The impact of the COVID-19 pandemic on health-related quality of life in head and neck cancer survivors:An observational cohort studs
Objective: To investigate the impact of the COVID-19 pandemic on physical, psychological, and social aspects of health-related quality of life (HRQOL) among head and neck cancer (HNC) survivors. Materials and methods: Prospectively collected data from the NETherlands QUality of life and BIomedical Cohort study in HNC were used. All patients were diagnosed and treated before the COVID-19 pandemic. Patient reported outcome measures (PROMs) collected 24 and 36 months after treatment (M24 and M36) were compared between survivors who completed both assessments before the COVID-19 pandemic and those who completed M24 before but M36 during the pandemic. Personal, clinical, physical, psychological, social, and lifestyle characteristics of the survivors assessed at baseline or M24 were investigated as potential effect modifiers. Results: In total, 318 HNC survivors were included, of which 199 completed both M24 and M36 before the COVID-19 pandemic and 119 completed M24 before but M36 during the pandemic. Changes in HRQOL between 24 and 36 months follow-up did not differ between the two groups for any of the PROMs. Nevertheless, in some subgroups of HNC survivors the COVID-19 pandemic negatively affected the course of HRQOL for several PROMs while it positively affected the course of HRQOL for other PROMs. Conclusions: The COVID-19 pandemic did not affect HRQOL in HNC survivors in general, but some subgroups were affected in a positive and others in a negative way.</p

Identifying client characteristics to predict homecare use more accurately:a Delphi-study involving nurses and homecare purchasing specialists
BackgroundCase-mix based prospective payment of homecare is being implemented in several countries to work towards more efficient and client-centred homecare. However, existing models can only explain a limited part of variance in homecare use, due to their reliance on health- and function-related client data. It is unclear which predictors could improve predictive power of existing case-mix models. The aim of this study was therefore to identify relevant predictors of homecare use by utilizing the expertise of district nurses and health insurers.MethodsWe conducted a two-round Delphi-study according to the RAND/UCLA Appropriateness Method. In the first round, participants assessed the relevance of eleven client characteristics that are commonly included in existing case-mix models for predicting homecare use, using a 9-Point Likert scale. Furthermore, participants were also allowed to suggest missing characteristics that they considered relevant. These items were grouped and a selection of the most relevant items was made. In the second round, after an expert panel meeting, participants re-assessed relevance of pre-existing characteristics that were assessed uncertain and of eleven suggested client characteristics. In both rounds, median and inter-quartile ranges were calculated to determine relevance.ResultsTwenty-two participants (16 district nurses and 6 insurers) suggested 53 unique client characteristics (grouped from 142 characteristics initially). In the second round, relevance of the client characteristics was assessed by 12 nurses and 5 health insurers. Of a total of 22 characteristics, 10 client characteristics were assessed as being relevant and 12 as uncertain. None was found irrelevant for predicting homecare use. Most of the client characteristics from the category ‘Daily functioning’ were assessed as uncertain. Client characteristics in other categories – i.e. ‘Physical health status’, ‘Mental health status and behaviour’, ‘Health literacy’, ‘Social environment and network’, and ‘Other’ – were more frequently considered relevant.ConclusionAccording to district nurses and health insurers, homecare use could be predicted better by including other more holistic predictors in case-mix classification, such as on mental functioning and social network. The challenge remains, however, to operationalize the new characteristics and keep stakeholders on board when developing and implementing case-mix classification for homecare prospective payment
HD 174884: a strongly eccentric, short-period early-type binary system discovered by CoRoT
Accurate photometric CoRoT space observations of a secondary seismological
target, HD 174884, led to the discovery that this star is an astrophysically
important double-lined eclipsing spectroscopic binary in an eccentric orbit (e
of about 0.3), unusual for its short (3.65705d) orbital period. The high
eccentricity, coupled with the orientation of the binary orbit in space,
explains the very unusual observed light curve with strongly unequal primary
and secondary eclipses having the depth ratio of 1-to-100 in the CoRoT 'seismo'
passband. Without the high accuracy of the CoRoT photometry, the secondary
eclipse, 1.5 mmag deep, would have gone unnoticed. A spectroscopic follow-up
program provided 45 high dispersion spectra. The analysis of the CoRoT light
curve was performed with an adapted version of PHOEBE that supports CoRoT
passbands. The final solution was obtained by simultaneous fitting of the light
and the radial velocity curves. Individual star spectra were derived by
spectrum disentangling. The uncertainties of the fit were derived by bootstrap
resampling and the solution uniqueness was tested by heuristic scanning. The
results provide a consistent picture of the system composed of two late B
stars. The Fourier analysis of the light curve fit residuals yields two
components, with orbital frequency multiples and an amplitude of about 0.1
mmag, which are tentatively interpreted as tidally induced pulsations. An
extensive comparison with theoretical models is carried out by means of the
Levenberg-Marquardt minimization technique and the discrepancy between models
and the derived parameters is discussed. The best fitting models yield a young
system age of 125 million years which is consistent with the eccentric orbit

A reduced-order strategy for 4D-Var data assimilation
This paper presents a reduced-order approach for four-dimensional variational
data assimilation, based on a prior EO F analysis of a model trajectory. This
method implies two main advantages: a natural model-based definition of a mul
tivariate background error covariance matrix , and an important
decrease of the computational burden o f the method, due to the drastic
reduction of the dimension of the control space. % An illustration of the
feasibility and the effectiveness of this method is given in the academic
framework of twin experiments for a model of the equatorial Pacific ocean. It
is shown that the multivariate aspect of brings additional
information which substantially improves the identification procedure. Moreover
the computational cost can be decreased by one order of magnitude with regard
to the full-space 4D-Var method
beta-decay study of Cu-77
A beta-decay study of Cu-77 has been performed at the ISOLDE mass separator
with the aim to deduce its beta-decay properties and to obtain spectroscopic
information on Zn-77. Neutron-rich copper isotopes were produced by means of
proton- or neutron-induced fission reactions on U-238. After the production,
Cu-77 was selectively laser ionized, mass separated and sent to different
detection systems where beta-gamma and beta-n coincidence data were collected.
We report on the deduced half-live, decay scheme, and possible spin assignment
of 77Cu
Universal corrections to scaling for block entanglement in spin-1/2 XX chains
We consider the R\'enyi entropies in the one dimensional spin-1/2
Heisenberg XX chain in a magnetic field. The case n=1 corresponds to the von
Neumann ``entanglement'' entropy. Using a combination of methods based on the
generalized Fisher-Hartwig conjecture and a recurrence relation connected to
the Painlev\'e VI differential equation we obtain the asymptotic behaviour,
accurate to order , of the R\'enyi entropies
for large block lengths . For n=1,2,3,10 this constitutes the 3,6,10,48
leading terms respectively. The o(1) contributions are found to exhibit a rich
structure of oscillatory behaviour, which we analyze in some detail both for

Magnetic Resonance Imaging-targeted Prostate Biopsy Compared with Systematic Prostate Biopsy in Biopsy-naïve Patients with Suspected Prostate Cancer
BACKGROUND: It remains uncertain whether transrectal ultrasound (TRUS)-guided systematic biopsies can be omitted and rely solely on multiparametric magnetic resonance imaging–targeted biopsies (MRI-TBx) in biopsy-naïve men suspected of prostate cancer (PCa). OBJECTIVE: To compare PCa detection in biopsy-naïve men between systematic biopsy and MRI-TBx. DESIGN, SETTING, AND PARTICIPANTS: A prospective cohort study was conducted in a Dutch teaching hospital. Consecutive patients with suspected PCa, no history of biopsy, and no clinical suspicion of metastasis underwent both TRUS-guided systematic biopsies and MRI-TBx by multiparametric magnetic resonance imaging (mpMRI)-ultrasound fusion, including sham biopsies in case of negative mpMRI. OUTCOME MEASUREMENTS AND STATISTICAL ANALYSIS: Clinically significant PCa (csPCa), defined as group ≥2 on the International Society of Urological Pathology grading, was detected. RESULTS AND LIMITATIONS: The overall prevalence of csPCa, irrespective of biopsy technique, was 37.4% (132/353) in our population. MRI-TBx were performed in 263/353 (74.5%) patients with suspicious mpMRI (Prostate Imaging Reporting and Data System [PI-RADS] ≥3). The detection rates for csPCa were 39.5% for MRI-TBx and 42.9% for systematic biopsies. The added values, defined as the additional percentages of patients with csPCa detected by adding one biopsy technique, were 8.7% for the systematic biopsies and 5.3% for MRI-TBx. In patients with nonsuspicious mpMRI, five cases (6%) of csPCa were found by systematic biopsies. CONCLUSIONS: This study in biopsy-naïve patients suspected for PCa showed that systematic biopsies have added value to MRI-TBx alone in patients with mpMRI PI-RADS >2. PATIENT SUMMARY: We studied magnetic resonance imaging (MRI)-guided prostate biopsy for diagnosing prostate cancer and compared it with the standard method of prostate biopsy. Standard systematic biopsies cannot be omitted in patients with suspicious MRI, as they add to the detection of significant prostate cancer
A replication study confirms the association of TNFSF4 (OX40L) polymorphisms with systemic sclerosis in a large European cohort
<p><b>Objectives</b> The aim of this study was to confirm the influence of TNFSF4 polymorphisms on systemic sclerosis (SSc) susceptibility and phenotypic features.</p>
<p><b>Methods</b> A total of 8 European populations of Caucasian ancestry were included, comprising 3014 patients with SSc and 3125 healthy controls. Four genetic variants of TNFSF4 gene promoter (rs1234314, rs844644, rs844648 and rs12039904) were selected as genetic markers.</p>
<p><b>Results</b> A pooled analysis revealed the association of rs1234314 and rs12039904 polymorphisms with SSc (OR 1.15, 95% CI 1.02 to 1.31; OR 1.18, 95% CI 1.08 to 1.29, respectively). Significant association of the four tested variants with patients with limited cutaneous SSc (lcSSc) was revealed (rs1234314 OR 1.22, 95% CI 1.07 to 1.38; rs844644 OR 0.91, 95% CI 0.83 to 0.99; rs844648 OR 1.10, 95% CI 1.01 to 1.20 and rs12039904 OR 1.20, 95% CI 1.09 to 1.33). Association of rs1234314, rs844648 and rs12039904 minor alleles with patients positive for anti-centromere antibodies (ACA) remained significant (OR 1.23, 95% CI 1.10 to 1.37; OR 1.12, 95% CI 1.01 to 1.25; OR 1.22, 95% CI 1.07 to 1.38, respectively). Haplotype analysis confirmed a protective haplotype associated with SSc, lcSSc and ACA positive subgroups (OR 0.88, 95% CI 0.82 to 0.96; OR 0.88, 95% CI 0.80 to 0.96; OR 0.86, 95% CI 0.77 to 0.97, respectively) and revealed a new risk haplotype associated with the same groups of patients (OR 1.14, 95% CI 1.03 to 1.26; OR 1.20, 95% CI 1.08 to 1.35; OR 1.23, 95% CI 1.07 to 1.42, respectively).</p>
<p><b>Conclusions</b> The data confirm the influence of TNFSF4 polymorphisms in SSc genetic susceptibility, especially in subsets of patients positive for lcSSc and ACA.</p>
