    Obstructive sleep apnoea syndrome (OSAS) is a consequence of repetitive oropharyngeal airway narrowing/closure during sleep resulting in chronic intermittent hypoxaemia [1]. OSAS is regarded as an independent risk factor for hypertension (HTN) development [2] and is associated with decreased cerebral blood flow [3], leading to daytime neuropsychological sequelae [1]. Blood pressure (BP) follows a circadian rhythm termed dipping and the absence of nocturnal BP dipping is associated with target-organ damage, cerebrovascular disease, myocardial remodelling and increased cardiovascular events/mortality [4]. Abnormal BP in OSAS typically manifests as reduced nocturnal BP dipping [5].     Nitrate-Rich Beetroot Juice Selectively Lowers Ambulatory Pressures and LDL Cholesterol in Uncontrolled but not Controlled Hypertension: a Pilot Study.

    Background Dietary nitrate has been shown to increase nitrate/nitrite levels in multiple populations, with potential blood pressure lowering effects. However, there are few reports among hypertensives. Aims We aimed to assess the effect of daily nitrate in subject with controlled hypertension vs. uncontrolled hypertension. Methods On day 0, hypertensives wore an ambulatory BP monitor (ABPM) for 24h and fasting blood was taken. Subjects then consumed concentrated beetroot juice (12.9mmol nitrate) for 14 consecutive days. On day 14 subjects consumed their last nitrate dose after fasting blood was drawn and again had an ABPM for 24h. Results According to baseline ABPM, 11 subjects had controlled BP while 8 had uncontrolled BP. There were similar, significant increases in serum nitrate/nitrite in both groups. We observed little change in BP variables among controlled hypertensives. However, there were reductions in BP variables in uncontrolled hypertensives where decreases in nighttime DBP (-6 ± 4.8mmHg), arterial stiffness (-0.08 ± 0.03 ambulatory arterial stiffness index) and LDL (-0.36 ± 0.42mmol/L) reached significance (p=003, 0.05 and 0.046 respectively). Conclusions Our results support the existing data suggesting an anti-hypertensive effect of nitratecontaining beetroot juice, but only among those with uncontrolled hypertension

    End of Project ReportThis study set out to determine the occurrence of diseases and pests in Irish mushroom units and their method of control using chemical, biological and other means of control. It also examined the role of a combination of these methods to enable control with minimal pesticide input. It was found that pesticides alone will never give effective disease and pest control and that they should only be considered an adjunct to the implementation of other methods. They include: (1) Exclusion (2) Containment of spread (3) Elimination. A major factor in good disease and pest control was found to be the implementation of a good programme of hygiene which must be followed from the time of filling a tunnel to the time of emptying after cropping.     Vitamin D as an Adjunctive Therapy in Asthma. Part 2: A Review of Human Studies

    Vitamin D deficiency (VDD) is highly prevalent worldwide, with adverse effects on bone health but also potentially other unfavorable consequences. VDD and asthma-incidence/severity share many common risk factors, including winter season, industrialization, poor diet, obesity, dark skin pigmentation, and high latitude. Multiple anatomical areas relevant to asthma contain both the enzyme responsible for producing activated vitamin D and the vitamin D receptor suggesting that activated vitamin D (1,25- dihydroxyvitamin D) may have important local effects at these sites. Emerging evidence suggests that VDD is associated with increased airway hyperresponsiveness, decreased pulmonary function, worse asthma control, and possibly decreased response to standard antiasthma therapy. However the effect is inconsistent with preliminary evidence from different studies suggesting vitamin D is both beneficial and detrimental to asthma genesis and severity. Current evidence suggests that supplementation with moderate doses of vitamin D may be appropriate for maintenance of bone health in asthmatics, particularly steroid users. However emerging data from an increasing number of randomized, controlled, intervention studies of vitamin D supplementation in pediatric and adult asthma are becoming available and should help determine the importance, if any of vitamin D for asthma pathogenesis.     Hospital Outcomes in Patients Hospitalized for COVID-19 Pneumonia: The Effect of SARS-CoV-2 Vaccination and Vitamin D Status

    SARS-CoV-2 vaccination promises to improve outcomes for patients with COVID-19 pneumonia (most notably those with advanced age and at high risk for severe disease). Here, we examine serum 25-Hydroxyvitamin D (25(OH)D) status and outcomes in both old (\u3e70 years) and young vaccinated (n = 80) and unvaccinated (n = 91) subjects, who were hospitalized due to COVID- 19 pneumonia in a single center (Connolly Hospital Dublin). Outcomes included ICU admission and mortality. Serum 25(OH)D levels were categorized as D30 (/L), D40 (30–49.99 nmol/L) and D50 (50 nmol/L). In multivariate analyses, D30 was independently associated with ICU admission (OR: 6.87 (95% CI: 1.13–41.85) (p = 0.036)) and mortality (OR: 24.81 (95% CI: 1.57–392.1) (p = 0.023)) in unvaccinated patients, even after adjustment for major confounders including age, sex, obesity and pre-existing diabetes mellitus. While mortality was consistently higher in all categories of patients over 70 years of age, the highest observed mortality rate of 50%, seen in patients over 70 years with a low vitamin D state (D30), appeared to be almost completely corrected by either vaccination, or having a higher vitamin D state, i.e., mortality was 14% for vaccinated patients over 70 years with D30 and 16% for unvaccinated patients over 70 years with a 25(OH)D level greater than 30 nmol/L. We observe that high mortality from COVID-19 pneumonia occurs in older patients, especially those who are unvaccinated or have a low vitamin D state. Recent vaccination or having a high vitamin D status are both associated with reduced mortality, although these effects do not fully mitigate the mortality risk associated with advanced age

    Vitamin D deficiency (VDD) is highly prevalent worldwide. The classical role for vitamin D is to regulate calcium absorption form the gastrointestinal tract and influence bone health. Recently vitamin D receptors and vitamin D metabolic enzymes have been discovered in numerous sites systemically supporting diverse extra-skeletal roles of vitamin D, for example in asthmatic disease. Further, VDD and asthma share several common risk factors including high latitude, winter season, industrialization, poor diet, obesity, and dark skin pigmentation. Vitamin D has been demonstrated to possess potent immunomodulatory effects, including effects on T cells and B cells as well as increasing production of antimicrobial peptides (e.g. cathelicidin). This immunomodulation may lead to asthma specific clinical benefits in terms of decreased bacterial/viral infections, altered airway smooth muscle-remodeling and efunction as well as modulation of response to standard anti-asthma therapy (e.g. glucocorticoids and immunotherapy). Thus, vitamin D and its deficiency have a number of biological effects that are potentially important in altering the course of disease pathogenesis and severity in asthma.
