    Self-Organizing Maps For Knowledge Discovery From Corporate Databases To Develop Risk Based Prioritization For Stagnation

    Stagnation or low turnover of water within water distribution systems may result in water quality issues, even for relatively short durations of stagnation / low turnover if other factors such as deteriorated aging pipe infrastructure are present. As leakage management strategies, including the creation of smaller pressure management zones, are implemented increasingly more dead ends are being created within networks and hence potentially there is an increasing risk to water quality due to stagnation / low turnover. This paper presents results of applying data driven tools to the large corporate databases maintained by UK water companies. These databases include multiple information sources such as asset data, regulatory water quality sampling, customer complaints etc. A range of techniques exist for exploring the interrelationships between various types of variables, with a number of studies successfully using Artificial Neural Networks (ANNs) to probe complex data sets. Self Organising Maps (SOMs), are a class of unsupervised ANN that perform dimensionality reduction of the feature space to yield topologically ordered maps, have been used successfully for similar problems to that posed here. Notably for this application, SOM are trained without classes attached in an unsupervised fashion. Training combines competitive learning (learning the position of a data cloud) and co-operative learning (self-organising of neighbourhoods). Specifically, in this application SOMs performed multidimensional data analysis of a case study area (covering a town for an eight year period). The visual output of the SOM analysis provides a rapid and intuitive means of examining covariance between variables and exploring hypotheses for increased understanding. For example, water age (time from system entry, from hydraulic modelling) in combination with high pipe specific residence time and old cast iron pipe were found to be strong explanatory variables. This derived understanding could ultimately be captured in a tool providing risk based prioritisation scores

    Background Findings from the RESTART trial suggest that starting antiplatelet therapy might reduce the risk of recurrent symptomatic intracerebral haemorrhage compared with avoiding antiplatelet therapy. Brain imaging features of intracerebral haemorrhage and cerebral small vessel diseases (such as cerebral microbleeds) are associated with greater risks of recurrent intracerebral haemorrhage. We did subgroup analyses of the RESTART trial to explore whether these brain imaging features modify the effects of antiplatelet therapy

    MRSA transmission dynamics among interconnected acute, intermediate-term, and long-term healthcare facilities in Singapore

P. and M. T. G. H.).Background: Methicillin-resistant Staphylococcus aureus (MRSA) is the most common healthcare-associated multidrug-resistant organism. Despite the interconnectedness between acute care hospitals (ACHs) and intermediate- and long-term care facilities (ILTCFs), the transmission dynamics of MRSA between healthcare settings is not well understood. Methods: We conducted a cross-sectional study in a network comprising an ACH and 5 closely affiliated ILTCFs in Singapore. A total of 1700 inpatients were screened for MRSA over a 6-week period in 2014. MRSA isolates underwent whole-genome sequencing, with a pairwise single-nucleotide polymorphism (Hamming distance) cutoff of 60 core genome single-nucleotide polymorphisms used to define recent transmission clusters (clades) for the 3 major clones. Results: MRSA prevalence was significantly higher in intermediate-term (29.9%) and long-term (20.4%) care facilities than in the ACH (11.8%) (P < .001). The predominant clones were sequence type [ST] 22 (n = 183; 47.8%), ST45 (n = 129; 33.7%), and ST239 (n = 26; 6.8%), with greater diversity of STs in ILTCFs relative to the ACH. A large proportion of the clades in ST22 (14 of 21 clades; 67%) and ST45 (7 of 13; 54%) included inpatients from the ACH and ILTCFs. The most frequent source of the interfacility transmissions was the ACH (n = 28 transmission events; 36.4%). Conclusions: MRSA transmission dynamics between the ACH and ILTCFs were complex. The greater diversity of STs in ILTCFs suggests that the ecosystem in such settings might be more conducive for intrafacility transmission events. ST22 and ST45 have successfully established themselves in ILTCFs.     A marketing research tool for destination marketing organizations’ logo design

    The literature widely acknowledges visual communication in the form of a logo as a potential marketing tool for attracting visitors. Focusing exclusively on logos, this study uses the Haitian Destination Marketing Organisation (DMO) logo as a case study to explain how the choice of colors of a logo can benefit from market research methods. In essence, the study develops an analytical framework and contributes to the body of meta-literature in tourism marketing research. The results of the research affect current marketing practices in tourism, because firms seemingly overlook some important elements when developing a new logo. This situation is the case of the Haitian DMO and possibly other DMOs in the world

    Health, education, and social care provision after diagnosis of childhood visual disability

    Aim: To investigate the health, education, and social care provision for children newly diagnosed with visual disability.Method: This was a national prospective study, the British Childhood Visual Impairment and Blindness Study 2 (BCVIS2), ascertaining new diagnoses of visual impairment or severe visual impairment and blindness (SVIBL), or equivalent vi-sion. Data collection was performed by managing clinicians up to 1-year follow-up, and included health and developmental needs, and health, education, and social care provision.Results: BCVIS2 identified 784 children newly diagnosed with visual impairment/SVIBL (313 with visual impairment, 471 with SVIBL). Most children had associated systemic disorders (559 [71%], 167 [54%] with visual impairment, and 392 [84%] with SVIBL). Care from multidisciplinary teams was provided for 549 children (70%). Two-thirds (515) had not received an Education, Health, and Care Plan (EHCP). Fewer children with visual impairment had seen a specialist teacher (SVIBL 35%, visual impairment 28%, χ2p < 0.001), or had an EHCP (11% vs 7%, χ2p < 0 . 01).Interpretation: Families need additional support from managing clinicians to access recommended complex interventions such as the use of multidisciplinary teams and educational support.     Genomic signatures of human and animal disease in the zoonotic pathogen Streptococcus suis

    Streptococcus suis causes disease in pigs worldwide and is increasingly implicated in zoonotic disease in East and South-East Asia. To understand the genetic basis of disease in S. suis, we study the genomes of 375 isolates with detailed clinical phenotypes from pigs and humans from the United Kingdom and Vietnam. Here, we show that isolates associated with disease contain substantially fewer genes than non-clinical isolates, but are more likely to encode virulence factors. Human disease isolates are limited to a single-virulent population, originating in the 1920 s when pig production was intensified, but no consistent genomic differences between pig and human isolates are observed. There is little geographical clustering of different S. suis subpopulations, and the bacterium undergoes high rates of recombination, implying that an increase in virulence anywhere in the world could have a global impact over a short timescale.Peer reviewe
