    Finishing the euchromatic sequence of the human genome

    The sequence of the human genome encodes the genetic instructions for human physiology, as well as rich information about human evolution. In 2001, the International Human Genome Sequencing Consortium reported a draft sequence of the euchromatic portion of the human genome. Since then, the international collaboration has worked to convert this draft into a genome sequence with high accuracy and nearly complete coverage. Here, we report the result of this finishing process. The current genome sequence (Build 35) contains 2.85 billion nucleotides interrupted by only 341 gaps. It covers ∼99% of the euchromatic genome and is accurate to an error rate of ∼1 event per 100,000 bases. Many of the remaining euchromatic gaps are associated with segmental duplications and will require focused work with new methods. The near-complete sequence, the first for a vertebrate, greatly improves the precision of biological analyses of the human genome including studies of gene number, birth and death. Notably, the human enome seems to encode only 20,000-25,000 protein-coding genes. The genome sequence reported here should serve as a firm foundation for biomedical research in the decades ahead

    Robotic surgery for cT2 kidney cancer: analysis of the National Cancer Database

    Robotic surgery for renal cell carcinoma (RCC) is increasingly adopted for cT1 disease, but its utilization for cT2 disease remains unexplored. We aimed to characterize the trend in robotic approach for cT2 RCC. The National Cancer Database was queried for patients who were diagnosed with cT2N0M0 RCC from 2010 to 2016 and underwent subsequent radical (RN) or partial (PN) nephrectomy. Analysis of treatment trends was performed and logistic regression (LR) undertaken for predictors of surgical approach. 21,258 patients met inclusion criteria for analysis; 1698 (8%) underwent a PN and 19,560 (92%) underwent RN. Use of robotics in PN increased 346% (12.3-42.6%) and 351% (6.2-21.8%) for RN during the studied time period. Robotic PN or RN was associated with shorter hospital stay compared to non-robotic approaches (p \u3c 0.001). Academic institutions were more likely to perform a robotic procedure and the uninsured were less likely to receive robotic approach. There was no association between age, sex, race, or income and surgical approach. On LR, robotic approach was independently associated with academic institutions and a more recent year of diagnosis. There was no significant difference in the rate of positive margins, 30-day readmission, or 30/90-day mortality between approaches. Robotic PN and RN is becoming an increasingly popular approach in the treatment of cT2 RCC. Utilization of robotics is associated with academic institutions and results in a shorter hospital stay without significant differences rate of positive margins, readmission rates, or 30/90-day mortality

    Analysis of Complications After Robot-Assisted Radical Cystectomy Between 2002-2021

    OBJECTIVE: To identify trends in complications following robot-assisted radical cystectomy (RARC) using a multi-institutional database, the International Robotic Cystectomy Consortium (IRCC). METHODS: A retrospective review of the IRCC database was performed (2976 patients, 26 institutions from 11 countries). Postoperative complications were categorized as overall or high grade (≥ Clavien Dindo III) and were further categorized based on type/organ site. Descriptive statistics was used to summarize the data. Multivariate analysis (MVA) was used to identify variables associated with overall and high-grade complications. Cochran-Armitage trend test was used to describe the trend of complications over time. RESULTS: 1777 (60%) patients developed postoperative complications following RARC, 51% of complications occurred within 30 days of RARC, 19% between 30-90 days, and 30% after 90 days. 835 patients (28%) experienced high-grade complications. Infectious complications (25%) were the most prevalent, while bleeding (1%) was the least. The incidence of complications was stable between 2002-2021. Gastrointestinal and neurologic postoperative complications increased significantly (P \u3c .01, for both) between 2005 and 2020 while thromboembolic (P = .03) and wound complications (P \u3c .01) decreased. On MVA, BMI (OR 1.03, 95%CI 1.01-1.05, P \u3c .01), prior abdominal surgery (OR 1.26, 95%CI 1.03-1.56, P = .03), receipt of neobladder (OR 1.52, 95%CI 1.17-1.99, P \u3c .01), positive nodal disease (OR 1.33, 95%CI 1.05-1.70, P = .02), length of inpatient stay (OR 1.04, 95%CI 1.02-1.05, P \u3c .01) and ICU admission (OR 1.67, 95%CI 1.36-2.06, P \u3c .01) were associated with high-grade complications. CONCLUSION: Overall and high-grade complications after RARC remained stable between 2002-2021. GI and neurologic complications increased, while thromboembolic and wound complications decreased
