    Evaluation des Bundesprogramms "Soziale Teilhabe am Arbeitsmarkt": Erster Zwischenbericht

    Das Bundesprogramm "Soziale Teilhabe am Arbeitsmarkt" zielt auf die Verbesserung der sozialen Teilhabe von arbeitsmarktfernen Langzeitleistungsbeziehenden im SGB II, die entweder mit Kindern in einer Bedarfsgemeinschaft leben und/oder auf Grund von gesundheitlichen Einschränkungen besonderer Förderung bedürfen. Es ist im Jahr 2015 gestartet und läuft bis zum 31.12.2018. Bislang nahmen knapp 10.000 Personen aus 105 Jobcentern an dem Programm teil. Eine Ausweitung des Programms hat zum Jahreswechsel 2016/17 stattgefunden. Der vorliegende Zwischenbericht stellt erste Ergebnisse der programmbegleitenden Evaluation vor.The programme "Social Inclusion in the Labour Market" (Soziale Teilhabe am Arbeitsmarkt) is a Federal labour market programme aiming to support social inclusion of persons with severe placement obstacles who are long-time receivers of Arbeitslosengeld II (minimum income support). The programme runs from 2015 to 31.12.2018 and focusses on persons who live together with children and/or who need special support due to health limitations. Until now, almost 10,000 persons from 105 Jobcenters participate in this programme. The programme will be extended from January 2017 on. This intermediate report presents first findings from the evaluation research team

    Meta-analysis uncovers genome-wide significant variants for rapid kidney function decline

    Rapid decline of glomerular filtration rate estimated from creatinine (eGFRcrea) is associated with severe clinical endpoints. In contrast to cross-sectionally assessed eGFRcrea, the genetic basis for rapid eGFRcrea decline is largely unknown. To help define this, we meta-analyzed 42 genome-wide association studies from the Chronic Kidney Diseases Genetics Consortium and United Kingdom Biobank to identify genetic loci for rapid eGFRcrea decline. Two definitions of eGFRcrea decline were used: 3 mL/min/1.73m(2)/year or more ("Rapid3"; encompassing 34,874 cases, 107,090 controls) and eGFRcrea decline 25% or more and eGFRcrea under 60 mL/min/1.73m(2) at follow-up among those with eGFRcrea 60 mL/min/1.73m(2) or more at baseline ("CKDi25"; encompassing 19,901 cases, 175,244 controls). Seven independent variants were identified across six loci for Rapid3 and/or CKDi25: consisting of five variants at four loci with genome-wide significance (near UMOD-PDILT (2), PRKAG2, WDR72, OR2S2) and two variants among 265 known eGFRcrea variants (near GATM, LARP4B). All these loci were novel for Rapid3 and/or CKDi25 and our bioinformatic follow-up prioritized variants and genes underneath these loci. The OR2S2 locus is novel for any eGFRcrea trait including interesting candidates. For the five genome-wide significant lead variants, we found supporting effects for annual change in blood urea nitrogen or cystatin-based eGFR, but not for GATM or (LARP4B). Individuals at high compared to those at low genetic risk (8-14 vs. 0-5 adverse alleles) had a 1.20-fold increased risk of acute kidney injury (95% confidence interval 1.08-1.33). Thus, our identified loci for rapid kidney function decline may help prioritize therapeutic targets and identify mechanisms and individuals at risk for sustained deterioration of kidney function

    Target genes, variants, tissues and transcriptional pathways influencing human serum urate levels.

    Elevated serum urate levels cause gout and correlate with cardiometabolic diseases via poorly understood mechanisms. We performed a trans-ancestry genome-wide association study of serum urate in 457,690 individuals, identifying 183 loci (147 previously unknown) that improve the prediction of gout in an independent cohort of 334,880 individuals. Serum urate showed significant genetic correlations with many cardiometabolic traits, with genetic causality analyses supporting a substantial role for pleiotropy. Enrichment analysis, fine-mapping of urate-associated loci and colocalization with gene expression in 47 tissues implicated the kidney and liver as the main target organs and prioritized potentially causal genes and variants, including the transcriptional master regulators in the liver and kidney, HNF1A and HNF4A. Experimental validation showed that HNF4A transactivated the promoter of ABCG2, encoding a major urate transporter, in kidney cells, and that HNF4A p.Thr139Ile is a functional variant.     Genetic loci and prioritization of genes for kidney function decline derived from a meta-analysis of 62 longitudinal genome-wide association studies

    Estimated glomerular filtration rate (eGFR) reflects kidney function. Progressive eGFR-decline can lead to kidney failure, necessitating dialysis or transplantation. Hundreds of loci from genome-wide association studies (GWAS) for eGFR help explain population cross section variability. Since the contribution of these or other loci to eGFR-decline remains largely unknown, we derived GWAS for annual eGFR-decline and meta-analyzed 62 longitudinal studies with eGFR assessed twice over time in all 343,339 individuals and in high-risk groups. We also explored different covariate adjustment. Twelve genome-wide significant independent variants for eGFR-decline unadjusted or adjusted for eGFR-baseline (11 novel, one known for this phenotype), including nine variants robustly associated across models were identified. All loci for eGFR-decline were known for cross-sectional eGFR and thus distinguished a subgroup of eGFR loci. Seven of the nine variants showed variant-by-age interaction on eGFR cross section (further about 350,000 individuals), which linked genetic associations for eGFR-decline with age-dependency of genetic cross-section associations. Clinically important were two to four-fold greater genetic effects on eGFR-decline in high-risk subgroups. Five variants associated also with chronic kidney disease progression mapped to genes with functional in-silico evidence (UMOD, SPATA7, GALNTL5, TPPP). An unfavorable versus favorable nine-variant genetic profile showed increased risk odds ratios of 1.35 for kidney failure (95% confidence intervals 1.03-1.77) and 1.27 for acute kidney injury (95% confidence intervals 1.08-1.50) in over 2000 cases each, with matched controls). Thus, we provide a large data resource, genetic loci, and prioritized genes for kidney function decline, which help inform drug development pipelines revealing important insights into the age-dependency of kidney function genetics

    Reducing the environmental impact of surgery on a global scale: systematic review and co-prioritization with healthcare workers in 132 countries

    Abstract Background Healthcare cannot achieve net-zero carbon without addressing operating theatres. The aim of this study was to prioritize feasible interventions to reduce the environmental impact of operating theatres. Methods This study adopted a four-phase Delphi consensus co-prioritization methodology. In phase 1, a systematic review of published interventions and global consultation of perioperative healthcare professionals were used to longlist interventions. In phase 2, iterative thematic analysis consolidated comparable interventions into a shortlist. In phase 3, the shortlist was co-prioritized based on patient and clinician views on acceptability, feasibility, and safety. In phase 4, ranked lists of interventions were presented by their relevance to high-income countries and low–middle-income countries. Results In phase 1, 43 interventions were identified, which had low uptake in practice according to 3042 professionals globally. In phase 2, a shortlist of 15 intervention domains was generated. In phase 3, interventions were deemed acceptable for more than 90 per cent of patients except for reducing general anaesthesia (84 per cent) and re-sterilization of ‘single-use’ consumables (86 per cent). In phase 4, the top three shortlisted interventions for high-income countries were: introducing recycling; reducing use of anaesthetic gases; and appropriate clinical waste processing. In phase 4, the top three shortlisted interventions for low–middle-income countries were: introducing reusable surgical devices; reducing use of consumables; and reducing the use of general anaesthesia. Conclusion This is a step toward environmentally sustainable operating environments with actionable interventions applicable to both high– and low–middle–income countries

    Ich möchte in meinem Vortrag kurz über unsere Bibliothek und ihre Aufgaben sowie über unsere Katalogisierung im SWB berichten. Dann möchte ich Ihnen etwas über HORIZON als solches und seinen Einsatz bei uns erzählen. Und zum Schluss ein Resümee ziehen und einen Ausblick geben. Bereits in der Projektphase des Landesmuseums wurden seit 1979 im großen Umfang Bücher erworben. Größtenteils geschah dies durch Schenkungen. Als das Museum 1990 eröffnet wurde, war der Bestand damals schon enorm gewachsen – zum großen Teil jedoch unbearbeitet. Unsere Bibliothek umfasst derzeit ca. 90.000 Bände und ist als systematische Freihandbibliothek mit angeschlossenem Magazin und Außendepot konzipiert. Wir verfügen über ca. 280 laufende Zeitschriften, 830 Videokassetten und 230 CD-ROMS/DVDs. Unser jährlicher Zuwachs an Bänden beträgt ca. 2.300 Bände. Hauptsächlich dienen wir unseren Museumsmitarbeitern und ihrer täglichen Arbeit. Insbesondere zur Ausstellungsvorbereitung und –begleitung, aber auch als wissenschaftliche Spezialbibliothek für unsere Wissenschaftler, sind wir fester Bestandteil im Arbeitsablauf des Museumsbetriebes. Unsere Bibliothek ist jedoch auch öffentlich und somit für die Bevölkerung Mannheims und der Umgebung ein zusätzliches Angebot. Unser Schwerpunkt richtet sich nach dem Schwerpunkt des Museums: Technik und Sozialgeschichte, sowie Landes und Unternehmensgeschichte des deutschen Südwestens. Als Service bieten wir Ausleihe und Fernleihe, nehmen Bestellwünsche entgegen und beraten gerne bei der Literaturrecherche. Im Jahr 2002 haben wir 3.368 Benutzer gezählt. Unsere Bibliothek katalogisiert seit 1991 im SWB. Bis zum heutigen Tag haben wir 51.354 Bestandsnachweise gemeldet. Unser Anteil am Eigenkatalogisat betrug im laufenden Jahr: 30%. Wir vergeben an jedem Titel Schlagwörter nach den RSWK. Seit Mitte dieses Jahres nutzten wir den SWB auch im Sinne der Erwerbungskatalogisierung. Unsere Daten werden einmal wöchentlich per Datenabzug von Konstanz in unser HORIZON-System eingespielt. Somit haben wir leider eine Verzögerung von 1 Woche, in der die Bücher zwar nachgewiesen, aber noch nicht entleihbar sind. Der SWB bietet für uns einen klaren Vorteil im Rahmen der Recherche, der Katalogisierung und der Fernleihbearbeitung. Zu guter Letzt haben wir es ihm zu verdanken, dass wir schnell unsere Albestände katalogisieren konnten und somit 1999 unseren Zettelkatalog abbrechen konnten

    Regulation of muscarinic acetylcholine receptor sequestration and function by β-arrestin

    After activation, agonist-occupied G protein-coupled receptors are phosphorylated by G protein-coupled receptor kinases and bind cytosolic β- arrestins, which uncouple the receptors from their cognate G proteins. Recent studies on the β2-adrenergic receptor have demonstrated that β-arrestin also targets the receptors to clathrin-coated pits for subsequent internalization and activation of mitogen-activated protein kinases. We and others have previously shown that muscarinic acetylcholine receptors (mAChRs) of the m1, m3, and m4 subtype require functional dynamin to sequester into HEK-293 tsA201 cells, whereas m2 mAChRs sequester in a dynamin-independent manner. To investigate the role of β-arrestin in mAChR sequestration, we determined the effect of overexpressing β-arrestin-1 and the dominant- negative inhibitor of β-arrestin-mediated receptor sequestration, β- arrestin-1 V53D, on mAChR sequestration and function. Sequestration of ml, m3, and m4 mAChRs was suppressed by 60-75% in cells overexpressing β- arrestin-1 V53D, whereas m2 mAChR sequestration was affected by less than 10%. In addition, overexpression of β-arrestin-1 V53D as well as dynamin K44A significantly suppressed m1 mAChR-mediated activation of mitogen- activated protein kinases. Finally, we investigated whether mAChRs sequester into clathrin-coated vesicles by overexpressing Hub, a dominant-negative clathrin mutant. Although sequestration of m1, m3, and m4 mAChRs was inhibited by 50-70%, m2 mAChR sequestration was suppressed by less than 10%. We conclude that m1, m3, and m4 mAChRs expressed in HEK-293 tsA201 cells sequester into clathrin-coated vesicles in a β-arrestin- and dynamin- dependent manner, whereas sequestration of m2 mAChRs in these cells is largely independent of these proteins.</p
