    Socioeconomic inequalities in health among Swedish adolescents - adding the subjective perspective

    Abstract Background Socioeconomic inequalities in adolescent health predict future inequalities in adult health. Subjective measures of socioeconomic status (SES) may contribute with an increased understanding of these inequalities. The aim of this study was to investigate socioeconomic health inequalities using both a subjective and an objective measure of SES among Swedish adolescents. Method Cross-sectional HBSC-data from 2002 to 2014 was used with a total sample of 23,088 adolescents aged 11–15 years. Three measures of self-rated health (dependent variables) were assessed: multiple health complaints, life satisfaction and health perception. SES was measured objectively by the Family Affluence Scale (FAS) and subjectively by “perceived family wealth” (independent variables). The trend for health inequalities was investigated descriptively with independent t-tests and the relationship between independent and dependent variables was investigated with multiple logistic regression analysis. Gender, age and survey year was considered as possible confounders. Results Subjective SES was more strongly related to health outcomes than the objective measure (FAS). Also, the relation between FAS and health was weakened and even reversed (for multiple health complaints) when subjective SES was tested simultaneously in regression models (FAS OR: 1.03, CI: 1.00;1.06 and subjective SES OR: 0.66, CI: 0.63;0.68). Conclusions The level of socioeconomic inequalities in adolescent health varied depending on which measure that was used to define SES. When focusing on adolescents, the subjective appraisals of SES is important to consider because they seem to provide a stronger tool for identifying inequalities in health for this group. This finding is important for policy makers to consider given the persistence of health inequalities in Sweden and other high-income countries

It is well documented that ephedra-based products are associated with various cardiovascular adverse effects. With new restrictions placed on such products, companies are now manufacturing caffeine-based ephedra-free herbal supplements. We present the case of 36-year old, previously healthy female who developed malignant hypertension and aortic dissection while taking various caffeine-based dietary supplements. Given the lack of research studies in regards to their safety and efficacy, judicious care should be taken with the use of dietary supplements, including those designated as ephedra-free

    Intraoperative PaO2 is not related to the development of surgical site infections after major cardiac surgery

    Producción CientíficaBackground: The perioperative use of high inspired oxygen fraction (FIO2) for preventing surgical site infections (SSIs) has demonstrated a reduction in their incidence in some types of surgery however there exist some discrepancies in this respect. The aim of this study was to analyze the relationship between PaO2 values and SSIs in cardiac patients. Methods: We designed a prospective study in which 1,024 patients undergoing cardiac surgery were analyzed. Results: SSIs were observed in 5.3% of patients. There was not significant difference in mortality at 30 days between patients with and without SSIs. In the uni and multivariate analysis no differences in function of the inspired oxygen fraction administrated were observed. Conclusions: We observed that the PaO2 in adult cardiac surgery patients was not related to SSI rate

    Fagerstrom test for nicotine dependence vs heavy smoking index in a general population survey

    <p>Abstract</p> <p>Background</p> <p>The Fagerström Test for Nicotine Dependence (FTND) is used for assessing nicotine dependence. A shorter test derived from the FTND used for the general population is the Heavy Smoking Index (HSI) (six questions vs. two). The objective of this study is to compare the validity of the HSI versus the FTND.</p> <p>Methods</p> <p>A survey of tobacco use in the general population was carried out in the northern Spanish region of Galicia using both the FTND and the HSI to study a representative sample of 1655 daily smokers. The HSI was compared with the FTND, considered the gold standard. Measures of sensitivity, specificity and predictive values were calculated. Concordance between the tests was also established (Cohen's kappa).</p> <p>Results</p> <p>Cohen's kappa showed good agreement between measures (Kappa = 0.7); specificity values were also high (Sp = 96.2%). Sensitivity analysis in females (Se = 62.3%) did not show good agreement.</p> <p>Conclusions</p> <p>The HSI can be used as a reasonably good screening test in order to identify daily smokers with high nicotine dependence. Nevertheless, for populations or subpopulations having low nicotine dependence, such as women, the FTND is more reliable.</p

    Individual rules for trail pattern formation in Argentine ants (Linepithema humile)

    We studied the formation of trail patterns by Argentine ants exploring an empty arena. Using a novel imaging and analysis technique we estimated pheromone concentrations at all spatial positions in the experimental arena and at different times. Then we derived the response function of individual ants to pheromone concentrations by looking at correlations between concentrations and changes in speed or direction of the ants. Ants were found to turn in response to local pheromone concentrations, while their speed was largely unaffected by these concentrations. Ants did not integrate pheromone concentrations over time, with the concentration of pheromone in a 1 cm radius in front of the ant determining the turning angle. The response to pheromone was found to follow a Weber's Law, such that the difference between quantities of pheromone on the two sides of the ant divided by their sum determines the magnitude of the turning angle. This proportional response is in apparent contradiction with the well-established non-linear choice function used in the literature to model the results of binary bridge experiments in ant colonies (Deneubourg et al. 1990). However, agent based simulations implementing the Weber's Law response function led to the formation of trails and reproduced results reported in the literature. We show analytically that a sigmoidal response, analogous to that in the classical Deneubourg model for collective decision making, can be derived from the individual Weber-type response to pheromone concentrations that we have established in our experiments when directional noise around the preferred direction of movement of the ants is assumed.Comment: final version, 9 figures, submitted to Plos Computational Biology (accepted

    New Physics in Bs -> J/psi phi: a General Analysis

    Recently, the CDF and D0 collaborations measured indirect CP violation in Bs -> J/psi phi and found a hint of a signal. If taken at face value, this can be interpreted as a nonzero phase of Bs-Bsbar mixing (beta_s), in disagreement with the standard model, which predicts that beta_s ~= 0. In this paper, we argue that this analysis may be incomplete. In particular, there can be new physics (NP) in the bbar -> sbar c cbar decay. If so, the value of beta_s is different than for the case in which NP is assumed to be present only in the mixing. We have examined several models of NP and found that, indeed, there can be significant contributions to the decay. These effects are consistent with measurements in B -> J/psi K* and Bd -> J/psi Ks. Due to the NP in the decay, polarization-dependent indirect CP asymmetries and triple-product asymmetries are predicted in Bs -> J/psi phi.Comment: 28 pages, JHEP, no figures.     The validation analysis of the INSHORE system: a precise and efficient coastal survey system

    Government and environmental entities are becoming increasingly concerned with qualifying and quantifying the erosion effects that are observed in sandy shores. Correspondingly, survey methodologies that gather data for such erosion studies are increasingly being demanded. The responsible entities are continually broadening their areas of interest, are concerned in the establishment of regular monitoring programmes and are demanding high accuracy from the geo-spatial data that is collected. The budget available for such monitoring activities, however, does not parallel the trend in the increasing demand for quality specifications. Survey methodologies need improvement to meet these requirements. We have developed a new land-based survey system-the INSHORE system-that is ideal for low cost, highly efficient and highly precise coastal surveys. The INSHORE system uses hi-tech hardware that is based on high-grade global positioning system (GPS) receivers and a laser distance sensor combined with advanced software algorithms. This system enables the determination of the ground coordinates of the surveyed areas with a precision of 1 to 2 cm, without having a sensor in contact with the ground surface. The absence of physical contact with the ground makes this system suitable for high-efficiency surveys. The accuracy of the positioning, which is based on advanced differential GPS processing, is enhanced by considering the estimated attitude of the GPS receiver holding structure and eliminates undesirable offsets.     The ROSAT International X-ray/Optical Survey (RIXOS): source catalogue

    We describe the ROSAT International X-ray/Optical Survey (RIXOS), a medium-sensitivity survey and optical identification of X-ray sources discovered in ROSAT high Galactic latitude fields (|b|>28°) and observed with the Position Sensitive Proportional Counter (PSPC) detector. The survey made use of the central 17 arcmin of each ROSAT field. A flux limit of 3×10−14 erg cm−2 s−1 (0.5–2 keV) was adopted for the survey, and a minimum exposure time of 8000 s was required for qualifying ROSAT observations. X-ray sources in the survey are therefore substantially above the detection threshold of each field used, and many contain enough counts to allow the X-ray spectral slope to be estimated. Spectroscopic observations of potential counterparts were obtained of all sources down to the survey limit in 64 fields, totalling a sky area of 15.77 deg2. Positive optical identifications are made for 94 per cent of the 296 sources thus examined. A further 18 fields (4.44 deg2), containing 105 sources above the 3×10−14 erg cm−2 s−1 survey limit, are completely optically identified to a higher flux of 8×10−14 erg cm−2 s−1 (0.5–2 keV). Optical spectroscopic data are supplemented by deep CCD imaging of many sources to reveal the morphology of the optical counterparts, and objects too faint to register on Sky Survey plates. The faintest optical counterparts have R∼22. This paper describes the survey method, and presents a catalogue of the RIXOS sources and their optical identifications. Finding charts based on Sky Survey data are given for each source, supplemented by CCD imaging where necessary

    Procalcitonin levels in acute exacerbation of COPD admitted in ICU: a prospective cohort study

    <p>Abstract</p> <p>Background</p> <p>Antibiotics are recommended for severe acute exacerbation of chronic obstructive pulmonary disease (AECOPD) admitted to intensive care units (ICU). Serum procalcitonin (PCT) could be a useful tool for selecting patients with a lower probability of developing bacterial infection, but its measurement has not been investigated in this population.</p> <p>Methods</p> <p>We conducted a single center prospective cohort study in consecutive COPD patients admitted to the ICU for AECOPD between September 2005 and September 2006. Sputum samples or tracheal aspirates were tested for the presence of bacteria and viruses. PCT levels were measured at the time of admittance, six hours, and 24 hours using a sensitive immunoassay.</p> <p>Results</p> <p>Thirty nine AECOPD patients were included, 31 of which (79%) required a ventilator support at admission. The median [25%–75% interquartile range] PCT level, assessed in 35/39 patients, was: 0.096 μg/L [IQR, 0.065 to 0.178] at the time of admission, 0.113 μg/L [IQR, 0.074 to 0.548] at six hours, and 0.137 μg/L [IQR, 0.088 to 0.252] at 24 hours. The highest PCT (PCTmax) levels were less than 0.1 μg/L in 14/35 (40%) patients and more than 0.25 μg/L in 10/35 (29%) patients, suggesting low and high probability of bacterial infection, respectively. Five species of bacteria and nine species of viruses were detected in 12/39 (31%) patients. Among the four patients positive for <it>Pseudomonas aeruginosa</it>, one had a PCTmax less than 0.25 μg/L and three had a PCTmax less than 0.1 μg/L. The one patient positive for <it>Haemophilus influenzae </it>had a PCTmax more than 0.25 μg/L. The presence or absence of viruses did not influence PCT at time of admission (0.068 vs 0.098 μg/L respectively, <it>P </it>= 0.80).</p> <p>Conclusion</p> <p>The likelihood of bacterial infection is low among COPD patients admitted to ICU for AECOPD (40% with PCT < 0.1 μg/L) suggesting a possible inappropriate use of antibiotics. Further studies are necessary to assess the impact of a procalcitonin-based therapeutic strategy in critically ill COPD patients.</p
