    Machine learning for large-scale wearable sensor data in Parkinson disease:concepts, promises, pitfalls, and futures

    For the treatment and monitoring of Parkinson's disease (PD) to be scientific, a key requirement is that measurement of disease stages and severity is quantitative, reliable, and repeatable. The last 50 years in PD research have been dominated by qualitative, subjective ratings obtained by human interpretation of the presentation of disease signs and symptoms at clinical visits. More recently, “wearable,” sensor-based, quantitative, objective, and easy-to-use systems for quantifying PD signs for large numbers of participants over extended durations have been developed. This technology has the potential to significantly improve both clinical diagnosis and management in PD and the conduct of clinical studies. However, the large-scale, high-dimensional character of the data captured by these wearable sensors requires sophisticated signal processing and machine-learning algorithms to transform it into scientifically and clinically meaningful information. Such algorithms that “learn” from data have shown remarkable success in making accurate predictions for complex problems in which human skill has been required to date, but they are challenging to evaluate and apply without a basic understanding of the underlying logic on which they are based. This article contains a nontechnical tutorial review of relevant machine-learning algorithms, also describing their limitations and how these can be overcome. It discusses implications of this technology and a practical road map for realizing the full potential of this technology in PD research and practice

    Towards a wearable system for predicting the freezing of gait in people affected by Parkinson's disease

    Some wearable solutions exploiting on-body acceleration sensors have been proposed to recognize Freezing of Gait (FoG) in people affected by Parkinson Disease (PD). Once a FoG event is detected, these systems generate a sequence of rhythmic stimuli to allow the patient restarting the march. While these solutions are effective in detecting FoG events, they are unable to predict FoG to prevent its occurrence. This paper fills in the gap by presenting a machine learning-based approach that classifies accelerometer data from PD patients, recognizing a pre-FOG phase to further anticipate FoG occurrence in advance. Gait was monitored by three tri-axial accelerometer sensors worn on the back, hip and ankle. Gait features were then extracted from the accelerometer's raw data through data windowing and non-linear dimensionality reduction. A k-nearest neighbor algorithm (k-NN) was used to classify gait in three classes of events: pre-FoG, no-FoG and FoG. The accuracy of the proposed solution was compared to state of-the-art approaches. Our study showed that: (i) we achieved performances overcoming the state-of-the-art approaches in terms of FoG detection, (ii) we were able, for the very first time in the literature, to predict FoG by identifying the pre-FoG events with an average sensitivity and specificity of, respectively, 94.1% and 97.1%, and (iii) our algorithm can be executed on resource-constrained devices. Future applications include the implementation on a mobile device, and the administration of rhythmic stimuli by a wearable device to help the patient overcome the FoG

    Real-Life Gait Performance as a Digital Biomarker for Motor Fluctuations: The Parkinson@Home Validation Study

    Background: Wearable sensors have been used successfully to characterize bradykinetic gait in patients with Parkinson disease (PD), but most studies to date have been conducted in highly controlled laboratory environments. Objective: This paper aims to assess whether sensor-based analysis of real-life gait can be used to objectively and remotely monitor motor fluctuations in PD. Methods: The Parkinson@Home validation study provides a new reference data set for the development of digital biomarkers to monitor persons with PD in daily life. Specifically, a group of 25 patients with PD with motor fluctuations and 25 age-matched controls performed unscripted daily activities in and around their homes for at least one hour while being recorded on video. Patients with PD did this twice: once after overnight withdrawal of dopaminergic medication and again 1 hour after medication intake. Participants wore sensors on both wrists and ankles, on the lower back, and in the front pants pocket, capturing movement and contextual data. Gait segments of 25 seconds were extracted from accelerometer signals based on manual video annotations. The power spectral density of each segment and device was estimated using Welch’s method, from which the total power in the 0.5- to 10-Hz band, width of the dominant frequency, and cadence were derived. The ability to discriminate between before and after medication intake and between patients with PD and controls was evaluated using leave-one-subject-out nested cross-validation. Results: From 18 patients with PD (11 men; median age 65 years) and 24 controls (13 men; median age 68 years), ≥10 gait segments were available. Using logistic LASSO (least absolute shrinkage and selection operator) regression, we classified whether the unscripted gait segments occurred before or after medication intake, with mean area under the receiver operator curves (AUCs) varying between 0.70 (ankle of least affected side, 95% CI 0.60-0.81) and 0.82 (ankle of most affected side, 95% CI 0.72-0.92) across sensor locations. Combining all sensor locations did not significantly improve classification (AUC 0.84, 95% CI 0.75-0.93). Of all signal properties, the total power in the 0.5- to 10-Hz band was most responsive to dopaminergic medication. Discriminating between patients with PD and controls was generally more difficult (AUC of all sensor locations combined: 0.76, 95% CI 0.62-0.90). The video recordings revealed that the positioning of the hands during real-life gait had a substantial impact on the power spectral density of both the wrist and pants pocket sensor. Conclusions: We present a new video-referenced data set that includes unscripted activities in and around the participants’ homes. Using this data set, we show the feasibility of using sensor-based analysis of real-life gait to monitor motor fluctuations with a single sensor location. Future work may assess the value of contextual sensors to control for real-world confounders

    Technology in Parkinson's disease:challenges and opportunities

    The miniaturization, sophistication, proliferation, and accessibility of technologies are enabling the capture of more and previously inaccessible phenomena in Parkinson's disease (PD). However, more information has not translated into a greater understanding of disease complexity to satisfy diagnostic and therapeutic needs. Challenges include noncompatible technology platforms, the need for wide-scale and long-term deployment of sensor technology (among vulnerable elderly patients in particular), and the gap between the "big data" acquired with sensitive measurement technologies and their limited clinical application. Major opportunities could be realized if new technologies are developed as part of open-source and/or open-hardware platforms that enable multichannel data capture sensitive to the broad range of motor and nonmotor problems that characterize PD and are adaptable into self-adjusting, individualized treatment delivery systems. The International Parkinson and Movement Disorders Society Task Force on Technology is entrusted to convene engineers, clinicians, researchers, and patients to promote the development of integrated measurement and closed-loop therapeutic systems with high patient adherence that also serve to (1) encourage the adoption of clinico-pathophysiologic phenotyping and early detection of critical disease milestones, (2) enhance the tailoring of symptomatic therapy, (3) improve subgroup targeting of patients for future testing of disease-modifying treatments, and (4) identify objective biomarkers to improve the longitudinal tracking of impairments in clinical care and research. This article summarizes the work carried out by the task force toward identifying challenges and opportunities in the development of technologies with potential for improving the clinical management and the quality of life of individuals with PD. © 2016 International Parkinson and Movement Disorder Society

    Intelligent diagnosis system based on artificial intelligence models for predicting freezing of gait in Parkinson’s disease

    IntroductionFreezing of gait (FoG) is a significant issue for those with Parkinson’s disease (PD) since it is a primary contributor to falls and is linked to a poor superiority of life. The underlying apparatus is still not understood; however, it is postulated that it is associated with cognitive disorders, namely impairments in executive and visuospatial functions. During episodes of FoG, patients may experience the risk of falling, which significantly effects their quality of life.MethodsThis research aims to systematically evaluate the effectiveness of machine learning approaches in accurately predicting a FoG event before it occurs. The system was tested using a dataset collected from the Kaggle repository and comprises 3D accelerometer data collected from the lower backs of people who suffer from episodes of FoG, a severe indication frequently realized in persons with Parkinson’s disease. Data were acquired by measuring acceleration from 65 patients and 20 healthy senior adults while they engaged in simulated daily life tasks. Of the total participants, 45 exhibited indications of FoG. This research utilizes seven machine learning methods, namely the decision tree, random forest, Knearest neighbors algorithm, LightGBM, and CatBoost models. The Gated Recurrent Unit (GRU)-Transformers and Longterm Recurrent Convolutional Networks (LRCN) models were applied to predict FoG. The construction and model parameters were planned to enhance performance by mitigating computational difficulty and evaluation duration.ResultsThe decision tree exhibited exceptional performance, achieving sensitivity rates of 91% in terms of accuracy, precision, recall, and F1- score metrics for the FoG, transition, and normal activity classes, respectively. It has been noted that the system has the capacity to anticipate FoG objectively and precisely. This system will be instrumental in advancing consideration in furthering the comprehension and handling of FoG

    Wearable inertial sensors for human movement analysis

    Introduction: The present review aims to provide an overview of the most common uses of wearable inertial sensors in the field of clinical human movement analysis.Areas covered: Six main areas of application are analysed: gait analysis, stabilometry, instrumented clinical tests, upper body mobility assessment, daily-life activity monitoring and tremor assessment. Each area is analyzed both from a methodological and applicative point of view. The focus on the methodological approaches is meant to provide an idea of the computational complexity behind a variable/parameter/index of interest so that the reader is aware of the reliability of the approach. The focus on the application is meant to provide a practical guide for advising clinicians on how inertial sensors can help them in their clinical practice.Expert commentary: Less expensive and more easy to use than other systems used in human movement analysis, wearable sensors have evolved to the point that they can be considered ready for being part of routine clinical routine

    Clinical Decision Support Systems with Game-based Environments, Monitoring Symptoms of Parkinson’s Disease with Exergames

    Parkinson’s Disease (PD) is a malady caused by progressive neuronal degeneration, deriving in several physical and cognitive symptoms that worsen with time. Like many other chronic diseases, it requires constant monitoring to perform medication and therapeutic adjustments. This is due to the significant variability in PD symptomatology and progress between patients. At the moment, this monitoring requires substantial participation from caregivers and numerous clinic visits. Personal diaries and questionnaires are used as data sources for medication and therapeutic adjustments. The subjectivity in these data sources leads to suboptimal clinical decisions. Therefore, more objective data sources are required to better monitor the progress of individual PD patients. A potential contribution towards more objective monitoring of PD is clinical decision support systems. These systems employ sensors and classification techniques to provide caregivers with objective information for their decision-making. This leads to more objective assessments of patient improvement or deterioration, resulting in better adjusted medication and therapeutic plans. Hereby, the need to encourage patients to actively and regularly provide data for remote monitoring remains a significant challenge. To address this challenge, the goal of this thesis is to combine clinical decision support systems with game-based environments. More specifically, serious games in the form of exergames, active video games that involve physical exercise, shall be used to deliver objective data for PD monitoring and therapy. Exergames increase engagement while combining physical and cognitive tasks. This combination, known as dual-tasking, has been proven to improve rehabilitation outcomes in PD: recent randomized clinical trials on exergame-based rehabilitation in PD show improvements in clinical outcomes that are equal or superior to those of traditional rehabilitation. In this thesis, we present an exergame-based clinical decision support system model to monitor symptoms of PD. This model provides both objective information on PD symptoms and an engaging environment for the patients. The model is elaborated, prototypically implemented and validated in the context of two of the most prominent symptoms of PD: (1) balance and gait, as well as (2) hand tremor and slowness of movement (bradykinesia). While balance and gait affections increase the risk of falling, hand tremors and bradykinesia affect hand dexterity. We employ Wii Balance Boards and Leap Motion sensors, and digitalize aspects of current clinical standards used to assess PD symptoms. In addition, we present two dual-tasking exergames: PDDanceCity for balance and gait, and PDPuzzleTable for tremor and bradykinesia. We evaluate the capability of our system for assessing the risk of falling and the severity of tremor in comparison with clinical standards. We also explore the statistical significance and effect size of the data we collect from PD patients and healthy controls. We demonstrate that the presented approach can predict an increased risk of falling and estimate tremor severity. Also, the target population shows a good acceptance of PDDanceCity and PDPuzzleTable. In summary, our results indicate a clear feasibility to implement this system for PD. Nevertheless, long-term randomized clinical trials are required to evaluate the potential of PDDanceCity and PDPuzzleTable for physical and cognitive rehabilitation effects

    Context-aware home monitoring system for Parkinson's disease patients : ambient and wearable sensing for freezing of gait detection

This PhD Thesis has been developed in the framework of, and according to, the rules of the Erasmus Mundus Joint Doctorate on Interactive and Cognitive Environments EMJD ICE [FPA no. 2010-0012]Parkinson’s disease (PD). It is characterized by brief episodes of inability to step, or by extremely short steps that typically occur on gait initiation or on turning while walking. The consequences of FOG are aggravated mobility and higher affinity to falls, which have a direct effect on the quality of life of the individual. There does not exist completely effective pharmacological treatment for the FOG phenomena. However, external stimuli, such as lines on the floor or rhythmic sounds, can focus the attention of a person who experiences a FOG episode and help her initiate gait. The optimal effectiveness in such approach, known as cueing, is achieved through timely activation of a cueing device upon the accurate detection of a FOG episode. Therefore, a robust and accurate FOG detection is the main problem that needs to be solved when developing a suitable assistive technology solution for this specific user group. This thesis proposes the use of activity and spatial context of a person as the means to improve the detection of FOG episodes during monitoring at home. The thesis describes design, algorithm implementation and evaluation of a distributed home system for FOG detection based on multiple cameras and a single inertial gait sensor worn at the waist of the patient. Through detailed observation of collected home data of 17 PD patients, we realized that a novel solution for FOG detection could be achieved by using contextual information of the patient’s position, orientation, basic posture and movement on a semantically annotated two-dimensional (2D) map of the indoor environment. We envisioned the future context-aware system as a network of Microsoft Kinect cameras placed in the patient’s home that interacts with a wearable inertial sensor on the patient (smartphone). Since the hardware platform of the system constitutes from the commercial of-the-shelf hardware, the majority of the system development efforts involved the production of software modules (for position tracking, orientation tracking, activity recognition) that run on top of the middle-ware operating system in the home gateway server. The main component of the system that had to be developed is the Kinect application for tracking the position and height of multiple people, based on the input in the form of 3D point cloud data. Besides position tracking, this software module also provides mapping and semantic annotation of FOG specific zones on the scene in front of the Kinect. One instance of vision tracking application is supposed to run for every Kinect sensor in the system, yielding potentially high number of simultaneous tracks. At any moment, the system has to track one specific person - the patient. To enable tracking of the patient between different non-overlapped cameras in the distributed system, a new re-identification approach based on appearance model learning with one-class Support Vector Machine (SVM) was developed. Evaluation of the re-identification method was conducted on a 16 people dataset in a laboratory environment. Since the patient orientation in the indoor space was recognized as an important part of the context, the system necessitated the ability to estimate the orientation of the person, expressed in the frame of the 2D scene on which the patient is tracked by the camera. We devised method to fuse position tracking information from the vision system and inertial data from the smartphone in order to obtain patient’s 2D pose estimation on the scene map. Additionally, a method for the estimation of the position of the smartphone on the waist of the patient was proposed. Position and orientation estimation accuracy were evaluated on a 12 people dataset. Finally, having available positional, orientation and height information, a new seven-class activity classification was realized using a hierarchical classifier that combines height-based posture classifier with translational and rotational SVM movement classifiers. Each of the SVM movement classifiers and the joint hierarchical classifier were evaluated in the laboratory experiment with 8 healthy persons. The final context-based FOG detection algorithm uses activity information and spatial context information in order to confirm or disprove FOG detected by the current state-of-the-art FOG detection algorithm (which uses only wearable sensor data). A dataset with home data of 3 PD patients was produced using two Kinect cameras and a smartphone in synchronized recording. The new context-based FOG detection algorithm and the wearable-only FOG detection algorithm were both evaluated with the home dataset and their results were compared. The context-based algorithm very positively influences the reduction of false positive detections, which is expressed through achieved higher specificity. In some cases, context-based algorithm also eliminates true positive detections, reducing sensitivity to the lesser extent. The final comparison of the two algorithms on the basis of their sensitivity and specificity, shows the improvement in the overall FOG detection achieved with the new context-aware home system.Esta tesis propone el uso de la actividad y el contexto espacial de una persona como medio para mejorar la detección de episodios de FOG (Freezing of gait) durante el seguimiento en el domicilio. La tesis describe el diseño, implementación de algoritmos y evaluación de un sistema doméstico distribuido para detección de FOG basado en varias cámaras y un único sensor de marcha inercial en la cintura del paciente. Mediante de la observación detallada de los datos caseros recopilados de 17 pacientes con EP, nos dimos cuenta de que se puede lograr una solución novedosa para la detección de FOG mediante el uso de información contextual de la posición del paciente, orientación, postura básica y movimiento anotada semánticamente en un mapa bidimensional (2D) del entorno interior. Imaginamos el futuro sistema de consciencia del contexto como una red de cámaras Microsoft Kinect colocadas en el hogar del paciente, que interactúa con un sensor de inercia portátil en el paciente (teléfono inteligente). Al constituirse la plataforma del sistema a partir de hardware comercial disponible, los esfuerzos de desarrollo consistieron en la producción de módulos de software (para el seguimiento de la posición, orientación seguimiento, reconocimiento de actividad) que se ejecutan en la parte superior del sistema operativo del servidor de puerta de enlace de casa. El componente principal del sistema que tuvo que desarrollarse es la aplicación Kinect para seguimiento de la posición y la altura de varias personas, según la entrada en forma de punto 3D de datos en la nube. Diseñamos un método para fusionar la información de seguimiento de posición del sistema de visión y los datos de inercia del smartphone para obtener la estimación de postura 2D del paciente en el mapa de la escena. Además, se propuso un método para la estimación de la posición del Smartphone en la cintura del paciente. La precisión de la estimación de la posición y la orientación se evaluó en un conjunto de datos de 12 personas. Finalmente, al tener disponible información de posición, orientación y altura, se realizó una nueva clasificación de actividad de seven-class utilizando un clasificador jerárquico que combina un clasificador de postura basado en la altura con clasificadores de movimiento SVM traslacional y rotacional. Cada uno de los clasificadores de movimiento SVM y el clasificador jerárquico conjunto se evaluaron en el experimento de laboratorio con 8 personas sanas. El último algoritmo de detección de FOG basado en el contexto utiliza información de actividad e información de texto espacial para confirmar o refutar el FOG detectado por el algoritmo de detección de FOG actual.     Modulation and modeling of anticipatory postural adjustments for gait initiation in persons with Parkinson's disease

    Parkinson’s disease (PD) is a movement disorder traditionally thought to be caused by the degeneration of striatal dopaminergic neurons in the substantia nigra. One of the most devastating symptoms of PD that can decrease mobility and substantially impair quality of life is freezing of gait (FOG). Currently, the treatments for the motor symptoms of PD (e.g., levodopa, deep brain stimulation) are ineffective at managing FOG as the disease progresses. These treatments only target the cortical-striatal pathways of volitional movement that are dependent on dopamine, whereas FOG may be caused by the degeneration of other non-dopaminergic subcortical nuclei that are involved with posture and gait control (e.g., the pedunculopontine nucleus). A well-characterized behavior observed in PD that could contribute to FOG is a diminished ability to properly coordinate anticipatory postural adjustments (APAs) prior to the first step. In particular, diminished muscle activation leads to impaired limb mechanics and slower, less-coordinated gait initiation. Sensory cues have been demonstrated to improve gait initiation behaviors, possibly because they provide relevant information for movement to the motor cortex through cerebellar-thalamo-parietal pathways that remain intact during the disease process. However, sensory cues are not always reliable or effective in all contexts and are unable to directly modulate the force production of the user. Forms of mechanical stimuli can amplify force production during APAs by directly modulating force production and providing relevant timing and magnitude information through afferent sensory pathways. To date, no mechanical assistance that mimics the desired motion during an APA provided at the ankle joint in the form of modest ankle torques has been tested. The overall research objective of this dissertation work was to test the hypothesis that mechanical assistance provided at the ankle joint can be an effective paradigm for facilitating the diminished gait initiation behaviors in persons with PD and FOG symptoms. Biomechanical measurements and mechanical modeling techniques were used to explore the neuromechanical factors (e.g., cognitive, sensorimotor, biomechanical) that could enable this type of intervention or therapy. (1) The first research objective was to provide proof of concept that mechanical assistance provided at the ankle through a powered ankle-foot orthosis can shorten and amplify APAs compared to self-initiated stepping in healthy young adults. (2) The second research objective was to test the hypothesis that mechanical assistance provided at the ankle by a wearable powered ankle-foot orthosis can directly shorten and amplify gait initiation APAs compared to self-initiated and acoustic cued stepping in persons with PD and FOG. (3) The third research objective was to evaluate how cue-induced modulation of APAs in persons with PD and FOG vary based on whether the external cue is initiated exogenously or is self-triggered. (4) The fourth research objective of this dissertation was to simulate the behaviors observed during the early phase of an APA for gait initiation in persons with PD and FOG using mechanical modeling techniques. Results from these studies may inform future interventions or therapies that can provide mechanical assistance at the ankle during gait initiation for persons with PD and FOG. Such interventions could increase mobility and promote independence, thereby improving quality of life and decreasing morbidity for these patients
