    Designing for Sustainability:Lessons Learned from Four Industrial Projects

    Scientific research addressing the relation between software and sustainability is slowly maturing in two focus areas, related to `sustainable software' and `software for sustainability'. The first is better understood and may include research foci like energy-efficient software and software maintainability. It most-frequently covers `technical' concerns. The second, `software for sustainability', is much broader in both scope and potential impact, as it entails how software can contribute to sustainability goals in any sector or application domain. Next to the technical concerns, it may also cover economic, social, and environmental sustainability. Differently from researchers, practitioners are often not aware or well-trained in all four types of software sustainability concerns. To address this need, in previous work we have defined the Sustainability-Quality Assessment Framework (SAF) and assessed its viability via the analysis of a series of software projects. Nevertheless, it was never used by practitioners themselves, hence triggering the question: What can we learn from the use of SAF in practice? To answer this question, we report the results of practitioners applying the SAF to four industrial cases. The results show that the SAF helps practitioners in (1) creating a sustainability mindset in their practices, (2) uncovering the relevant sustainability-quality concerns for the software project at hand, and (3) reasoning about the inter-dependencies and trade-os of such concerns as well as the related short- and long-term implications. Next to improvements for the SAF, the main lesson for us as researchers is the missing explicit link between the SAF and the (technical) architecture design

    Variability of Care and Access to Transplantation for Children with Biliary Atresia Who Need a Liver Replacement

    Background & Aims: Biliary atresia (BA) is the commonest single etiology indication for liver replacement in children. As timely access to liver transplantation (LT) remains challenging for small BA children (with prolonged waiting time being associated with clinical deterioration leading to both preventable pre- and post-transplant morbidity and mortality), the care pathway of BA children in need of LT was analyzed—from diagnosis to LT—with particular attention to referral patterns, timing of referral, waiting list dynamics and need for medical assistance before LT. Methods: International multicentric retrospective study. Intent-to-transplant study analyzing BA children who had indication for LT early in life (aged < 3 years at the time of assessment), over the last 5 years (2016–2020). Clinical and laboratory data of 219 BA children were collected from 8 transplant centers (6 in Europe and 2 in USA). Results: 39 patients underwent primary transplants. Children who underwent Kasai in a specialist -but not transplant- center were older at time of referral and at transplant. At assessment for LT, the vast majority of children already were experiencing complication of cirrhosis, and the majority of children needed medical assistance (nutritional support, hospitalization, transfusion of albumin or blood) while waiting for transplantation. Severe worsening of the clinical condition led to the need for requesting a priority status (i.e., Peld Score exception or similar) for timely graft allocation for 76 children, overall (35%). Conclusions: As LT currently results in BA patient survival exceeding 95% in many expert LT centers, the paradigm for BA management optimization and survival have currently shifted to the pre-LT management. The creation of networks dedicated to the timely referral to a pediatric transplant center and possibly centralization of care should be considered, in combination with implementing all different graft type surgeries in specialist centers (including split and living donor LTs) to achieve timely LT in this vulnerable population

    ERIS: revitalising an adaptive optics instrument for the VLT

    ERIS is an instrument that will both extend and enhance the fundamental diffraction limited imaging and spectroscopy capability for the VLT. It will replace two instruments that are now being maintained beyond their operational lifetimes, combine their functionality on a single focus, provide a new wavefront sensing module that makes use of the facility Adaptive Optics System, and considerably improve their performance. The instrument will be competitive with respect to JWST in several regimes, and has outstanding potential for studies of the Galactic Center, exoplanets, and high redshift galaxies. ERIS had its final design review in 2017, and is expected to be on sky in 2020.     Central Retinal Artery Occlusion: Current Practice, Awareness and Prehospital Delays in Switzerland

    BACKGROUND AND PURPOSE: Central retinal artery occlusion (CRAO) often leads to permanent monocular blindness. Hence, early recognition and rapid re-perfusion is of paramount importance. This study aims to describe prehospital pathways in CRAO compared to stroke and study the knowledge about CRAO. METHODS: (1) Description of baseline characteristics, prehospital pathways/delays, and acute treatment (thrombolysis/thrombectomy vs. standard of care) of patients with CRAO and ischemic stroke registered in the Swiss Stroke Registry. (2) Online survey about CRAO knowledge amongst population, general practitioners (GPs) and ophthalmologists in Eastern Switzerland. RESULTS: Three hundred and ninety seven CRAO and 32,816 ischemic stroke cases were registered from 2014 until 2019 in 20 Stroke Centers/Units in Switzerland. In CRAO, 25.6% arrived at the hospital within 4 h of symptom onset and had a lower rate of emergency referrals. Hence, the symptom-to-door time was significantly longer in CRAO compared to stroke (852 min. vs. 300 min). The thrombolysis/thrombectomy rate was 13.2% in CRAO and 30.9% in stroke. 28.6% of the surveyed population recognized CRAO-symptoms, 55.4% of which would present directly to the emergency department in contrast to 90.0% with stroke symptoms. Almost 100% of the ophthalmologist and general practitioners recognized CRAO as a medical emergency and 1/3 of them considered IV thrombolysis a potentially beneficial therapy. CONCLUSIONS: CRAO awareness of the general population and physician awareness about the treatment options as well as the non-standardized prehospital organization, seems to be the main reason for the prehospital delays and impedes treating CRAO patients. Educational efforts should be undertaken to improve awareness about CRAO

    Process-evaluation of tropospheric humidity simulated by general circulation models using water vapor isotopologues: 1. comparison between models and observations

    [1] The goal of this study is to determine how H2O and HDO measurements in water vapor can be used to detect and diagnose biases in the representation of processes controlling tropospheric humidity in atmospheric general circulation models (GCMs). We analyze a large number of isotopic data sets (four satellite, sixteen ground-based remote-sensing, five surface in situ and three aircraft data sets) that are sensitive to different altitudes throughout the free troposphere. Despite significant differences between data sets, we identify some observed HDO/H2O characteristics that are robust across data sets and that can be used to evaluate models. We evaluate the isotopic GCM LMDZ, accounting for the effects of spatiotemporal sampling and instrument sensitivity. We find that LMDZ reproduces the spatial patterns in the lower and mid troposphere remarkably well. However, it underestimates the amplitude of seasonal variations in isotopic composition at all levels in the subtropics and in midlatitudes, and this bias is consistent across all data sets. LMDZ also underestimates the observed meridional isotopic gradient and the contrast between dry and convective tropical regions compared to satellite data sets. Comparison with six other isotope-enabled GCMs from the SWING2 project shows that biases exhibited by LMDZ are common to all models. The SWING2 GCMs show a very large spread in isotopic behavior that is not obviously related to that of humidity, suggesting water vapor isotopic measurements could be used to expose model shortcomings.     Process-evaluation of tropospheric humidity simulated by general circulation models using water vapor isotopic observations: 2. Using isotopic diagnostics to understand the mid and upper tropospheric moist bias in the tropics and subtropics

    Evaluating the representation of processes controlling tropical and subtropical tropospheric relative humidity (RH) in atmospheric general circulation models (GCMs) is crucial to assess the credibility of predicted climate changes. GCMs have long exhibited a moist bias in the tropical and subtropical mid and upper troposphere, which could be due to the mis-representation of cloud processes or of the large-scale circulation, or to excessive diffusion during water vapor transport. The goal of this study is to use observations of the water vapor isotopic ratio to understand the cause of this bias. We compare the three-dimensional distribution of the water vapor isotopic ratio measured from space and ground to that simulated by several versions of the isotopic GCM LMDZ. We show that the combined evaluation of RH and of the water vapor isotopic composition makes it possible to discriminate the most likely cause of RH biases. Models characterized either by an excessive vertical diffusion, an excessive convective detrainment or an underestimated in situ cloud condensation will all produce a moist bias in the free troposphere. However, only an excessive vertical diffusion can lead to a reversed seasonality of the free tropospheric isotopic composition in the subtropics compared to observations. Comparing seven isotopic GCMs suggests that the moist bias found in many GCMs in the mid and upper troposphere most frequently results from an excessive diffusion during vertical water vapor transport.
