    Community-Led Total Sanitation: A Mixed-Methods Systematic Review of Evidence and Its Quality

    Background: Community-led total sanitation (CLTS) is a widely applied rural behavior change approach for ending open defecation. However, evidence of its impact is unclear. Objectives: We conducted a systematic review of journal-published and gray literature to a) assess evidence quality, b) summarize CLTS impacts, and c) identify factors affecting implementation and effectiveness. Methods: Eligible studies were systematically screened and selected for analysis from searches of seven databases and 16 websites. We developed a framework to appraise literature quality. We qualitatively analyzed factors enabling or constraining CLTS, and summarized results from quantitative evaluations. Discussion: We included 200 studies (14 quantitative evaluations, 29 qualitative studies, and 157 case studies). Journal-published literature was generally of higher quality than gray literature. Fourteen quantitative evaluations reported decreases in open defecation, but did not corroborate the widespread claims of open defecation–free (ODF) villages found in case studies. Over one-fourth of the literature overstated conclusions, attributing outcomes and impacts to interventions without an appropriate study design. We identified 43 implementation- and community-related factors reportedly affecting CLTS. This analysis revealed the importance of adaptability, structured posttriggering activities, appropriate community selection, and further research on combining and sequencing CLTS with other interventions. Conclusions: The evidence base on CLTS effectiveness available to practitioners, policy makers, and program managers to inform their actions is weak. Our results highlight the need for more rigorous research on CLTS impacts as well as applied research initiatives that bring researchers and practitioners together to address implementation challenges to improve rural sanitation efforts

    Factors that impact on access to water and sanitation for older adults and people with disability in rural South Africa: An occupational justice perspective

    Limited access to water and sanitation is a risk to health, dignity, and ability to engage in occupations. This article aims to: 1) discuss the current and historical factors affecting access to water and sanitation in rural South Africa, and 2) explore the occupational implications of water access, particularly for older adults and people with disability in rural South Africa. A literature review was carried out through searching JSTOR, Scopus, and MEDLINE databases and using framework analysis to interpret the retrieved documents. This paper also reports a thematic analysis of semi-structured interviews, conducted in 2012 in a rural area of South Africa. Environmental, political, social-economic and attitudinal factors were identified as impacting water access and occupation, in both the documentary analysis and the semi-structured interviews. Due to South Africa’s history, injustice has occurred in the forms of occupational apartheid and occupational deprivation. We argue that supply systems must enable people to easily access more water than is essential for survival, so that people can participate in meaningful and productive occupations. Therefore, access to water should be considered part of an occupational right. Recognising this right will be an integral step in ensuring that water supplies are improved to support better livelihoods, and to achieve economic and social empowerment, and quality of life for all, in line with many of the United Nations’ new Sustainable Development Goals

    Using smart pumps to help deliver universal access to safe and affordable drinking water

    It is estimated that broken water pumps impact 62 million people in sub-Saharan Africa. Over the last 20 years, broken handpumps have represented US$1·2–1·5 billion of lost investment in this region, with 30–40% of rural water systems failing prematurely. While the contributory factors are complex and multi-faceted, the authors consider that improved post-construction monitoring strategies for remote water projects, which rely on smart pumps to monitor operational performance in place of physical site visits, may address some of these problems and help reduce the heavy time and resource demands on stakeholders associated with traditional monitoring strategies. As such, smart pumps could play a significant role in improving project monitoring and might subsequently help deliver universal access to safe and affordable drinking water by 2030, which constitutes one of the key targets of United Nations sustainable development goal 6 and is embedded in some national constitutions

    Understanding the drivers of sanitation behaviour in riverine communities of Niger Delta, Nigeria: the case of Odi and Kaiama communities

    Recent reports show that between 1990 and 2012, among countries, Nigeria had the highest increase in the absolute number of open defecators. Bayelsa State makes a huge contribution to these numbers as almost 70% of residents lack access to improved sanitation. The adoption of Community-Led Total Sanitation (CLTS) as national policy has improved sanitation in the country, but progress has been slower or non-existent in riverine communities where open defecation persists. In communities where defecation is directly into water bodies evidence of the practice is hidden, which may hinder attempts to trigger community-wide behaviour change. This study evaluates sanitation in two riverine communities of Bayelsa State. It applied qualitative multiple-case methodology using observations, interviews and focus group discussions to explore existing sanitation behaviours. Field notes and interview transcripts were analysed using NVivo and qualitative content analysis. Findings highlight physical, cultural and economic drivers inhibiting the successful implementation of CLTS in riverine communities of Bayelsa State. Findings suggest that to be effective, sanitation interventions in riverine communities, where defecation is predominantly into water bodies, may need to make use of modified tools and also be coupled more strongly with development of supply chains for appropriate sanitation goods and services

    Can 'functionlaity' save the community management model of rural water supply?

    As attention increasingly turns to the sustainability of rural water supplies - and not simply overall levels of coverage or access - water point functionality has become a core concern for development practitioners and national governments, especially in Sub-Saharan Africa. Within the long-enduring Community-Based Management (CBM) model this has resulted in increased scrutiny of the “functionality” of the local water point committee (WPC) or similar community management organisation. This paper reviews the literature written from both practice-focused and critical-academic perspectives and identifies three areas that pose challenges to our understanding of water point functionality as it relates to CBM. These concern the relative neglect of (i) the local institutional and socio-economic landscape, (ii) broader governance processes and power dynamics, and (iii) the socio-technical interface. By examining these three areas, the paper engages with the specific issue of WPC functionality, whilst also considering broader issues relating to the framing of problems in development and the methodological and disciplinary ways that these are addressed. Furthermore, by focusing on community management of rural water points, the paper lays the ground for a more substantial critique of the continuing persistence of the CBM model as a central development strategy
