    "Detachment of the carinal hook following endobronchial intubation with a double lumen tube"

    <p>Abstract</p> <p>Background</p> <p>Carinal hooks increases difficulty at endotracheal intubation. Amputation of the carinal hook during passage and malpositioning of the tube to the hook are some of the potential problems related with left-sided Carlens double lumen tube (DLT). This article reports an amputation of the hook during a difficult selective intubation and aimed at calling the attention to complications associated with DLTs and the importance of fiberoptic bronchoscopy.</p> <p>Case presentation</p> <p>A 68 year-old woman was scheduled for right-sided thoracotomy in whom blind DLT insertion was performed. Narrowed trachea causes difficulty in rotating the DLT 90° counter-clockwise. After carinal hook was noticed upon visual inspection of the DLT, fiberoptic bronchoscopy was used to remove the missing part (with the use of forceps) from the right mainstem bronchus.</p> <p>Conclusion</p> <p>Insertion of DLTs with carinal hook is associated with technical problems and potentially life-threatening hazards have discouraged their use. Fiberoptic evaluation and repositioning solves most of the problems. Although amputation of the carinal hook has not been previously reported, clinicians should be alert. This case report emphasizes the utility of the fiberoptic bronchoscopy in the operating theatre for placement, positioning and inspection of the carinal hook DLT.</p

    A decentralized spectrum allocation and partitioning scheme for a two-tier macro-femtocell network with downlink beamforming

    This article examines spectrum allocation and partitioning schemes to mitigate cross-tier interference under downlink beamforming environments. The enhanced SIR owing to beamforming allows more femtocells to share their spectrum with the macrocell and accordingly improves overall spectrum efficiency. We first design a simplified centralized scheme as the optimum and then propose a practical decentralized algorithm that determines which femtocells to use the full or partitioned spectrum with acceptable control overhead. To exploit limited information of the received signal strength efficiently, we consider two types of probabilistic femtocell base station (HeNB) selection policies. They are equal selection and interference weighted selection policies, and we drive their outage probabilities for a macrocell user. Through performance evaluation, we demonstrate that the outage probability and the cell capacity in our decentralized scheme are significantly better than those in a conventional cochannel deployment scheme. Furthermore, we show that the cell utility in our proposed scheme is close to that in the centralized scheme and better than that in the spectrum partitioning scheme with a fixed ratio.open0

    <p>Abstract</p> <p>Introduction</p> <p>Osteonecrosis of the femoral head is a progressive disease that leads to femoral head collapse and osteoarthritis. Our goal in treating osteonecrosis is to preserve, not to replace, the femoral head.</p> <p>Case presentation</p> <p>We present the case of a patient with bilateral osteonecrosis of the femoral head treated with autologous cultured osteoblast injection.</p> <p>Conclusion</p> <p>Although our experience is limited to one patient, autologous cultured osteoblast transplantation appears to be effective for treating the osteonecrosis of femoral head.</p

    Quantum Vacuum Experiments Using High Intensity Lasers

    The quantum vacuum constitutes a fascinating medium of study, in particular since near-future laser facilities will be able to probe the nonlinear nature of this vacuum. There has been a large number of proposed tests of the low-energy, high intensity regime of quantum electrodynamics (QED) where the nonlinear aspects of the electromagnetic vacuum comes into play, and we will here give a short description of some of these. Such studies can shed light, not only on the validity of QED, but also on certain aspects of nonperturbative effects, and thus also give insights for quantum field theories in general.Comment: 9 pages, 8 figur

    Performance of the CMS Cathode Strip Chambers with Cosmic Rays

    The Cathode Strip Chambers (CSCs) constitute the primary muon tracking device in the CMS endcaps. Their performance has been evaluated using data taken during a cosmic ray run in fall 2008. Measured noise levels are low, with the number of noisy channels well below 1%. Coordinate resolution was measured for all types of chambers, and fall in the range 47 microns to 243 microns. The efficiencies for local charged track triggers, for hit and for segments reconstruction were measured, and are above 99%. The timing resolution per layer is approximately 5 ns

    Performance and Operation of the CMS Electromagnetic Calorimeter

    The operation and general performance of the CMS electromagnetic calorimeter using cosmic-ray muons are described. These muons were recorded after the closure of the CMS detector in late 2008. The calorimeter is made of lead tungstate crystals and the overall status of the 75848 channels corresponding to the barrel and endcap detectors is reported. The stability of crucial operational parameters, such as high voltage, temperature and electronic noise, is summarised and the performance of the light monitoring system is presented
