    On exact categories and applications to triangulated adjoints and model structures

    We show that Quillen's small object argument works for exact categories under very mild conditions. This has immediate applications to cotorsion pairs and their relation to the existence of certain triangulated adjoint functors and model structures.     Coordinated and tailored work rehabilitation: a randomized controlled trial with economic evaluation undertaken with workers on sick leave due to musculoskeletal disorders

    Introduction In Denmark, the magnitude and impact of work disability on the individual worker and society has prompted the development of a new "coordinated and tailored work rehabilitation" (CTWR) approach. The aim of this study was to compare the effects of CTWR with conventional case management (CCM) on return-to-work of workers on sick leave due to musculoskeletal disorders (MSDs). Methods The study was a randomized controlled trial with economic evaluation undertaken with workers on sick leave for 4-12 weeks due to MSDs. CTWR consists of a work disability screening by an interdisciplinary team followed by the collaborative development of a RTW plan. The primary outcome variable was registered cumulative sickness absence hours during 12 months follow-up. Secondary outcomes were work status as well as pain intensity and functional disability, measured at baseline, 3 and 12 months follow-up. The economic evaluation (intervention costs, productivity loss, and health care utilization costs) was based on administrative data derived from national registries. Results For the time intervals 0-6 months, 6-12 months, and the entire follow-up period, the number of sickness absence hours was significantly lower in the CTWR group as compared to the control group. The total costs saved in CTWR participants compared to controls were estimated at US 1,366perpersonat6monthsfollowupandUS 1,366 per person at 6 months follow-up and US 10,666 per person at 12 months follow-up. Conclusions Workers on sick leave for 4-12 weeks due to MSD who underwent "CTWR" by an interdisciplinary team had fewer sickness absence hours than controls. The economic evaluation showed that-in terms of productivity loss-CTWR seems to be cost saving for the society

    Gamma-Ray Emissions from Pulsars: Spectra of the TEV Fluxes from Outer-Gap Accelerators

    We study the gamma-ray emissions from an outer-magnetospheric potential gap around a rotating neutron star. Migratory electrons and positrons are accelerated by the electric field in the gap to radiate copious gamma-rays via curvature process. Some of these gamma-rays materialize as pairs by colliding with the X-rays in the gap, leading to a pair production cascade. Imposing the closure condition that a single pair produces one pair in the gap on average, we explicitly solve the strength of the acceleration field and demonstrate how the peak energy and the luminosity of the curvature-radiated, GeV photons depend on the strength of the surface blackbody and the power-law emissions. Some predictions on the GeV emission from twelve rotation-powered pulsars are presented. We further demonstrate that the expected pulsed TeV fluxes are consistent with their observational upper limits. An implication of high-energy pulse phase width versus pulsar age, spin, and magnetic moment is discussed.Comment: Revised to compute absolute TeV spectra (22 pages, 9 figures

    Mobile Robotic Painting of Texture

    Robotic painting is well-established in controlled factory environments, but there is now potential for mobile robots to do functional painting tasks around the everyday world. An obvious first target for such robots is painting a uniform single color. A step further is the painting of textured images. Texture involves a varying appearance, and requires that paint is delivered accurately onto the physical surface to produce the desired effect. Robotic painting of texture is relevant for architecture and in themed environments. A key challenge for robotic painting of texture is to take a desired image as input, and to generate the paint commands to as closely as possible create the desired appearance, according to the robotic capabilities. This paper describes a deep learning approach to take an input ink map of a desired texture, and infer robotic paint commands to produce that texture. We analyze the trade-offs between quality of reconstructed appearance and ease of execution. Our method is general for different kinds of robotic paint delivery systems, but the emphasis here is on spray painting. More generally, the framework can be viewed as an approach for solving a specific class of inverse imaging problems

    Parametric hazard rate models for long-term sickness absence

    PURPOSE: In research on the time to onset of sickness absence and the duration of sickness absence episodes, Cox proportional hazard models are in common use. However, parametric models are to be preferred when time in itself is considered as independent variable. This study compares parametric hazard rate models for the onset of long-term sickness absence and return to work. METHOD: Prospective cohort study on sickness absence with four follow-up years of 53,830 employees working in the private sector in the Netherlands. The time to onset of long-term (>6 weeks) sickness absence and return to work were modelled by parametric hazard rate models. RESULTS: The exponential parametric model with a constant hazard rate most accurately described the time to onset of long-term sickness absence. Gompertz-Makeham models with monotonically declining hazard rates best described return to work. CONCLUSIONS: Parametric models offer more possibilities than commonly used models for time-dependent processes as sickness absence and return to work. However, the advantages of parametric models above Cox models apply mainly for return to work and less for onset of long-term sickness absence

    Occupational and leisure time physical activity in contrasting relation to ambulatory blood pressure

    Background: While moderate and vigorous leisure time physical activities are well documented to decrease the risk for cardiovascular disease, several studies have demonstrated an increased risk for cardiovascular disease in workers with high occupational activity. Research on the underlying causes to the contrasting effects of occupational and leisure time physical activity on cardiovascular health is lacking. The aim of this study was to examine the relation of objective and self-report measures of occupational and leisure time physical activity with 24-h ambulatory systolic blood pressure (BP). Methods: Results for self-reported physical activity are based on observations in 182 workers (60% male, mean age 51 years), while valid objective physical activity data were available in 151 participants. The usual level of physical activity was assessed by 5 items from the Job Content Questionnaire (high physical effort, lifting heavy loads, rapid physical activity, awkward body positions and awkward positions of head or arms at work) and one item asking about the general level of physical activity during non-working time. On a regular working day, participants wore an ambulatory BP monitor and an accelerometer physical activity monitor during 24 h. Associations were examined by means of Analysis of Covariance. Results: Workers with an overall high level of self-reported occupational physical activity as well as those who reported to often lift heavy loads at work had a higher mean systolic BP at work, at home and during sleep. However, no associations were observed between objectively measured occupational physical activity and BP. In contrast, those with objectively measured high proportion of moderate and vigorous leisure time physical activity had a significantly lower mean systolic BP during daytime, while no differences were observed according to self-reported level of leisure time physical activity. Conclusions: These findings suggest that workers reporting static occupational physical activities, unlike general physically demanding tasks characterized by dynamic movements of large muscle groups, are related to a higher daily systolic BP, while high objective levels of moderate and vigorous leisure time physical activity are related to lower daytime systolic BP. Ambulatory systolic BP may be a physiological explanatory factor for the contrasting effects of occupational and leisure time physical activity

    Gravitational Quenching in Massive Galaxies and Clusters by Clumpy Accretion

    We consider a simple gravitational-heating mechanism for the long-term quenching of cooling flows and star formation in massive dark-matter haloes hosting ellipticals and clusters. The virial shock heating in haloes >10^12 Mo triggers quenching in 10^12-13 Mo haloes (Birnboim, Dekel & Neistein 2007). We show that the long-term quenching in haloes >Mmin~7x10^12 Mo could be due to the gravitational energy of cosmological accretion delivered to the inner-halo hot gas by cold flows via ram-pressure drag and local shocks. Mmin is obtained by comparing the gravitational power of infall into the potential well with the overall radiative cooling rate. The heating wins if the gas inner density cusp is not steeper than r^-0.5 and if the masses in the cold and hot phases are comparable. The effect is stronger at higher redshifts, making the maintenance easier also at later times. Clumps >10^5 Mo penetrate to the inner halo with sufficient kinetic energy before they disintegrate, but they have to be <10^8 Mo for the drag to do enough work in a Hubble time. Pressure confined ~10^4K clumps are stable against their own gravity and remain gaseous once below the Bonnor-Ebert mass ~10^8 Mo. They are also immune to tidal disruption. Clumps in the desired mass range could emerge by thermal instability in the outer halo if the conductivity is not too high. Alternatively, such clumps may be embedded in dark-matter subhaloes if the ionizing flux is ineffective, but they separate from their subhaloes by ram pressure before entering the inner halo. Heating by dynamical friction becomes dominant for massive satellites, which can contribute up to one third of the total gravitational heating.     Auditing the Management of Vaccine-Preventable Disease Outbreaks: The Need for a Tool

    Public health activities, especially infectious disease control, depend on effective teamwork. We present the results of a pilot audit questionnaire aimed at assessing the quality of public health services in the management of VPD outbreaks. Audit questionnaire with three main areas indicators (structure, process and results) was developed. Guidelines were set and each indicator was assessed by three auditors. Differences in indicator scores according to median size of outbreaks were determined by ANOVA (significance at p≤0.05). Of 154 outbreaks; eighteen indicators had a satisfactory mean score, indicator “updated guidelines” and “timely reporting” had a poor mean score (2.84±106 and 2.44±1.67, respectively). Statistically significant differences were found according to outbreak size, in the indicators “availability of guidelines/protocol updated less than 3 years ago” (p = 0.03) and “days needed for outbreak control” (p = 0.04). Improving availability of updated guidelines, enhancing timely reporting and adequate recording of control procedures taken is needed to allow for management assessment and improvement
