Influenza Vaccination Coverage among Adults with Asthma: Findings from the 2000 Behavioral Risk Factor Surveillance System
The prevalence of asthma has increased substantially in the United States since 1980 (1). The primary clinical and public health strategy to reduce the burden of asthma is to prevent exacerbation of the disease, of which upper and lower respiratory tract infections are a major cause (2â5). One such infectionâinfluenzaâ and its complications are potentially preventable with annual vaccination, which is a safe and effective method (6,7), including among persons with asthma (8). Because persons with asthma are at increased risk of complications from influenza, they are advised to receive annual influenza vaccinations (9), unless there are contraindications. Little is known about how adults with asthma or health care providers comply with the annual influenza vaccination recommendations issued by the Advisory Committee on Immunization Practices (9). Among children with asthma, however, vaccination levels are low (10 â15). We therefore sought to assess the influenza vaccination coverage among adults with asthma, as well as to identify predictors of receiving such a vaccination
Sinistral shear during Middle Jurassic emplacement of the Matancilla Plutonic Complex in northern Chile (25.4⊠S) as evidence of oblique plate convergence during the early Andean orogeny
component of oblique subduction into an intra-arc shear zone. We document a shear zone at latitude 25.4⊠S near Taltal, Chile that was associated with intrusion of the Matancilla Plutonic Complex at ~169 Ma to evaluate intra-arc deformation and possible tectonic plate configurations during this time period. Polyphase folding of Paleozoic metasedimentary rocks is overprinted by mylonitic fabrics that are most extensive in a zone up to 1.4 km wide in the thermal aureole of the granodioritic Matancilla pluton, where contact metamorphic andalusite porphyroblasts are synkinematic with fabric development. Mylonite in metasedimentary rocks is overprinted by a ~130 Ma granodiorite (zircon UâPb) and by ~133 Ma postkinematic monazite (UâPb). Within the Jurassic Matancilla granodiorite, pervasive ductile shear occurs along the intrusive contact while centimeter-scale discrete high-strain zones throughout the pluton are associated with focused hydrothermal alteration and reaction weakening. Mylonitic foliation in the metasedimentary rocks and within the pluton strikes N- to NE and dips steeply, while stretching lineations are subhorizontal on average. Kinematic indicators record dominantly sinistral shear, though some dextral or symmetric indicators and S \u3e L fabrics suggest a component of coaxial strain and flattening. Sinistral strike-slip kinematics in the Matancilla shear zone may indicate that Middle Jurassic convergence had sinistral obliquity that was locally partitioned into the contemporaneous magmatic arc. Sinistral-oblique convergence would require the Phoenix- Farallon spreading center to be north of ~25⊠S in the Middle Jurassic, providing a constraint to plate reconstructions during the early Andean orogeny

The resummation of inter-jet energy flow for gaps-between-jets processes at HERA
We calculate resummed perturbative predictions for gaps-between-jets
processes and compare to HERA data. Our calculation of this non-global
observable needs to include the effects of primary gluon emission (global
logarithms) and secondary gluon emission (non-global logarithms) to be correct
at the leading logarithm (LL) level. We include primary emission by calculating
anomalous dimension matrices for the geometry of the specific event definitions
and estimate the effect of non-global logarithms in the large limit. The
resulting predictions for energy flow observables are consistent with

A Multi-telescope Campaign on FRB 121102: Implications for the FRB Population
We present results of the coordinated observing campaign that made the first
subarcsecond localization of a Fast Radio Burst, FRB 121102. During this
campaign, we made the first simultaneous detection of an FRB burst by multiple
telescopes: the VLA at 3 GHz and the Arecibo Observatory at 1.4 GHz. Of the
nine bursts detected by the Very Large Array at 3 GHz, four had simultaneous
observing coverage at other observatories. We use multi-observatory constraints
and modeling of bursts seen only at 3 GHz to confirm earlier results showing
that burst spectra are not well modeled by a power law. We find that burst
spectra are characterized by a ~500 MHz envelope and apparent radio energy as
high as erg. We measure significant changes in the apparent
dispersion between bursts that can be attributed to frequency-dependent
profiles or some other intrinsic burst structure that adds a systematic error
to the estimate of DM by up to 1%. We use FRB 121102 as a prototype of the FRB
class to estimate a volumetric birth rate of FRB sources Mpc yr, where is the number of bursts per
source over its lifetime. This rate is broadly consistent with models of FRBs
from young pulsars or magnetars born in superluminous supernovae or long
gamma-ray bursts, if the typical FRB repeats on the order of thousands of times

Higgs Interference Effects in \Pg \Pg \to \PZ\PZ and their Uncertainty
Interference between the Standard Model Higgs boson and continuum
contributions is considered in the heavy-mass scenario. Results are available
at leading order for the background. It is discussed how to combine the result
with the next-to-next-to-leading order Higgs production cross-section and a

Experiences of men with breast cancer: an exploratory focus group study
Management and care of men with breast cancer is based on that developed for women. Our study reports that men have specific issues regarding certain aspects of their breast cancer experience, including diagnosis, disclosure, support and gender-specific information, and offers suggestions for improved patient care
A galaxy populations study of a radio-selected protocluster at z~3.1
We present a population study of several types of galaxies within the
protocluster surrounding the radio galaxy MRC0316-257 at z~3.1. In addition to
the known population of Ly_alpha emitters (LAEs) and [OIII] emitters, we use
colour selection techniques to identify protocluster candidates that are Lyman
break galaxies (LBG) and Balmer break galaxies (BBGs). The radio galaxy field
contains an excess of LBG candidates, with a surface density 1.6\pm0.3 times
larger than found for comparable blank fields. This surface overdensity
corresponds to an LBG volume overdensity of ~8\pm4. The BBG photometric
redshift distribution peaks at the protocluster's redshift, but we detect no
significant surface overdensity of BBG. This is not surprising because a volume
overdensity similar to the LBGs would have resulted in a surface density of
~1.2 that found in the blank field. This could not have been detected in our
sample. Masses and star formation rates of the candidate protocluster galaxies
are determined using SED fitting. These properties are not significantly
different from those of field galaxies. The galaxies with the highest masses
and star formation rates are located near the radio galaxy, indicating that the
protocluster environment influences galaxy evolution at z~3. We conclude that
the protocluster around MRC0316-257 is still in the early stages of formation.Comment: 19 pages, 20 figures, accepted for publication in MNRA
Appropriate disclosure of a diagnosis of dementia : identifying the key behaviours of 'best practice'
Background: Despite growing evidence that many people with dementia want to know their diagnosis, there is wide variation in attitudes of professionals towards disclosure. The disclosure of the diagnosis of dementia is increasingly recognised as being a process rather than a one-off behaviour. However, the different behaviours that contribute to this process have not been comprehensively defined. No intervention studies to improve diagnostic disclosure in dementia have been reported to date. As part of a larger study to develop an intervention to promote appropriate disclosure, we sought to identify important disclosure behaviours and explore whether supplementing a literature review with other methods would result in the identification of new behaviours. Methods: To identify a comprehensive list of behaviours in disclosure we conducted a literature review, interviewed people with dementia and informal carers, and used a consensus process involving health and social care professionals. Content analysis of the full list of behaviours was carried out. Results: Interviews were conducted with four people with dementia and six informal carers. Eight health and social care professionals took part in the consensus panel. From the interviews, consensus panel and literature review 220 behaviours were elicited, with 109 behaviours over-lapping. The interviews and consensus panel elicited 27 behaviours supplementary to the review. Those from the interviews appeared to be self-evident but highlighted deficiencies in current practice and from the panel focused largely on balancing the needs of people with dementia and family members. Behaviours were grouped into eight categories: preparing for disclosure; integrating family members; exploring the patient's perspective; disclosing the diagnosis; responding to patient reactions; focusing on quality of life and well-being; planning for the future; and communicating effectively. Conclusion: This exercise has highlighted the complexity of the process of disclosing a diagnosis of dementia in an appropriate manner. It confirms that many of the behaviours identified in the literature (often based on professional opinion rather than empirical evidence) also resonate with people with dementia and informal carers. The presence of contradictory behaviours emphasises the need to tailor the process of disclosure to individual patients and carers. Our combined methods may be relevant to other efforts to identify and define complex clinical practices for further study.This project is funded by UK Medical Research Council, Grant reference number G0300999
