Prognostication of Bell’s palsy: a new perspective
Background: Bell’s palsy is considered as the most frequent cranial neuropathy. Early and adequate risk stratification may help both the patients and the treating physicians in taking informed decisions regarding treatment and understanding their outcomes. We aimed to formulate accessible and sensitive methods of risk stratification in Bell’s palsy by utilizing electrophysiological and hematological parameters.
Methods: We prospectively followed up 101 patients with Bell’s palsy over a period of 18 months. Electrophysiological parameters were measured thrice i.e., on the first evaluation and after the first week and first month. The N/L, P/L ratio, erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP) were documented in steroid naïve cases. Patients were graded in severity based on the Sunnybrook and House Brackmann systems.
Results: The mean SB and HB grades at admission were 53.89±24.725 and 3.92±1.04 indicating moderate severity. The mean N/L, P/L ratios and ESR on the first day was 3.46±3.45, 145.42±162.84 and 22.51±21.105 respectively. There was no statistical correlation with severity at any time point. The mean CMAP indices on the 1st day, 1st week and 1st month were 0.585±0.31,0.43±0.26 and 0.45±0.23 respectively. The CMAP index at 1 month was correlating with severity. Blink amplitude ratios were correlating with the HB scores at 1 week and 1 month (p<0.0001 both) and the SB score at 1 month (p<0.0001).
Conclusions: Hematological parameters were not correlated to disease severity. However, electrophysiological parameters are correlated to disease severity at one week and one month. Blink amplitude ratio may be a useful indicator for risk stratification of Bell’s palsy patients
Electromagnetic Dissociation as a Tool for Nuclear Structure and Astrophysics
Coulomb dissociation is an especially simple and important reaction
mechanism. Since the perturbation due to the electric field of the (target)
nucleus is exactly known, firm conclusions can be drawn from such measurements.
Electromagnetic matrixelements and astrophysical S-factors for radiative
capture processes can be extracted from experiments. We describe the basic
elements of the theory of nonrelativistic and relativistic electromagnetic
excitation with heavy ions. This is contrasted to electromagnetic excitation
with leptons (electrons), with their small electric charge and the absence of
strong interactions. We discuss various approaches to the study of higher order
electromagnetic effects and how these effects depend on the basic parameters of
the experiment. The dissociation of neutron halo nuclei is studied in a zero
range model using analytical methods. We also review ways how to treat nuclear
interactions, show their characteristics and how to avoid them (as far as
possible). We review the experimental results from a theoretical point of view.
Of special interest for nuclear structure physics is the appearence of low
lying electric dipole strength in neutron rich nuclei. Applications of Coulomb
dissociation to some selected radiative capture reactions relevant for nuclear
astrophysics are discussed. The Coulomb dissociation of 8B is relevant for the
solar neutrino problem. The potential of the method especially for future
investigations of (medium) heavy exotic nuclei for nuclear structure and
astrophysics is explored. We conclude that the Coulomb dissociation mechanism
is theoretically well understood, the potential difficulties are identified and
can be taken care of. Many interesting experiments have been done in this field
and many more are expected in the future.Comment: review article accepted for publication in "Prog. in Part. and Nucl.

The Drakensberg Declaration on the Control of Rheumatic Fever and Rheumatic Heart Disease in Africa
This paper reviews some research studies on tillage methods influencing soil and moisture conservation in the eastern African countries of Kenya, Tanzania, Malawi and Ethiopia during the past four decades. Most of these studies were conducted in marginal rainfall (semi arid ) areas and on shallow soils of various textures (sandy clay loam, sandy clay, clay and loam). The studies were meant to establish the effects of tillage and residue management practices on physico-chemical soil properties (i.e. structure, bulk density, soil moisture and organic matter contents), runoff and infiltration. This review emphasizes the importance of appropriate tillage and residue management methods (contour bunds and terraces, minimum tillage, tied ridging, mulching and conventional tillage) in providing soil conditions favourable for soil moisture conservation and subsequent crop performance and yield on smallholder farm
