    Perspectives on ethnic and racial disparities in Alzheimer\u27s disease and related dementias: Update and areas of immediate need

    Alzheimer\u27s disease and related dementias (ADRDs) are a global crisis facing the aging population and society as a whole. With the numbers of people with ADRDs predicted to rise dramatically across the world, the scientific community can no longer neglect the need for research focusing on ADRDs among underrepresented ethnoracial diverse groups. The Alzheimer\u27s Association International Society to Advance Alzheimer\u27s Research and Treatment (ISTAART; alz.org/ISTAART) comprises a number of professional interest areas (PIAs), each focusing on a major scientific area associated with ADRDs. We leverage the expertise of the existing international cadre of ISTAART scientists and experts to synthesize a cross-PIA white paper that provides both a concise “state-of-the-science” report of ethnoracial factors across PIA foci and updated recommendations to address immediate needs to advance ADRD science across ethnoracial populations. © 2018 The Author

    Identification of carbon dioxide in an exoplanet atmosphere

    Carbon dioxide (CO2) is a key chemical species that is found in a wide range of planetary atmospheres. In the context of exoplanets, CO2 is an indicator of the metal enrichment (that is, elements heavier than helium, also called ‘metallicity’), and thus the formation processes of the primary atmospheres of hot gas giants. It is also one of the most promising species to detect in the secondary atmospheres of terrestrial exoplanets. Previous photometric measurements of transiting planets with the Spitzer Space Telescope have given hints of the presence of CO2, but have not yielded definitive detections owing to the lack of unambiguous spectroscopic identification. Here we present the detection of CO2 in the atmosphere of the gas giant exoplanet WASP-39b from transmission spectroscopy observations obtained with JWST as part of the Early Release Science programme. The data used in this study span 3.0–5.5 micrometres in wavelength and show a prominent CO2 absorption feature at 4.3 micrometres (26-sigma significance). The overall spectrum is well matched by one-dimensional, ten-times solar metallicity models that assume radiative–convective–thermochemical equilibrium and have moderate cloud opacity. These models predict that the atmosphere should have water, carbon monoxide and hydrogen sulfide in addition to CO2, but little methane. Furthermore, we also tentatively detect a small absorption feature near 4.0 micrometres that is not reproduced by these models

    Effect of angiotensin-converting enzyme inhibitor and angiotensin receptor blocker initiation on organ support-free days in patients hospitalized with COVID-19

    IMPORTANCE Overactivation of the renin-angiotensin system (RAS) may contribute to poor clinical outcomes in patients with COVID-19. Objective To determine whether angiotensin-converting enzyme (ACE) inhibitor or angiotensin receptor blocker (ARB) initiation improves outcomes in patients hospitalized for COVID-19. DESIGN, SETTING, AND PARTICIPANTS In an ongoing, adaptive platform randomized clinical trial, 721 critically ill and 58 non–critically ill hospitalized adults were randomized to receive an RAS inhibitor or control between March 16, 2021, and February 25, 2022, at 69 sites in 7 countries (final follow-up on June 1, 2022). INTERVENTIONS Patients were randomized to receive open-label initiation of an ACE inhibitor (n = 257), ARB (n = 248), ARB in combination with DMX-200 (a chemokine receptor-2 inhibitor; n = 10), or no RAS inhibitor (control; n = 264) for up to 10 days. MAIN OUTCOMES AND MEASURES The primary outcome was organ support–free days, a composite of hospital survival and days alive without cardiovascular or respiratory organ support through 21 days. The primary analysis was a bayesian cumulative logistic model. Odds ratios (ORs) greater than 1 represent improved outcomes. RESULTS On February 25, 2022, enrollment was discontinued due to safety concerns. Among 679 critically ill patients with available primary outcome data, the median age was 56 years and 239 participants (35.2%) were women. Median (IQR) organ support–free days among critically ill patients was 10 (–1 to 16) in the ACE inhibitor group (n = 231), 8 (–1 to 17) in the ARB group (n = 217), and 12 (0 to 17) in the control group (n = 231) (median adjusted odds ratios of 0.77 [95% bayesian credible interval, 0.58-1.06] for improvement for ACE inhibitor and 0.76 [95% credible interval, 0.56-1.05] for ARB compared with control). The posterior probabilities that ACE inhibitors and ARBs worsened organ support–free days compared with control were 94.9% and 95.4%, respectively. Hospital survival occurred in 166 of 231 critically ill participants (71.9%) in the ACE inhibitor group, 152 of 217 (70.0%) in the ARB group, and 182 of 231 (78.8%) in the control group (posterior probabilities that ACE inhibitor and ARB worsened hospital survival compared with control were 95.3% and 98.1%, respectively). CONCLUSIONS AND RELEVANCE In this trial, among critically ill adults with COVID-19, initiation of an ACE inhibitor or ARB did not improve, and likely worsened, clinical outcomes.     Predictive assessment of models for dynamic functional connectivity

    In neuroimaging, it has become evident that models of dynamic functional connectivity (dFC), which characterize how intrinsic brain organization changes over time, can provide a more detailed representation of brain function than traditional static analyses. Many dFC models in the literature represent functional brain networks as a meta-stable process with a discrete number of states; however, there is a lack of consensus on how to perform model selection and learn the number of states, as well as a lack of understanding of how different modeling assumptions influence the estimated state dynamics. To address these issues, we consider a predictive likelihood approach to model assessment, where models are evaluated based on their predictive performance on held-out test data. Examining several prominent models of dFC (in their probabilistic formulations) we demonstrate our framework on synthetic data, and apply it on two real-world examples: a face recognition EEG experiment and resting-state fMRI. Our results evidence that both EEG and fMRI are better characterized using dynamic modeling approaches than by their static counterparts, but we also demonstrate that one must be cautious when interpreting dFC because parameter settings and modeling assumptions, such as window lengths and emission models, can have a large impact on the estimated states and consequently on the interpretation of the brain dynamics

    <i>In situ</i> powder neutron diffraction study of non-stoichiometric phase formation during the hydrogenation of Li<sub>3</sub>N

    The hydrogenation of Li3N at low chemical potential has been studied in situ by time-of-flight powder neutron diffraction and the formation of a nonstoichiometric Li4_2xNH phase and Li4NH observed. The results are interpreted in terms of a model for the reaction pathway involving the production of Li4NH and Li2NH, which subsequently react together to form Li4_2xNH. Possible mechanisms for the production of Li4NH from the hydrogenation of Li3N are discussed

    Knowledge on STIs / HIV / AIDS, Stigma-Discrimination and sexual behaviors AMONG students of the University of Dschang, in Cameroon

    This study reports the results of a survey conducted among students of the University of Dschang, on STI/HIV/AIDS, the Stigma-Discrimination pair and sexual behaviors. Methods: This was a descriptive cross-sectional study conducted during the months of January and February 2017. We collected data by a face to face questionnaire administered to students of the University of Dschang. The codification, process and analysis have been done using the software EPI-Info 7.3.1.1, with the threshold of significance set at 0.05. Results: A total of 520 individuals participated in this survey, with more than half (62.7%, n = 326/520) aged between 20-30 years, with a male/female sex ratio of 1,031. The vast majority (83%, n = 418/520) of them were in the undergraduate cycle. The main modes of transmission cited included: the combination of items such as blood transfusion / soiled objects / Mother-to-child transmission/ unprotected sex for 36.3% of respondents (n = 186/516) although 21.9 % (n = 112/516) of them admitted not knowing any modes of transmission of STIs / HIV. Also, 74.2% (n = 386/520) of respondents were sexually active, with the estimated age of sex debut being over 18 years (53.9%; 208/386) for half of them. Of the 36.2% (n = 186/514) respondents who reported knowing someone with STI / HIV and AIDS, the first feeling they had with regards to them was pity (86.6%; n = 386/446), followed by fear in 11.7% (n = 52/446) of respondents but yet 40.2% (n = 208/518) admitted they have never heard of discrimination. Conclusion: Stigma and discrimination in the student milieu remains a major barrier to students’ development who, becoming sexually active at a very early age, are more exposed to the risk of contracting STI / HIV. Sensitization actions should be implemented within university campuses and reproductive health courses for young adults, included in the academic curriculum for a significant reduction in the number of new infection
