    Demonstration of Optical Nonlinearity in InGaAsP/InP Passive Waveguides

    We report on the study of the third-order nonlinear optical interactions in Inx_{x}Ga1x_{1-x}Asy_{y}P1y_{1-y}/InP strip-loaded waveguides. The material composition and waveguide structures were optimized for enhanced nonlinear optical interactions. We performed self-phase modulation, four-wave mixing and nonlinear absorption measurements at the pump wavelength 1568 nm in our waveguides. The nonlinear phase shift of up to 2.5π2.5\pi has been observed in self-phase modulation experiments. The measured value of the two-photon absorption coefficient α2\alpha_2 was 15 cm/GW. The four-wave mixing conversion range, representing the wavelength difference between maximally separated signal and idler spectral components, was observed to be 45 nm. Our results indicate that InGaAsP has a high potential as a material platform for nonlinear photonic devices, provided that the operation wavelength range outside the two-photon absorption window is selected

    Design and control of a parallel linkage wrist for robotic microsurgery

    This paper presents the design and control of a teleoperated robotic system for dexterous micromanipulation tasks at the meso-scale, specifically open microsurgery. Robotic open microsurgery is an unexplored yet potentially a high impact area of surgical robotics. Microsurgical operations, such as microanastomosis of blood vessels and reattachment of nerve fibers, require high levels of manual dexterity and accuracy that surpass human capabilities. A 3-DoF robotic wrist is designed and built based on a spherical five-bar mechanism. The wrist is attached to a 3-axis commercial off-the-shelf linear stage, achieving a fully dexterous system. Design requirements are determined using motion data collected during a simulated microanastomosis operation. The wrist design is optimized to maximize workspace and manipulability. The system is teleoperated using a haptic device, and has the required bandwidth to replicate microsurgical motions. The system was successfully used in a micromanipulation task to stack 1 mm-diameter metal spheres. The micromanipulation system presented here may improve surgical outcomes during open microsurgery by offering better accuracy and dexterity to surgeons.Engineering and Applied Science

    The CAFA challenge reports improved protein function prediction and new functional annotations for hundreds of genes through experimental screens

    Background The Critical Assessment of Functional Annotation (CAFA) is an ongoing, global, community-driven effort to evaluate and improve the computational annotation of protein function. Results Here, we report on the results of the third CAFA challenge, CAFA3, that featured an expanded analysis over the previous CAFA rounds, both in terms of volume of data analyzed and the types of analysis performed. In a novel and major new development, computational predictions and assessment goals drove some of the experimental assays, resulting in new functional annotations for more than 1000 genes. Specifically, we performed experimental whole-genome mutation screening in Candida albicans and Pseudomonas aureginosa genomes, which provided us with genome-wide experimental data for genes associated with biofilm formation and motility. We further performed targeted assays on selected genes in Drosophila melanogaster, which we suspected of being involved in long-term memory. Conclusion We conclude that while predictions of the molecular function and biological process annotations have slightly improved over time, those of the cellular component have not. Term-centric prediction of experimental annotations remains equally challenging; although the performance of the top methods is significantly better than the expectations set by baseline methods in C. albicans and D. melanogaster, it leaves considerable room and need for improvement. Finally, we report that the CAFA community now involves a broad range of participants with expertise in bioinformatics, biological experimentation, biocuration, and bio-ontologies, working together to improve functional annotation, computational function prediction, and our ability to manage big data in the era of large experimental screens.Peer reviewe

    Impact of opioid-free analgesia on pain severity and patient satisfaction after discharge from surgery: multispecialty, prospective cohort study in 25 countries

    Background: Balancing opioid stewardship and the need for adequate analgesia following discharge after surgery is challenging. This study aimed to compare the outcomes for patients discharged with opioid versus opioid-free analgesia after common surgical procedures.Methods: This international, multicentre, prospective cohort study collected data from patients undergoing common acute and elective general surgical, urological, gynaecological, and orthopaedic procedures. The primary outcomes were patient-reported time in severe pain measured on a numerical analogue scale from 0 to 100% and patient-reported satisfaction with pain relief during the first week following discharge. Data were collected by in-hospital chart review and patient telephone interview 1 week after discharge.Results: The study recruited 4273 patients from 144 centres in 25 countries; 1311 patients (30.7%) were prescribed opioid analgesia at discharge. Patients reported being in severe pain for 10 (i.q.r. 1-30)% of the first week after discharge and rated satisfaction with analgesia as 90 (i.q.r. 80-100) of 100. After adjustment for confounders, opioid analgesia on discharge was independently associated with increased pain severity (risk ratio 1.52, 95% c.i. 1.31 to 1.76; P < 0.001) and re-presentation to healthcare providers owing to side-effects of medication (OR 2.38, 95% c.i. 1.36 to 4.17; P = 0.004), but not with satisfaction with analgesia (beta coefficient 0.92, 95% c.i. -1.52 to 3.36; P = 0.468) compared with opioid-free analgesia. Although opioid prescribing varied greatly between high-income and low- and middle-income countries, patient-reported outcomes did not.Conclusion: Opioid analgesia prescription on surgical discharge is associated with a higher risk of re-presentation owing to side-effects of medication and increased patient-reported pain, but not with changes in patient-reported satisfaction. Opioid-free discharge analgesia should be adopted routinely

    This was originally written as a term paper for the Center for Homeland Defense and Security at the Naval Postgraduate School (www.chds.us).A communication method successfully demonstrated using readily available amateur (ham) radio equipment with minor modifications, mounted on a tethered helium-filled balloon or high-lift kite to establish ad hoc two way radio communications in remote areas, radio dead zones and in the case of a repeater failure. Tested on both VHF and UHF bands, one person can setup in 15 minutes. Cost is approximately $2,000.00. Minimum wind requirement is approximately 6 mph for a kite. Maximum wind for tethered spherical helium balloon is approximately 20 mph

    CHDS State/LocalThe VI is under-prepared for major natural disasters and terrorism. Many emergency plans are incomplete, based on inaccurate information, wrongly optimistic and assume an infrastructure that does not exist. The VI is at risk to a direct terrorist attack and also subject to whatever secondary consequences result from a stateside attack, because virtually one hundred percent of food, fuel, medicine, and oil used to make electricity and potable water - and tourists, the economy's lifeblood -- are imported. Also, (1) geographically, help is not readily available from any neighboring city or state; (2) the VI's first responders are likely to be affected by the same disaster they are responding to and unable to assist; (3) the VI must assume more of the emergency preparedness functions that would typically be shared between neighboring communities; (4) the VI must have more funding than similar communities on the mainland; and (5) preparedness planning must include the importing of mainland resources. The purpose of this thesis is to point out how the VI is different from the mainland U.S. This thesis documents vulnerabilities and shows how the Government's existing infrastructure is inadequate. Included is discussion of how the police, social issues, and existing VI Government leadership affect the vulnerabilities. It examines existing preparedness plans. One chapter discusses how management can harm Homeland Security while another chapter deals with change and proposes a Terrorism and Natural Disasters Planning Group.
