    The impact of the cultural backlash on the rise of european national populism : the fear of ethnic disappearance : Cross-Country Study of Austria and Hungary

    National Populists are winning seats around Europe and liberal democracy finds itself increasingly undermined. According to the report Democracy in Crisis (2020)1 , the number of countries that have suffered democratic setbacks surpasses the number having achieved gains in this realm. This dissertation for the MA Program in Governance, Leadership and Democracy Studies will reflect on how the cultural backlash is fueling white identity resentments through the fear of ethnic change. According to Kaufmann (2018), in order to understand the phenomenon of national populism, the issue of migration “is central and ethnic change is the story.” (Kaufmann 2018,11). According to Goodwin and Eatwell (2018) the national populist phenomenon reflects deep­rooted fears about immigration and hyper ethnic change (Goodwin and Eatwell 2018,132). People feel instinctively negative about how immigration could threaten their national identity, and consequently lead to the destruction of the wider group identity, calling into question the future of the white majority (Taub 2016; Kaufmann 2018; Eatwell and Goodwin 2018)

    Experimental Study of the Influence of Inlet and Outlet Conditions on the Flow Pattern of a Rectangular Shallow Reservoir

    This article presents an analysis of the influence of symmetrical and asymmetrical positioning of the inlet and outlet channels on flow pattern inside a rectangular shallow reservoir, by means of a series of experiments with clear water in the laboratory. The experimental facility was built in the Hydraulic Research Center (CPH) of the Federal University of Minas Gerais (UFMG), which consisted of a 3 m long and 2 m wide rectangular basin, whose maximum depth was 0.30 m. The inlet and outlet channels were 1.0 m long, 0.125 m wide and 0.30 m deep. Three different positions were provided for the installation of inlet and outlet channels in their respective transverse walls: left, center and right. Six different geometric combinations of the inlet and outlet channels were experimentally tested, each under three different flow rates: 0.50, 1.25 and 3.40 l/s. Considering the results related to the unique axis-symmetric configuration, the main jet crossed the basin from upstream to downstream, developing a meander-like path for the lower flow discharge. Moreover, it has been noticed the formation of two large eddies in opposite directions, one on each side of the main jet. For the higher flow rates, however, the flow pattern was asymmetrical with the main jet diverting to the right and forming a large counter clockwise vortex.     Introdução: O carcinoma adenóide cístico primário de pulmão é uma neoplasia torácica de crescimento lento e com malignidade de baixo grau. Representa 0,04 a 0,2% de todos os tumores pulmonares, sendo considerada uma neoplasia rara. Apresentação do caso: Paciente, sexo masculino, 49 anos de idade, foi admitido no Hospital Santa Casa de Misericórdia de Goiania, apresentando tosse persistente, dispneia, hemoptise e cerca de 4 episódios previos de pneumonia; negou comorbidades, etilismo e tabagismo, uso de medicamentos e alergias conhecidas. Discussão: O adenocarcinoma pulmonar possui como subtipo o carcinoma adenóide cístico. Anteriormente conhecido como cilindroma. Têm baixo grau de malignidade principalmente pelo seu lento crescimento e curso clínico estendido. Conclusão: Por se tratar de uma baixa malignidade, a sobrevida a longo prazo é satisfatoriamente prevista

    Introdução: O velum interpositum (VI) é uma membrana no subaracnóideo formado por uma invaginação da pia-máter preenchido por líquido cefalorraquidiano (LCR). Sua formação ocorre durante o período embrionário e regridem posteriormente. A persistência dessa estrutura primitiva pode acarretar em sua dilatação, sendo denominado cavum veli interpositum (CVI) e, se maior que 10 mm em medida transversal axial, cisto cavum veli interpositum. Sua prevalência é maior em recém nascidos e prematuros. Apresentação do caso: paciente do sexo masculino, recém-nascido de 10 dias, foi admitido no Hospital Materno Infantil (HMI), por quadro de vômitos intermitentes, associado a febre de 39.5ºC. A mãe relata prematuridade de 31 semanas, trabalho de parto prolongado, cesariana e apresentação pélvica. RN teve um episódio convulsivo minutos antes da chegada à unidade. Nega outras queixas. A ressonância magnética destaca-se como exame padrão ouro, seguido da tomografia computadorizada de crânio. Discussão: Os cistos de CVI não apontam fisiopatologia e quadro clínico bem definidos, mas os conhecimentos acerca de sua localização são importantes para correlacionar sinais e sintomas neurológicos que condizem com efeito de massa, sendo um diagnóstico diferencial de lesões císticas intracranianas da linha média. A terapêutica ainda é restrita, mas há muitos relatos de casos em que a técnica de fenestração endoscópica minimamente invasiva foi preconizada para o tratamento dos cistos de CVI. Conclusão: Quanto ao prognóstico, em âmbito radiológico, estudos demonstram redução do cisto e do efeito de massa em imagem de ressonância magnética pós-operatória. E, apesar da resposta clínica ser dependente se os sintomas são decorrentes direta ou indiretamente do cisto, também demonstraram melhora nessa esfera

    O tropicalismo arranca em 1967, através do corpo: a música de Caetano Veloso e Gilberto Gil, os vestíveis de Hélio Oiticica, as propostas teatrais de José Celso Martinez Corrêa e os cenários de Hélio Eichbauer. Hoje as coisas são um pouco mais complexas. Em tempo de redes sociais, os aspirantes ao poder fazem uso da sua imediatez para suscitarem reações epidérmicas, superficiais, populistas e de grande instantaneidade. A boçalidade triunfa nas caixas de comentários, e com mais alguns perfis falsificados podem manipular-se plebiscitos, movimentos secessionistas, ou, e também censurar-se exposições de arte. Nesta variação do fascismo, a epiderme eletrificada das redes sociais estrutura-se como uma poderosa arena onde se aparenta uma falsa democracia. Talvez a arte continue a ser um reduto para reflexão, mas vemos que a censura se manifesta hoje de modo talvez mais eficaz, silenciando artistas e professores, através da pressão mediatizada, da emoção do momento.     ANÁLISE COMPARATIVA DO USO DA ESTATINA E DO ÁCIDO NICOTÍNICO NO TRATAMENTO DA SÍNDROME METABÓLICA

    Considering metabolic syndrome (MS) as a pathophysiological condition characterized by arterial hypertension, central obesity, insulin resistance, dyslipidemia, and hyperglycemia, this study aims to compare the use of statins and nicotinic acid in the treatment of MS. To this end, a comparative analysis based on relevant clinical and epidemiological studies was conducted. Thus, it is observed that statins significantly reduce LDL cholesterol by about 50% and decrease the risk of cardiovascular events by 25% to 35%, with an odds ratio (OR) of improvement between 0.70 and 0.80. Statins are widely available and accessible, especially in generic versions, and have a relatively manageable adverse effect profile. In contrast, nicotinic acid reduces LDL cholesterol by 15% to 25%, increases HDL cholesterol, and reduces triglycerides, but with less consistent efficacy in preventing cardiovascular events (OR ranging from 0.90 to 1.10) and significant side effects, such as skin flushing and hepatotoxicity. The cost and availability of nicotinic acid are more limited compared to statins. It is concluded that statins are the preferred choice for the treatment of MS due to their robust efficacy and accessibility, while nicotinic acid may be considered in specific cases, with caution due to its adverse effects.Considerando a síndrome metabólica (SM) como uma condição fisiopatológica caracterizada por hipertensão arterial, obesidade central, resistência à insulina, dislipidemia e hiperglicemia, este estudo objetiva comparar o uso de estatinas e ácido nicotínico no tratamento da SM. Para tanto, procede-se a uma análise comparativa baseada em estudos clínicos e epidemiológicos relevantes. Desse modo, observa-se que as estatinas reduzem significativamente o LDL-colesterol em cerca de 50% e diminuem o risco de eventos cardiovasculares em 25% a 35%, com odds ratio (OR) de melhora entre 0,70 e 0,80. As estatinas são amplamente disponíveis e acessíveis, especialmente nas versões genéricas, e apresentam um perfil de efeitos adversos relativamente manejável. Em contraste, o ácido nicotínico reduz o LDL-colesterol em 15% a 25%, aumenta o HDL-colesterol e reduz os triglicerídeos, mas com uma eficácia menos consistente na prevenção de eventos cardiovasculares (OR variando de 0,90 a 1,10) e efeitos colaterais significativos, como rubor cutâneo e hepatotoxicidade. O custo e a disponibilidade do ácido nicotínico são mais limitados em comparação às estatinas. Conclui-se que as estatinas são a escolha preferida para o tratamento da SM devido à sua eficácia robusta e acessibilidade, enquanto o ácido nicotínico pode ser considerado em casos específicos, com precaução devido aos seus efeitos adversos

    Atualmente, observa-se que a tuberculose pulmonar constitui um importante problema de Saúde Pública no mundo, uma vez que esse agravo apresentou, em 2015, 10,4 milhões de casos, dos quais, mais de um milhão de pessoas vieram a óbito. Sob essa perspectiva, o presente artigo tem como objetivo traçar um perfil epidemiológico dos casos de Tuberculose Pulmonar notificados no município de Serra Talhada, entre os anos de 2007 a 2017. Foi realizado um estudo de série histórica observacional do tipo transversal, no intervalo de tempo de 2007 a 2017.  No período investigado o número de casos de tuberculose pulmonar foi de 246 casos, o local que teve a maior prevalência foi Serra Talhada, 287 por 100 mil habitantes. Diante dos dados apresentados, é imprescindível concluir, portanto, que esse estudo corrobora o perfil epidemiológico brasileiro para a Tuberculose Pulmonar, o qual indica variabilidade nos índices de acometimento durante o período analisado

    COVID-19 symptoms at hospital admission vary with age and sex: results from the ISARIC prospective multinational observational study

    Background: The ISARIC prospective multinational observational study is the largest cohort of hospitalized patients with COVID-19. We present relationships of age, sex, and nationality to presenting symptoms. Methods: International, prospective observational study of 60 109 hospitalized symptomatic patients with laboratory-confirmed COVID-19 recruited from 43 countries between 30 January and 3 August 2020. Logistic regression was performed to evaluate relationships of age and sex to published COVID-19 case definitions and the most commonly reported symptoms. Results: ‘Typical’ symptoms of fever (69%), cough (68%) and shortness of breath (66%) were the most commonly reported. 92% of patients experienced at least one of these. Prevalence of typical symptoms was greatest in 30- to 60-year-olds (respectively 80, 79, 69%; at least one 95%). They were reported less frequently in children (≤ 18 years: 69, 48, 23; 85%), older adults (≥ 70 years: 61, 62, 65; 90%), and women (66, 66, 64; 90%; vs. men 71, 70, 67; 93%, each P < 0.001). The most common atypical presentations under 60 years of age were nausea and vomiting and abdominal pain, and over 60 years was confusion. Regression models showed significant differences in symptoms with sex, age and country. Interpretation: This international collaboration has allowed us to report reliable symptom data from the largest cohort of patients admitted to hospital with COVID-19. Adults over 60 and children admitted to hospital with COVID-19 are less likely to present with typical symptoms. Nausea and vomiting are common atypical presentations under 30 years. Confusion is a frequent atypical presentation of COVID-19 in adults over 60 years. Women are less likely to experience typical symptoms than men

    Catálogo Taxonômico da Fauna do Brasil: setting the baseline knowledge on the animal diversity in Brazil

    The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. The importance of the CTFB is evidenced by the elevated number of citations in the scientific literature in diverse areas of biology, law, anthropology, education, forensic science, and veterinary science, among others

    Rationale, study design, and analysis plan of the Alveolar Recruitment for ARDS Trial (ART): Study protocol for a randomized controlled trial

    Background: Acute respiratory distress syndrome (ARDS) is associated with high in-hospital mortality. Alveolar recruitment followed by ventilation at optimal titrated PEEP may reduce ventilator-induced lung injury and improve oxygenation in patients with ARDS, but the effects on mortality and other clinical outcomes remain unknown. This article reports the rationale, study design, and analysis plan of the Alveolar Recruitment for ARDS Trial (ART). Methods/Design: ART is a pragmatic, multicenter, randomized (concealed), controlled trial, which aims to determine if maximum stepwise alveolar recruitment associated with PEEP titration is able to increase 28-day survival in patients with ARDS compared to conventional treatment (ARDSNet strategy). We will enroll adult patients with ARDS of less than 72 h duration. The intervention group will receive an alveolar recruitment maneuver, with stepwise increases of PEEP achieving 45 cmH(2)O and peak pressure of 60 cmH2O, followed by ventilation with optimal PEEP titrated according to the static compliance of the respiratory system. In the control group, mechanical ventilation will follow a conventional protocol (ARDSNet). In both groups, we will use controlled volume mode with low tidal volumes (4 to 6 mL/kg of predicted body weight) and targeting plateau pressure <= 30 cmH2O. The primary outcome is 28-day survival, and the secondary outcomes are: length of ICU stay; length of hospital stay; pneumothorax requiring chest tube during first 7 days; barotrauma during first 7 days; mechanical ventilation-free days from days 1 to 28; ICU, in-hospital, and 6-month survival. ART is an event-guided trial planned to last until 520 events (deaths within 28 days) are observed. These events allow detection of a hazard ratio of 0.75, with 90% power and two-tailed type I error of 5%. All analysis will follow the intention-to-treat principle. Discussion: If the ART strategy with maximum recruitment and PEEP titration improves 28-day survival, this will represent a notable advance to the care of ARDS patients.
