    Glucoregulatory Consequences and Cardiorespiratory Parameters in Rats Exposed to Chronic–Intermittent Hypoxia: Effects of the Duration of Exposure and Losartan

    Background: Obstructive sleep apnea (OSA) is associated with glucose intolerance. Both chronic sleep disruption and recurrent blood oxygen desaturations (chronic–intermittent hypoxia, CIH) may cause, or exacerbate, metabolic derangements. Methods: To assess the impact of CIH alone, without accompanying upper airway obstructions, on the counter-regulatory response to glucose load and cardiorespiratory parameters, we exposed adult male Sprague-Dawley rats to CIH or sham room air exchanges for 10 h/day for 7, 21, or 35 days and then, 1 day after conclusion of CIH exposure, conducted intravenous glucose-tolerance tests (ivgtt) under urethane anesthesia. Additional rats underwent 35 days of CIH followed by 35 days of regular housing, or had 35 day-long CIH exposure combined with daily administration of the type 1 angiotensin II receptor antagonist, losartan (15 mg/kg, p.o.), and then were also subjected to ivgtt. Results: Compared with the corresponding control groups, CIH rats had progressively reduced glucose-stimulated insulin release and impaired glucose clearance, only mildly elevated heart rate and/or arterial blood pressure and slightly reduced respiratory rate. The differences in insulin release between the CIH and sham-treated rats disappeared in the rats normally housed for 35 days after 35 days of CIH/sham exposure. The losartan-treated rats had improved insulin sensitivity, with no evidence of suppressed insulin release in the CIH group. Conclusion: In adult rats, the glucose-stimulated insulin release is gradually suppressed with the duration of exposure to CIH, but the effect is reversible. Elimination of the detrimental effect of CIH on insulin release by losartan suggests that CIH disrupts glucoregulation through angiotensin/catecholaminergic pathways. Accordingly, treatment with continuous positive airway pressure may ameliorate pre-diabetic conditions in OSA patients, in part, by reducing sympathoexcitatory effects of recurrent nocturnal hypoxia

    Calling time on responsible drinking: A qualitative study of perceptions of information on alcohol product labels

    Objectives: This study aimed to explore (a) how people interpret responsible drinking messages on alcohol product labels, and (b) the acceptability of including health information on labels. Design: Qualitative interviews. Methods: Face-to-face semi-structured interviews were conducted with 20 people aged 21-63; 18 were classified risky drinkers using the AUDIT-C. They were shown three sets of alcohol product labels: one including three responsible drinking messages (drink responsibly), one with three positively worded messages (drinking less reduces risks) and one with three negatively worded messages (drinking more increases risks). Health messages included information about cancer, liver and heart disease. Results: Thematic analysis identified three themes: ambiguity about alcohol labelling; identifying oneself as responsible; and acceptability of enhanced product labelling. Participants were critical of responsible drinking messages and wary of conflicting information in the media. They positioned themselves as responsible, knowledgeable drinkers. They did not appear to support the inclusion of health information on labels; however, novel information was considered more impactful. Conclusions: Responsible drinking messages were seen by our sample as an alcohol industry ploy. Although health messages about cancer were seen as potentially impactful, the ability of consumers to position themselves as unproblematic drinkers means that they may not see the information as relevant to themselevs. Understanding factors that increase the personal relevance of messages is needed, alongside an exploration of a wider range of methods for alcohol health communicatio

    Galaxies in Southern Bright Star Fields I. Near-infrared imaging

    As a prerequisite for cosmological studies using adaptive optics techniques, we have begun to identify and characterize faint sources in the vicinity of bright stars at high Galactic latitudes. The initial phase of this work has been a program of K_s imaging conducted with SOFI at the ESO NTT. From observations of 42 southern fields evenly divided between the spring and autumn skies, we have identified 391 additional stars and 1589 galaxies lying at separations 60" from candidate guide stars in the magnitude range 9.0 R 12.4. When analyzed as a "discrete deep field" with 131 arcmin^2 area, our dataset gives galaxy number counts that agree with those derived previously over the range 16 K_s 20.5. This consistency indicates that in the aggregate, our fields should be suitable for future statistical studies.     The strong transformation of spiral galaxies infalling into massive clusters at z~0.2

    We describe two peculiar galaxies falling into the massive galaxy clusters Abell 1689 (z~0.18) and 2667 (z~0.23) respectively. Hubble Space Telescope images show extraordinary trails composed of bright blue knots (-16.5<M<-11.5 mag) and stellar streams associated with each of these systems. Combining optical, near and mid-infrared and radio observations we prove that while both galaxies show similar extended trails of star-forming knots, their recent star formation histories are different. One (~L*) is experiencing a strong burst of star formation, appearing as a rare example of a luminous infrared cluster galaxy. In comparison, the other (~ 0.1 L*) has recently ceased its star formation activity. Our model suggests that the morphologies and star formation in these galaxies have been influenced by the combined action of tidal interaction (likely with the cluster potential) and of ram pressure with the intracluster medium. These results can be used to gain more insights to the origin of S0s, dwarf and ultra-compact dwarf (UCD) cluster galaxies

    Further Sunyaev-Zel'dovich observations of two Planck ERCSC clusters with the Arcminute Microkelvin Imager

    We present follow-up observations of two galaxy clusters detected blindly via the Sunyaev-Zel'dovich (SZ) effect and released in the Planck Early Release Compact Source Catalogue. We use the Arcminute Microkelvin Imager, a dual-array 14-18 GHz radio interferometer. After radio source subtraction, we find a SZ decrement of integrated flux density -1.08+/-0.10 mJy toward PLCKESZ G121.11+57.01, and improve the position measurement of the cluster, finding the centre to be RA 12 59 36.4, Dec +60 04 46.8, to an accuracy of 20 arcseconds. The region of PLCKESZ G115.71+17.52 contains strong extended emission, so we are unable to confirm the presence of this cluster via the SZ effect.Comment: 4 tables, 3 figures, revised after referee's comments and resubmitted to MNRA

    Microwave observations of spinning dust emission in NGC6946

    We report new cm-wave measurements at five frequencies between 15 and 18GHz of the continuum emission from the reportedly anomalous "region 4" of the nearby galaxy NGC6946. We find that the emission in this frequency range is significantly in excess of that measured at 8.5GHz, but has a spectrum from 15-18GHz consistent with optically thin free-free emission from a compact HII region. In combination with previously published data we fit four emission models containing different continuum components using the Bayesian spectrum analysis package radiospec. These fits show that, in combination with data at other frequencies, a model with a spinning dust component is slightly preferred to those that possess better-established emission mechanisms.Comment: submitted MNRA

    The influence of socioeconomic status on presentation and outcome of acute kidney injury

    Aim Although socioeconomic background is known to impact on the incidence and progression of chronic kidney disease, its influence of on the presentation and outcome for acute kidney injury is not known and is the subject of this study. Design The Welsh National electronic AKI reporting system was used to identify all cases of AKI in patients >18 years of age between March 2015 and November 2017. Methods Socioeconomic classification of patients was derived from the Welsh Index Multiple Deprivation score (WIMD). Patients were grouped according to the WIMD score by their postcode, and the ranked data were categorized into percentiles and correlated with incidence and measures of AKI severity and outcome. Results Date was collected on a total of 57 654 patients. Increased deprivation was associated with higher AKI incidence rates, more episodes of AKI per patient and more severe AKI at presentation. In contrast 90-day mortality was highest in the most affluent areas. Mortality in affluent areas was driven by increased patient age. Corrected for age 90-day mortality was higher in areas of increased deprivation. Conclusion This study highlights that AKI incidence presentation and outcomes are adversely affected by social deprivation. Further studies are required to understand the extent to which these differences reflect patient related factors or regional differences in provision and access to care

    Linear dynamics of weakly viscous accretion disks: A disk analog of Tollmien-Schlichting waves

    This paper discusses new perspectives and approaches to the problem of disk dynamics where, in this study, we focus on the effects of viscous instabilities influenced by boundary effects. The Boussinesq approximation of the viscous large shearing box equations is analyzed in which the azimuthal length scale of the disturbance is much larger than the radial and vertical scales. We examine the stability of a non-axisymmetric potential vorticity mode, i.e. a PV-anomaly. in a configuration in which buoyant convection and the strato-rotational instability do not to operate. We consider a series of boundary conditions which show the PV-anomaly to be unstable both on a finite and semi-infinite radial domains. We find these conditions leading to an instability which is the disk analog of Tollmien-Schlichting waves. When the viscosity is weak, evidence of the instability is most pronounced by the emergence of a vortex sheet at the critical layer located away from the boundary where the instability is generated. For some boundary conditions a necessary criterion for the onset of instability for vertical wavelengths that are a sizable fraction of the layer's thickness and when the viscosity is small is that the appropriate Froude number of the flow be greater than one. This instability persists if more realistic boundary conditions are applied, although the criterion on the Froude number is more complicated. The unstable waves studied here share qualitative features to the instability seen in rotating Blasius boundary layers. The implications of these results are discussed.
