    Welcome to Commoning Ethnography

    What does it mean to suggest that ethnography might be a site, source, or scene of commoning? What does combining the idea of commoning with the practice of ethnography allow us to think about or to do that we might not otherwise? Why create a journal dedicated to such an obviously paradoxical thought experiment? This editorial introduction outlines the impetus behind this project, our aspirations for it, and considers some of the questions it raises. It also offers a brief synopsis of the scope of the first issue. &nbsp

    Development of a core set of outcome measures for OAB treatment

Introduction and hypothesis: Standardized measures enable the comparison of outcomes across providers and treatments giving valuable information for improving care quality and efficacy. The aim of this project was to define a minimum standard set of outcome measures and case-mix factors for evaluating the care of patients with overactive bladder (OAB). Methods: The International Consortium for Health Outcomes Measurement (ICHOM) convened an international working group (WG) of leading clinicians and patients to engage in a structured method for developing a core outcome set. Consensus was determined by a modified Delphi process, and discussions were supported by both literature review and patient input. Results: The standard set measures outcomes of care for adults seeking treatment for OAB, excluding residents of long-term care facilities. The WG focused on treatment outcomes identified as most important key outcome domains to patients: symptom burden and bother, physical functioning, emotional health, impact of symptoms and treatment on quality of life, and success of treatment. Demographic information and case-mix factors that may affect these outcomes were also included. Conclusions: The standardized outcome set for evaluating clinical care is appropriate for use by all health providers caring for patients with OAB, regardless of specialty or geographic location, and provides key data for quality improvement activities and research

    Diglossia and identity in Northeast Thailand: Linguistic, social, and political hierarchy

    The paper explores diglossic relations between Central Thai and phasa isan, a variety officially known as a dialect of Thai, but linguistically close to Lao. Phasa isan is spoken by almost one-third of Thailand's population but its speakers in the Northeast are often stigmatized as uneducated and backward. We conducted field research mainly among university students in Ubon Ratchathani, a northeastern border province, by drawing upon data from survey questionnaires, reflective essays, interviews, and field observations. The findings suggest a transitional diglossic relationship in which Central Thai is the High and phasa isan the Low variety. These relationships are discussed in terms of nationalism, social hierarchy, and language maintenance and shift

    The role of pulmonary arterial stiffness in COPD

    AbstractCOPD is the second most common cause of pulmonary hypertension, and is a common complication of severe COPD with significant implications for both quality of life and mortality. However, the use of a rigid diagnostic threshold of a mean pulmonary arterial pressure (mPAP) of ≥25mHg when considering the impact of the pulmonary vasculature on symptoms and disease is misleading. Even minimal exertion causes oxygen desaturation and elevations in mPAP, with right ventricular hypertrophy and dilatation present in patients with mild to moderate COPD with pressures below the threshold for diagnosis of pulmonary hypertension. This has significant implications, with right ventricular dysfunction associated with poorer exercise capability and increased mortality independent of pulmonary function tests.The compliance of the pulmonary artery (PA) is a key component in decoupling the right ventricle from the pulmonary bed, allowing the right ventricle to work at maximum efficiency and protecting the microcirculation from large pressure gradients. PA stiffness increases with the severity of COPD, and correlates well with the presence of exercise induced pulmonary hypertension. A curvilinear relationship exists between PA distensibility and mPAP and pulmonary vascular resistance (PVR) with marked loss of distensibility before a rapid rise in mPAP and PVR occurs with resultant right ventricular failure. This combination of features suggests PA stiffness as a promising biomarker for early detection of pulmonary vascular disease, and to play a role in right ventricular failure in COPD. Early detection would open this up as a potential therapeutic target before end stage arterial remodelling occurs

    Towards an Ethnography Commons

    How might the notion of an ethnography commons transform ethnographic research practice and pedagogy? In this paper, I consider how the concept of the commons, in all of its messiness, might provide a way of not only addressing questions surrounding the boundaries of ethnographic research and knowledge that have been fundamental to anthropology since Writing Culture (Clifford and Marcus 1986), but also for crafting more transformative research and social interventions into the world itself. I do so first by considering how contemporary structures of capitalism are shaping the university, our research, and our relationships with our students. Then, I trace the ways in which the debates about the boundaries of ethnography have transformed research and pedagogy over the last 20 years. Finally, I conclude by suggesting a number of potential trajectories for acting on the promise of the commons through ethnographic teaching and research
