    Enhanced recovery protocol in laparoscopic liver surgery

    Introduction Enhanced recovery protocols (ERP) accelerate recovery and shorten postoperative hospital stay. This increased knowledge of ERPs has also gradually implemented into liver surgery. However, in laparoscopic liver surgery (LLS), the experience of optimized perioperative care protocols is still limited. Methods We prospectively studied the implementation of multimodal ERP principles to LLS in the first 100 consecutive patients. Opioid-sparing multimodal pain management was applied together with early mobilization already in the postoperative care unit (PACU). Drains and catheters were avoided and per oral intake was initiated promptly. Primary pain control was achieved with iv NSAIDS, low-dose opioid and corticosteroids. Combination of per oral ibuprofen and long-acting tramadol was routinely administered shortly after operation. The multiprofessional adherence to the protocol was also evaluated. Results Investigated LLS was performed during Aug 2016-Apr 2019. Operations were done due to malignancy in 83 (83%) of cases, mostly for colorectal liver metastases (n = 52, 52%). Forty-eight (48%) of the operated patients were female. Median age was 65 years (range 17-91). The American Society of Anaesthesiologists Physical Status (ASA) classification median was three. Median postoperative hospital stay was 2 days (range 1-8 days). More than seventy percent of patients were discharged by the second postoperative day and nearly ninety percent by the third postoperative day. Complications after surgery were few. The new ERP elements were adopted in most of the cases. Conclusions ERP was introduced safely and effectively after LLS. The adherence to the ERP was good. Routine discharge 1-2 days after LLS is realistic and achievable.Peer reviewe

    Impact of Machine Learning Assistance on the Quality of Life Prediction for Breast Cancer Patients

    Proper and well-timed interventions may improve breast cancer patient adaptation, resilience and quality of life (QoL) during treatment process and time after disease. The challenge is to identify those patients who would benefit most from a particular intervention. The aim of this study was to measure whether the machine learning prediction incorporated in the clinical decision support system (CDSS) improves clinicians' performance to predict patients' QoL during treatment process. We conducted an experimental setup in which six clinicians used CDSS and predicted QoL for 60 breast cancer patients. Each patient was evaluated both with and without the aid of machine learning prediction. The clinicians were also open-ended interviewed to investigate the usage and perceived benefits of CDSS with the machine learning prediction aid. Clinicians' performance to evaluate the patients' QoL was higher with the aid of machine learning predictions than without the aid. AUROC of clinicians was .777 (95% CI .691 - .857) with the aid and .755 (95% CI .664 - .840) without the aid. When the machine learning model's prediction was correct, the average accuracy (ACC) of the clinicians was .788 (95% CI .739 - .838) with the aid and .717 (95% CI .636 - .798) without the aid.Peer reviewe

    IKÄPIHA – ikäihmisten pitkäaikaispalvelujen harmonisointi maakunnissa

    Yhdenvertaisuus on keskeinen arvo suomalaisessa sote-palvelujärjestelmässä, ja sille on myös lainsäädännölliset perusteet. Ikääntyneiden palvelujen käytössä on alueellisia vaihteluja. Tämä johtuu ainakin osittain muusta kuin ikärakenteesta tai sairastavuudesta. Selvityksen tarkoituksena oli tarkastella ikääntyneiden pitkäaikaispalvelujen nykyrakenteita ja arvioida palvelujen myöntämisperusteiden harmonisoinnin vaikutuksia palvelujen peittävyyteen ja kustannuksiin maakunnissa. Raportissa kuvataan palvelujen harmonisoitumista perustuen muutamaan maakuntaan, joissa on jo nykyisin yksi maakunnallinen järjestäjäorganisaatio sekä arvioidaan harmonisoinnin potentiaalisia vaikutuksia laskennallisesti. Selvitys osoitti, että harmonisoinnilla olisi merkittäviä vaikutuksia sekä palvelujen peittävyyksiin että kustannuksiin. Selvityksessä tarkastellun esimerkkipalvelurakenteen mukaisesti toteutettuna harmonisointi tuottaisi nykyisellä väestörakenteella noin puolen miljardin euron vuosittaiset kustannussäästöt koko maan tasolla yhteen laskettuna. Laskelma on kuitenkin vain teoreettinen, koska se perustuu mallinnettuihin kustannuksiin. Lisäksi säästöjen aikaan saaminen edellyttäisi todellisuudessa mittavaa kehitystyötä palvelurakenteiden keventämiseksi. Harmonisoituminen, jonka havaittiin jo osittain tapahtuneen palvelujaan yhdenmukaistaneissa maakunnissa, tarkoittaisi nykyistä yhdenmukaisempia palvelurakenteita ja yhdenvertaisempaa palveluihin pääsyä, mikä olisi jo itsessään arvokas lopputulos. Harmonisointi on yksi osatekijä ikääntyvän väestön palvelujen järjestämisessä yhdenvertaisesti ja taloudellisesti kestävästi, mutta palvelutarpeisiin ja kustannuksiin on jatkossa pyrittävä vaikuttamaan yhtäaikaisesti monin eri tavoin.Tämä julkaisu on toteutettu osana valtioneuvoston selvitys- ja tutkimussuunnitelman toimeenpanoa.     Amyloid-beta 42 (A beta 42) and phosphorylated tau (pTau) levels in cerebrospinal fluid (CSF) reflect core features of the pathogenesis of Alzheimer's disease (AD) more directly than clinical diagnosis. Initiated by the European Alzheimer & Dementia Biobank (EADB), the largest collaborative effort on genetics underlying CSF biomarkers was established, including 31 cohorts with a total of 13,116 individuals (discovery n = 8074; replication n = 5042 individuals). Besides the APOE locus, novel associations with two other well-established AD risk loci were observed; CR1 was shown a locus for A beta 42 and BIN1 for pTau. GMNC and C16orf95 were further identified as loci for pTau, of which the latter is novel. Clustering methods exploring the influence of all known AD risk loci on the CSF protein levels, revealed 4 biological categories suggesting multiple A beta 42 and pTau related biological pathways involved in the etiology of AD. In functional follow-up analyses, GMNC and C16orf95 both associated with lateral ventricular volume, implying an overlap in genetic etiology for tau levels and brain ventricular volume.Peer reviewe

We first perform a large genetic association study by merging all available case-control datasets and by-proxy study results (discovery n = 409,435 and validation size n = 58,190). Here, we add six variants associated with Alzheimer's disease risk (near APP, CHRNE, PRKD3/NDUFAF7, PLCG2 and two exonic variants in the SHARPIN gene). Assessment of the polygenic risk score and stratifying by APOE reveal a 4 to 5.5 years difference in median age at onset of Alzheimer's disease patients in APOE ɛ4 carriers. Because of this study, the underlying mechanisms of APP can be studied to refine the amyloid cascade and the polygenic risk score provides a tool to select individuals at high risk of Alzheimer's disease

    Rare coding variants in PLCG2, ABI3, and TREM2 implicate microglial-mediated innate immunity in Alzheimer's disease

    We identified rare coding variants associated with Alzheimer’s disease (AD) in a 3-stage case-control study of 85,133 subjects. In stage 1, 34,174 samples were genotyped using a whole-exome microarray. In stage 2, we tested associated variants (P<1×10-4) in 35,962 independent samples using de novo genotyping and imputed genotypes. In stage 3, an additional 14,997 samples were used to test the most significant stage 2 associations (P<5×10-8) using imputed genotypes. We observed 3 novel genome-wide significant (GWS) AD associated non-synonymous variants; a protective variant in PLCG2 (rs72824905/p.P522R, P=5.38×10-10, OR=0.68, MAFcases=0.0059, MAFcontrols=0.0093), a risk variant in ABI3 (rs616338/p.S209F, P=4.56×10-10, OR=1.43, MAFcases=0.011, MAFcontrols=0.008), and a novel GWS variant in TREM2 (rs143332484/p.R62H, P=1.55×10-14, OR=1.67, MAFcases=0.0143, MAFcontrols=0.0089), a known AD susceptibility gene. These protein-coding changes are in genes highly expressed in microglia and highlight an immune-related protein-protein interaction network enriched for previously identified AD risk genes. These genetic findings provide additional evidence that the microglia-mediated innate immune response contributes directly to AD development

    Multiancestry analysis of the HLA locus in Alzheimer's and Parkinson's diseases uncovers a shared adaptive immune response mediated by HLA-DRB1*04 subtypes

This article contains supporting information online at https://www.pnas.org/lookup/suppl/doi:10.1073/pnas.2302720120/-/DCSupplemental.Across multiancestry groups, we analyzed Human Leukocyte Antigen (HLA) associations in over 176,000 individuals with Parkinson's disease (PD) and Alzheimer's disease (AD) versus controls. We demonstrate that the two diseases share the same protective association at the HLA locus. HLA-specific fine-mapping showed that hierarchical protective effects of HLA-DRB1*04 subtypes best accounted for the association, strongest with HLA-DRB1*04:04 and HLA-DRB1*04:07, and intermediary with HLA-DRB1*04:01 and HLA-DRB1*04:03. The same signal was associated with decreased neurofibrillary tangles in postmortem brains and was associated with reduced tau levels in cerebrospinal fluid and to a lower extent with increased Aβ42. Protective HLA-DRB1*04 subtypes strongly bound the aggregation-prone tau PHF6 sequence, however only when acetylated at a lysine (K311), a common posttranslational modification central to tau aggregation. An HLA-DRB1*04-mediated adaptive immune response decreases PD and AD risks, potentially by acting against tau, offering the possibility of therapeutic avenues.This work was supported by the Michael J.
