    Perfil clínico periodontal de homens usuários de esteróides anabolizantes androgênicos

Desta forma, derivados sintéticos deste hormônio (Esteroides Anabolizantes Androgênicos - EAAs) são amplamente usados por atletas e fisiculturistas. Entretanto, o uso inadequado dos EAAs pode gerar efeitos colaterais, entre eles toxicidade hepática, acne, alterações comportamentais e podem alterar os tecidos periodontais. O objetivo deste estudo foi avaliar a condição periodontal de homens usuários de EAAs. Foram recrutados 15 homens, com idades igual ou superior a 18 anos, em academias de Curitiba, PR, Brasil, que reportassem uso de EAAs no momento da entrevista (grupo "caso"). O grupo controle foi composto por outros 15 homens, pareados por idade e hábito (frequentadores de academia) e que nunca fizeram uso de EAAs. Todos os pacientes foram avaliados clinicamente quanto a Índice de Placa (IP), Sangramento Marginal (SM), Profundidade de Sondagem (PS), Nível Clínico de Inserção (NIC) e Sangramento à Sondagem (SS). Considerou-se "periodontite" a presen.a de pelo menos dois sítios com PS maior que 3mm, com perda de inserção e SS no mesmo sítio. A idade dos pacientes variou entre 23 a 40 anos (grupo caso: 30,07±4,4 anos; grupo controle:28,1±4,6 anos), sendo a maioria de etnia branca (grupo caso: 92,5%; grupo controle: 100%) e com conclusão do ensino médio (grupo caso: 60%; grupo controle: 73,3%). O grupo caso apresentou significativamente mais sítios com doen.a periodontal, com PS?4mm e PS?4mm com SS e mais sítios com SS (p0,05). O uso de EAAs pode afetar a saúde periodontal, principalmente quanto aos parâmetros inflamatórios como PS e SM. Palavras-chave: Testosterona, anabolizante, periodonto, inflamação.Abstract: Testosterone is the main male sex hormone and promotes muscle growth among its many functions. In this way, synthetic derivatives of this hormone (Androgenetic Anabolic Steroids - AAS) are widely used by athletes and bodybuilders. However, inadequate use of AAS can generate side effects, including liver toxicity, acne, behavioral changes and may alter periodontal tissues. The objective of this study was to evaluate the periodontal status of the men using AAS. Fifteen men, aged 18 years and over, were recruited from gyms in Curitiba, PR, Brazil, who reported use of AAS at the time of the interview (case group). The control group consisted of 15 other men, matched by age and habit (gymgoers) who had never used AAS. All patients were clinically evaluated for the Plaque Index (PI), Marginal Bleeding (MB), Probing Pocket Depth (PD), Clinical Attachment Loss (CAL) and Bleeding on Probing (BoP). The presence of at least two sites with PD greater than 3mm, with attachment loss and BoP at the same site, was considered "periodontitis". The patients' ages ranged from 23 to 40 years (case group: 30.07 ± 4.4 years, control group: 28.1 ± 4.6 years), most of them white (case group: 92.5%, control group: 100%) and with high school education (case group: 60%, control group: 73.3%). The case group presented significantly more sites with periodontal disease, PD ?4 mm with our without BoP (p 0.05). The use of AAS can affect periodontal health, especially in relation to inflammatory paraments such as PD and MB. Key words: Testosterone, anabolic agents, periodontium, inflammation

    Accuracy of biplane x-ray imaging combined with model-based tracking for measuring in-vivo patellofemoral joint motion

    <p>Abstract</p> <p>Background</p> <p>Accurately measuring <it>in-vivo</it> motion of the knee's patellofemoral (PF) joint is challenging. Conventional measurement techniques have largely been unable to accurately measure three-dimensional, <it>in-vivo</it> motion of the patella during dynamic activities. The purpose of this study was to assess the accuracy of a new model-based technique for measuring PF joint motion.</p> <p>Methods</p> <p>To assess the accuracy of this technique, we implanted tantalum beads into the femur and patella of three cadaveric knee specimens and then recorded dynamic biplane radiographic images while manually flexing and extending the specimen. The position of the femur and patella were measured from the biplane images using both the model-based tracking system and a validated dynamic radiostereometric analysis (RSA) technique. Model-based tracking was compared to dynamic RSA by computing measures of bias, precision, and overall dynamic accuracy of four clinically-relevant kinematic parameters (patellar shift, flexion, tilt, and rotation).</p> <p>Results</p> <p>The model-based tracking technique results were in excellent agreement with the RSA technique. Overall dynamic accuracy indicated errors of less than 0.395 mm for patellar shift, 0.875° for flexion, 0.863° for tilt, and 0.877° for rotation.</p> <p>Conclusion</p> <p>This model-based tracking technique is a non-invasive method for accurately measuring dynamic PF joint motion under <it>in-vivo</it> conditions. The technique is sufficiently accurate in measuring clinically relevant changes in PF joint motion following conservative or surgical treatment.</p

    Subcortical brain volume, regional cortical thickness, and cortical surface area across disorders: findings from the ENIGMA ADHD, ASD, and OCD Working Groups

    Objective Attention-deficit/hyperactivity disorder (ADHD), autism spectrum disorder (ASD), and obsessive-compulsive disorder (OCD) are common neurodevelopmental disorders that frequently co-occur. We aimed to directly compare all three disorders. The ENIGMA consortium is ideally positioned to investigate structural brain alterations across these disorders. Methods Structural T1-weighted whole-brain MRI of controls (n=5,827) and patients with ADHD (n=2,271), ASD (n=1,777), and OCD (n=2,323) from 151 cohorts worldwide were analyzed using standardized processing protocols. We examined subcortical volume, cortical thickness and surface area differences within a mega-analytical framework, pooling measures extracted from each cohort. Analyses were performed separately for children, adolescents, and adults using linear mixed-effects models adjusting for age, sex and site (and ICV for subcortical and surface area measures). Results We found no shared alterations among all three disorders, while shared alterations between any two disorders did not survive multiple comparisons correction. Children with ADHD compared to those with OCD had smaller hippocampal volumes, possibly influenced by IQ. Children and adolescents with ADHD also had smaller ICV than controls and those with OCD or ASD. Adults with ASD showed thicker frontal cortices compared to adult controls and other clinical groups. No OCD-specific alterations across different age-groups and surface area alterations among all disorders in childhood and adulthood were observed. Conclusion Our findings suggest robust but subtle alterations across different age-groups among ADHD, ASD, and OCD. ADHD-specific ICV and hippocampal alterations in children and adolescents, and ASD-specific cortical thickness alterations in the frontal cortex in adults support previous work emphasizing neurodevelopmental alterations in these disorders

    Retrospective evaluation of whole exome and genome mutation calls in 746 cancer samples

    Funder: NCI U24CA211006Abstract: The Cancer Genome Atlas (TCGA) and International Cancer Genome Consortium (ICGC) curated consensus somatic mutation calls using whole exome sequencing (WES) and whole genome sequencing (WGS), respectively. Here, as part of the ICGC/TCGA Pan-Cancer Analysis of Whole Genomes (PCAWG) Consortium, which aggregated whole genome sequencing data from 2,658 cancers across 38 tumour types, we compare WES and WGS side-by-side from 746 TCGA samples, finding that ~80% of mutations overlap in covered exonic regions. We estimate that low variant allele fraction (VAF < 15%) and clonal heterogeneity contribute up to 68% of private WGS mutations and 71% of private WES mutations. We observe that ~30% of private WGS mutations trace to mutations identified by a single variant caller in WES consensus efforts. WGS captures both ~50% more variation in exonic regions and un-observed mutations in loci with variable GC-content. Together, our analysis highlights technological divergences between two reproducible somatic variant detection efforts

Foram recrutados 15 homens, com idades igual ou superior a 18 anos, em academias de Curitiba, PR, Brasil, que reportassem uso de EAAs no momento da entrevista (grupo "caso"). O grupo controle foi composto por outros 15 homens, pareados por idade e hábito (frequentadores de academia) e que nunca fizeram uso de EAAs. Todos os pacientes foram avaliados clinicamente quanto a Índice de Placa (IP), Sangramento Marginal (SM), Profundidade de Sondagem (PS), Nível Clínico de Inserção (NIC) e Sangramento à Sondagem (SS). Considerou-se "periodontite" a presen.a de pelo menos dois sítios com PS maior que 3mm, com perda de inserção e SS no mesmo sítio. A idade dos pacientes variou entre 23 a 40 anos (grupo caso: 30,07±4,4 anos; grupo controle:28,1±4,6 anos), sendo a maioria de etnia branca (grupo caso: 92,5%; grupo controle: 100%) e com conclusão do ensino médio (grupo caso: 60%; grupo controle: 73,3%). O grupo caso apresentou significativamente mais sítios com doen.a periodontal, com PS?4mm e PS?4mm com SS e mais sítios com SS (p0,05). O uso de EAAs pode afetar a saúde periodontal, principalmente quanto aos parâmetros inflamatórios como PS e SM. Palavras-chave: Testosterona, anabolizante, periodonto, inflamação.Abstract: Testosterone is the main male sex hormone and promotes muscle growth among its many functions. In this way, synthetic derivatives of this hormone (Androgenetic Anabolic Steroids - AAS) are widely used by athletes and bodybuilders. However, inadequate use of AAS can generate side effects, including liver toxicity, acne, behavioral changes and may alter periodontal tissues. The objective of this study was to evaluate the periodontal status of the men using AAS. Fifteen men, aged 18 years and over, were recruited from gyms in Curitiba, PR, Brazil, who reported use of AAS at the time of the interview (case group). The control group consisted of 15 other men, matched by age and habit (gymgoers) who had never used AAS. All patients were clinically evaluated for the Plaque Index (PI), Marginal Bleeding (MB), Probing Pocket Depth (PD), Clinical Attachment Loss (CAL) and Bleeding on Probing (BoP). The presence of at least two sites with PD greater than 3mm, with attachment loss and BoP at the same site, was considered "periodontitis". The patients' ages ranged from 23 to 40 years (case group: 30.07 ± 4.4 years, control group: 28.1 ± 4.6 years), most of them white (case group: 92.5%, control group: 100%) and with high school education (case group: 60%, control group: 73.3%). The case group presented significantly more sites with periodontal disease, PD ?4 mm with our without BoP (p 0.05).     Can Sustainable Livestock Systems and Alternative Proteins Address the Climate Crisis? Presentations

    Climate change is severely impacting our food systems, making it increasingly challenging to provide food security and healthy diets for all people. At the same time, food systems contribute 25–30% of all greenhouse gas emissions. The livestock sector alone contributes about half of these GHGs, and has a large land and water footprint. The livestock sector is also the fastest growing agricultural sector in Africa and other parts of the Global South, where access to healthy, affordable milk, meat, and egg remains low. Clearly food system adaptation, including in the livestock sector, must deliver both mitigation and healthy diets. The CGIAR’s 2023 Achieving Agriculture Breakthrough report identifies seven key technological areas and approaches for driving real change in the agriculture sector. Three of these are related to livestock: reducing methane emissions from livestock, livestock breeding for climate resilience, and alternative proteins to replace traditional animal-source proteins. The third seminar of the CGIAR Policy Seminar Series on Strengthening Food Systems will delve into findings and recommendations around sustainable livestock technologies, healthy diets, methane emissions, and alternative proteins, including promising initiatives and technologies that can be pursued to address climate change and healthy diets

    SARS-CoV-2 in saliva, viremia and seroprevalence for COVID-19 surveillance at a single hematopoietic stem cell transplantation center: a prospective cohort study

    This prospective cohort study aims to analyze the surveillance of COVID-19 at a single hematopoietic stem cell transplantation (HSCT) center in Brazil, in 29 patients undergoing allogeneic HSCT and 57 healthcare workers (nurses and dentists), through viral shedding of SARS-CoV-2 in saliva and plasma and seroprevalence of anti-SARS-CoV-2 IgG. In addition, we report two cases with prolonged persistent detection of SARS-CoV-2 without seroconversion. The sample collection was performed seven times for patients and five times for healthcare workers. Only two patients tested positive for SARS-CoV-2 in their saliva and plasma samples (6.9%) without seroconversion. All healthcare workers were asymptomatic and none tested positive. Two patients (6.9%) and four nurses (8%) had positive serology. No dentists had positive viral detection or positive serology. Our results reflect a low prevalence of positive RT-PCR and seroprevalence of SARS-CoV-2 in patients and healthcare workers at a single HSCT center. Results have also corroborated how the rigorous protocols adopted in transplant centers were even more strengthened in this pandemic scenario

    Residual tricuspid regurgitation after tricuspid transcatheter edge-to-edge repair: Insights into the EuroTR registry.

    AIMS Data on the prognostic impact of residual tricuspid regurgitation (TR) after tricuspid transcatheter edge-to-edge repair (T-TEER) are scarce. The aim of this analysis was to evaluate 2-year survival and symptomatic outcomes of patients in relation to residual TR after T-TEER. METHODS AND RESULTS Using the large European Registry of Transcatheter Repair for Tricuspid Regurgitation (EuroTR registry) we investigated the impact of residual TR on 2-year all-cause mortality and New York Heart Association (NYHA) functional class at follow-up. The study further identified predictors for residual TR ≥3+ using a logistic regression model. The study included a total of 1286 T-TEER patients (mean age 78.0 ± 8.9 years, 53.6% female). TR was successfully reduced to ≤1+ in 42.4%, 2+ in 40.0% and 3+ in 14.9% of patients at discharge, while 2.8% remained with TR ≥4+ after the procedure. Residual TR ≥3+ was an independent multivariable predictor of 2-year all-cause mortality (hazard ratio 2.06, 95% confidence interval 1.30-3.26, p = 0.002). The prevalence of residual TR ≥3+ was four times higher in patients with higher baseline TR (vena contracta >11.1 mm) and more severe tricuspid valve tenting (tenting area >1.92 cm2). Of note, no survival difference was observed in patients with residual TR ≤1+ versus 2+ (76.2% vs. 73.1%, p = 0.461). The rate of NYHA functional class ≥III at follow-up was significantly higher in patients with residual TR ≥3+ (52.4% vs. 40.5%, p < 0.001). Of note, the degree of TR reduction significantly correlated with the extent of symptomatic improvement (p = 0.012). CONCLUSIONS T-TEER effectively reduced TR severity in the majority of patients. While residual TR ≥3+ was associated with worse outcomes, no differences were observed for residual TR 1+ versus 2+. Symptomatic improvement correlated with the degree of TR reduction
