    Risk stratification of Barrett's oesophagus using a non-endoscopic sampling method coupled with a biomarker panel: a cohort study

    Background Barrett's oesophagus predisposes to adenocarcinoma. However, most patients with Barrett's oesophagus will not progress and endoscopic surveillance is invasive, expensive, and fraught by issues of sampling bias and the subjective assessment of dysplasia. We investigated whether a non-endoscopic device, the Cytosponge, could be coupled with clinical and molecular biomarkers to identify a group of patients with low risk of progression suitable for non-endoscopic follow-up. Methods In this multicentre cohort study (BEST2), patients with Barrett's oesophagus underwent the Cytosponge test before their surveillance endoscopy. We collected clinical and demographic data and tested Cytosponge samples for a molecular biomarker panel including three protein biomarkers (P53, c-Myc, and Aurora kinase A), two methylation markers (MYOD1 and RUNX3), glandular atypia, and TP53 mutation status. We used a multivariable logistic regression model to compute the conditional probability of dysplasia status. We selected a simple model with high classification accuracy and applied it to an independent validation cohort. The BEST2 study is registered with ISRCTN, number 12730505. Findings The discovery cohort consisted of 468 patients with Barrett's oesophagus and intestinal metaplasia. Of these, 376 had no dysplasia and 22 had high-grade dysplasia or intramucosal adenocarcinoma. In the discovery cohort, a model with high classification accuracy consisted of glandular atypia, P53 abnormality, and Aurora kinase A positivity, and the interaction of age, waist-to-hip ratio, and length of the Barrett's oesophagus segment. 162 (35%) of 468 of patients fell into the low-risk category and the probability of being a true non-dysplastic patient was 100% (99% CI 96–100) and the probability of having high-grade dysplasia or intramucosal adenocarcinoma was 0% (0–4). 238 (51%) of participants were classified as of moderate risk; the probability of having high-grade dysplasia was 14% (9–21). 58 (12%) of participants were classified as high-risk; the probability of having non-dysplastic endoscopic biopsies was 13% (5–27), whereas the probability of having high-grade dysplasia or intramucosal adenocarcinoma was 87% (73–95). In the validation cohort (65 patients), 51 were non-dysplastic and 14 had high-grade dysplasia. In this cohort, 25 (38%) of 65 patients were classified as being low-risk, and the probability of being non-dysplastic was 96·0% (99% CI 73·80–99·99). The moderate-risk group comprised 27 non-dysplastic and eight high-grade dysplasia cases, whereas the high-risk group (8% of the cohort) had no non-dysplastic cases and five patients with high-grade dysplasia. Interpretation A combination of biomarker assays from a single Cytosponge sample can be used to determine a group of patients at low risk of progression, for whom endoscopy could be avoided. This strategy could help to avoid overdiagnosis and overtreatment in patients with Barrett's oesophagus. Funding Cancer Research UK

    Identification and validation of multiple cell surface markers of clinical-grade adipose-derived mesenchymal stromal cells as novel release criteria for good manufacturing practice-compliant production

    Background: Clinical translation of mesenchymal stromal cells (MSCs) necessitates basic characterization of the cell product since variability in biological source and processing of MSCs may impact therapeutic outcomes. Although expression of classical cell surface markers (e.g., CD90, CD73, CD105, and CD44) is used to define MSCs, identification of functionally relevant cell surface markers would provide more robust release criteria and options for quality control. In addition, cell surface expression may distinguish between MSCs from different sources, including bone marrow-derived MSCs and clinical-grade adipose-derived MSCs (AMSCs) grown in human platelet lysate (hPL). Methods: In this work we utilized quantitative PCR, flow cytometry, and RNA-sequencing to characterize AMSCs grown in hPL and validated non-classical markers in 15 clinical-grade donors. Results: We characterized the surface marker transcriptome of AMSCs, validated the expression of classical markers, and identified nine non-classical markers (i.e., CD36, CD163, CD271, CD200, CD273, CD274, CD146, CD248, and CD140B) that may potentially discriminate AMSCs from other cell types. More importantly, these markers exhibit variability in cell surface expression among different cell isolates from a diverse cohort of donors, including freshly prepared, previously frozen, or proliferative state AMSCs and may be informative when manufacturing cells. Conclusions: Our study establishes that clinical-grade AMSCs expanded in hPL represent a homogeneous cell culture population according to classical markers,. Additionally, we validated new biomarkers for further AMSC characterization that may provide novel information guiding the development of new release criteria

    A América Latina vem experimentando uma nova era de declarada fé dos governos no mito do desenvolvimento, em articulação com a expansão de políticas extrativistas exportadoras em um contexto de renovada dependência. A face mais dramática do extrativismo na região tem sido a crescente presença de corporações mineiras transnacionais apoiadas por governos nacionais e regionais e por instituições internacionais financeiras e de apoio ao desenvolvimento, e intensamente resistidas por movimentos sociais populares. Neste artigo apresentamos o caso de Andalgalá (uma pequena cidade na Província de Catamarca, na Argentina) e as lutas do povo contra corporações mineiras transnacionais e seus aliados. Na tradição da Filosofia da Libertação e do método ana-dialético de Dussel, nos engajamos com o que tem sido denominado "comunidades argentinas do NÃO", expressando sua oposição a formas neocoloniais de desenvolvimento e gestão. Neste artigo estamos especificamente interessados em compreender como dois dispositivos gerencialistas usados pelas corporações mineiras, responsabilidade social corporativa (RSC) e pactos de governança, impactam a luta do povo. Acima de tudo, este artigo oferece instantâneos de batalhas na linha de frente do extrativismo. Esperamos ter dado voz àquelas pessoas que normalmente não são ouvidas, criando um espaço para suas visões sobre um tipo diferente de desenvolvimento.</jats:p

    Metabolic Rift or Metabolic Shift? Dialectics, Nature, and the World-Historical Method

    Abstract In the flowering of Red-Green Thought over the past two decades, metabolic rift thinking is surely one of its most colorful varieties. The metabolic rift has captured the imagination of critical environmental scholars, becoming a shorthand for capitalism’s troubled relations in the web of life. This article pursues an entwined critique and reconstruction: of metabolic rift thinking and the possibilities for a post-Cartesian perspective on historical change, the world-ecology conversation. Far from dismissing metabolic rift thinking, my intention is to affirm its dialectical core. At stake is not merely the mode of explanation within environmental sociology. The impasse of metabolic rift thinking is suggestive of wider problems across the environmental social sciences, now confronted by a double challenge. One of course is the widespread—and reasonable—sense of urgency to evolve modes of thought appropriate to an era of deepening biospheric instability. The second is the widely recognized—but inadequately internalized—understanding that humans are part of nature

    Digenean trematodes of Seriolella porosa (Pisces, Centrolophidae) in San Matías Gulf, Argentina

    Seriolella porosa (Silver warehou) is an important fish resource in the Argentine Sea. No previous studies have been carried out to date on its parasite fauna. The present study aims to (i) identify the digenean trematodes that parasitize Silver warehou, (ii) estimate parasite frequency and abundance, and (iii) establish their relationship with fish size and sex. A total of 100 specimens of S. porosa from San Matías Gulf, Argentina were examined between September and October 2006. Host size and sex were determined. Parasites extracted from the stomach lumen were fixed, conserved in 5% formalin, and stained with Langeron's hydrochloric carmine. The following parameters were also determined: dispersion coefficient, prevalence, intensity, and parasite abundance. Differences in parasitism were assessed using non-parametric tests. Four adult digenean species were found in the stomach, namely Lecithocladium cristatum, Aponurus laguncula, Elytrophalloides oatesi and Gonocerca cf. phycidis. L. cristatum was not only dominant but also the most prevalent and abundant species. None of the digeneans showed interspecific association with each other and therefore they might follow different patterns of colonization (i. e., different intermediate hosts, seasonal and/or spatial distribution of the infective stages). Significant differences were observed among the digeneans analyzed in respect to the sex of the host. Results from this study show that Silver warehou plays a key role as a final host of a significant number of digeneans in the ecosystem.     Optimal Distribution of the Fundamental Non-efficient Load Current Terms Between the Energy Gateways Connected in Grid-tied Microgrids

    [EN] Main objective of this paper is the optimal distribution of the fundamental non-efficient load current terms between the inverters ¿Energy Gateways (EGs)¿ connected in grid-tied microgrids (MGs). The main feature of the presented approach is the use of the EGs as controlled current sources that can compensate fundamental non-efficient load current terms in addition to the generation of fundamental positive-sequence active current, avoiding the use of shunt active power filters. The approach relies on the so-called System of Constant References (SoCR) that is based on the symmetrical components decomposition and dq0 transformation. SoCR procedure decouples efficient and non-efficient components of the instantaneous load currents, transforming all of them into six constant references. The optimization algorithm uses a new approach for the calculation of the peak currents in each phase, avoiding non-convex problems when determining the currents of the EGs considering their operating limits. A medium-power size MG that includes photovoltaic and wind generators, as well as, a battery energy storage system is considered to evaluate the capabilities of the proposal. There were evaluated four scenarios: baseline, balanced distribution, proportional distribution, and optimal distribution. All scenarios, except optimal distribution scenario, surpass the current limits for the EGs connected.     Los contratos petroleros de concesión con grandes ventajas para el capital extranjero fueron la modalidad que predomine desde 1905. A partir de los años 40 compañías multinacionales buscaron petróleo en la Orinoquia colombiana. En los años 70 los expertos mostraron las bondades petrolíferas del subsuelo orinoquense. La eliminación del sistema de concesión y la nueva forma de asociación estimularon al capital extranjero. El contrato de asociación es una herramienta que ha usado el país desde 1974 para vincular capital extranjero de riesgo y tecnología privada internacional en la exploración de petróleo. Bajo esta modalidad se han encontrado los yacimientos petrolíferos en la Orinoquia. La filosofía del contrato de asociación es que el socio privado realice bajo su propio nesgó las inversiones exploratorias. En caso de encontrarse un campo productivo, su explotación la hace asociado y no individualmente, bajo la dirección de un comité ejecutivo en el que están representados la Nación, con Ecopetrol, y la empresa asociada
