    Implementación de la metodología del ciclo de deming en la gestión de procesos operativos de un taller automotriz

    La presente tesis es un trabajo de investigación y aplicación que tiene como propósito determinar la influencia que tiene la implementación del Ciclo de Deming en los procesos operativos de la empresa Renuevo en la ciudad de Trujillo, con el fin de que se pueda tener una mejor gestión de estos y mejorar el servicio y la productividad de los trabajadores. En la investigación, se aplica del Ciclo de Deming, utilizando herramientas tales como la metodología de las 5S y las Fichas de Control de Calidad, de los cuales en una primera instancia se procedió a identificar las diversas falencias existentes en los procesos operativos de la empresa, permitiéndonos tener un plan de acción para su mejora. Al aplicar dichas metodologías, se obtuvieron mejoras tanto en los procesos operativos de la empresa, como en la forma de laborar de los trabajadores, es así que se redujeron los retrabajos identificados en más del 70%. Además, en la aplicación de las 5S se obtuvo un incremento de 49% a 85% de cumplimiento para la última semana de aplicación. Asimismo, se pudo reducir un 10% en el tiempo total de los procesos operativos. Por último, mediante el análisis financiero se confirmó que es factible llevar a cabo la implementación del Ciclo de Deming, ya que se obtuvo un Valor Actual Neto de S/.64,872.27 Nuevo Soles y una Tasa de Retorno de 287%. También se encontró un Costo Beneficio de 4.77 y un periodo de recuperación de inversión de 4 meses.This thesis is a research and application that aims to determine the influence of the implementation of the Deming Cycle in the operating processes of the company Renuevo in the city of Trujillo, in order to have a better management of these and, therefore, improve the service and productivity of workers. In the research, some tools of the Deming Cycle are applied, such as the methodology of the 5S and the Quality Control Sheets, from which in a first instance we proceeded to identify the various flaws existing in the company's operational processes, allowing us to have an action plan for its improvement. When applying these methodologies, improvements were obtained both in the company's operating processes and in the way of working of the workers, so that the rework identified in more than 70% was reduced. In addition, in the application of the 5S there was an increase of 49% to 85% compliance for the last week of application. Likewise, it was possible to reduce 10% in the total time of the operational processes. Finally, through the financial analysis it was confirmed that it is feasible to carry out the implementation of the Deming Cycle, since a Net Present Value of S/. 64,872.27 Nuevos Soles and a Return Rate of 287% was obtained. A Benefit Cost of 4.77 and an investment recovery period of 4 months were also found

    En el presente trabajo se tiene como finalidad desarrollar una mejor alternativa que sea óptima para la empresa, de esta manera lograr una solución para una mejor capacidad de almacenamiento. En esta investigación se analiza a la empresa Destilería la Caravedo S.R.L., con la finalidad de optar por una alternativa optima que rinda una mayor capacidad de almacenamiento. De esta manera, se evaluó como objetivo general proponer una óptima capacidad de almacenamiento de piscos para lograr la sostenibilidad en el nivel de servicio de la empresa. Por un lado, como primer objetivo específico es realizar un diagnóstico de la situación actual del almacén, el cual está ubicado en Lurín. Asimismo, se analizó como segundo objetivo específico elaborar un análisis de costos financieros, donde se considera revisar las alternativas propuestas como las del almacén actual, las cuales se evaluaron alquiler de almacén, compra de un almacén y por último la ampliación del almacén en Ica. Por otro lado, como el tercer objetivo específico es brindar una alternativa de solución rentable para la organización a largo plazo, el cual se llegó analizar proyecciones hasta el año 2028.The purpose of this work is to develop a better alternative that is optimal for the company, in this way achieving a solution for better storage capacity. In this research, the company Destilería la Caravedo S.R.L. is analyzed, with the aim of opting for an optimal alternative that provides greater storage capacity. In this way, the general objective of proposing an optimal pisco storage capacity to achieve sustainability in the company's service level was evaluated. On the one hand, the first specific objective is to carry out a diagnosis of the current situation of the warehouse, which is located in Lurín. Likewise, the second specific objective was analyzed to prepare a financial cost analysis, where it is considered to review the alternative proposals such as those of the current warehouse, which are evaluated in warehouse rental, purchase of a warehouse and finally the expansion of the warehouse in Ica .     Challenges in dementia care: Comparing key issues from Brazil and the United Kingdom

    The United Kingdom-Brazil Dementia Workshop took place in July 2019 in the city of Belo Horizonte, MG, Brazil, with an interdisciplinary group of health and care professionals from the United Kingdom and from Brazil to address challenges in diagnosis, public perception and care of dementia. The aim of this article is to present the results identified in relation to challenges in the care of dementia, including recommendations that could potentially guide local and State/Municipal authorities and care services for people with dementia in the future. Four key issues were prioritised to identify challenges and generate possible solutions in Brazil and the United Kingdom: I) limitations of current health systems; II) continuous and long-term support for family carers (pre-diagnosis, mourning); III) support for people with advanced dementia and end-of-life care; IV) support for people with young-onset dementia. In both countries, carers feel left without post-diagnostic support; information on the progression of dementia is lacking and some people do not even have a specific diagnosis; encouraging and providing training for carers best manage some of the symptoms is imperative; preparation for end of life care and support carers after the death of their loved ones remains highly needed; strengthening services and qualification of health professionals, also creating protocols to guide dementia-related services represent a common challenge to overcome. The authors outline recommendations according to the issues identified to assist future formulation of adequate policies and services for people with dementia and carers

    Protocol for a cluster randomised controlled feasibility study of Prehospital Optimal Shock Energy for Defibrillation (POSED)

    Aims: The Prehospital Optimal Shock Energy for Defibrillation (POSED) study will assess the feasibility of conducting a cluster randomised controlled study of clinical effectiveness in UK ambulance services to identify the optimal shock energy for defibrillation. Methods: POSED is a pragmatic, allocation concealed, open label, cluster randomised, controlled feasibility study. Defibrillators within a single UK ambulance service will be randomised in an equal ratio to deliver one of three shock strategies 120–150–200 J, 150–200–200 J, 200–200–200 J. Consecutive adults (18 years or over) presenting with out of hospital cardiac arrest requiring defibrillation will be eligible. The study plans to enrol 90 patients (30 in each group). Patients (or their relatives for non-survivors) will be informed about trial participation after the initial emergency has resolved. Survivors will be invited to consent to participate in follow-up (i.e., at 30 days or discharge). The primary feasibility outcome is the proportion of eligible patients who receive the randomised study intervention. Secondary feasibility outcomes will include recruitment rate, adherence to allocated treatment and data completeness. Clinical outcomes will include Return of an Organised Rhythm (ROOR) at 2 minutes post-shock, refibrillation rate, Return of Spontaneous Circulation (ROSC) at hospital handover, survival and neurological outcome at 30 days. Conclusion: The POSED study will assess the feasibility of a large-scale trial and explore opportunities to optimise the trial protocol

    A systematic review of the evidence for single stage and two stage revision of infected knee replacement

    BACKGROUND: Periprosthetic infection about the knee is a devastating complication that may affect between 1% and 5% of knee replacement. With over 79 000 knee replacements being implanted each year in the UK, periprosthetic infection (PJI) is set to become an important burden of disease and cost to the healthcare economy. One of the important controversies in treatment of PJI is whether a single stage revision operation is superior to a two-stage procedure. This study sought to systematically evaluate the published evidence to determine which technique had lowest reinfection rates. METHODS: A systematic review of the literature was undertaken using the MEDLINE and EMBASE databases with the aim to identify existing studies that present the outcomes of each surgical technique. Reinfection rate was the primary outcome measure. Studies of specific subsets of patients such as resistant organisms were excluded. RESULTS: 63 studies were identified that met the inclusion criteria. The majority of which (58) were reports of two-stage revision. Reinfection rated varied between 0% and 41% in two-stage studies, and 0% and 11% in single stage studies. No clinical trials were identified and the majority of studies were observational studies. CONCLUSIONS: Evidence for both one-stage and two-stage revision is largely of low quality. The evidence basis for two-stage revision is significantly larger, and further work into direct comparison between the two techniques should be undertaken as a priority

    Upregulation of PKD1L2 provokes a complex neuromuscular disease in the mouse

    Following a screen for neuromuscular mouse mutants, we identified ostes, a novel N-ethyl N-nitrosourea-induced mouse mutant with muscle atrophy. Genetic and biochemical evidence shows that upregulation of the novel, uncharacterized transient receptor potential polycystic (TRPP) channel PKD1L2 (polycystic kidney disease gene 1-like 2) underlies this disease. Ostes mice suffer from chronic neuromuscular impairments including neuromuscular junction degeneration, polyneuronal innervation and myopathy. Ectopic expression of PKD1L2 in transgenic mice reproduced the ostes myopathic changes and, indeed, caused severe muscle atrophy in Tg(Pkd1l2)/Tg(Pkd1l2) mice. Moreover, double-heterozygous mice (ostes/+, Tg(Pkd1l2)/0) suffer from myopathic changes more profound than each heterozygote, indicating positive correlation between PKD1L2 levels and disease severity. We show that, in vivo, PKD1L2 primarily associates with endogenous fatty acid synthase in normal skeletal muscle, and these proteins co-localize to costameric regions of the muscle fibre. In diseased ostes/ostes muscle, both proteins are upregulated, and ostes/ostes mice show signs of abnormal lipid metabolism.     Developing Transdisciplinary Approaches to Sustainability Challenges: The Need to Model Socio-Environmental Systems in the Longue Durée

    Human beings are an active component of every terrestrial ecosystem on Earth. Although our local impact on the evolution of these ecosystems has been undeniable and extensively documented, it remains unclear precisely how our activities are altering them, in part because ecosystems are dynamic systems structured by complex, non-linear feedback processes and cascading effects. We argue that it is only by studying human–environment interactions over timescales that greatly exceed the lifespan of any individual human (i.e., the deep past or longue durée), we can hope to fully understand such processes and their implications. In this article, we identify some of the key challenges faced in integrating long-term datasets with those of other areas of sustainability science, and suggest some useful ways forward. Specifically, we (a) highlight the potential of the historical sciences for sustainability science, (b) stress the need to integrate theoretical frameworks wherein humans are seen as inherently entangled with the environment, and (c) propose formal computational modelling as the ideal platform to overcome the challenges of transdisciplinary work across large, and multiple, geographical and temporal scales. Our goal is to provide a manifesto for an integrated scientific approach to the study of socio-ecological systems over the long term
