    Elective Cancer Surgery in COVID-19-Free Surgical Pathways During the SARS-CoV-2 Pandemic: An International, Multicenter, Comparative Cohort Study.

    PURPOSE: As cancer surgery restarts after the first COVID-19 wave, health care providers urgently require data to determine where elective surgery is best performed. This study aimed to determine whether COVID-19-free surgical pathways were associated with lower postoperative pulmonary complication rates compared with hospitals with no defined pathway. PATIENTS AND METHODS: This international, multicenter cohort study included patients who underwent elective surgery for 10 solid cancer types without preoperative suspicion of SARS-CoV-2. Participating hospitals included patients from local emergence of SARS-CoV-2 until April 19, 2020. At the time of surgery, hospitals were defined as having a COVID-19-free surgical pathway (complete segregation of the operating theater, critical care, and inpatient ward areas) or no defined pathway (incomplete or no segregation, areas shared with patients with COVID-19). The primary outcome was 30-day postoperative pulmonary complications (pneumonia, acute respiratory distress syndrome, unexpected ventilation). RESULTS: Of 9,171 patients from 447 hospitals in 55 countries, 2,481 were operated on in COVID-19-free surgical pathways. Patients who underwent surgery within COVID-19-free surgical pathways were younger with fewer comorbidities than those in hospitals with no defined pathway but with similar proportions of major surgery. After adjustment, pulmonary complication rates were lower with COVID-19-free surgical pathways (2.2% v 4.9%; adjusted odds ratio [aOR], 0.62; 95% CI, 0.44 to 0.86). This was consistent in sensitivity analyses for low-risk patients (American Society of Anesthesiologists grade 1/2), propensity score-matched models, and patients with negative SARS-CoV-2 preoperative tests. The postoperative SARS-CoV-2 infection rate was also lower in COVID-19-free surgical pathways (2.1% v 3.6%; aOR, 0.53; 95% CI, 0.36 to 0.76). CONCLUSION: Within available resources, dedicated COVID-19-free surgical pathways should be established to provide safe elective cancer surgery during current and before future SARS-CoV-2 outbreaks

    Surgical site infections after emergency hernia repair: substudy from the Management of Acutely Symptomatic Hernia (MASH) study

    Introduction Acutely symptomatic abdominal wall and groin hernias (ASH) are a common acute surgical presentation. There are limited data to guide decisions related to surgical repair technique and use of antibiotics, which can be driven by increased risk of surgical site infection (SSI) in this group. This study aims to report rates of SSI following ASH repair and explore the use of patient-reported outcome measure reporting in this setting. Methods An 18-week, UK-based, multicentre prospective cohort study (NCT04197271) recruited adults with ASH. This study reports operatively managed patients. Data on patient characteristics, inpatient management, quality of life, complications, and wound healing (Bluebelle score) were collected. Descriptive analyses were performed to estimate event rates of SSI and regression analysis explored the relationship between Bluebelle scores and SSI. The 30 and 90-day follow-up visits assessed complications and quality of life. Results The MASH study recruited 273 patients, of whom 218 were eligible for this study, 87.2 per cent who underwent open repair. Mesh was used in 123 patients (50.8 per cent). Pre- and postoperative antibiotics were given in 163 (67.4 per cent) and 28 (11.5 per cent) patients respectively. There were 26 reported SSIs (11.9 per cent). Increased BMI, incisional, femoral, and umbilical hernia were associated with higher rates of SSI (P = 0.006). In 238 patients, there was a difference in healthy utility values at 90 days between patients with and without SSI (P = 0.025). Also, when analysing 191 patients with Bluebelle scores, those who developed an SSI had higher Bluebelle values (P < 0.001). Conclusion SSI is frequent in repair of acutely symptomatic hernia and correlates with BMI and site of hernia

    Breast cancer management pathways during the COVID-19 pandemic: outcomes from the UK ‘Alert Level 4’ phase of the B-MaP-C study

    Abstract: Background: The B-MaP-C study aimed to determine alterations to breast cancer (BC) management during the peak transmission period of the UK COVID-19 pandemic and the potential impact of these treatment decisions. Methods: This was a national cohort study of patients with early BC undergoing multidisciplinary team (MDT)-guided treatment recommendations during the pandemic, designated ‘standard’ or ‘COVID-altered’, in the preoperative, operative and post-operative setting. Findings: Of 3776 patients (from 64 UK units) in the study, 2246 (59%) had ‘COVID-altered’ management. ‘Bridging’ endocrine therapy was used (n = 951) where theatre capacity was reduced. There was increasing access to COVID-19 low-risk theatres during the study period (59%). In line with national guidance, immediate breast reconstruction was avoided (n = 299). Where adjuvant chemotherapy was omitted (n = 81), the median benefit was only 3% (IQR 2–9%) using ‘NHS Predict’. There was the rapid adoption of new evidence-based hypofractionated radiotherapy (n = 781, from 46 units). Only 14 patients (1%) tested positive for SARS-CoV-2 during their treatment journey. Conclusions: The majority of ‘COVID-altered’ management decisions were largely in line with pre-COVID evidence-based guidelines, implying that breast cancer survival outcomes are unlikely to be negatively impacted by the pandemic. However, in this study, the potential impact of delays to BC presentation or diagnosis remains unknown

    Thermal performance characterization of a photovoltaic driven domestic refrigerator

    This paper presents the cool-down, warm-up and steady state performance of a 100 W AC operated domestic refrigerator powered by a field of photovoltaic panels, a battery bank and an inverter. It is shown that there is no degradation in the performance when a non-sinusoidal waveform AC source is used to operate the refrigerator although it may involve only a slight additional heating of the hermetic compressor. Thermal mapping of the temperatures at various points on the refrigerator is provided for steady state, cool-down, warm-up, periodic opening of the door and ice making. An energy flow diagram is given for a steady state sunny day operation. Major sources of losses are identified

    Photovoltaic Panel-Generator Based Autonomous Power Source For Small Refrigeration Units

    This article describes an autonomous power source for a domestic refrigeration unit which is powered by a field of photovoltaic panels backed-up by a generator set. Salient design features and results from some of the tests on the unit are presented. Methodologies for reliable and efficient operation of the refrigerator have been evolved. A finite time delay between cut-out and cut-in of the compressor, changes in invertor design to meet the demands at start and at run of the motor, choice of battery capacity so as to eliminate the need for a power conditioner are found to result in energy conservation. The entire unit has been made from indigenously available components and uses minimal electronic controls. Such units have applications for the storage of vaccines and life saving medicines which require uninterrupted refrigeration, in medical shops, rural health centres, veterinary laboratories, etc

    The objective of this paper is to describe salient features of a small refrigerator powered by a field of photovoltaic panels backed up by a petrol-start, kerosene-run portable generator set. Such units find applications in medical shops, veterinary applications and storage of vaccines in primary health centers. The modular arrangement allows use of certain components as a part of an uninterrupted power supply system for such refrigerators. Some preliminary results on temperature distributions and performance data are also presented

    Diurnal and environmental characterization of solar photovoltaic panels using a PC-AT add on plug in card

    This article describes the development of an instrumentation system for characterizing photovoltaic panels. It uses an add on card (which contains DAC, ADC and digital input/output ports) in conjunction with a PC-AT. The panels in a field are selected in succession through a set of relays actuated by the digital output port. The selected panel is loaded in discreet steps by a transistor loading circuit which is activated by the digital to analog convertor to trace the most significant portion of the I-V curve. The data shows that the power output is invariably lower than that specified by the manufacturer. The reasons for drop in output have been analyzed. It is shown that these effects can be represented by the curvature of I-V characteristic at the maximum power point (OP). The loss of power due to accumulation of dust and the increase in temperature of the panels can be significant
