Patient perception and attitudes toward magnetic resonance imaging safety
BackgroundMagnetic resonance imaging (MRI) scanners use strong, static and fast magnetic fields to form images. Due to rapid developments in MRI technology, several accidents have been recorded in hospitals worldwide as a result of insufficient knowledge about the dangers of MRI on the part of the patient or a failure to follow safety guidelines. This study evaluates patients’ perception and attitudes about MRI safety.AimsThis is a cross sectional study to evaluate the perception and attitudes of patients regarding MRI safety procedures.MethodsA 21 items questionnaire was collected from 119 patients in the MRI waiting area before the commencement of examination. Data were analysed using Statistical Package for the Social Sciences (SPSS) software (version 22.0, IBM Corp, Armonk, New York). The odds (OR) and 95 per cent confidence interval (CI) were used for analysis, the level of significance was set at p=0.05 using Chi-Square test to evaluate the relationship among the variables in the questionnaire.ResultsThe responses were collected from the patients and their relatives (46 male (38.6 per cent) and 73 female (61.4 per cent)). Approximately 71 per cent of the participants have already read or heard about MRI and the related safety aspects. 76 per cent of overall participants stated that they are aware of the need for preparation before an MRI exam with more awareness of MRI safety issues among younger patients (88 per cent). In this instance, females showed a higher level of knowledge (26 per cent) compared to males (11 per cent) with p=0.035.ConclusionPatients reported insufficient information about MRI safety which may increase the potential for accidents
Predicting breast cancer recurrence using principal component analysis as feature extraction: an unbiased comparative analysis
Breast cancer recurrence is among the most noteworthy fears faced by women. Nevertheless, with modern innovations in data mining technology, early recurrence prediction can help relieve these fears. Although medical information is typically complicated, and simplifying searches to the most relevant input is challenging, new sophisticated data mining techniques promise accurate predictions from high-dimensional data. In this study, the performances of three established data mining algorithms: Naïve Bayes (NB), k-nearest neighbor (KNN), and fast decision tree (REPTree), adopting the feature extraction algorithm, principal component analysis (PCA), for predicting breast cancer recurrence were contrasted. The comparison was conducted between models built in the absence and presence of PCA. The results showed that KNN produced better prediction without PCA (F-measure = 72.1%), whereas the other two techniques: NB and REPTree, improved when used with PCA (F-measure = 76.1% and 72.8%, respectively). Anti-inflammatory and anti-oxidant properties of Ipomoea nil (Linn.) Roth significantly alleviates cigarette smoke (CS)-induced acute lung injury via possibly inhibiting the NF-κB pathway
Acute respiratory distress syndrome (ARDS), a serious manifestation of acute lung injury (ALI), is a debilitating inflammatory lung disease that is caused by multiple risk factors. One of the primary causes that can lead to ALI/ARDS is cigarette smoke (CS) and its primary mode of action is via oxidative stress. Despite extensive research, no appropriate therapy is currently available to treat ALI/ARDS, which means there is a dire need for new potential approaches. In our study we explored the protective effects of 70 % methanolic-aqueous extract of Ipomoea nil (Linn.) Roth, named as In.Mcx against CS-induced ALI mice models and RAW 264.7 macrophages because Ipomoea nil has traditionally been used to treat breathing irregularities. Male Swiss albino mice (20–25 ± 2 g) were subjected to CS for 10 uninterrupted days in order to establish CS-induced ALI murine models. Dexamethasone (1 mg/kg), In.Mcx (100 200, and 300 mg/kg) and normal saline (10 mL/kg) were given to respective animal groups, 1 h before CS-exposure. 24 h after the last CS exposure, the lungs and bronchoalveolar lavage fluid (BALF) of all euthanized mice were harvested. Altered alveolar integrity and elevated lung weight-coefficient, total inflammatory cells, oxidative stress, expression of pro-inflammatory cytokines (IL-1β and IL-6) and chemokines (KC) were significantly decreased by In.Mcx in CS-exposed mice. In.Mcx also revealed significant lowering IL-1β, IL-6 and KC expression in CSE (4 %)-activated RAW 264.7 macrophage. Additionally, In.Mcx showed marked enzyme inhibition activity against Acetylcholinesterase, Butyrylcholinesterase and Lipoxygenase. Importantly, In.Mcx dose-dependently and remarkably suppressed the CS-induced oxidative stress via not only reducing the MPO, TOS and MDA content but also improving TAC production in the lungs. Accordingly, HPLC analysis revealed the presence of many important antioxidant components. Finally, In.Mcx showed a marked decrease in the NF-κB expression both in in vivo and in vitro models. Our findings suggest that In.Mcx has positive therapeutic effects against CS-induced ALI via suppressing uncontrolled inflammatory response, oxidative stress, lipoxygenase and NF-κB p65 pathway

Healthcare unit managers play a crucial role in promoting the importance of paramedics and nurses in critical care. They are responsible for ensuring the health and well-being of nurses and paramedics, as well as maintaining high-quality care. However, there is a lack of research focused on the actions and organizational strategies employed by healthcare unit managers to support nurses, as well as the specific working environments that facilitate such support. This paper utilizes a mixed methods approach, combining qualitative interviews and quantitative surveys with healthcare unit managers and nurses. The findings highlight the crucial role of healthcare unit managers\u27 accessibility to their nursing staff in promoting nurses. Additionally, the study emphasizes the importance of responsive support from the care unit managers\u27 superiors, administration, and managerial colleagues in creating favorable working conditions. The care unit manager\u27s own support was highly supported by the superior manager, and this support was positively connected with nurses
Seroprevalence of antibodies to SARS-CoV-2 among blood donors in the early month of the pandemic in Saudi Arabia
Background: Serologic testing provides better understanding of SARS-CoV-2 prevalence and its transmission. This study was an investigation of the prevalence of antibodies to SARS-CoV-2 among blood donors in Saudi Arabia.
Objective: To estimate the seroprevalence of anti-SARS-CoV-2 antibodies among blood donors in Saudi Arabia during the early phase of the COVID-19 pandemic.
Methods: Serology results and epidemiological data were analyzed for 837 adult blood donors, with no confirmed SARS-CoV-2 infection, in Saudi Arabia from 20th to 25th May 2020. Seroprevalence was determined using electrochemical immunoassay to detect anti-SARS-CoV-2 antibodies.
Results: The overall seroprevalence of anti-SARS-CoV-2 antibodies was 1.4% (12/837). Non-citizens had higher seroprevalence compared with citizens (OR 13.6, p = 0.001). Secondary education was significantly associated with higher seroprevalence compared with higher education (OR 6.8, p = 0.005). The data showed that the highest seroprevalence was in Makkah (8.1%). Uisng Makkah seroprevalence as the reference, the seroprevalence in other areas was: Madinah 4.1% (OR 0.48, 95% CI 0.12-1.94), Jeddah 2.3% (OR 0.27, 95% CI 0.31-2.25), and Qassim 2.9 % (OR 0.34, 95% CI 0.04-2.89) and these were not statistically different from seroprevalence in the Makkah region.
Conclusions: At the early months of the COVID-19 pandemic in Saudi Arabia, the seroprevalence of antibodies to SARS-CoV-2 among blood donors was low, but was higher among non-citizens. These findings may indicate that non-citizens and less educated individuals may be less attentive to preventive measures. Monitoring seroprevalence trends over time require repeated sampling
Exome-wide association study to identify rare variants influencing COVID-19 outcomes : Results from the Host Genetics Initiative

Previously, the COVID-19 Host Genetics Initiative genome-wide association study used common variants to identify multiple loci associated with COVID-19 outcomes. However, variants with the largest impact on COVID-19 outcomes are expected to be rare in the population. Hence, studying rare variants may provide additional insights into disease susceptibility and pathogenesis, thereby informing therapeutics development. Here, we combined whole-exome and whole-genome sequencing from 21 cohorts across 12 countries and performed rare variant exome-wide burden analyses for COVID-19 outcomes. In an analysis of 5,085 severe disease cases and 571,737 controls, we observed that carrying a rare deleterious variant in the SARS-CoV-2 sensor toll-like receptor TLR7 (on chromosome X) was associated with a 5.3-fold increase in severe disease (95% CI: 2.75–10.05, p = 5.41x10-7). This association was consistent across sexes. These results further support TLR7 as a genetic determinant of severe disease and suggest that larger studies on rare variants influencing COVID-19 outcomes could provide additional insights.Peer reviewe
Burnout among surgeons before and during the SARS-CoV-2 pandemic: an international survey
Background: SARS-CoV-2 pandemic has had many significant impacts within the surgical realm, and surgeons have been obligated to reconsider almost every aspect of daily clinical practice. Methods: This is a cross-sectional study reported in compliance with the CHERRIES guidelines and conducted through an online platform from June 14th to July 15th, 2020. The primary outcome was the burden of burnout during the pandemic indicated by the validated Shirom-Melamed Burnout Measure. Results: Nine hundred fifty-four surgeons completed the survey. The median length of practice was 10 years; 78.2% included were male with a median age of 37 years old, 39.5% were consultants, 68.9% were general surgeons, and 55.7% were affiliated with an academic institution. Overall, there was a significant increase in the mean burnout score during the pandemic; longer years of practice and older age were significantly associated with less burnout. There were significant reductions in the median number of outpatient visits, operated cases, on-call hours, emergency visits, and research work, so, 48.2% of respondents felt that the training resources were insufficient. The majority (81.3%) of respondents reported that their hospitals were included in the management of COVID-19, 66.5% felt their roles had been minimized; 41% were asked to assist in non-surgical medical practices, and 37.6% of respondents were included in COVID-19 management. Conclusions: There was a significant burnout among trainees. Almost all aspects of clinical and research activities were affected with a significant reduction in the volume of research, outpatient clinic visits, surgical procedures, on-call hours, and emergency cases hindering the training.
