Fresh-Register Automata
What is a basic automata-theoretic model of computation with names and fresh-name generation? We introduce Fresh-Register Automata (FRA), a new class of automata which operate on an infinite alphabet of names and use a finite number of registers to store fresh names, and to compare incoming names with previously stored ones. These finite machines extend Kaminski and Francezâs Finite-Memory Automata by being able to recognise globally fresh inputs, that is, names fresh in the whole current run. We exam-ine the expressivity of FRAâs both from the aspect of accepted languages and of bisimulation equivalence. We establish primary properties and connections between automata of this kind, and an-swer key decidability questions. As a demonstrating example, we express the theory of the pi-calculus in FRAâs and characterise bisimulation equivalence by an appropriate, and decidable in the finitary case, notion in these automata

High strength nanocrystalline CuâCo alloys with high tensile ductility
A supersaturated single-phase Cuâ26 at.% Co alloy was produced by high-pressure torsion deformation, leading to a nanocrystalline microstructure with a grain size smaller than 100 nm. The nonequilibrium solid solution decomposed during subsequent isothermal annealing. In situ high-energy X-ray diffraction was used to map changes linked to the separating phases, and the development of a nanoscale CuâCo composite structure was observed. To gain further information about the relationship of the microstructure and the mechanical properties after phase separation, uniaxial tensile tests were conducted on as-deformed and isothermally annealed samples. Based on the in situ diffraction data, different isothermal annealing temperatures were chosen. Miniaturized tensile specimens with a round cross section were tested, and an image-based data evaluation method enabled the evaluation of true stressâstrain curves and strain hardening behavior. The main results are as follows: all microstructural states showed high strength and ductility, which was achieved by a combination of strain-hardening and strain-rate hardening
Reproducibility and its confounders of CMR feature tracking myocardial strain analysis in patients with suspected myocarditis.
OBJECTIVES
Cardiovascular magnetic resonance feature tracking (CMR-FT) is an emerging technique for assessing myocardial strain with valuable diagnostic and prognostic potential. However, the reproducibility of biventricular CMR-FT analysis in a large cardiovascular population has not been assessed. Also, evidence of confounders impacting reader reproducibility for CMR-FT in patients is unknown and currently limits the clinical implementation of this technique.
METHODS
From a dual-center database of patients referred to CMR for suspected myocarditis, 125 patients were randomly selected to undergo biventricular CMR-FT analysis for 2-dimensional systolic and diastolic measures, with additional 3-dimensional analysis for the left ventricle. All image analysis was replicated by a single reader and by a second reader for intra- and inter-reader analysis (Circle Cardiovascular Imaging). Reliability was tested with intraclass correlation (ICC) tests, and the impact of imaging confounders on agreement was assessed through multivariable analysis.
RESULTS
Left and right ventricular ejection fractions were reduced in 34% and 37% of the patients, respectively. Good to excellent reliability was shown for 2D (all ICCâ>â0.85) and 3D (all ICCâ>â0.70) peak strain and early diastolic strain rate for both ventricles in longitudinal orientation as well as circumferential orientations for the left ventricle. An increased slice number improved agreement while the presence of pericardial effusion compromised diastolic strain rate agreement, and arrhythmia compromised right ventricular agreement.
CONCLUSION
In a large clinical cohort, we could show CMR-FT yields excellent inter-reader and intra-reader reproducibility. Multi-parametric CMR-FT of the right and left ventricles appears to be a robust tool in cardiovascular patients referred to CMR.

Predictive value of cardiac magnetic resonance right ventricular longitudinal strain in patients with suspected myocarditis
BACKGROUND
Recent evidence underlined the importance of right (RV) involvement in suspected myocarditis. We aim to analyze the possible incremental prognostic value from RV global longitudinal strain (GLS) by CMR.
METHODS
Patients referred for CMR, meeting clinical criteria for suspected myocarditis and no other cardiomyopathy were enrolled in a dual-center register cohort study. Ejection fraction (EF), GLS and tissue characteristics were assessed in both ventricles to assess their association to first major adverse cardiovascular events (MACE) including hospitalization for heart failure (HF), ventricular tachycardia (VT), recurrent myocarditis and death.
RESULTS
Among 659 patients (62.8% male; 48.1â±â16.1 years), RV GLS was impaired (>â-â15.4%) in 144 (21.9%) individuals, of whom 76 (58%), 108 (77.1%), 27 (18.8%) and 40 (32.8%) had impaired right ventricular ejection fraction (RVEF), impaired left ventricular ejection fraction (LVEF), RV late gadolinium enhancement (LGE) or RV edema, respectively. After a median observation time of 3.7 years, 45 (6.8%) patients were hospitalized for HF, 42 (6.4%) patients died, 33 (5%) developed VT and 16 (2.4%) had recurrent myocarditis. Impaired RV GLS was associated with MACE (HRâ=â1.07, 95% CI 1.04-1.10; pâ<â0.001), HF hospitalization (HRâ=â1.17, 95% CI 1.12-1.23; pâ<â0.001), and death (HRâ=â1.07, 95% CI 1.02-1.12; pâ=â0.004), but not with VT and recurrent myocarditis in univariate analysis. RV GLS lost its association with outcomes, when adjusted for RVEF, LVEF, LV GLS and LV LGE extent.
CONCLUSION
RV strain is associated with MACE, HF hospitalization and death but has neither independent nor incremental prognostic value after adjustment for RV and LV function and tissue characteristics. Therefore, assessing RV GLS in the setting of myocarditis has only limited value
Improvement of antibiotic prescription in outpatient care: a cluster-randomized intervention study using a sentinel surveillance network of physicians
Objectives To assess the effectiveness of implementing guidelines, coupled with individual feedback, on antibiotic prescribing behaviour of primary care physicians in Switzerland. Methods One hundred and forty general practices from a representative Swiss sentinel network of primary care physicians participated in this cluster-randomized prospective intervention study. The intervention consisted of providing guidelines on treatment of respiratory tract infections (RTIs) and uncomplicated lower urinary tract infections (UTIs), coupled with sustained, regular feedback on individual antibiotic prescription behaviour during 2 years. The main aims were: (i) to increase the percentage of prescriptions of penicillins for all RTIs treated with antibiotics; (ii) to increase the percentage of trimethoprim/sulfamethoxazole prescriptions for all uncomplicated lower UTIs treated with antibiotics; (iii) to decrease the percentage of quinolone prescriptions for all cases of exacerbated COPD (eCOPD) treated with antibiotics; and (iv) to decrease the proportion of sinusitis and other upper RTIs treated with antibiotics. The study was registered at ClinicalTrials.gov (NCT01358916). Results While the percentage of antibiotics prescribed for sinusitis or other upper RTIs and the percentage of quinolones prescribed for eCOPD did not differ between the intervention group and the control group, there was a significant increase in the percentage of prescriptions of penicillins for all RTIs treated with antibiotics [57% versus 49%, ORâ=â1.42 (95% CI 1.08-1.89), Pâ=â0.01] and in the percentage of trimethoprim/sulfamethoxazole prescriptions for all uncomplicated lower UTIs treated with antibiotics [35% versus 19%, ORâ=â2.16 (95% CI 1.19-3.91), Pâ=â0.01] in the intervention group. Conclusions In our setting, implementing guidelines, coupled with sustained individual feedback, was not able to reduce the proportion of sinusitis and other upper RTIs treated with antibiotics, but increased the use of recommended antibiotics for RTIs and UTIs, as defined by the guideline
Formalizing of Category Theory in Agda
The generality and pervasiness of category theory in modern mathematics makes
it a frequent and useful target of formalization. It is however quite
challenging to formalize, for a variety of reasons. Agda currently (i.e. in
2020) does not have a standard, working formalization of category theory. We
document our work on solving this dilemma. The formalization revealed a number
of potential design choices, and we present, motivate and explain the ones we
picked. In particular, we find that alternative definitions or alternative
proofs from those found in standard textbooks can be advantageous, as well as
"fit" Agda's type theory more smoothly. Some definitions regarded as equivalent
in standard textbooks turn out to make different "universe level" assumptions,
with some being more polymorphic than others. We also pay close attention to
engineering issues so that the library integrates well with Agda's own standard
library, as well as being compatible with as many of supported type theories in
Agda as possible
Genome Sequencing of SHH Medulloblastoma Predicts Genotype-Related Response to Smoothened Inhibition
SummarySmoothened (SMO) inhibitors recently entered clinical trials for sonic-hedgehog-driven medulloblastoma (SHH-MB). Clinical response is highly variable. To understand the mechanism(s) of primary resistance and identify pathways cooperating with aberrant SHH signaling, we sequenced and profiled a large cohort of SHH-MBs (n = 133). SHH pathway mutations involved PTCH1 (across all age groups), SUFU (infants, including germline), and SMO (adults). Children >3 years old harbored an excess of downstream MYCN and GLI2 amplifications and frequent TP53 mutations, often in the germline, all of which were rare in infants and adults. Functional assays in different SHH-MB xenograft models demonstrated that SHH-MBs harboring a PTCH1 mutation were responsive to SMO inhibition, whereas tumors harboring an SUFU mutation or MYCN amplification were primarily resistant
Conservation decisions under pressure: Lessons from an exercise in rapid response to wildlife disease
Novel outbreaks of emerging pathogens require rapid responses to enable successful mitigation. We simulated a 1âday emergency meeting where experts were engaged to recommend mitigation strategies for a new outbreak of the amphibian fungal pathogen Batrachochytrium salamandrivorans. Despite the inevitable uncertainty, experts suggested and discussed several possible strategies. However, their recommendations were undermined by imperfect initial definitions of the objectives and scope of management. This problem is likely to arise in most realâworld emergency situations. The exercise thus highlighted the importance of clearly defining the context, objectives, and spatialâtemporal scale of mitigation decisions. Managers are commonly under pressure to act immediately. However, an iterative process in which experts and managers cooperate to clarify objectives and uncertainties, while collecting more information and devising mitigation strategies, may be slightly more time consuming but ultimately lead to better outcomes
Sarcoma classification by DNA methylation profiling
Sarcomas are malignant soft tissue and bone tumours affecting adults, adolescents and children. They represent a morphologically heterogeneous class of tumours and some entities lack defining histopathological features. Therefore, the diagnosis of sarcomas is burdened with a high inter-observer variability and misclassification rate. Here, we demonstrate classification of soft tissue and bone tumours using a machine learning classifier algorithm based on array-generated DNA methylation data. This sarcoma classifier is trained using a dataset of 1077 methylation profiles from comprehensively pre-characterized cases comprising 62 tumour methylation classes constituting a broad range of soft tissue and bone sarcoma subtypes across the entire age spectrum. The performance is validated in a cohort of 428 sarcomatous tumours, of which 322 cases were classified by the sarcoma classifier. Our results demonstrate the potential of the DNA methylation-based sarcoma classification for research and future diagnostic applications
