    Retrospective cohort analysis of Spanish national trends of coronary artery bypass grafting and percutaneous coronary intervention from 1998 to 2017

    Introduction Spain is one of the countries with the lowest rates of revascularisation and highest ratio of percutaneous coronary intervention (PCI) to coronary artery bypass grafting (CABG). Objectives To investigate the changes and trends in the two revascularisation procedures between 1998 and 2017 in Spain. Design Retrospective cohort study. Analysis of in-hospital outcomes. Setting Minimum basic data set from the Spanish National Department of Health: mandatory database collecting information of patients who are attended in the Spanish public National Health System. Participants 603 976 patients who underwent isolated CABG or PCI in the Spanish National Health System. The study period was divided in four 5-year intervals. Patients with acute myocardial infarction on admission were excluded. Primary and secondary outcomes We investigated the volume of procedures nationwide, the changes of the risk profile of patients and in-hospital mortality of both techniques. Results We observed a 2.2-fold increase in the rate of any type of myocardial revascularisation per million inhabitants-year: 357 (1998) to 776 (2017). 93 682 (15.5%) had a coronary surgery. PCI to CABG ratio rose from 2.2 (1998-2002) to 8.1 (2013-2017). Charlson's index increased by 0.8 for CABG and 1 for PCI. The median annual volume of PCI/hospital augmented from 136 to 232, while the volume of CABG was reduced from 137 to 74. In the two decades, we detected a significant reduction of CABG in-hospital mortality (6.5% vs 2.6%, p<0.001) and a small increase in PCI (1.2% vs 1.5%, p<0.001). Risk adjusted mortality rate was reduced for both CABG (1.51 vs 0.48, p<0.001), and PCI (1.42 vs 1.05, p<0.001).     Estudio de la celularidad en las diferentes regiones musculares de rodaballos diploides y triploides, Scophthalmus maximus L.

    Se analizó la celularidad muscular en cuatro zonas del miotomo: epiaxial superior, hipoaxial superior, epiaxial interior e hipoaxial inlerior, en2 grupos de rodaballos: 7 diploides y 7 triploides. En diploides, las zonas epiaxiales (superior e interior) mostraron mayores valores del tamaOO (p0,05) que las hipoaxiales, siendo tales valores mayores en hembras que en machos. En triploides, ~ distribución librilar mostró algunas dilerencias respecto a diploides, observándose mayores valores del tamaño librilar en las regiones epie hipoaxiales de la zona superior del pez, mientras que el menor número y densidad de fibras fue encontrado en la región epiaxial superior. En relación al sexo, el tamaño librilar lue menor en hembras que en machos triploides, mientras que el resto de parámetros musculares mostraron generalmente mayores valores en hembras.     Meshless electrophysiological modeling of cardiac resynchronization therapy—benchmark analysis with finite-element methods in experimental data

    Computational models of cardiac electrophysiology are promising tools for reducing the rates of non-response patients suitable for cardiac resynchronization therapy (CRT) by optimizing electrode placement. The majority of computational models in the literature are mesh-based, primarily using the finite element method (FEM). The generation of patient-specific cardiac meshes has traditionally been a tedious task requiring manual intervention and hindering the modeling of a large number of cases. Meshless models can be a valid alternative due to their mesh quality independence. The organization of challenges such as the CRT-EPiggy19, providing unique experimental data as open access, enables benchmarking analysis of different cardiac computational modeling solutions with quantitative metrics. We present a benchmark analysis of a meshless-based method with finite-element methods for the prediction of cardiac electrical patterns in CRT, based on a subset of the CRT-EPiggy19 dataset. A data assimilation strategy was designed to personalize the most relevant parameters of the electrophysiological simulations and identify the optimal CRT lead configuration. The simulation results obtained with the meshless model were equivalent to FEM, with the most relevant aspect for accurate CRT predictions being the parameter personalization strategy (e.g., regional conduction velocity distribution, including the Purkinje system and CRT lead distribution). © 2022 by the authors.     The economic geography of the meso-global spaces: integrating multinationals and clusters at the local-global level

In the case of the latter, the literature, produced within the global production networks (GPNs) and global value chain frameworks, does not address the central role of firms, especially multinationals which co-locate and connect territories along GPNs. This paper develops a cross-field conceptual integration in order to enrich the EG perspective, using qualitative research methodology to test the framework.     The role of a firm's absorptive capacity and the technology transfer process in clusters: How effective are technology centres in low-tech clusters?

    This paper analyses how the internal resources of small- and medium-sized enterprises determine access (learning processes) to technology centres (TCs) or industrial research institutes (innovation infrastructure) in traditional low-tech clusters. These interactions basically represent traded (market-based) transactions, which constitute important sources of knowledge in clusters. The paper addresses the role of TCs in low-tech clusters, and uses semi-structured interviews with 80 firms in a manufacturing cluster. The results point out that producer–user interactions are the most frequent; thus, the higher the sector knowledge-intensive base, the more likely the utilization of the available research infrastructure becomes. Conversely, the sectors with less knowledge-intensive structures, i.e. less absorptive capacity (AC), present weak linkages to TCs, as they frequently prefer to interact with suppliers, who act as transceivers of knowledge. Therefore, not all the firms in a cluster can fully exploit the available research infrastructure, and their AC moderates this engagement. In addition, the existence of TCs is not sufficient since the active role of a firm's search strategies to undertake interactions and conduct openness to available sources of knowledge is also needed.     Efecto de la triploidía sobre los parámetros musculares y nutricionales del rodaballo (Scophthalmus maximus, L.) después de su maduración sexual

    Con el objetivo de una mejor comprensión de la calidad final del rodaballo triploide (Scophtha/mus maximus, L.), la celularidad musculary los parámetros de calidad de la carne fueron estudiados en 7 rodaballos diploides y 7 lriploides de 33 meses de edad, todos ejemplares decultivoy de un tamaño similar. La celularidad del músculo blanco se evaluó por medio del número y diámetro de las fibras musculares. Para determinarla calidad de la carne fueron analizados diversos parámetros físico-químicos (humedad, proteína, grasa y ácidos grasas totales) y se determinaIOO las propiedades mecánicas de la lextura (dureza, elasticidad, masticabilidad, cohe~ividad y gomosidad) con un texturómetro. El mecanismode crecimiento muscular difirió entre los dos grupos, de tal manera que el tamaño de las fibras fue mayor (p<O,OOI)en los triploides , mientrasquela densidad de fibras musculares fue mayor en el grupo de los diploides, lo que demuestra que las diferencias genéticas produjeron dtlerente celularidad muscular en ambos grupos. No se encontraron diferencias significativas en las propiedades texturales ni en los parámetros decalidad de la carne entre diploides y triploides.In order lo a beller understanding of Ihe final qualily of lriploid turbot (Scophthalmus maximus, L.), muscle cellularily and qualily parametersoflhe flesh were studied on 7 diploid and 7 triploid 33 months old farmed turbot of similar size. White muscle cellularily was evaluated by meansofIhe number and diameter of muscle fibers. lo ascertain the flesh qualily, several physlco-cbemlcal parameters (moisture, protein, total fat andfalty acids) were analyzed, and textural mechanical properties (hardness, springiness, chewiness, cohesiveness, gumminess) were determinedwitha texturometer. Muscle growth mechanism differed between both groups, such that muscie fibres size was greater (p<O,OOI)in triploid specimens, wihereas muscle fibre density was higher in diploid group, thus showing mat genetic differences produced different muscie cellularity in bothgroups. No significant differences were found in textural properties and flesh qualily parameters between diploid and triploid groups

    Las empresas han utilizado diversas herramientas que permiten que los operarios contribuyan al proceso de mejora continua. Entre las herramientas más usadas podemos destacar los sistemas de sugerencias tanto individuales como en grupo. En esta comunicación haremos un repaso de las principales características de ambos sistemas y los modos habituales de implantación. Nuestra ponencia pretende intentar responder a estas preguntas de investigación. ¿Qué resultados se derivan de la implantación de sistemas de sugerencias individuales o en grupo? ¿Cuál de los dos sistemas es más beneficioso para la empresa? ¿Qué problemas surgen durante el funcionamiento de estos programas? Para ello, analizaremos los datos de un caso de empresa industrial donde hemos recogido los datos históricos de 5 años de aplicación de un programa de mejora continua. Ambos programas han demostrado ser provechosos para la empresa, aunque las posibilidades de los sistemas de grupo parecen ser significativamente mayores

    Unraveling the effect of silent, intronic and missense mutations on VWF splicing: contribution of next generation sequencing in the study of mRNA

    Large studies in von Willebrand disease patients, including Spanish and Portuguese registries, led to the identification of >250 different mutations. It is a challenge to determine the pathogenic effect of potential splice site mutations on VWF mRNA. This study aimed to elucidate the true effects of 18 mutations on VWF mRNA processing, investigate the contribution of next-generation sequencing to in vivo mRNA study in von Willebrand disease, and compare the findings with in silico prediction. RNA extracted from patient platelets and leukocytes was amplified by RT-PCR and sequenced using Sanger and next generation sequencing techniques. Eight mutations affected VWF splicing: c.1533+1G>A, c.5664+2T>C and c.546G>A (p.=) prompted exon skipping; c.3223-7_3236dup and c.7082-2A>G resulted in activation of cryptic sites; c.3379+1G>A and c.7437G>A) demonstrated both molecular pathogenic mechanisms simultaneously; and the p.Cys370Tyr missense mutation generated two aberrant transcripts. Of note, the complete effect of three mutations was provided by next generation sequencing alone because of low expression of the aberrant transcripts. In the remaining 10 mutations, no effect was elucidated in the experiments. However, the differential findings obtained in platelets and leukocytes provided substantial evidence that four of these would have an effect on VWF levels. In this first report using next generation sequencing technology to unravel the effects of VWF mutations on splicing, the technique yielded valuable information. Our data bring to light the importance of studying the effect of synonymous and missense mutations on VWF splicing to improve the current knowledge of the molecular mechanisms behind von Willebrand disease. clinicaltrials.gov identifier:02869074
