Nas Ășltimas dĂ©cadas, os avanços mĂ©dico-tecnolĂłgicos destacaram a imprescindibilidade do diagnĂłstico precoce, bem como o rĂĄpido inĂ­cio do tratamento, para a obtenção dos melhores resultados no combate a essa doença. A PolĂ­tica de Atenção em Oncologia envolve as trĂȘs esferas de Governo, que atuam em redes estaduais ou regionais, alĂ©m de açÔes que vĂŁo desde promoção, prevenção, diagnĂłstico, tratamento, reabilitação atĂ© cuidados paliativos, e fazem com que a regulação em oncologia seja fator determinante nesse processo. Este relato de experiĂȘncia teve por objetivo analisar as funcionalidades das plataformas digitais SIGA e CROSS na regulação em oncologia no municĂ­pio de SĂŁo Paulo e apontar oportunidades de melhoria. SIGA Ă© um sistema informatizado municipal e CROSS Ă© estadual. Ambos tĂȘm como caracterĂ­stica a integração das informaçÔes, agendamento de consultas, exames e procedimentos, e possibilitam otimizar os recursos de alta complexidade existentes. Como pontos de melhoria, destacamos a incorporação de um fluxograma de acompanhamento passo a passo do paciente oncolĂłgico, a necessidade de uma maior interface entre as duas plataformas e a integração entre central telefĂŽnica e Sistema SIGA

    Azitromicina no tratamento da leishmaniose mucosa

    This report describes three elderly patients with mucosal form of American tegumentary leishmaniasis associated with chronic cardiopathy. Due to the known toxicity of classical drugs with activity against Leishmania sp., the patients received three oral courses of azithromycin therapy in single 500 mg daily dose during ten days, every other month. All lesions healed after the third series. One of the patients relapsed and a new series of azithromycin was prescribed. Azithromycin may be an alternative drug for the treatment of leishmaniasis in special situations due to its optimal mucosal and intraphagocyte concentration, single daily posology, high tolerance and oral administration. The mechanism of this drug on Leishmania sp. is unknown at present.O presente relato descreve trĂȘs pacientes idosos com leishmaniose mucosa de longa evolução, os quais eram portadores de cardiopatia crĂŽnica. Pela alta freqĂŒĂȘncia de efeitos secundĂĄrios e tĂłxicos dos medicamentos utilizados no tratamento clĂĄssico, esses pacientes receberam azitromicina. Este medicamento foi administrado pela via oral, em dose Ășnica diĂĄria de 500 mg, durante dez dias, em trĂȘs sĂ©ries com intervalo de um mĂȘs. Em todos, houve cicatrização das lesĂ”es depois da terceira sĂ©rie. Um dos pacientes apresentou recidiva apĂłs seis meses e uma nova sĂ©rie de azitromicina fez regredir novamente o quadro. Azitromicina pode ser uma alternativa para o tratamento das leishmanioses, principalmente pela concentração adequada em mucosas e nos fagĂłcitos, posologia Ășnica diĂĄria, boa tolerĂąncia e administração oral

    Use of the Google Docs Computational Tool as a Strategy to Reduce Waiting Time for Elective Surgeries in a Municipality of the State of SĂŁo Paulo

    No Ăąmbito do Sistema Único de SaĂșde, o acesso da população a procedimentos cirĂșrgicos eletivos de mĂ©dia complexidade, de modo geral, Ă© burocrĂĄtico e demorado. Neste sentido, o presente relato tĂ©cnico teve como objetivo descrever a experiĂȘncia do Complexo Regulador da SaĂșde de um municĂ­pio do Estado de SĂŁo Paulo utilizando a ferramenta computacional Google Docs como estratĂ©gia para redução do tempo de espera nas filas de cirurgias eletivas. Trata-se de um estudo descritivo quantitativo, realizado em um hospital filantrĂłpico do municĂ­pio em estudo. A coleta de informaçÔes ocorreu pela exportação de dados fornecidos pelo Sistema Nacional de Regulação – SISREG, de uma amostra por conveniĂȘncia. Mensurou-se o tempo mĂ©dio de espera entre a primeira consulta com o mĂ©dico cirurgiĂŁo e a realização de cirurgias eletivas antes e apĂłs a implantação do projeto de utilização da ferramenta computacional Google Docs. A partir da anĂĄlise dos resultados, foi possĂ­vel verificar a redução do tempo mĂ©dio de espera por cirurgias eletivas em 30,4% e ganho produtivo no processo de acesso a cirurgias eletivas no hospital do estudo. Integra-se Ă  conclusĂŁo desse trabalho, a sugestĂŁo da implantação deste projeto nos demais hospitais do municĂ­pio.In general, within the scope of the Unified Health System, the population's access to elective surgical procedures of medium complexity, is bureaucratic and time-consuming. In this context, this technical report has aimed to describe the experience of the Health Regulatory Complex of a municipality of the State of SĂŁo Paulo using the Google Docs computational tool as a strategy to reduce the waiting time in the queues of elective surgeries. This is a quantitative descriptive study, carried out in a philanthropic hospital of the municipality under study. The data collection was done by exportation from the Brazilian System of Regulation - SISREG, using a convenience sample. The average waiting time between the first consultation with the medical surgeon and the carry out of elective surgeries before and after the implementation of the project of using Google Docs computational tool was measured. From the analysis of the results, it was possible to verify a reduction of the average waiting time for elective surgeries in 30.4% and productive gain in the process of access to elective surgeries in the study hospital. According to the positive results observed in this study, we suggest  the extension of this project to other hospitals of the municipality

    Liver stiffness is not associated with short- and long-term plasma HIV RNA replication in immunocompetent patients with HIV infection and with HIV/HCV coinfection

    Background:Human immunodeficiency virus (HIV) may be directly responsible for liver damage but there are contrasting data regarding the influence of detectable plasma viremia. We analyzed the influence of plasma HIV RNA (pHIV) detectability and of other clinical and viro-immunological variables on liver stiffness (LS) measurement in adult immunocompetent HIV-monoinfected patients and in patients coinfected with hepatitis C virus (HCV). Methods: Logistic regression analysis was performed using the value of LS>7.1 kPa as the dependent variable. A linear regression model was applied using LS measurement after log 10 transformation (lkpa) as the dependent variable and we analyzed the predicted values versus the observed lkpa values; pHIV was classified as detectable or undetectable in the 12- and 36-month study periods before LS measurement. Results: We studied 251 patients (178 with HIV monoinfection), most of whom were on antiviral treatment; 36-month study time was available for 154 subjects. The mean CD4+ cell count was 634 cells/mm3 in HIV-monoinfected patients and 606 cells/mm3 in coinfected patients. No difference in LS was found between patients with detectable or undetectable pHIV in either the 12- or the 36-month study period before transient elastography. The mean LS was higher in HIV/HCV coinfected patients (P<0.0001) than in the HIV-monoinfected subjects; lkpa was positively correlated with HCV coinfection (P<0.0001) and aspartate aminotransferase levels (P<0.0001). Detectable pHIV failed to reach significance. Eight HIV-monoinfected patients had a predicted LS measurement lower than the observed one, while eight patients had the opposite result. Conclusion: LS was not correlated with ongoing HIV replication during the 12- and 36-month study periods in immunocompetent HIV-monoinfected and HIV/HCV-coinfected patients

    Molecular analysis of post-harvest withering in grape by AFLP transcriptional profiling

    Post-harvest withering of grape berries is used in the production of dessert and fortified wines to alter must quality characteristics and increase the concentration of simple sugars. The molecular processes that occur during withering are poorly understood, so a detailed transcriptomic analysis of post-harvest grape berries was carried out by AFLP-transcriptional profiling analysis. This will help to elucidate the molecular mechanisms of berry withering and will provide an opportunity to select markers that can be used to follow the drying process and evaluate different drying techniques. AFLP-TP identified 699 withering-specific genes, 167 and 86 of which were unique to off-plant and on-plant withering, respectively. Although similar molecular events were revealed in both withering processes, it was apparent that off-plant withering induced a stronger dehydration stress response resulting in the high level expression of genes involved in stress protection mechanisms, such as dehydrin and osmolite accumulation. Genes involved in hexose metabolism and transport, cell wall composition, and secondary metabolism (particularly the phenolic and terpene compound pathways) were similarly regulated in both processes. This work provides the first comprehensive analysis of the molecular events underpinning post-harvest withering and could help to define markers for different withering processes

    The WSES/SICG/ACOI/SICUT/AcEMC/SIFIPAC guidelines for diagnosis and treatment of acute left colonic diverticulitis in the elderly

    Acute left colonic diverticulitis (ALCD) in the elderly presents with unique epidemiological features when compared with younger patients. The clinical presentation is more nuanced in the elderly population, having higher in-hospital and postoperative mortality. Furthermore, geriatric comorbidities are a risk factor for complicated diverticulitis. Finally, elderly patients have a lower risk of recurrent episodes and, in case of recurrence, a lower probability of requiring urgent surgery than younger patients. The aim of the present work is to study age-related factors that may support a unique approach to the diagnosis and treatment of this problem in the elderly when compared with the WSES guidelines for the management of acute left-sided colonic diverticulitis. During the 1 degrees Pisa Workshop of Acute Care & Trauma Surgery held in Pisa (Italy) in September 2019, with the collaboration of the World Society of Emergency Surgery (WSES), the Italian Society of Geriatric Surgery (SICG), the Italian Hospital Surgeons Association (ACOI), the Italian Emergency Surgery and Trauma Association (SICUT), the Academy of Emergency Medicine and Care (AcEMC) and the Italian Society of Surgical Pathophysiology (SIFIPAC), three panel members presented a number of statements developed for each of the four themes regarding the diagnosis and management of ALCD in older patients, formulated according to the GRADE approach, at a Consensus Conference where a panel of experts participated. The statements were subsequently debated, revised, and finally approved by the Consensus Conference attendees. The current paper is a summary report of the definitive guidelines statements on each of the following topics: diagnosis, management, surgical technique and antibiotic therapy.Peer reviewe

    A regional audit system for stillbirth: A way to better understand the phenomenon

    Background: Implementation of high-quality national audits for perinatal mortality are needed to improve the registration of all perinatal deaths and the identification of the causes of death. This study aims to evaluate the implementation of a Regional Audit System for Stillbirth in Emilia-Romagna Region, Italy. Methods: For each stillbirth ( 65 22 weeks of gestation, 65 500 g) occurred between January 1, 2014 to December 1, 2016 (n = 332), the same diagnostic workup was performed and a clinical record with data about mother and stillborn was completed. Every case was discussed in a multidisciplinary local audit to assess both the cause of death (ReCoDe classification) and the quality of care. Data were reviewed by the Regional Audit Group. Stillbirth rates, causes of death and the quality of care were established for each case. Results: Total stillbirth rate was 3.09 per 1000 births (332/107,528). Late stillbirth rate was 2.3 per 1000 (251/107,087). Sixteen stillbirths were not registered by the Regional Birth Register. The most prevalent cause of death was placental disorder (33.3%), followed by fetal (17.6%), cord (14.2%) and maternal disorders (7.6%). Unexplained cases were 14%. Compared to local audits, the regional group attributed different causes of death in 17% of cases. At multivariate analysis, infections were associated with early stillbirths (OR 3.38, CI95% 1.62-7.03) and intrapartum cases (OR 6.64, CI95% 2.61-17.02). Placental disorders were related to growth restriction (OR 1.89, CI95% 1.06-3.36) and were more frequent before term (OR 1.86, CI95% 1.11-3.15). Stillbirths judged possibly/probably preventable with a different management (10.9%) occurred more frequently in non-Italian women and were mainly related to maternal disorders (OR 6.64, CI95% 2.61-17.02). Conclusions: Regional Audit System for Stillbirth improves the registration of stillbirth and allows to define the causes of death. Moreover, sub-optimal care was recognized, allowing to identify populations which could benefit from preventive measures

    The rapid spread of SARS-COV-2 Omicron variant in Italy reflected early through wastewater surveillance

    The SARS-CoV-2 Omicron variant emerged in South Africa in November 2021, and has later been identified worldwide, raising serious concerns. A real-time RT-PCR assay was designed for the rapid screening of the Omicron variant, targeting characteristic mutations of the spike gene. The assay was used to test 737 sewage samples collected throughout Italy (19/21 Regions) between 11 November and 25 December 2021, with the aim of assessing the spread of the Omicron variant in the country. Positive samples were also tested with a real-time RT-PCR developed by the European Commission, Joint Research Centre (JRC), and through nested RT-PCR followed by Sanger sequencing. Overall, 115 samples tested positive for Omicron SARS-CoV-2 variant. The first occurrence was detected on 7 December, in Veneto, North Italy. Later on, the variant spread extremely fast in three weeks, with prevalence of positive wastewater samples rising from 1.0% (1/104 samples) in the week 5-11 December, to 17.5% (25/143 samples) in the week 12-18, to 65.9% (89/135 samples) in the week 19-25, in line with the increase in cases of infection with the Omicron variant observed during December in Italy. Similarly, the number of Regions/Autonomous Provinces in which the variant was detected increased from one in the first week, to 11 in the second, and to 17 in the last one. The presence of the Omicron variant was confirmed by the JRC real-time RT-PCR in 79.1% (91/115) of the positive samples, and by Sanger sequencing in 66% (64/97) of PCR amplicons. In conclusion, we designed an RT-qPCR assay capable to detect the Omicron variant, which can be successfully used for the purpose of wastewater-based epidemiology. We also described the history of the introduction and diffusion of the Omicron variant in the Italian population and territory, confirming the effectiveness of sewage monitoring as a powerful surveillance tool

    Management of acute diverticulitis with pericolic free gas (ADIFAS). an international multicenter observational study

    Background: There are no specific recommendations regarding the optimal management of this group of patients. The World Society of Emergency Surgery suggested a nonoperative strategy with antibiotic therapy, but this was a weak recommendation. This study aims to identify the optimal management of patients with acute diverticulitis (AD) presenting with pericolic free air with or without pericolic fluid. Methods: A multicenter, prospective, international study of patients diagnosed with AD and pericolic-free air with or without pericolic free fluid at a computed tomography (CT) scan between May 2020 and June 2021 was included. Patients were excluded if they had intra-abdominal distant free air, an abscess, generalized peritonitis, or less than a 1-year follow-up. The primary outcome was the rate of failure of nonoperative management within the index admission. Secondary outcomes included the rate of failure of nonoperative management within the first year and risk factors for failure. Results: A total of 810 patients were recruited across 69 European and South American centers; 744 patients (92%) were treated nonoperatively, and 66 (8%) underwent immediate surgery. Baseline characteristics were similar between groups. Hinchey II-IV on diagnostic imaging was the only independent risk factor for surgical intervention during index admission (odds ratios: 12.5, 95% CI: 2.4-64, P =0.003). Among patients treated nonoperatively, at index admission, 697 (94%) patients were discharged without any complications, 35 (4.7%) required emergency surgery, and 12 (1.6%) percutaneous drainage. Free pericolic fluid on CT scan was associated with a higher risk of failure of nonoperative management (odds ratios: 4.9, 95% CI: 1.2-19.9, P =0.023), with 88% of success compared to 96% without free fluid ( P &lt;0.001). The rate of treatment failure with nonoperative management during the first year of follow-up was 16.5%. Conclusion: Patients with AD presenting with pericolic free gas can be successfully managed nonoperatively in the vast majority of cases. Patients with both free pericolic gas and free pericolic fluid on a CT scan are at a higher risk of failing nonoperative management and require closer observation

    Infected pancreatic necrosis: outcomes and clinical predictors of mortality. A post hoc analysis of the MANCTRA-1 international study

    : The identification of high-risk patients in the early stages of infected pancreatic necrosis (IPN) is critical, because it could help the clinicians to adopt more effective management strategies. We conducted a post hoc analysis of the MANCTRA-1 international study to assess the association between clinical risk factors and mortality among adult patients with IPN. Univariable and multivariable logistic regression models were used to identify prognostic factors of mortality. We identified 247 consecutive patients with IPN hospitalised between January 2019 and December 2020. History of uncontrolled arterial hypertension (p = 0.032; 95% CI 1.135-15.882; aOR 4.245), qSOFA (p = 0.005; 95% CI 1.359-5.879; aOR 2.828), renal failure (p = 0.022; 95% CI 1.138-5.442; aOR 2.489), and haemodynamic failure (p = 0.018; 95% CI 1.184-5.978; aOR 2.661), were identified as independent predictors of mortality in IPN patients. Cholangitis (p = 0.003; 95% CI 1.598-9.930; aOR 3.983), abdominal compartment syndrome (p = 0.032; 95% CI 1.090-6.967; aOR 2.735), and gastrointestinal/intra-abdominal bleeding (p = 0.009; 95% CI 1.286-5.712; aOR 2.710) were independently associated with the risk of mortality. Upfront open surgical necrosectomy was strongly associated with the risk of mortality (p &lt; 0.001; 95% CI 1.912-7.442; aOR 3.772), whereas endoscopic drainage of pancreatic necrosis (p = 0.018; 95% CI 0.138-0.834; aOR 0.339) and enteral nutrition (p = 0.003; 95% CI 0.143-0.716; aOR 0.320) were found as protective factors. Organ failure, acute cholangitis, and upfront open surgical necrosectomy were the most significant predictors of mortality. Our study confirmed that, even in a subgroup of particularly ill patients such as those with IPN, upfront open surgery should be avoided as much as possible.
