    Knowledge Transfer through Public-Private Development Partnerships

    This article aims to add to the body of literature on knowledge transfer in Technical and Vocational Education and Training (TVET) focusing on the core stakeholders’ experiences in Public-Private Development Partnership (PPDP) in Ethiopia and Zambia. The qualitative case study comparatively examines practices and barriers to promoting knowledge transfer in two TVET programmes in Ethiopia and Zambia— both applied a similar PPDP model. The study draws on data generated through semi-structured interviews based on narratives perspective. Key informants were purposively selected from among stakeholders who were directly involved in the two initiatives. The data analysis process follows thematic analysis. The results show that anchoring the PPDP initiatives with the nationally mandated Ministry and broadening coalition with the private sector are critical in transferring knowledge into the national TVET systems.  Knowledge transfer through PPDPs is challenged by difficulties of retaining staff the initiatives have invested and partnering with a model school isolated from a TVET system that the PPDP intends to influence.  This article points to consideration of local schools’ prior experience of delivering similar programmes in order to insert new programmes into TVET systems. In Ethiopia, new TVET interventions are installed in a country and school not familiar with the knowledge field contrary to the programme in Zambia which was built on the existing programme. The successful transfer of PPDP interventions and their sustainability, it is argued, requires implementation of the initiatives with sound knowledge of the local context, and application of adaptive practices that encourage collaboration with diversified and committed local stakeholders

    Fecal short chain fatty acids in children living on farms and a link between valeric acid and protection from eczema

    Children growing up on farms have low rates of allergy, but the mechanism for this protective effect has not been fully elucidated. Short chain fatty acids (SCFAs) produced by the gut microbiota may play a role in protection from allergy. We measured fecal SCFA levels in samples collected from 28 farming and 37 control children over the first 3\ua0years of life using gas chromatography. Data on diet and other host factors were recorded and allergy was diagnosed at 8\ua0years of age. Among all children, median propionic and butyric acid concentration increased over the first 3\ua0years, and longer SCFAs typically appeared by 1\ua0year of age. Farm children had higher levels of iso-butyric, iso-valeric and valeric acid at 3\ua0years of age than rural controls. In addition, children with elder siblings had higher levels of valeric acid at 3\ua0years of age, and dietary factors also affected SCFA pattern. High levels of valeric acid at 3\ua0years of age were associated with low rate of eczema at 8\ua0years of age. The fecal SCFA pattern in farm children suggests a more rapid maturation of the gut microbiota. Valeric acid or associated microbes may have protective potential against eczema

    Prenatal phthalate exposures and anogenital distance in Swedish boys

    BACKGROUND: Phthalates are used as plasticizers in soft polyvinyl chloride (PVC) and in a large number of consumer products. Because of reported health risks, diisononyl phthalate (DiNP) has been introduced as a replacement for di(2-ethylhexyl) phthalate (DEHP) in soft PVC. This raises concerns because animal data suggest that DiNP may have antiandrogenic properties similar to those of DEHP. The anogenital distance (AGD)-the distance from the anus to the genitals-has been used to assess reproductive toxicity. OBJECTIVE: The objective of this study was to examine the associations between prenatal phthalate exposure and AGD in Swedish infants. METHODS: AGD was measured in 196 boys at 21 months of age, and first-trimester urine was analyzed for 10 phthalate metabolites of DEP (diethyl phthalate), DBP (dibutyl phthalate), DEHP, BBzP (benzylbutyl phthalate), as well as DiNP and creatinine. Data on covariates were collected by questionnaires. RESULTS: The most significant associations were found between the shorter of two AGD measures (anoscrotal distance; AGDas) and DiNP metabolites and strongest for oh-MMeOP [mono(4-methyl-7-hydroxyloctyl) phthalate] and oxo-MMeOP [mono-(2-ethyl-5-oxohexyl) phthalate]. However, the AGDas reduction was small (4%) in relation to more than an interquartile range increase in DiNP exposure. CONCLUSIONS: These findings call into question the safety of substituting DiNP for DEHP in soft PVC, particularly because a shorter male AGD has been shown to relate to male genital birth defects in children and impaired reproductive function in adult males and the fact that human levels of DiNP are increasing globally

    Body composition in older acute stroke patients after treatment with individualized, nutritional supplementation while in hospital

    <p>Abstract</p> <p>Background</p> <p>Individualized, nutritional support reduced undernutrition among older stroke patients and improved quality of life in our recent randomized, controlled trial. Weight control thus seems to be important after stroke, and methods for monitoring nutritional status need to be simple and non-invasive. Here we aimed to assess if the nutritional intervention altered body composition in men and women in this study cohort, and also to examine the correlation between the methods for assessing body-, fat- and fat-free mass.</p> <p>Methods</p> <p>Acute stroke patients > 65 years at nutritional risk were randomized to either individualized, nutritional treatment with energy- and protein rich supplementation (intervention, n = 58) or routine, nutritional care (control, n = 66) while in hospital. Body composition was assessed with anthropometry and bioelectrical impedance. The follow-up period was three months.</p> <p>Results</p> <p>During the first week while in hospital, weight loss was smaller in the intervention group compared with the controls (P = 0.013). After three months weight- and fat loss were significant in both men and women. Whereas no significant differences were found in changes in body composition between the male study groups, in the women both weight loss (P = 0.022) and fat loss (P = 0.005) was smaller in the intervention group compared with the controls. A high correlation (r = 0.87) between mid upper arm circumference (MUAC) and body mass index (BMI) was found.</p> <p>Conclusions</p> <p>Individualized nutritional support to older stroke patients in hospital was beneficial for maintaining an adequate body mass and body composition the first week and seemed to have a preventive effect on fat loss among women, but not among men after three months. Measurement of MUAC may be used in the assessment of nutritional status when BMI cannot be obtained.</p> <p>Trial registration</p> <p>This trial is registered with ClinicalTrials.gov, number NCT00163007.</p

    COVID-19 and Precarious Employment: Consequences of the Evolving Crisis

    The world of work is facing an ongoing pandemic and an economic downturn with severe effects worldwide. Workers trapped in precarious employment (PE), both formal and informal, are among those most affected by the COVID-19 pandemic. Here we call attention to at least 5 critical ways that the consequences of the crisis among workers in PE will be felt globally: (a) PE will increase, (b) workers in PE will become more precarious, (c) workers in PE will face unemployment without being officially laid off, (d) workers in PE will be exposed to serious stressors and dramatic life changes that may lead to a rise in diseases of despair, and (e) PE might be a factor in deterring the control of or in generating new COVID-19 outbreaks. We conclude that what we really need is a new social contract, where the work of all workers is recognized and protected with adequate job contracts, employment security, and social protection in a new economy, both during and after the COVID-19 crisis

    Associations between depressive symptoms and disease progression in older patients with chronic kidney disease: results of the EQUAL study

    Background Depressive symptoms are associated with adverse clinical outcomes in patients with end-stage kidney disease; however, few small studies have examined this association in patients with earlier phases of chronic kidney disease (CKD). We studied associations between baseline depressive symptoms and clinical outcomes in older patients with advanced CKD and examined whether these associations differed depending on sex. Methods CKD patients (&gt;= 65 years; estimated glomerular filtration rate &lt;= 20 mL/min/1.73 m(2)) were included from a European multicentre prospective cohort between 2012 and 2019. Depressive symptoms were measured by the five-item Mental Health Inventory (cut-off &lt;= 70; 0-100 scale). Cox proportional hazard analysis was used to study associations between depressive symptoms and time to dialysis initiation, all-cause mortality and these outcomes combined. A joint model was used to study the association between depressive symptoms and kidney function over time. Analyses were adjusted for potential baseline confounders. Results Overall kidney function decline in 1326 patients was -0.12 mL/min/1.73 m(2)/month. A total of 515 patients showed depressive symptoms. No significant association was found between depressive symptoms and kidney function over time (P = 0.08). Unlike women, men with depressive symptoms had an increased mortality rate compared with those without symptoms [adjusted hazard ratio 1.41 (95% confidence interval 1.03-1.93)]. Depressive symptoms were not significantly associated with a higher hazard of dialysis initiation, or with the combined outcome (i.e. dialysis initiation and all-cause mortality). Conclusions There was no significant association between depressive symptoms at baseline and decline in kidney function over time in older patients with advanced CKD. Depressive symptoms at baseline were associated with a higher mortality rate in men

    The child who starts school soon becomes aware that it is important to learn to read and write. When it is educated in written literacy, pictures are used in reading textbooks and other school materials. The child is also asked to draw and talk about pictures in other contexts. Against this background, the aim of the study is to investigate the importance of pictures for children’s learning to read and write. The aim includes a description and analysis of the existing reading and writing discourse. The study problemises relational aspects of school practice, research and teacher education. This problemisation visualises a need for both a change and a development of the field of reading and writing. The thesis starts with a knowledge survey of the field of reading and writing. The survey provides the basis of a text study in which 11 Swedish theses, published between 1996 and 2003, are discussed and related to report materials published by The Swedish National Agency for School Improvement and the Board of Education. The text study is followed by an analysis of pictures, where the empirical material consists both of pictures taken from reading textbooks and reading diagnoses and pictures produced by children. The total result shows that there is consensus about the importance of different methods being applied through balanced reading programmes. At the same time it seems as if the field includes two discourses, a reading and a writing discourse. The dominant reading discourse is characterised by a strong emphasis on the teacher’s competence in terms of being able to see, create progression, arrange in order, settle up and correct for the purpose of providing good preconditions for learning. Pictures are given here a clearly subordinate role. As for the writing discourse, there is a clear shift from the teacher to the child as a meaning carrying and meaning creating subject. To some extent pictures are then recognised as a complementary form of mediation in connection with writing. In the reading discourse it is clear that pictures are traditionally used as illustrations of texts for the purpose of proficiency training. In that case pictures are given a complex role. On the one hand they are criticised for being ambiguous. This ambiguity can lead the child away from effective decoding. On the other hand it is presupposed, paradoxically enough, that there is a one-to-one relationship between picture and text. In contrast, children themselves seem to be able to apply a well-integrated use of picture and text where the intention is superordinate to the convention. The picture-text relation is then expansive: through the child’s involvement the interplay between words and pictures is functional and creates meaning. The thesis shows that a widened text concept, and consideration for the forms of production and presentation that characterise our culture, may yield knowledge contributing to developing the field of reading and writing. In order for this development to be possible, a complementary description of what characterises the respective discourse is required. In this description the concepts of communication and language, society and context and understanding and action function as an active intertriad for handling the teaching of reading and writing not as two separate fields but as a broadened, and at the same genuinely inclusive, field of knowledge

    Pedagogernas perspektiv pÄ möjligheten att bedriva konstruktiv konflikthantering i förskolan

    Denna studie syftar till att genom intervju ta del av nÄgra förskolepedagogers uppfattningar om konflikt och konflikthantering dÀr empirin analyseras med Cohens konfliktpyramid. Studiens syfte Àr att bidra med ökad kunskap om strategier och pÄverkande faktorer för att frÀmja konstruktiv konflikthantering i förskolan. Studien visar att förskolepedagogerna ser konflikter som naturligt och konflikthantering Àr en del av det pedagogiska uppdraget. Resultatet visar att förskolepedagogernas uppfattning Àr att bÄde barnens och pedagogens konfliktlösningsförmÄga kan pÄverka en konfliktsituation.  Resultatet visar att förskolepedagogerna anvÀnder olika metoder och strategier, bland annat drama och modellering, för att utveckla barnens förmÄga att konstruktivt hantera konflikter. Förskolepedagogerna uppger Àven att den faktiska konflikten anvÀnds som lÀrtillfÀlle dÀr de strÀvar efter att anvÀnda dialog och medling som en strategi. Resultatet visar att deltagande förskolepedagogerna anser att det Àr viktigt att undvika strategier som hindrar en utveckling av barnen egna konflikthanteringsförmÄgor. Resultatet visar att förskolepedagogerna aktivt förebygger konflikter i syfte att frÀmja en stöttande och trygg miljö i förskolan. Förskolepedagogernas uppfattning Àr att konflikthantering pÄverkas negativt av stress och kompetensbrist. Kollektiva reflektioner och organisatoriskt stöd lyfts fram av förskolepedagogerna som framgÄngsfaktorer
