    Review article: the global emergence of Helicobacter pylori antibiotic resistance.

    BackgroundHelicobacter pylori is one of the most prevalent global pathogens and can lead to gastrointestinal disease including peptic ulcers, gastric marginal zone lymphoma and gastric carcinoma.AimTo review recent trends in H. pylori antibiotic resistance rates, and to discuss diagnostics and treatment paradigms.MethodsA PubMed literature search using the following keywords: Helicobacter pylori, antibiotic resistance, clarithromycin, levofloxacin, metronidazole, prevalence, susceptibility testing.ResultsThe prevalence of bacterial antibiotic resistance is regionally variable and appears to be markedly increasing with time in many countries. Concordantly, the antimicrobial eradication rate of H. pylori has been declining globally. In particular, clarithromycin resistance has been rapidly increasing in many countries over the past decade, with rates as high as approximately 30% in Japan and Italy, 50% in China and 40% in Turkey; whereas resistance rates are much lower in Sweden and Taiwan, at approximately 15%; there are limited data in the USA. Other antibiotics show similar trends, although less pronounced.ConclusionsSince the choice of empiric therapies should be predicated on accurate information regarding antibiotic resistance rates, there is a critical need for determination of current rates at a local scale, and perhaps in individual patients. Such information would not only guide selection of appropriate empiric antibiotic therapy but also inform the development of better methods to identify H. pylori antibiotic resistance at diagnosis. Patient-specific tailoring of effective antibiotic treatment strategies may lead to reduced treatment failures and less antibiotic resistance

    Observation of liquid–liquid phase transitions in ethane at 300 K

    We have conducted Raman spectroscopy experiments on liquid ethane (C2H6) at 300 K, obtaining a large amount of data at very high resolution. This has enabled the observation of Raman peaks expected but not previously observed in liquid ethane and a detailed experimental study of the liquid that was not previously possible. We have observed a transition between rigid and nonrigid liquid states in liquid ethane at ca. 250 MPa corresponding to the recently proposed Frenkel line, a dynamic transition between rigid liquid (liquidlike) and nonrigid liquid (gaslike) states beginning in the subcritical region and extending to arbitrarily high pressure and temperature. The observation of this transition in liquid (subcritical) ethane allows a clear differentiation to be made between the Frenkel line (beginning in the subcritical region at higher density than the boiling line) and the Widom lines (emanating from the critical point and not existing in the subcritical region). Furthermore, we observe a narrow transition at ca. 1000 MPa to a second rigid liquid state. We propose that this corresponds to a state in which orientational order must exist to achieve the expected density and can view the transition in analogy to the transition in the solid state away from the orientationally disordered phase I to the orientationally ordered phases II and III

    Gas turbulence modulation in a two-fluid model for gas-solid flows

    Recent rapid progress in the theoretical and experimental study of turbulence modulation has led to greater understanding of the physics of particle-gas turbulence interactions. A new two-fluid model incorporating these advances for relatively dilute gas-solid flows containing high-inertia particles is established. The effect of aerodynamic forces upon the particulate stresses is considered in this kinetic theory-based model, and the influence of the particles on the turbulent gas is addressed: the work associated with drag forces contributes to the gas turbulent energy, and the space occupied by particles restricts the turbulent length scale. The interparticle length scale, which is usually ignored, has been incorporated into a new model for determining the turbulent length scale. This model also considers the transport effect on the turbulent length scale. Simulation results for fully developed steady flows in vertical pipes are compared with a wide range of published experimental data and, generally, good agreement is shown. This comprehensive and validated model accounts for many of the interphase interactions that have been shown to be important

    Mobile health service for HIV screening and care in resource-constrained setting

    Objective: This review paper aims at demonstrating that mobile health services for HIV infection in resource-constrained countries may be particularly useful for HIV screening and treatment of HIV disease and associated co-morbidities, especially for people who have limited access to fixed health facilities, including remote or nomadic populations and socially marginalised people.Data source: PubMed database was used to retrieve appropriate literature related to the following MeSH terms: HIV, testing, counselling, mobile counselling and testing, routine offer of counselling and testing, provider-initiated testing and counselling,home-based counselling and testing, decentralisation.Study selection: All articles that met these inclusion criteria and described work conducted in sub-Saharan Africa were considered. Articles were reviewed for information pertaining to mobile health facilities in the field of HIV infection. Wealso reviewed available articles describing alternative approaches to HIV screening and care delivery in resource-limited settings without time period restrictions up to December 2015.Data synthesis: Data collected were analysed and the results related to the aim of the study.Conclusions: The development of mobile services has benefitted from the  simplification of laboratory tests, including reliable rapid diagnostic tests for HIV, and “point of care” (“POC”) tests for CD4 enumeration. The mobile strategy aims to reach more patients,particularly those living in remote areas, to reduce loss-to-follow-up, and to improve patient outcomes. With a reduction in HIV-related stigma and associated discrimination by using these services, the mobile strategy may assist decentralisation of programs devoted to HIV screening, anti-retroviral treatment and HIV care

    Cognitive Control Errors in Nonhuman Primates Resembling Those in Schizophrenia Reflect Opposing Effects of NMDA Receptor Blockade on Causal Interactions Between Cells and Circuits in Prefrontal and Parietal Cortices

    Background: The causal biology underlying schizophrenia is not well understood, but it is likely to involve a malfunction in how neurons adjust synaptic connections in response to patterns of activity in networks. We examined statistical dependencies between neural signals at the cell, local circuit, and distributed network levels in prefrontal and parietal cortices of monkeys performing a variant of the AX continuous performance task paradigm. We then quantified changes in the pattern of neural interactions across levels of scale following NMDA receptor (NMDAR) blockade and related these changes to a pattern of cognitive control errors closely matching the performance of patients with schizophrenia. Methods: We recorded the spiking activity of 1762 neurons along with local field potentials at multiple electrode sites in prefrontal and parietal cortices concurrently, and we generated binary time series indicating the presence or absence of spikes in single neurons or local field potential power above or below a threshold. We then applied causal discovery analysis to the time series to detect statistical dependencies between the signals (causal interactions) and compared the pattern of these interactions before and after NMDAR blockade. Results: Global blockade of NMDAR produced distinctive and frequently opposite changes in neural interactions at the cell, local circuit, and network levels in prefrontal and parietal cortices. Cognitive control errors were associated with decreased interactions at the cell level and with opposite changes at the network level in prefrontal and parietal cortices. Conclusions: NMDAR synaptic deficits change causal interactions between neural signals at different levels of scale that correlate with schizophrenia-like deficits in cognitive control

    VECTORS of change in the marine environment: Ecosystem and economic impacts and management implications

    Human use of the European marine environment is increasing and diversifying. This is creating new mechanisms for human induced-changes in marine life which need to be understood and quantified as well as the impact of these changes on ecosystems, their structures (e.g. biodiversity) and functioning (e.g. productivity), and the social and economic consequences that arise. The current and emerging pressures are multiple and interacting, arising, for example, from transport, platforms for renewable and nonrenewable energy, exploitation of living and non-living resources, agricultural and industrial discharges, together with wider environmental changes (including climate change). Anticipating the future consequences of these pressures and vectors of change for marine life and of adaptation and mitigation measures (such as the introduction of new technologies and structures, new ballast water practices, ocean and offshore wind energy devices and new fishing strategies) is a prerequisite to the development and implementation of strategies, policies and regulations to manage the marine environment, such as the IMO Convention on ballast water management and the EU Maritime Policy and Marine Strategy Framework Directive

    Phototrophic Fe(II)-oxidation in the chemocline of a ferruginous meromictic lake

Precambrian Banded Iron Formation (BIF) deposition was conventionally attributed to the precipitation of iron-oxides resulting from the abiotic reaction of ferrous iron (Fe(II)) with photosynthetically produced oxygen. Earliest traces of oxygen date from 2.7 Ga, thus raising questions as to what may have caused BIF precipitation before oxygenic photosynthesis evolved. The discovery of anoxygenic phototrophic bacteria thriving through the oxidation of Fe(II) has provided support for a biological origin for some BIFs, but despite reports suggesting that anoxygenic phototrophs may oxidize Fe(II) in the environment, a model ecosystem of an ancient ocean where they are demonstrably active was lacking. Here we show that anoxygenic phototrophic bacteria contribute to Fe(II) oxidation in the water column of the ferruginous sulfate-poor, meromictic lake La Cruz (Spain). We observed in-situ photoferrotrophic activity through stimulation of phototrophic carbon uptake in the presence of Fe(II), and determined light-dependent Fe(II)-oxidation by the natural chemocline microbiota. Moreover, a photoferrotrophic bacterium most closely related to Chlorobium ferrooxidans was enriched from the ferruginous water column. Our study for the first time demonstrates a direct link between anoxygenic photoferrotrophy and the anoxic precipitation of Fe(III)-oxides in a ferruginous water column, providing a plausible mechanism for the bacterial origin of BIFs before the advent of free oxygen. However, photoferrotrophs represent only a minor fraction of the anoxygenic phototrophic community with the majority apparently thriving by sulfur cycling, despite the very low sulfur content in the ferruginous chemocline of Lake La Cruz

    Freeze-Drying of Mononuclear Cells Derived from Umbilical Cord Blood Followed by Colony Formation

    BACKGROUND: We recently showed that freeze-dried cells stored for 3 years at room temperature can direct embryonic development following cloning. However, viability, as evaluated by membrane integrity of the cells after freeze-drying, was very low; and it was mainly the DNA integrity that was preserved. In the present study, we improved the cells' viability and functionality after freeze-drying. METHODOLOGY/PRINCIPAL FINDINGS: We optimized the conditions of directional freezing, i.e. interface velocity and cell concentration, and we added the antioxidant EGCG to the freezing solution. The study was performed on mononuclear cells (MNCs) derived from human umbilical cord blood. After freeze-drying, we tested the viability, number of CD34(+)-presenting cells and ability of the rehydrated hematopoietic stem cells to differentiate into different blood cells in culture. The viability of the MNCs after freeze-drying and rehydration with pure water was 88%-91%. The total number of CD34(+)-presenting cells and the number of colonies did not change significantly when evaluated before freezing, after freeze-thawing, and after freeze-drying (5.4 x 10(4)+/-4.7, 3.49 x 10(4)+/-6 and 6.31 x 10(4)+/-12.27 cells, respectively, and 31+/-25.15, 47+/-45.8 and 23.44+/-13.3 colonies, respectively). CONCLUSIONS: This is the first report of nucleated cells which have been dried and then rehydrated with double-distilled water remaining viable, and of hematopoietic stem cells retaining their ability to differentiate into different blood cells
