    Import 11/07/2012Cieľom mojej Diplomovej práce bolo navrhnúť a implementovať odmeňovací systéme pre jednu z pobočiek nemenovanej banky. Práca je rozdelená do viacerých logických celkov, ktoré na seba nadväzujú. Aby som mohol aplikáciu vyhotoviť bol nutné si naštudovať všetky problémové oblasti. Ďalej bolo nutné vykonať analýzu súčasného stavu pobočky a jej bonusového systému. Po analýze bolo možné vykonať návrh nového systému pomocou UML. Tento návrh mi poslúžil k realizácii systému v prostredí Microsoft Access 2003. Aplikácia bola vyvíjaná agilne, tak aby vyhovela meniacim sa požiadavkám zákazníka. Pridaná hodnota aplikácie pre pobočku je hlavne v analýze dát, ktoré tento systém produkuje. Hodnotenie pracovníkov sa zautomatizovalo a prinieslo tak úspory pre administrátora aplikácie.The aim of my thesis was to design and implement a reward system for one of the branches of an unnamed bank. The work is divided into several logical units that are interlinked. So I could make an application was necessary to study the all problem areas. It was necessary to analyze the current state of the branch and its bonus system. After analysis it was possible to make the new system design using UML. This proposal has served me to the realization of the system in Microsoft Access 2003. The application was developed agile, so to comply changing customer requirements. Added value applications for the office is mainly in the data analysis, the system produces.     Cieľom diplomovej práce je návrh objektu administratívnej budovy s bytovou jednotkou, ktorý sa nachádza v obci Mestečko na parcele 557/5 na rovinatom teréne. Práca zahŕňa spracovanie stavebnej časti a návrh vykurovania objektu. Stavebné riešenie bolo navrhnuté s ohľadom na stavebnú tepelnú techniku a energetiku budovy. Budova má tri nadzemné podlažia, pričom sú tu spojené viaceré prevádzky. V objekte je skladová hala spojená s administratívnou budovou kde sa nachádza bytová jednotka s možnosťou príležitostného obývania. Hala je opláštená so sendvičových panelov. Administratívna budova je postavená z pórobetónových tvaroviek, stropy sú navrhnuté ako monolitické železobetónové dosky. Zdrojom tepla pre celý objekt je automatický kotol na peletky. Vykurovanie je navrhnuté pomocou oceľových doskových telies.The main aim of this thesis is preparation of proposal for an office building with dwelling units which is located in the municipality of Mestečko, plot 557/5, on flat grounds. The paper includes a plan of construction works as well as scheme of object’s heating technology. The project has been designed with respect to isolation technique and energy consumption-related attributes. The building has three over ground floors consisting of several connected establishments. In the object, there is a warehouse connected with the office section where a dwelling unit for occasional tenant is located as well. The warehouse is isolated by particular type of panels. The office building is constructed with aerated concrete while the ceilings are designed as monolithic reinforced concrete slabs.     A Pilot Study of Mifepristone in Combat-Related PTSD

    Background. We obtained pilot data to examine the clinical and neuroendocrine effects of short-term mifepristone treatment in male veterans with PTSD. Methods. Eight male veterans with military-related PTSD completed a randomized, double-blind trial of one week of treatment with mifepristone (600 mg/day) or placebo. The primary clinical outcome measures were improvement in PTSD symptoms and dichotomously defined clinical responder status as measured by the CAPS at one-month follow-up. Additional outcome measures included self-reported measures of PTSD symptom severity, CAPS-2 symptom subscale scores, and morning plasma cortisol and ACTH levels. Results. Mifepristone was associated with significant improvements in total CAPS-2 score. At one-month follow-up, all four veterans in the mifepristone group and one of four veterans in the placebo group achieved clinical response; three of four veterans in the mifepristone group and one of four veterans in the mifepristone group remitted. Mifepristone treatment was associated with acute increases in cortisol and ACTH levels and decreases in cytosolic glucocorticoid receptor number in lymphocytes. Conclusions. Further controlled trials of the effects of mifepristone and their durability are indicated in PTSD. If effective, a short-term pharmacological treatment in PTSD could have myriad uses

    Mood instability, mental illness and suicidal ideas : results from a household survey

    Purpose: There is weak and inconsistent evidence that mood instability (MI) is associated with depression, post traumatic stress disorder (PTSD) and suicidality although the basis of this is unclear. Our objectives were first to test whether there is an association between depression and PTSD, and MI and secondly whether MI exerts an independent effect on suicidal thinking over and above that explained by common mental disorders. Methods: We used data from the Adult Psychiatric Morbidity Survey 2007 (N = 7,131). Chi-square tests were used to examine associations between depression and PTSD, and MI, followed by regression modelling to examine associations between MI and depression, and with PTSD. Multiple logistic regression analyses were used to assess the independent effect of MI on suicidal thinking, after adjustment for demographic factors and the effects of common mental disorder diagnoses. Results: There are high rates of MI in depression and PTSD and the presence of MI increases the odds of depression by 10.66 [95 % confidence interval (CI) 7.51–15.13] and PTSD by 8.69 (95 % CI 5.90–12.79), respectively, after adjusting for other factors. Mood instability independently explained suicidal thinking, multiplying the odds by nearly five (odds ratio 4.82; 95 % CI 3.39–6.85), and was individually by some way the most important single factor in explaining suicidal thoughts. Conclusions: MI is strongly associated with depression and PTSD. In people with common mental disorders MI is clinically significant as it acts as an additional factor exacerbating the risk of suicidal thinking. It is important to enquire about MI as part of clinical assessment and treatment studies are required

    Relation between Corticosterone and Fear-related Behavior in Mice Selectively Bred for High or Low Alcohol Preference

    Blunted cortisol responses to stress or trauma have been linked with genetic (familial) risk for both alcoholism and post-traumatic stress-disorder (PTSD). Mouse lines selectively bred for high (HAP) or low (LAP) alcohol preference may be a relevant model of genetic risk for co-morbid alcoholism and PTSD in humans. HAP mice show greater fear-potentiated startle (FPS), a model used to study PTSD, than LAP mice. The relation between corticosterone (CORT) and FPS behavior was explored in four experiments. Naïve male and female HAP2 and LAP2 mice received fear-conditioning or control treatments and CORT levels were measured before and immediately after fear-conditioning or FPS testing. In two other experiments, HAP2 mice received CORT (1.0, 5.0, or 10.0 mg/kg) or a glucocorticoid receptor antagonist (mifepristone; 25.0 and 50.0 mg/kg) 30 min before fear-conditioning. HAP2 mice exposed to fear-conditioning and to control foot shock exposures showed lower CORT after the fear-conditioning and FPS testing sessions than LAP2 mice. A trend toward higher FPS was seen in HAP2 mice pretreated with 10.0 mg/kg CORT and CORT levels were the lowest in this group, suggesting negative feedback inhibition of CORT release. Mifepristone did not alter FPS. Overall, these results are consistent with data in humans and rodents indicating that lower cortisol/CORT levels after stress are associated with PTSD/PTSD-like behavior. These findings in HAP2 and LAP2 mice suggest that a blunted CORT response to stress may be a biological marker for greater susceptibility to develop PTSD in individuals with increased genetic risk for alcoholism

    Recent research on Gulf War illness and other health problems in veterans of the 1991 Gulf War: Effects of toxicant exposures during deployment

    Veterans of Operation Desert Storm/Desert Shield - the 1991 Gulf War (GW) - are a unique population who returned from theater with multiple health complaints and disorders. Studies in the U.S. and elsewhere have consistently concluded that approximately 25-32% of this population suffers from a disorder characterized by symptoms that vary somewhat among individuals and include fatigue, headaches, cognitive dysfunction, musculoskeletal pain, and respiratory, gastrointestinal and dermatologic complaints. Gulf War illness (GWI) is the term used to describe this disorder. In addition, brain cancer occurs at increased rates in subgroups of GW veterans, as do neuropsychological and brain imaging abnormalities. Chemical exposures have become the focus of etiologic GWI research because nervous system symptoms are prominent and many neurotoxicants were present in theater, including organophosphates (OPs), carbamates, and other pesticides; sarin/cyclosarin nerve agents, and pyridostigmine bromide (PB) medications used as prophylaxis against chemical warfare attacks. Psychiatric etiologies have been ruled out. This paper reviews the recent literature on the health of 1991 GW veterans, focusing particularly on the central nervous system and on effects of toxicant exposures. In addition, it emphasizes research published since 2008, following on an exhaustive review that was published in that year that summarizes the prior literature (RACGWI, 2008). We conclude that exposure to pesticides and/or to PB are causally associated with GWI and the neurological dysfunction in GW veterans. Exposure to sarin and cyclosarin and to oil well fire emissions are also associated with neurologically based health effects, though their contribution to development of the disorder known as GWI is less clear. Gene-environment interactions are likely to have contributed to development of GWI in deployed veterans. The health consequences of chemical exposures in the GW and other conflicts have been called "toxic wounds" by veterans. This type of injury requires further study and concentrated treatment research efforts that may also benefit other occupational groups with similar exposure-related illnesses

    Distinguishing PTSD, Complex PTSD, and Borderline Personality Disorder: A latent class analysis

    Background: There has been debate regarding whether Complex Posttraumatic Stress Disorder (Complex PTSD) is distinct from Borderline Personality Disorder (BPD) when the latter is comorbid with PTSD. Objective: To determine whether the patterns of symptoms endorsed by women seeking treatment for childhood abuse form classes that are consistent with diagnostic criteria for PTSD, Complex PTSD, and BPD. Method: A latent class analysis (LCA) was conducted on an archival dataset of 280 women with histories of childhood abuse assessed for enrollment in a clinical trial for PTSD. Results: The LCA revealed four distinct classes of individuals: a Low Symptom class characterized by low endorsements on all symptoms; a PTSD class characterized by elevated symptoms of PTSD but low endorsement of symptoms that define the Complex PTSD and BPD diagnoses; a Complex PTSD class characterized by elevated symptoms of PTSD and self-organization symptoms that defined the Complex PTSD diagnosis but low on the symptoms of BPD; and a BPD class characterized by symptoms of BPD. Four BPD symptoms were found to greatly increase the odds of being in the BPD compared to the Complex PTSD class: frantic efforts to avoid abandonment, unstable sense of self, unstable and intense interpersonal relationships, and impulsiveness. Conclusions: Findings supported the construct validity of Complex PTSD as distinguishable from BPD. Key symptoms that distinguished between the disorders were identified, which may aid in differential diagnosis and treatment planning

    Factors Affecting Grammatical and Lexical Complexity of Long-Term L2 Speakers’ Oral Proficiency

    There remains considerable disagreement about which factors drive second language (L2) ultimate attainment. Age of onset (AO) appears to be a robust factor, lending support to theories of maturational constraints on L2 acquisition. The present study is an investigation of factors that influence grammatical and lexical complexity at the stage of L2 ultimate attainment. Grammatical and lexical complexity were assessed in 102 spontaneous oral interviews. Interviewees' AOs ranged from 7 to 17 years old. Multifactorial analyses yielded consistently significant effects of gender and level of education for grammatical and lexical complexity. Additionally, native language use at work was a significant predictor for lexical complexity; conversely, AO did not emerge as a significant factor. We conclude that grammatical and lexical complexity at the stage of L2 ultimate attainment is the result of a complex interplay of variables that are general to language learning and performance rather than L2 specific

    Model-Based Therapeutic Correction of Hypothalamic-Pituitary-Adrenal Axis Dysfunction

    The hypothalamic-pituitary-adrenal (HPA) axis is a major system maintaining body homeostasis by regulating the neuroendocrine and sympathetic nervous systems as well modulating immune function. Recent work has shown that the complex dynamics of this system accommodate several stable steady states, one of which corresponds to the hypocortisol state observed in patients with chronic fatigue syndrome (CFS). At present these dynamics are not formally considered in the development of treatment strategies. Here we use model-based predictive control (MPC) methodology to estimate robust treatment courses for displacing the HPA axis from an abnormal hypocortisol steady state back to a healthy cortisol level. This approach was applied to a recent model of HPA axis dynamics incorporating glucocorticoid receptor kinetics. A candidate treatment that displays robust properties in the face of significant biological variability and measurement uncertainty requires that cortisol be further suppressed for a short period until adrenocorticotropic hormone levels exceed 30% of baseline. Treatment may then be discontinued, and the HPA axis will naturally progress to a stable attractor defined by normal hormone levels. Suppression of biologically available cortisol may be achieved through the use of binding proteins such as CBG and certain metabolizing enzymes, thus offering possible avenues for deployment in a clinical setting. Treatment strategies can therefore be designed that maximally exploit system dynamics to provide a robust response to treatment and ensure a positive outcome over a wide range of conditions. Perhaps most importantly, a treatment course involving further reduction in cortisol, even transient, is quite counterintuitive and challenges the conventional strategy of supplementing cortisol levels, an approach based on steady-state reasoning
