Incidence of lymph node metastases in clinical early-stage mucinous and seromucinous ovarian carcinoma: a retrospective cohort study
Objective:
The use of lymph node sampling during staging procedures in clinical early-stage mucinous ovarian carcinoma (MOC) is an ongoing matter of debate. Furthermore, the incidence of lymph node metastases (LNM) in MOC in relation to tumour grade (G) is unknown. We aimed to determine the incidence of LNM in clinical early-stage MOC per tumour grade.
Design:
Retrospective study with data from the Dutch Pathology Registry (PALGA).
Setting:
The Netherlands, 2002â2012.
Population or sample:
Patients with MOC.
Methods:
Histology reports on patients with MOC diagnosed in the Netherlands between 2002 and 2012 were obtained from PALGA. Reports were reviewed for diagnosis, tumour grade and presence of LNM. Clinical data, surgery reports and radiology reports of patients with LNM were retrieved from hospital files.
Main outcome measures:
Incidence of LNM, disease-free survival (DFS).
Results:
Of 915 patients with MOC, 426 underwent lymph node sampling. Cytoreductive surgery was performed in 267 patients. The other 222 patients received staging without lymph node sampling. In eight of 426 patients, LNM were discovered by sampling. In four of 190 (2.1%) patients with G1 MOC, LNM were present, compared with one of 115 (0.9%) patients with G2 MOC and three of 22 (13.6%) patients with G3 MOC. Tumour grade was not specified in 99 patients. Patients with clinical early-stage MOC had no DFS benefit from lymph node sampling.
Conclusions:
LNM are rare in early-stage G1 and G2 MOC without clinical suspicion of LNM. Therefore, lymph node sampling can be omitted in these patients
Psychological interventions for the Fear of Public Speaking: A Meta-analysis.
Background: Fear of Public Speaking (FoPS) or public speaking anxiety is a type of social anxiety and the single most commonly feared situation in the population. FoPS is disabling with negative occupational, academic, and social consequences, reported by up to one third of the population. FoPS in adolescence and adulthood is associated with an increased risk of developing generalized social anxiety disorder with further impairments. Since the last review on FoPS, a significant number of randomized controlled trials (RCTs) have been conducted assessing the effects of novel interventions with innovative modes of delivery. Objectives: The objectives of the present meta-analysis are to (1) examine the short and long-term effects of psychological interventions aimed at FoPS on FoPS and generalized social anxiety; (2) assess whether differences exist between technology-assisted modes of delivery (e.g., Internet-delivered therapies) and more traditional modes of delivering treatment (e.g., face-to-face therapies); (3) investigate whether differences in effect exist between theoretical frameworks; (4) inspect the differences in effect size between self-report measures and other measures (i.e., physiological and behavioral); (5) examine the effects of psychological interventions aimed at FoPS on secondary outcome measures (e.g., depression); and (6) investigate whether a âsleeper effectâ is present for psychological interventions for FoPS and generalized social anxiety. Methods: The study investigates the effects of psychological interventions for FoPS through a quantitative meta-analysis of RCTs, using a random-effects model. Results: A total of 30 RCTs with 1,355 participants were included through systematic searches of PsycINFO, MEDLINE, Web of Science, and Cochrane Library. The majority of the studies investigated the effects of cognitive or behavioral interventions. Nearly half of the studies used active control groups (e.g., attention placebo), whereas the other half used passive (e.g., waitlist) controls. The overall effect of psychological interventions for FoPS across 62 interventions was 0.74 (Hedges g; 95% CI: 0.61â0.87) with low to moderate heterogeneity. No difference in effect was found across theoretical frameworks. The effects based on self-report measures were larger compared to physiological and behavioral outcomes. Effects were robust against both active and passive control groups. Furthermore, psychological interventions for FoPS had a small to moderate effect on generalized social anxiety disorder (g = 0.35; 95% CI: 0.22â0.48). The effect of psychological interventions aimed at FoPS at follow-up was large (g = 1.11, 95% CI: 0.90â1.31) and moderate to large for generalized social anxiety (g = 0.70, 95% CI: 0.59â0.80). A sleeper effect was found for cognitive and behavioral interventions, indicating that patients continued to improve after treatment termination. There were some indications of publication bias. Conclusions: Psychological interventions are effective in reducing FoPS. Interventions using technology-assisted modes of delivery are equally effective as traditional face-to-face interventions in reducing FoPS. This finding highlights an opportunity to increase access to evidence-based treatments through technology-delivered interventions, which can be implemented at schools, in primary care and specialist mental health care. Moreover, psychological interventions aimed at FoPS have an effect on generalized social anxiety. Further implications are discussed.publishedVersio

The Mid-Infrared Properties of X-ray Sources
We combine the results of the Spitzer IRAC Shallow Survey and the Chandra
XBootes Survey of the 8.5 square degrees Bootes field of the NOAO Deep Wide-
Field Survey to produce the largest comparison of mid-IR and X-ray sources to
date. The comparison is limited to sources with X-ray fluxes >8x10-15 erg
cm-2s-1 in the 0.5-7.0 keV range and mid-IR sources with 3.6 um fluxes brighter
than 18.4 mag (12.3 uJy). In this most sensitive IRAC band, 85% of the 3086
X-ray sources have mid-IR counterparts at an 80% confidence level based on a
Bayesian matching technique. Only 2.5% of the sample have no IRAC counterpart
at all based on visual inspection. Even for a smaller but a significantly
deeper Chandra survey in the same field, the IRAC Shallow Survey recovers most
of the X-ray sources. A majority (65%) of the Chandra sources detected in all
four IRAC bands occupy a well-defined region of IRAC [3.6] - [4.5] vs [5.8] -
[8.0] color-color space. These X-ray sources are likely infrared luminous,
unobscured type I AGN with little mid-infrared flux contributed by the AGN host
galaxy. Of the remaining Chandra sources, most are lower luminosity type I and
type II AGN whose mid-IR emission is dominated by the host galaxy, while
approximately 5% are either Galactic stars or very local galaxies.Comment: Accepted for publication in Ap
AEGIS-X: The Chandra Deep Survey of the Extended Groth Strip
We present the AEGIS-X survey, a series of deep Chandra ACIS-I observations
of the Extended Groth Strip. The survey comprises pointings at 8 separate
positions, each with nominal exposure 200ks, covering a total area of
approximately 0.67 deg2 in a strip of length 2 degrees. We describe in detail
an updated version of our data reduction and point source detection algorithms
used to analyze these data. A total of 1325 band-merged sources have been found
to a Poisson probability limit of 4e-6, with limiting fluxes of 5.3e-17
erg/cm2/s in the soft (0.5-2 keV) band and 3.8e-16 erg/cm2/s in the hard (2-10
keV) band. We present simulations verifying the validity of our source
detection procedure and showing a very small, <1.5%, contamination rate from
spurious sources. Optical/NIR counterparts have been identified from the DEEP2,
CFHTLS, and Spitzer/IRAC surveys of the same region. Using a likelihood ratio
method, we find optical counterparts for 76% of our sources, complete to
R(AB)=24.1, and, of the 66% of the sources that have IRAC coverage, 94% have a
counterpart to a limit of 0.9 microJy at 3.6 microns (m(AB)=23.8). After
accounting for (small) positional offsets in the 8 Chandra fields, the
astrometric accuracy of the Chandra positions is found to be 0.8 arcsec RMS,
however this number depends both on the off-axis angle and the number of
detected counts for a given source. All the data products described in this

Deep Low-Frequency Radio Observations of the NOAO Bootes Field: I. Data Reduction and Catalog Construction
In this article we present deep, high-resolution radio interferometric
observations at 153 MHz to complement the extensively studied NOAO Bootes
field. We provide a description of the observations, data reduction and source
catalog construction. From our single pointing GMRT observation of ~12 hours we
obtain a high-resolution (26" x 22") image of ~11.3 square degrees, fully
covering the Bootes field region and beyond. The image has a central noise
level of ~1.0 mJy/beam, which rises to 2.0-2.5 mJy/beam at the field edge,
placing it amongst the deepest ~150 MHz surveys to date. The catalog of 598
extracted sources is estimated to be ~92 percent complete for >10 mJy sources,
while the estimated contamination with false detections is <1 percent. The low
RMS position uncertainty of 1.24" facilitates accurate matching against
catalogs at optical, infrared and other wavelengths. Differential source counts
are determined down to <~10 mJy. There is no evidence for flattening of the
counts towards lower flux densities as observed in deep radio surveys at higher
frequencies, suggesting that our catalog is dominated by the classical
radio-loud AGN population that explains the counts at higher flux densities.
Combination with available deep 1.4 GHz observations yields an accurate
determination of spectral indices for 417 sources down to the lowest 153 MHz
flux densities, of which 16 have ultra-steep spectra with spectral indices
below -1.3. We confirm that flattening of the median spectral index towards low
flux densities also occurs at this frequency. The detection fraction of the
radio sources in NIR Ks-band is found to drop with radio spectral index, which
is in agreement with the known correlation between spectral index and redshift

Work Package Report 6: Shared, plural and cultural values of ecosystems â Summary
Key findings:
Finding 1: Shared values resulting from deliberative, group-based valuation are different from individual values. Case study evidence suggests that they are more informed, considered, confident and reflective of participantsâ deeper-held, transcendental values. Deliberated, group-based monetary values may be a better reflection of real welfare impacts than non-deliberated individual values, if derived through a carefully designed and managed process. Although more research is needed to expand the currently small evidence base on deliberative monetary methods, group deliberation has the potential to significantly enhance elicitation of values.
Finding 2: The ethical, moral and justice dimensions of many environmental issues necessitate approaches that allow for the elicitation of shared and plural values. Key ethical concerns include: 1) providing a space and opportunity for people to identify values that they may find difficult to articulate (e.g. spiritual, identity); 2) recognising that some values cannot be traded without discussion and negotiation (e.g. the legal or felt rights of local people, intrinsic values of other species); and 3) understanding that it is often difficult to isolate valuation from decision-making processes because people feel there are strong ethical or moral issues at stake that need to be debated (e.g. the justice of the process, fairness in the distribution of benefits or disbenefits, responsibility, and issues of sustainability and future generations).
Finding 3: Catalyst and/or conflict points can play a key role in the emergence and articulation of values at a societal or community level that have not previously been outwardly or explicitly articulated. Catalyst and conflict points can be symbolic and are often linked to wider contested issues and meanings about who is involved in decision-making, whose voice counts and who receives the benefits or disbenefits of environmental change. These catalyst points can potentially be connected to feelings of powerlessness that give rise to concern and protest. By recognising transcendental societal and communal values (the deeper-held and overarching values held by society and communities), it becomes possible to make these values explicit and incorporate them in decision-making to better anticipate and manage conflicts.
Finding 4: There is a diversity of ways in which shared, plural, cultural and social values are used, but they are rarely conceptualised. The UK NEAFO provides a clear theoretical framework that distinguishes and categorises different dimensions and types of shared values. The proposed range of value types was both identifiable and distinguishable within case study results. This suggests that the framework provides a useful basis for operationalizing shared values for decision-making.
Finding 5: Shared and social values in the sense of value to society is conceptualised very differently by conventional economics and other disciplines. Neoclassical economists have generally undertaken valuation by equating social value with the aggregate of individual values. They consider values as fundamentally commensurable. In contrast, literature from other disciplines consistently considers values as plural, not just in the sense that multiple things have value, but also that there are multiple dimensions to value that cannot necessarily be captured in a single metric. Within mainstream economics, the difficulties associated with commensurability and aggregating values have long been recognised, but have also been neglected. An interesting area for future debate between economic and non-economic views on values may be the normative nature of value-aggregation.
Finding 6: A mixed method approach is required to elicit the multiple dimensions of shared values and to translate deeper-held, transcendental values into contextual values and preferences. Monetary valuation is limited to quantifying values. Other methods are needed to understand their meaning or content, and the communal, societal and transcendental values that underpin them. Psychometric, non-analytical and interpretive methods (e.g. storytelling) can reveal those shared values. They can be combined with deliberative-analytical methods (e.g. deliberative monetary valuation and multi-criteria analysis) to provide a comprehensive valuation that can quantify values, understand their individual and shared meanings and significance, and better include ethical dimensions.
Finding 7: Deliberative and social learning processes help people to understand the values held by others; they can lead to increased sharing of values and/or to greater acceptance of the decisions emerging from such processes. Deliberation clearly affects what values participants express compared to non-deliberated processes. There is also a growing body of theoretical and empirical research suggesting that deliberation has the potential to affect how people understand and shape the values of others. Although rarely considered in the economic literature, the concept of social learning helps to explain some of the processes involved in deliberation. The extent to which deliberation or social learning helps participants express and shape values will depend upon the frequency and depth of interactions and the timescale over which interactions occur. Only a shift in cultural values (e.g. less emphasis on material wealth), reflected in other societal institutions (e.g. changes in the indicators used to measure national progress) is likely to achieve sustainable outcomes in the long-term.
Finding 8: Media analysis is a promising avenue for characterising different types of shared values at a large scale, as well as assessing the conflicts between the communal values of different sectors of society. There has been a marked increase in public interest in environmental issues over the last decade, which is reflected in their increased media coverage. Media content and discourse analysis is able to distinguish and characterise the plurality of cultural, societal and transcendental values and their interrelationships, and can offer a picture of the self- and other-regarding values that underpin environmental issues and conflicts. Social media can provide a further forum for understanding societal and communal values surrounding environmental issues.
Finding 9: Aesthetic and spiritual values of ecosystems have a strong non-instrumental component. While they benefit human well-being, they should not simply be classified as just âservicesâ or âbenefitsâ. Many spiritual discourses about nature resist talk of consequentialist benefits and economic analysis. These discourses counter assertions of the disenchantment of the world, which is associated with an instrumental environmental ethic and the commodification of nature. Allowing the possibility of enchantment can be a richer way of understanding our experience of nature and alerts us to the limitations of using economic models for valuation and informing decisions about these profound cultural ecosystem âservicesâ. Faith communities have experience of using these non-utilitarian values in their own decision-making and provide models that could be adapted for use in environmental decision-making.
Finding 10: Subjective well-being measures provide a useful means of assessing âintangibleâ cultural ecosystem services and their benefits. Different user groups associate common elements of subjective well-being with environmental settings, providing opportunities for development of standardised measures. In the UK NEAFO, key facets of well-being associated with places in nature across different user groups included: engagement with nature (incorporating elements of connectedness, getting to know nature and the beauty of nature, and taking care of a place); therapeutic benefits (including physical and mental aspects of health); place identity (including a sense of place and belonging); spiritual value (in the sense of feeling connected or responsible to something larger than oneself); social bonding with others; and transformative and memorable experiences. Further empirical work with different user groups and environmental settings would allow for the continued development of a standardised tool for large-scale non-monetary assessment of cultural ecosystem services
