    In this article, we present some archaeological testimonies of glass lamps and wick holders located in contexts of the 5th-7th centuries, in the episcopal group of Barcelona and in that of Egara (Terrassa). Likewise, the meaning that lamps and lighting had in cult buildings, their status and consideration through written texts and the theological symbolism of light are addressed.En este artículo presentamos algunos testimonios arqueológicos de lámparas de vidrio y portamechas localizados en contextos de los siglos V-VII, en el grupo episcopal de Barcelona y en el de Egara (Terrassa). Asimismo, se aborda el significado que tenían las lámparas y la iluminación en los edificios de culto, su estatus y consideración a través de los textos escritos y el simbolismo teológico de la luz

    Impact of the implementation of best practice guidelines on nurse's evidence-based practice and on nurses' work environment: research protocol

    Abstract Aim: To determine the impact of the Best Practice Spotlight Organization® initia- tive on nurses' perception of their work environment and their attitudes to evidence- based practice. Design: Quasi-experimental, multicentre study. The intervention is the participation in Best Prectice Spotilight Organizations to implement Best Practice Guidelines. Methods: The study will include seven centres in the interventional group and 10 in the non-equivalent control group, all of them belonging to the Spanish national health system. The Practice Environment Scale of the Nursing Work Index, and the Health Sciences Evidence-Based Practice Questionnaire will be administered to a sample of 1,572 nurses at the beginning of the programme and at 1 year. This 3-year study started in April 2018 and will continue until December 2021. Statistical analy- ses will be carried out using the SPSS 25.0. This project was approved by the Drug Research Ethics Committee of the Parc de Salut Mar and registered in Clinical Trials. Discussion: The study findings will show the current state of nurses' perception of their work environment and attitudes to evidence-based practice, and possible changes in these parameters due to the programme. Impact: The findings could provide a strong argument for health policymakers to scale up the Best Practice Spotlight Organization® initiative in the Spanish national health system

    Impact of opioid-free analgesia on pain severity and patient satisfaction after discharge from surgery: multispecialty, prospective cohort study in 25 countries

    Background: Balancing opioid stewardship and the need for adequate analgesia following discharge after surgery is challenging. This study aimed to compare the outcomes for patients discharged with opioid versus opioid-free analgesia after common surgical procedures.Methods: This international, multicentre, prospective cohort study collected data from patients undergoing common acute and elective general surgical, urological, gynaecological, and orthopaedic procedures. The primary outcomes were patient-reported time in severe pain measured on a numerical analogue scale from 0 to 100% and patient-reported satisfaction with pain relief during the first week following discharge. Data were collected by in-hospital chart review and patient telephone interview 1 week after discharge.Results: The study recruited 4273 patients from 144 centres in 25 countries; 1311 patients (30.7%) were prescribed opioid analgesia at discharge. Patients reported being in severe pain for 10 (i.q.r. 1-30)% of the first week after discharge and rated satisfaction with analgesia as 90 (i.q.r. 80-100) of 100. After adjustment for confounders, opioid analgesia on discharge was independently associated with increased pain severity (risk ratio 1.52, 95% c.i. 1.31 to 1.76; P < 0.001) and re-presentation to healthcare providers owing to side-effects of medication (OR 2.38, 95% c.i. 1.36 to 4.17; P = 0.004), but not with satisfaction with analgesia (beta coefficient 0.92, 95% c.i. -1.52 to 3.36; P = 0.468) compared with opioid-free analgesia. Although opioid prescribing varied greatly between high-income and low- and middle-income countries, patient-reported outcomes did not.Conclusion: Opioid analgesia prescription on surgical discharge is associated with a higher risk of re-presentation owing to side-effects of medication and increased patient-reported pain, but not with changes in patient-reported satisfaction.     Heterozygous Variants in KMT2E Cause a Spectrum of Neurodevelopmental Disorders and Epilepsy.

    We delineate a KMT2E-related neurodevelopmental disorder on the basis of 38 individuals in 36 families. This study includes 31 distinct heterozygous variants in KMT2E (28 ascertained from Matchmaker Exchange and three previously reported), and four individuals with chromosome 7q22.2-22.23 microdeletions encompassing KMT2E (one previously reported). Almost all variants occurred de novo, and most were truncating. Most affected individuals with protein-truncating variants presented with mild intellectual disability. One-quarter of individuals met criteria for autism. Additional common features include macrocephaly, hypotonia, functional gastrointestinal abnormalities, and a subtle facial gestalt. Epilepsy was present in about one-fifth of individuals with truncating variants and was responsive to treatment with anti-epileptic medications in almost all. More than 70% of the individuals were male, and expressivity was variable by sex; epilepsy was more common in females and autism more common in males. The four individuals with microdeletions encompassing KMT2E generally presented similarly to those with truncating variants, but the degree of developmental delay was greater. The group of four individuals with missense variants in KMT2E presented with the most severe developmental delays. Epilepsy was present in all individuals with missense variants, often manifesting as treatment-resistant infantile epileptic encephalopathy. Microcephaly was also common in this group. Haploinsufficiency versus gain-of-function or dominant-negative effects specific to these missense variants in KMT2E might explain this divergence in phenotype, but requires independent validation.     Abordajes Terapéuticos No Farmacológicos para el Tratamiento de la Cefalea Tensional. Revisión bibliográfica.

    Introducción: La cefalea tensional (CT) es la más prevalente dentro de las cefaleas primarias. Representa un impacto económico alto y una repercusión importante en la vida de los pacientes que las sufren y en el sistema público de salud. Objetivos: Conocer el número y fechas de publicación de los estudios realizados en el tratamiento de la CT. Material y Métodos: Se procedió a una búsqueda bibliográfica en PubMed recurriendo al uso de las palabras clave contenidas en dichas publicaciones referidas específicamente a CT y se realizó una búsqueda incluyendo los términos tension-type headache y manual therapy, manipulation therapy, physical therapy, vertebral manipulation, cervical manipulation, osteopathy, osteopathic treatment, osteopathic medicine, osteopathic manipulative treatment, osteopathic manipulative. Se encontraron y analizaron un total de 5 estudios que cumplieron los criterios de selección. Resultados: Los resultados encontrados tras la revisión mostraron que respecto a la cefalea tensional (Tension-type headache) existen 2.506 estudios desde 1947 hasta la actualidad. Los resultados mostraron hasta 15 estudios que comenzaron en 1977 hasta el 2009 cuando se asoció a osteopatía (osteopathy) como concepto general resultaron; con tratamiento osteopático (osteopathic treatment) 14 estudios desde 1993 hasta 2011; con medicina osteopática (osteopathic medicine) resultaron 8 estudios desde 1993 hasta 2009; con tratamiento osteopático manipulativo (osteopathic manipulative treatment) 11 estudios desde 1993 hasta 2009; y con osteopatía manipulativa (osteopathic manipulative) 8 estudios desde 1993 hasta 2011. Conclusiones: Existe escasez de estudios que relacionen la terapia osteopática y la CT. Respecto a la aplicación del tratamiento osteopático, junto y por separado, resulta eficaz, pero es necesario que los estudios tengan mayor calidad metodológica

    Prospective One-Year Follow-Up of Sensory Processing in Phelan–McDermid Syndrome

    Background: Phelan–McDermid syndrome (PMS) is caused by the loss (deletion) of a small portion of chromosome 22 in a region designated q13.3 (22q13.3 deletion). PMS is one of the most common genetic forms of autism spectrum disorder (ASD) in which sensory reactivity difficulties have been described on limited occasions. Methods: The objective of this study is to identify whether changes in sensory reactivity skills occur after one year of follow-up in a group of 44 participants diagnosed with PMS. All participants completed the Short Sensory Profile (SSP). Two-factor ANOVA tests were performed with repeated measures for the study of the evolution of the scores. Results: Participants with PMS showed significant changes after one year of follow-up in sensory reactivity skills associated with tactile hyperreactivity (p = 0.003). The rest of the study variables did not show significant differences compared to the baseline assessment, showing definite differences associated with patterns of hypo-responsiveness and sensory seeking, low/weak energy, and difficulties in auditory filtering. Conclusions: Understanding the evolution of sensory reactivity skills can facilitate the adjustment to behavioral changes in people with PMS and design-targeted interventions to address sensory reactivity challenges

    The Impact of Social Distancing Due to COVID-19 on Activities of Daily Living in Parkinson’s Disease

    Introduction: To explore the impact of the lockdown and social distancing measures, applied for one year, due to the COVID-19 pandemic on Activities of Daily Living in patients with Parkinson’s disease, as well as to determine the association between daily performance and tasks requiring more manipulative dexterity. Methods: Data collection was carried out between 18 January and 22 March 2021 through telephone interviews. Patients were recruited from associations of patients with Parkinson’s disease in Spain. A questionnaire was designed including items from standardized tools of the Activities of Daily Living Questionnaire to measure the level of independence and from the Dexterity Questionnaire for manipulative dexterity. Results: There were 126 participants aged 36–89 years, 58% of whom were male. The results of our study reveal a significant decline in almost all the ADLs assessed. There is a moderate correlation between the degree of dependence in ADLs and the difficulty in performing activities requiring manipulative dexterity. Conclusions: Social isolation related to the COVID-19 pandemic and its consequences may have contributed to an increase in the deterioration of manipulative ability, leading to a loss of ability to perform ADLs. These results show specific needs to be considered in the rehabilitation treatment of these patients
