    Use of Item Response Analysis to Investigate Measurement Properties and Clinical Validity of Data for the Dynamic Gait Index

    Background and Purpose. The Dynamic Gait Index (DGI) is a standardized clinical assessment that aids in evaluating a subject’s ability to modify gait in response to changing demands. The purpose of this study was to use Rasch measurement theory to examine whether the DGI rating scale meets suggested psychometric guidelines, whether the hierarchical order of DGI tasks is consistent with a clinically logical testing procedure, and whether the DGI represents a unidimensional construct. Subjects. Subjects were 84 community-dwelling male veterans (age range=64–88 years; mean±SD=75±6.47 years). Methods. Data were retrieved retrospectively from the participants’ clinical records. The Rasch measurement model with the WINSTEPS program was used in this study because it offers distinct advantages over traditional psychometric approaches. Results. Overall, the DGI showed sound item psychometric properties. Each of the original 4 rating scale categories appeared to distinctly identify subjects at different ability levels. The analysis revealed a clear item difficulty hierarchical order that is generally consistent with clinical expectations. In addition, fit statistics and principal components analysis indicated that the 8 items of the DGI appear to represent a single construct. Discussion and Conclusion. The results suggest that the rating scale of the DGI is used appropriately for community-dwelling older subjects with balance problems. The findings support the continued use of this well-constructed scale for clinical and research assessment in a community-dwelling population of older subjects.     Feasibility of Electromyography-Triggered Neuromuscular Stimulation as an Adjunct to Constraint-Induced Movement Therapy

    Background and Purpose. The purpose of this case report is to explore the feasibility of electromyography-triggered neuromuscular stimulation (EMG-stim) as an adjunct to constraint-induced movement therapy (CIMT). Case Description. The patient was a 72-year-old man, 10 years poststroke, who did not meet traditional CIMT criteria. The EMG-stim was applied to the wrist extensors of the patient\u27s weaker arm for one half of the CIMT training hours. Outcomes. The intervention was feasible for this individual. Improvements were observed in motor behavior, quality and amount of use, muscle activity, wrist range of motion, and reaction time of the more-affected extremity. These improvements were paralleled by a change in the size and location of the extensor digitorum communis muscle representation in the primary motor cortex, as measured by transcranial magnetic stimulation mapping. Discussion. These changes suggest that using EMG-stim as an adjunct to CIMT should be further investigated in individuals who have low functional abilities following stroke

    ABSTRACT This fabrication process includes three major steps, i.e., fabrication of glassy carbon molds with arrays of micro size holes, glass compression molding to create micro cylinders on glass substrate, and reheating to form microlens arrays. As compared to traditional polymer microlens arrays, glass microlens arrays are more reliable and therefore could be used in more critical applications. In this research, microlens arrays with different surface geometries were successfully fabricated on P-SK57 (T g = 493 ºC) glass substrate using a combination of compression molding and thermal reflow process. The major parameters that influence the final lens shape, including reheating temperature and holding time, were studied to establish a suitable fabrication process.     Intratumor Heterogeneity of the Estrogen Receptor and the Long-term Risk of Fatal Breast Cancer.

    Background:Breast cancer patients with estrogen receptor (ER)-positive disease have a continuous long-term risk for fatal breast cancer, but the biological factors influencing this risk are unknown. We aimed to determine whether high intratumor heterogeneity of ER predicts an increased long-term risk (25 years) of fatal breast cancer. Methods:The STO-3 trial enrolled 1780 postmenopausal lymph node-negative breast cancer patients randomly assigned to receive adjuvant tamoxifen vs not. The fraction of cancer cells for each ER intensity level was scored by breast cancer pathologists, and intratumor heterogeneity of ER was calculated using Rao's quadratic entropy and categorized into high and low heterogeneity using a predefined cutoff at the second tertile (67%). Long-term breast cancer-specific survival analyses by intra-tumor heterogeneity of ER were performed using Kaplan-Meier and multivariable Cox proportional hazard modeling adjusting for patient and tumor characteristics. Results:A statistically significant difference in long-term survival by high vs low intratumor heterogeneity of ER was seen for all ER-positive patients (P < .001) and for patients with luminal A subtype tumors (P = .01). In multivariable analyses, patients with high intratumor heterogeneity of ER had a twofold increased long-term risk as compared with patients with low intratumor heterogeneity (ER-positive: hazard ratio [HR] = 1.98, 95% confidence interval [CI] = 1.31 to 3.00; luminal A subtype tumors: HR = 2.43, 95% CI = 1.18 to 4.99). Conclusions:Patients with high intratumor heterogeneity of ER had an increased long-term risk of fatal breast cancer. Interestingly, a similar long-term risk increase was seen in patients with luminal A subtype tumors. Our findings suggest that intratumor heterogeneity of ER is an independent long-term prognosticator with potential to change clinical management, especially for patients with luminal A tumors

    Performance measurement and KPIs for remanufacturing

    The paper provides a brief background to remanufacturing and the general use of Performance Measurement and Key Performance Indicators (KPIs) before introducing selected and newly formulated KPIs designed specifically for remanufacturing. Their relationships with the remanufacturing challenges faced by two contrasting remanufacturing businesses and the wider reman industry are described in detail. Subsets of KPIs forming a ‘Balanced Scorecard’ for each of the two remanufacturing cases conclude the paper. They arise through close working with Centro Ricerche FIAT (CRF) and SKF, and are triangulated by literature review and wider expert interviews. The two businesses represent contrasting remanufacturing scenarios: well-established high-volume low-margin automotive engine remanufacturing by the OEM ( >1000 units per year, < €10 k per unit) verses low-volume high-value wind turbine gearbox reman by an independent start-up ( €100 k per unit). The 10 general production engineering KPIs selected for the reman KPI toolbox are as follows: Work In Progress (WIP), Overall Equipment Effectiveness (OEE), Lead Time (LT), Cycle Time (CT), Hours Per Unit (HPU), Product Margin (PM), Quotation Accuracy (QA), Number of Concessions (NC), Number of managed mBOMs (BOM), and Personnel Saturation (PS). The Eco KPIs selected are: Material Used (MU), Recycled Material Used (RMU), Direct Energy Consumption (ECD), Indirect Energy Consumption (ECI), Water Withdrawal (WW), Green House Gas emissions (GHG), Total Waste (TW) by weight. The 8 Remanufacturing KPIs compiled and formulated as part of this research are: Core / Product Ratio (CPR), Core / Product Value Ratio (CPV), New Component Costs (NCC), Component Salvage Rate (SRC), Product Salvage Rate (SRP), Core Disposal Rate (CDR), Core Class Accuracy (CCA), and Core Class Distribution (CCD)

    A diagnosis on torque reversals in 4U 1626-67

    Several X-ray pulsars have been observed to experience torque reversals, which provide important observational clues to the interaction between the neutron star magnetic field and the accretion disk. We review the current models proposed for the torque reversals and discuss their viability based on the observations of the quasi-periodic oscillations (QPOs) in 4U 1626-67. Most of these models seem to be incompatible with the evolution of the QPO frequencies if they are interpreted in terms of the beat frequency model. We suggest that winds or outflows from the neutron star and the accretion disk may play an important role in accounting for the spin-down in disk-fed neutron stars.Comment: 7 pages, accepted for publication in A&

    Gerakan Kemandirian Pangan Melalui Program Desa Mandiri Pangan: Analisis Kinerja Dan Kendala

    Program Desa Mandiri Pangan (Demapan) dilaksanakan tahun 2006 dan sampaisekarang masih berlanjut. Program dirancang dalam kurun waktu 4 tahun (tahap) dengan tujuan akhir mengurangi rawan pangan di perdesaan (mandiri). Sudah 158 desa masuk tahap kemandirian dan diharapkan desa yang sudah mandiri bisa membina 3 desa lainnya dalam program Gerakan Kemandirian Pangan. Tetapi dalam pelaksanaannya program ini tidak berhasil menjadikan seluruh desa mandiri menjadi desa inti, hal ini disebabkan kelembagaan Demapannya tidak aktif. Sementara kelembagaan tersebut akan menjadi narasumber dan tempat magang desa yang akan dibina (replikasi). Kelembagaan tersebut adalah Kelompok Afinitas (KA), Lembaga Keuangan Desa (LKD) dan Tim Pangan Desa (TPD). Bagi desa yang sudah tidak aktif kelembagaannya, maka program tidak perlu dilanjutkan. Sebaliknya bagi desa yang kelembagaannya kurang aktif, maka diperlukan kegiatan pemberdayaan kelembagaan. Selain itu diperlukan dana dari pemerintah untuk pengurus dalam menggerakkan lembaga TPD dan LKD. Dana itu diinisiasi dahulu dari Badan Ketahanan Pangan Kementerian Pertanian yang dimasukkan dalam dana kegiatan Gerakan Kemandirian Pangan. Tahun berikutnya dana tersebut diharapkan dari dana APBD Tk I dan APBD Tk II. Selanjutnya untuk mensukseskan kegiatan Demapan maka peranan BKP Provinsi perlu ditingkatkan lagi agar masalah kabupaten tidak melakukan monev kegiatan dengan baik dan seringnya pergantian SDM di tingkat kabupaten bisa diatasi

    The Herschel Reference Survey: Dust in Early-Type Galaxies and Across the Hubble Sequence

    We present Herschel observations of 62 Early-Type Galaxies (ETGs), including 39 galaxies morphologically classified as S0+S0a and 23 galaxies classified as ellipticals using SPIRE at 250, 350 and 500 microns (and PACS 100 and 160 microns for 19 sources) as part of the volume-limited Herschel Reference Survey. We detect dust emission in 24% of the ellipticals and 62% of the S0s. The mean temperature of the dust is 23.9+/-0.8 K, warmer than that found for late-type galaxies in the Virgo Cluster. Including the non-detections, the mean dust mass is log(Mdust) = 5.9+/-0.1 and 5.2+/-0.1 Msun for the S0s and elliptical galaxies respectively. The mean dust-to-stellar mass is log(Mdust/Mstar) = -4.4+/-0.1 (S0s) and -5.8+/-0.1 (ellipticals). Virtually all the galaxies lie close to the red sequence yet the large number of detections of cool dust, the gas-to-dust ratios and the ratios of far-infrared to radio emission all suggest that many ETGs contain a cool interstellar medium similar to that in late-type galaxies. The mean dust-to-stellar mass ratio for S0s is approximatly a factor of ten less than for early-type spirals and the sizes of the dust sources in the S0s are also much smaller. We show that the difference cannot be explained by either the different bulge-to-disk ratios or environmental effects such as ram-pressure stripping. The wide range in the dust-to-stellar mass ratio for ETGs and the lack of a correlation between dust mass and optical luminosity suggest that much of the dust in the ETGs detected by Herschel has been acquired as the result of gravitational interactions; these interactions are unlikely to have had a major effect on the stellar masses of the ETGs. The Herschel observations tentatively suggest that in the most massive ETGs, the mass of the interstellar medium is unconnected to the evolution of the stellar populations.Comment: 28 Pages, 12 Figures.     Reversal of oncogene transformation and suppression of tumor growth by the novel IGF1R kinase inhibitor A-928605

    BACKGROUND: The insulin-like growth factor (IGF) axis is an important signaling pathway in the growth and survival of many cell and tissue types. This pathway has also been implicated in many aspects of cancer progression from tumorigenesis to metastasis. The multiple roles of IGF signaling in cancer suggest that inhibition of the pathway might yield clinically effective therapeutics. METHODS: We describe A-928605, a novel pyrazolo [3,4-d]pyrimidine small molecule inhibitor of the receptor tyrosine kinases (IGF1R and IR) responsible for IGF signal transduction. This compound was first tested for its activity and selectivity via conventional in vitro kinome profiling and cellular IGF1R autophosphorylation. Additionally, cellular selectivity and efficacy of A-928605 were analyzed in an IGF1R oncogene-addicted cell line by proliferation, signaling and microarray studies. Finally, in vivo efficacy of A-928605 was assessed in the oncogene-addicted cell line and in a neuroblastoma model as a single agent as well as in combination with clinically approved therapeutics targeting EGFR in models of pancreatic and non-small cell lung cancers. RESULTS: A-928605 is a selective IGF1R inhibitor that is able to abrogate activation of the pathway both in vitro and in vivo. This novel compound dosed as a single agent is able to produce significant growth inhibition of neuroblastoma xenografts in vivo. A-928605 is also able to provide additive effects when used in combination with clinically approved agents directed against EGFR in non-small cell lung and human pancreatic tumor models. CONCLUSION: These results suggest that a selective IGF1R inhibitor such as A-928605 may provide a useful clinical therapeutic for IGF pathway affected tumors and warrants further investigation
