Relation of Iron Status to Prognosis After Acute Coronary Syndrome
Iron deficiency has been extensively researched and is associated with adverse outcomes in heart failure. However, to our knowledge, the temporal evolution of iron status has not been previously investigated in patients with acute coronary syndrome (ACS). Therefore, we aimed to explore the temporal pattern of repeatedly measured iron, ferritin, transferrin, and transferrin saturation (TSAT) in relation to prognosis post-ACS. BIOMArCS (BIOMarker study to identify the Acute risk of a Coronary Syndrome) is a prospective, multicenter, observational cohort study conducted in The Netherlands between 2008 and 2015. A total of 844 patients with post-ACS were enrolled and underwent high-frequency (median 17) blood sampling during 1 year follow-up. Biomarkers of iron status were measured batchwise in a central laboratory. We analyzed 3 patient subsets, including the case-cohort (n = 187). The primary endpoint (PE) was a composite of cardiovascular mortality and repeat nonfatal ACS, including unstable angina pectoris requiring revascularization. The association between iron status and the PE was analyzed using multivariable joint models. Mean age was 63 years; 78% were men, and >50% had iron deficiency at first sample in the case-cohort. After adjustment for a broad range of clinical variables, 1 SD decrease in log-iron was associated with a 2.2-fold greater risk of the PE (hazard ratio 2.19, 95% confidence interval 1.34 to 3.54, p = 0.002). Similarly, 1 SD decrease in log-TSAT was associated with a 78% increased risk of the PE (hazard ratio 1.78, 95% confidence interval 1.17 to 2.65, p = 0.006). Ferritin and transferrin were not associated with the PE. Repeated measurements of iron and TSAT predict risk of adverse outcomes in patients with post-ACS during 1 year follow-up. Attitude toward contraception and abortion among Curaçao women. Ineffective contraception due to limited sexual education?
Background In Curaçao is a high incidence of unintended pregnancies and induced abortions. Most of the induced abortions in Curaçao are on request of the woman and performed by general practitioners. In Curaçao, induced abortion is strictly prohibited, but since 1999 there has been a policy of connivance. We present data on the relevance of economic and socio-cultural factors for the high abortion-rates and the ineffective use of contraception. Methods Structured interviews to investigate knowledge and attitudes toward sexuality, contraception and abortion and reasons for ineffective use of contraceptives among women, visiting general practitioners. Results Of 158 women, 146 (92%) participated and 82% reported that their education on sexuality and about contraception was of good quality. However 'knowledge of reliable contraceptive methods' appeared to be - in almost 50% of the cases - false information, misjudgements or erroneous views on the chance of getting pregnant using coitus interruptus and about the reliability and health effects of oral contraceptive pills. Almost half of the interviewed women had incorrect or no knowledge about reliability of condom use and IUD. 42% of the respondents risked by their behavior an unplanned pregnancy. Most respondents considered abortion as an emergency procedure, not as contraception. Almost two third experienced emotional, physical or social problems after the abortion. Conclusions Respondents had a negative attitude toward reliable contraceptives due to socio-cultural determined ideas about health consequences and limited sexual education. Main economic factors were costs of contraceptive methods, because most health insurances in Curaçao do not cover contraceptives. To improve the effective use of reliable contraceptives, more adequate information should be given, targeting the wrong beliefs and false information. The government should encourage health insurance companies to reimburse contraceptives. Furthermore, improvement of counseling during the abortion procedure is important
The inner wind of IRC+10216 revisited: New exotic chemistry and diagnostic for dust condensation in carbon stars
Aims. We model the chemistry of the inner wind of the carbon star IRC+10216
and consider the effect of periodic shocks induced by the stellar pulsation on
the gas to follow the non-equilibrium chemistry in the shocked gas layers. We
consider a very complete set of chemical families, including hydrocarbons and
aromatics, hydrides, halogens and phosphorous-bearing species. Derived
abundances are compared to the latest observational data from large surveys and
Herschel. Results. The shocks induce a non-equilibrium chemistry in the dust
formation zone of IRC+10216 where the collision destruction of CO in the
post-shock gas triggers the formation of O-bearing species (H2O, SiO). Most of
the modelled abundances agree very well with the latest values derived from
Herschel data on IRC+10216. Hydrides form a family of abundant species that are
expelled into the intermediate envelope. In particular, HF traps all the atomic
fluorine in the dust formation zone. Halogens are also abundant and their
chemistry is independent of the C/O ratio of the star. Therefore, HCl and other
Cl-bearing species should also be present in the inner wind of O-rich AGB or
supergiant stars. We identify a specific region ranging from 2.5 R* to 4 R*,
where polycyclic aromatic hydrocarbons form and grow. The estimated carbon
dust-to-gas mass ratio derived from the mass of aromatics ranges from 1.2 x
10^(-3) to 5.8 x 10^{-3} and agrees well with existing observational values.
The aromatic formation region is located outside hot layers where SiC2 is
produced as a bi-product of silicon carbide dust synthesis. Finally, we predict
that some molecular lines will show flux variation with pulsation phase and
time (e.g., H2O) while other species will not (e.g., CO). These variations
merely reflect the non-equilibrium chemistry that destroys and reforms
molecules over a pulsation period in the shocked gas of the dust formation

Brain enhancement through cognitive training: A new insight from brain connectome
Owing to the recent advances in neurotechnology and the progress in understanding of brain cognitive functions, improvements of cognitive performance or acceleration of learning process with brain enhancement systems is not out of our reach anymore, on the contrary, it is a tangible target of contemporary research. Although a variety of approaches have been proposed, we will mainly focus on cognitive training interventions, in which learners repeatedly perform cognitive tasks to improve their cognitive abilities. In this review article, we propose that the learning process during the cognitive training can be facilitated by an assistive system monitoring cognitive workloads using electroencephalography (EEG) biomarkers, and the brain connectome approach can provide additional valuable biomarkers for facilitating leaners' learning processes. For the purpose, we will introduce studies on the cognitive training interventions, EEG biomarkers for cognitive workload, and human brain connectome. As cognitive overload and mental fatigue would reduce or even eliminate gains of cognitive training interventions, a real-time monitoring of cognitive workload can facilitate the learning process by flexibly adjusting difficulty levels of the training task. Moreover, cognitive training interventions should have effects on brain sub-networks, not on a single brain region, and graph theoretical network metrics quantifying topological architecture of the brain network can differentiate with respect to individual cognitive states as well as to different individuals' cognitive abilities, suggesting that the connectome is a valuable approach for tracking the learning progress. Although only a few studies have exploited the connectome approach for studying alterations of the brain network induced by cognitive training interventions so far, we believe that it would be a useful technique for capturing improvements of cognitive function
Muon Track Reconstruction and Data Selection Techniques in AMANDA
The Antarctic Muon And Neutrino Detector Array (AMANDA) is a high-energy
neutrino telescope operating at the geographic South Pole. It is a lattice of
photo-multiplier tubes buried deep in the polar ice between 1500m and 2000m.
The primary goal of this detector is to discover astrophysical sources of high
energy neutrinos. A high-energy muon neutrino coming through the earth from the
Northern Hemisphere can be identified by the secondary muon moving upward
through the detector. The muon tracks are reconstructed with a maximum
likelihood method. It models the arrival times and amplitudes of Cherenkov
photons registered by the photo-multipliers. This paper describes the different
methods of reconstruction, which have been successfully implemented within
AMANDA. Strategies for optimizing the reconstruction performance and rejecting
background are presented. For a typical analysis procedure the direction of
