    Brachyspira pilosicoli bloodstream infections: Case report and review of the literature

    Brachyspira pilosicoli is the etiologic agent of human and animal intestinal spirochetosis and is rarely implicated as a cause of bacteremia. Here, we describe the case of a B. pilosicoli spirochetemia in a 53-year-old male patient suffering from cardiogenic shock. This fastidious bacterium was isolated from blood, likely after translocation from the intestinal tract. Blood cultures were positive after 5 days of incubation (one day after the patient's death), highlighting the problem of the recovery of such type of fastidious bacterium. Identification was achieved by molecular methods (16S rRNA sequencing). A review of the English literature found only 8 cases of bacteremia caused by B. pilosicoli, mostly in immunocompromised or critically ill patients. Finally, difficulties in rapid and accurate diagnosis of B. pilosicoli bloodstream infections, in vitro antimicrobial susceptibility of human clinical isolates, and therapeutic options are discussed

    Reduced melt on debris-covered glaciers: investigations from Changri Nup Glacier, Nepal

    International audienceApproximately 25 % of the glacierized area in the Everest region is covered by debris, yet the surface mass balance of debris-covered portions of these glaciers has not been measured directly. In this study, ground-based measurements of surface elevation and ice depth are combined with terrestrial photogrammetry, unmanned aerial vehicle (UAV) and satellite elevation models to derive the surface mass balance of the debris-covered tongue of Changri Nup Glacier, located in the Everest region. Over the debris-covered tongue, the mean elevation change between 2011 and 2015 is −0.93 m year−1 or −0.84 m water equivalent per year (w.e. a−1). The mean emergence velocity over this region, estimated from the total ice flux through a cross section immediately above the debris-covered zone, is +0.37 m w.e. a−1. The debris-covered portion of the glacier thus has an area-averaged mass balance of −1.21 ± 0.2 m w.e. a−1 between 5240 and 5525 m above sea level (m a.s.l.). Surface mass balances observed on nearby debris-free glaciers suggest that the ablation is strongly reduced (by ca. 1.8 m w.e. a−1) by the debris cover. The insulating effect of the debris cover has a larger effect on total mass loss than the enhanced ice ablation due to supraglacial ponds and exposed ice cliffs. This finding contradicts earlier geodetic studies and should be considered for modelling the future evolution of debris-covered glaciers

    Ice cliff contribution to the tongue-wide ablation of Changri Nup Glacier, Nepal, central Himalaya

    Ice cliff backwasting on debris-covered glaciers is recognized as an important mass-loss process that is potentially responsible for the debris-cover anomaly, i.e. the fact that debris-covered and debris-free glacier tongues appear to have similar thinning rates in the Himalaya. In this study, we quantify the total contribution of ice cliff backwasting to the net ablation of the tongue of Changri Nup Glacier, Nepal, between 2015 and 2017. Detailed backwasting and surface thinning rates were obtained from terrestrial photogrammetry collected in November 2015 and 2016, unmanned air vehicle (UAV) surveys conducted in November 2015, 2016 and 2017, and Pléiades tri-stereo imagery obtained in November 2015, 2016 and 2017. UAV- and Pléiades-derived ice cliff volume loss estimates were 3&thinsp;% and 7&thinsp;% less than the value calculated from the reference terrestrial photogrammetry. Ice cliffs cover between 7&thinsp;% and 8&thinsp;% of the total map view area of the Changri Nup tongue. Yet from November 2015 to November 2016 (November 2016 to November 2017), ice cliffs contributed to 23±5&thinsp;% (24±5&thinsp;%) of the total ablation observed on the tongue. Ice cliffs therefore have a net ablation rate 3.1±0.6 (3.0±0.6) times higher than the average glacier tongue surface. However, on Changri Nup Glacier, ice cliffs still cannot compensate for the reduction in ablation due to debris-cover. In addition to cliff enhancement, a combination of reduced ablation and lower emergence velocities could be responsible for the debris-cover anomaly on debris-covered tongues.</p

    Fatal interstitial lung disease associated with oral erlotinib therapy for lung cancer

    <p>Abstract</p> <p>Background</p> <p>Erlotinib is a Human Epidermal Growth Factor Receptor Type 1/tyrosine kinase (EGFR) inhibitor which is used for non-small-cell lung cancer treatment. Despite that erlotinib is considered to have a favorable safety profile, adverse events such as interstitial lung disease (ILD) were reported in pivotal studies. The authors report the first histologically confirmed case of fatal ILD associated with erlotinib therapy.</p> <p>Case Presentation</p> <p>The medical record of a patient who developed fatal ILD after receiving erlotinib treatment was reviewed to identify the cause of death and other factors potentially contributive to this adverse outcome. A 55-year-old smoker with no evidence of pre-existing interstitial disease developed bilateral ILD and respiratory failure which could be explained only as a toxicity of erlotinib. He had a history of stage IV left upper lobe squamous-cell carcinoma for which he had received three successive regimens of chemotherapy (ifosfamide plus gemcitabine, docetaxel, mitomycin plus navelbine), followed five months later by erlotinib. At initiation of erlotinib treatment there were no radiological signs suggestive of ILD disease or apparent clinical signs of respiratory distress. While the patient completed two months with erlotinib therapy he developed bilateral interstitial infiltrates; despite discontinuation of erlotinib he was admitted with respiratory failure two weeks later. Diagnostic work up for other causes of pneumonitis including infectious diseases, congestive cardiac failure and pulmonary infraction was negative. Empiric treatment with oxygene, corticosteroids and later with cyclophosphamide was ineffective and the patient progressively deteriorated and died. The clinical and post-mortem examination findings are presented and the possible association relationship between erlotinib induced ILD and previous chemotherapy is discussed.</p> <p>Conclusion</p> <p>Physicians should be alert to the fact that erlotinib related ILD, although infrequent, is potential fatal. The association between selective EGFR-inhibitors and ILD should be further investigated.</p

    Coeval extensional shearing and lateral underflow during Late Cretaceous core complex development in the Niğde Massif, Central Anatolia, Turkey

    27 p.International audienceThe Nig¢de Massif, at the southern tip of the Central Anatolian Crystalline Complex, consists of two structural units. Foliations in the lower unit define a dome cored by migmatites. The contact between the two units bears all the elements of a ductile to brittle extensional detachment. Hence the Nig¢de Massif represents an extensional metamorphic core complex. Top-to-NE/ENE shearing at higher levels of the lower unit relates to displacement along the detachment. Deeper levels of the lower unit display hightemperature top-to-SSW ductile shearing. The two shearing deformations show a difference in the mean trend of stretching lineations of up to 58. New 40Ar/39Ar ages combined with previously published data enable us to infer that the two shears were contemporaneous. In our favored interpretation, oblique shearing in the core of the dome reflects lateral underflow, i.e., horizontal flowing of the lower crust in a direction highly oblique to the direction of extension. As a result of the interaction between lateral underflow and downdip shearing along the overlying detachment, distinct structural domains are expected to exist within the migmatitic part of the core complex, with observed counterparts in the Nig¢de dome. Lateral underflow may reflect ''inward'' flow on the scale of the core complex. Regional-scale channel flow is an alternative that would better account for the record of non-coaxial deformation in the core of the dome. More generally, we suspect that the development of lateral underflow in a metamorphic core complex more likely reflects regional channel flow, rather than local inward flow

    Unusual manifestation of Erdheim-Chester disease

    <p>Abstract</p> <p>Background</p> <p>Erdheim-Chester disease (ECD) is a rare multisystem non-Langerhans cell histiocytosis that is characterized histologically by xanthogranulomatous infiltrates and radiologically by symmetrical sclerosis of long bones. The xanthomatous process is characterized by prominent foamy histiocytes staining positive for CD68, occasionally for PS100 and negative for S100 and CD1a. Gastroenterological involvement is exceedingly rare.</p> <p>Case Presentation</p> <p>This case report describes the case of a 69-year-old man who presented otherwise well to the gastroenterology department with unspecific abdominal symptoms, nausea, vomiting and weight loss. ECD involving the gastrointestinal tract was confirmed clinically, radiologically and histologically.</p> <p>Conclusion</p> <p>Gastroenterological manifestation of ECD is rare but should be considered in the differential diagnosis in patients presenting with evidence of multi-organ disease and typical radiological features of Erdheim-Chester disease elsewhere.</p

    Cross-correlating Planck tSZ with RCSLenS weak lensing: implications for cosmology and AGN feedback

    We present measurements of the spatial mapping between (hot) baryons and the total matter in the Universe, via the cross-correlation between the thermal Sunyaev–Zeldovich (tSZ)map from Planck and the weak gravitational lensing maps from theRed Cluster Sequence Lensing Survey (RCSLenS). The cross-correlations are performed on the map level where all the sources (including diffuse intergalactic gas) contribute to the signal. We consider two configurationspace correlation function estimators, ξ y–κ and ξ y–γt , and a Fourier-space estimator, Cy–κ , in our analysis. We detect a significant correlation out to 3◦ of angular separation on the sky. Based on statistical noise only, we can report 13σ and 17σ detections of the cross-correlation using the configuration-space y–κ and y–γ t estimators, respectively. Including a heuristic estimate of the sampling variance yields a detection significance of 7σ and 8σ, respectively. A similar level of detection is obtained from the Fourier-space estimator, Cy–κ . As each estimator probes different dynamical ranges, their combination improves the significance of the detection. We compare our measurements with predictions from the cosmo-OverWhelmingly Large Simulations suite of cosmological hydrodynamical simulations, where different galactic feedback models are implemented. We find that a model with considerable active galactic nuclei (AGN) feedback that removes large quantities of hot gas from galaxy groups and Wilkinson Microwave Anisotropy Probe 7-yr best-fitting cosmological parameters provides the bestmatch to the measurements. All baryonic models in the context of a Planck cosmology overpredict the observed signal. Similar cosmological conclusions are drawn when we employ a halo model with the observed ‘universal’ pressure profile

    Prime Focus Spectrograph (PFS) for the Subaru Telescope: Overview, recent progress, and future perspectives

    PFS (Prime Focus Spectrograph), a next generation facility instrument on the 8.2-meter Subaru Telescope, is a very wide-field, massively multiplexed, optical and near-infrared spectrograph. Exploiting the Subaru prime focus, 2394 reconfigurable fibers will be distributed over the 1.3 deg field of view. The spectrograph has been designed with 3 arms of blue, red, and near-infrared cameras to simultaneously observe spectra from 380nm to 1260nm in one exposure at a resolution of ~1.6-2.7A. An international collaboration is developing this instrument under the initiative of Kavli IPMU. The project is now going into the construction phase aiming at undertaking system integration in 2017-2018 and subsequently carrying out engineering operations in 2018-2019.
