu- ja hoitosuosituksia sekä aikaisempia tutkimuksia psykiatristen potilaiden kokemuksista. Teoria-osassa käsiteltiin psykiatrista hoitotyötä ja psykoosisairauksia sekä hoitosuhdetyöskentelyä prosessina. Opinnäytetyö toteutettiin laadullisella tutkimusmenetelmällä. Analyysimenetelmänä käytettiin aineistolähtöistä sisällönanalyysia, jonka avulla aineisto tiivistettiin siten, että tutkittavia asioita voitiin kuvata.
Tuloksista selvisi kokemuksien ristiriitaisuus. Toisaalta potilas koki itsensä seesteiseksi ja tasapainoiseksi, sekä saamansa hoidon hyväksi. Toisaalta hän toi esille syyllisyyden ja arvottomuuden tunteita. Hoitohenkilökunnan hän koki toisinaan epäluotettavana ja kohtelun epäasiallisena. Potilaalla oli useita konkreettisia ehdotuksia sekä toiveita. Kehittämisehdotukset oli suunnattu pääasiassa osaston toimintaan, hoitoon ja muiden potilaiden viihtyvyyteen. Toiveet koskivat lähinnä potilasta itseään ja hänen hoitoaan. Tuloksia voitaisiin hyödyntää lisäämään hoitohenkilökunnan ymmärrystä psyykkisesti sairaan henkilön sisäisestä maailmasta ja kokemuksesta. Aineistosta nousseiden kehitysehdotusten myötä eri hoitotahot voisivat myös pohtia potilaidensa osallistamista hoidon suunnitteluun ja kehittämiseen.
Jatkotutkimuksella olisi mielenkiintoista selvittää, miten hoitajat yleisesti kokevat ja ymmärtävät psyykkisesti sairaita potilaita. Tärkeää olisi myös kuulla hoitajien kokemus siitä, miten hoito- ja laatusuosituksia toteutetaan käytännön hoitotyössä, ja kuinka hyödyllisiksi ne koetaan.The purpose of the Bachelor’s thesis was to study a certain patient’s experiences of their illness and psychiatric care. The data is based on written letters by the patient over the years. This thesis was commissioned by a psychiatric hospital in Southern Finland. The aim of this Bachelor’s thesis was to make the patient’s voice heard and to bring out the development requirements of psychiatric care.
The results of the thesis can be utilized to increase the understanding of nursing staff.
The theory was based on literature, psychiatric nursing quality and treatment recommendations, and previous studies of the experiences of psychiatric patients. In the theory part psychiatric nursing and psychosis illnesses and working on nurse-patient relationship as a process are dealt with. The thesis was carried out by qualitative research method. Data oriented content analysis was used as an analytical method to summarize the material so that the matters to be examined can be described.
The results highlighted the inconsistency of experience. On the other hand, the patient felt serene, balanced and benefited from the care. On the other hand, he raised feelings of guilt and worthlessness. Sometimes, the patient experienced the nursing staff as untrustworthy and that his treatment was inappropriate. The patient had several concrete suggestions and requests. The proposals for development were mainly directed at the operation of the ward, care and other patients' comfort. The requests mainly concerned the patients themselves and their care.
The results could be utilized to increase the understanding of the nursing staff in relation to the internal world and experience of a mentally ill person. The development suggestions could be used by various nursing homes to reflect on the involvement of their patients in the planning and development of care. It would be interesting to see a further study on how nurses feel and understand the mentally ill. It would also be important to hear the nurses' experience on how treatment and quality recommendations are implemented in practical nursing and how useful they are from the nurses` perspective
Erilainen Peliveli: dokumentti pienellä tuotantoryhmällä
Opinnäytetyöni mediatyö oli 40-minuuttinen dokumenttielokuva seinäjokisesta salibandyseurasta Seinäjoen Peliveljistä eli SPV:stä. Dokumentissa kerrotaan seuran historiasta ja kristillisistä arvoista sen takana, seuran nykyisestä tilasta sekä menestyksekkään joukkueen kaudesta 2016–2017 salibandyliigassa. Halusimme selvittää, miten kristilliset arvot ovat vaikuttaneet seuran alkuun ja taipaleeseen urheilumaailmassa.
Kirjallisessa osuudessa kerron lyhyesti aiheena olleesta seurasta, käyn hie-man läpi dokumenttielokuvia yleisesti sekä dokumenttien haasteellista objektiivi-suutta. Keskityn dokumenttielokuviin erilaisten tuotantovaiheiden kautta ja kerron oman tuotantomme etenemisestä ideasta käsikirjoitukseen ja tuotannon kautta leikkauspöydältä valmiiksi dokumentiksi
Intern kontroll med hjälp av ekonomisystemet Lemonsoft
Detta arbete kommer att behandla intern kontroll i ekonomisystemet Lemonsoft. Arbetets syfte är att läsaren ska förstå innebörden av intern kontroll samt att ge en uppfattning om hur ett ekonomisystem kan förbättra ett företags interna kontroll.
Den teoretiska referensramen behandlar intern kontroll och riskhantering samt de risker som en bristande intern kontroll kan medföra. I arbetets teoridel behandlas även den interna kontrollens betydelse i två olika processer; inköpsfaktura processen och försäljningsfaktura processen. För att läsaren lättare ska förstå arbetets empiriska del, behandlar den teoretiska referensramen även begreppet ekonomisystem. Den empiriska delen behandlar tillvägagångssättet i de båda processerna i ekonomisystemet Lemonsoft.
I slutsatsen följer en sammanfattning över teorin och empirin. Där redogörs för hur den interna kontrollen i ett ekonomisystem syns i de båda processerna samt hur ett ekonomisystem kan stöda den interna kontrollen i ett företag.Tämä opinnäytetyö käsittelee Lemonsoftin toiminnanohjausjärjestelmän sisäistä valvontaa. Työn tarkoituksena on esitellä lukijalle sisäisen valvonnan merkitystä ja antaa käsitys siitä, kuinka kirjanpitojärjestelmä voi parantaa yrityksen sisäistä valvontaa.
Työ koostuu kahdesta osasta. Ensimmäinen osa on teoreettinen osuus, joka käsittelee sisäistä valvontaa, riskienhallintaa sekä riskejä, joita sisäisen valvonnan puute voi aiheuttaa. Työn teoriassa käsitellään myös sisäisen valvonnan merkitystä kahdessa eri prosessissa; osto- ja myyntilaskuprosesseissa. Jotta lukija ymmärtäisi helpommin työn empiirisen osan, teoria käsittelee myös toiminnanohjausjärjestelmää. Empiirinen osa käsittelee edellä mainittuja prosesseja Lemonsoftin talousjärjestelmässä sisäisen valvonnan näkökannasta.
Työn johtopäätös on yhteenveto työn teoreettisesta ja empiirisestä osasta. Johtopäätöksessä kuvataan, kuinka kirjanpitojärjestelmän sisäinen valvonta näkyy molemmissa prosesseissa ja kuinka kirjanpitojärjestelmä voi tukea yrityksen sisäistä valvontaa.This thesis focuses on internal control in Lemonsoft. The purpose of the work is to present to the reader the meaning of internal control and to explain how an accounting system can improve a company's internal control.
The theoretical framework deals with internal control and risk management as well as the risks that a lack of internal control can cause. The theory of the thesis also focuses on the importance of internal control in two different processes; the purchase invoice process and the sales invoice process. The theoretical framework also focuses on the concept of financial system, which helps the reader to more easily understand the empirical part of the work. The empirical part deals with these two processes in Lemonsoft and how they work.
In the conclusion follows a summary of theory and empiricism. It describes how internal control in an accounting system works in both processes and how an accounting system can support internal control in a company
LXR regulation of hepatic gene expression and pituitary dependent hormones.
The liver X receptors (LXRs) are ligand activated transcription factors
involved in the regulation of metabolic genes. LXRs are considered
potential drug targets to treat metabolic disorders, however, unwanted
side effects, as revealed from rodent studies, include increased
lipogenesis in the liver and increased plasma triglyceride levels. In
this respect, it is of utmost importance to carry out studies also in
human cell systems. In addition to LXR, growth hormone (GH), a peptide
hormone originating from the pituitary and acting through its cognate
cell surface receptor, has effects on cholesterol, lipid and glucose
metabolism and a cross talk between LXR and GH signaling is conceivable.
The GH secretion pattern is sexually dimorphic resulting in sex
characteristic expression of many GH target genes in the liver and
further characterization of hepatic genes with regard to dependency on
sex specific GH secretion will help to understand liver physiology and
molecular mechanisms. Furthermore, whether LXR activation affects
hypothalamo-pituitary-hormonal axes had not been fully elucidated and was
considered of interest to explore.
In Paper I, we characterized the akr1b7 gene expression in rat liver as
female specific and dependent on the female GH secretion pattern. AKR1B7
is a protein involved in detoxification of lipid peroxidation- and
steroidogenesis byproducts. The effect of GH was exerted directly on the
hepatocyte but required ongoing protein synthesis, as shown in studies on
primary rat hepatocytes in culture. Akr1b7 is in mice described as an LXR
target gene. Surprisingly, treatment of rat hepatocytes with the
synthetic LXR agonist T0901317 did not induce akr1b7, while co-treatment
with GH and T0901317 resulted in reduced GH induced akr1b7 expression.
T0901317 also interfered with the GH induced expression of the female
specific CYP2C12, suggesting that LXR activation interferes with
signaling of the female GH pattern in regulation of female specific liver
genes.
In Paper II, we used human primary hepatocytes and the synthetic LXR
ligand GW3965 to identify human LXR target genes by microarray.
LXR-regulation of genes playing important roles in intermediary
metabolism was confirmed by qRT-PCR and data suggested that
LXR-activation renders the liver less responsive to insulin. Comparative
experiments on human and rat hepatocytes uncovered major species
differences with bearings on the LXRs as drug targets in humans; the
adipocyte differentiation related protein (ADFP), a protein strongly
correlated with hepatic lipid storage, was induced in human but not in
rat hepatocytes. This indicates a higher risk for development of hepatic
steatosis in human than in rat livers upon pharmacological LXR targeting.
In Paper III, we studied the effects of the LXR agonist T0901317 on
hypothalamo-pituitaryhormonal axes in male rats. After one week of
exposure the 24 h GH secretion profile was marginally affected, just a
small reduction in peak frequency and number was detected. On the other
hand, T0901317 treatment resulted in decreased T3 levels, and T4 levels
tended to increase, resulting in significantly changed T3/T4 ratio. The
underlying cause could be decreased hepatic conversion of T4 to T3, the
active thyroid hormone, as deiodinase 1 expression in liver was decreased
after LXR activation. Hypothyroidism has metabolic consequences for the
individual and must be considered as an unacceptable side effect of
pharmacological LXR agonism if occurring also in human.
In Paper IV, the molecular mechanism of LXR regulation of the human ADFP
gene, identified as an LXR target gene in Paper II, was studied in human
hepatocytes. A difference in activation of the gene with two commonly
used synthetic LXR agonists, GW3965 and T0901317, was evident; while
GW3965 induced the gene, T0901317 did not. Consistent with gene
expression data, ChIP assays showed recruitment of GW3965-activated LXR
to the 3 -UTR containing a putative LXRE and to the promoter region,
while T0901317-activated LXR was not recruited to these regions.
Luciferase reporter assays confirmed LXR responsiveness of the putative
LXRE. Interestingly, both LXR ligands induced the construct,
demonstrating the importance of chromatin structure and suggesting the
formation of a chromatin loop in the regulation of the human ADFP gene by
certain LXR agonists.
In conclusion, the studies presented in this thesis reveal important
species differences in response to LXR activation. Ligand- and gene
specific effects are demonstrated as well as LXR effects on pituitary
hormone axes. In addition, we show cross talk between LXR and GH
signaling. This urgently calls for experimental models based on human
basis and refined strategies in developing LXR agonists for treatment of
metabolic disorders
