    Patients with concomitant cardiac and hematologic disorders presenting for noncardiac surgery are challenging. Anemic patients with cardiac disease should be approached in a methodical fashion. Transfusion triggers and target should be based on underlying symptomatology. The approach to anticoagulation management in patients with artificial heart valves, cardiac devices, or severe heart failure in the operative setting must encompass a complete understanding of the rationale of a patient\u27s therapy as well as calculate the risk of changing this regimen. This article focuses common disorders and discusses strategies to optimize care in patients with coexisting cardiac and hematologic disease

    Automatic coding of short text responses via clustering in educational assessment

    Automatic coding of short text responses opens new doors in assessment. We implemented and integrated baseline methods of natural language processing and statistical modelling by means of software components that are available under open licenses. The accuracy of automatic text coding is demonstrated by using data collected in the Programme for International Student Assessment (PISA) 2012 in Germany. Free text responses of 10 items with Formula responses in total were analyzed. We further examined the effect of different methods, parameter values, and sample sizes on performance of the implemented system. The system reached fair to good up to excellent agreement with human codings Formula Especially items that are solved by naming specific semantic concepts appeared properly coded. The system performed equally well with Formula and somewhat poorer but still acceptable down to Formula Based on our findings, we discuss potential innovations for assessment that are enabled by automatic coding of short text responses.     STOL aircraft transient ground effects. Part 1: Fundamental analytical study

    The first phases of a fundamental analytical study of STOL ground effects were presented. Ground effects were studied in two dimensions to establish the importance of nonlinear effects, to examine transient aspects of ascent and descent near the ground, and to study the modelling of the jet impingement on the ground. Powered lift system effects were treated using the jet-flap analogy. The status of a three-dimensional jet-wing ground effect method was presented. It was shown, for two-dimensional unblown airfoils, that the transient effects are small and are primarily due to airfoil/freestream/ground orientation rather than to unsteady effects. The three-dimensional study showed phenomena similar to the two-dimensional results. For unblown wings, the wing/freestream/ground orientation effects were shown to be of the same order of magnitude as for unblown airfoils. This may be used to study the nonplanar, nonlinear, jet-wing ground effect

    ATS-6 solar cell experiment/improvement

    ATS 6 solar cell flight experiment data through 2 years of synchronous orbit operation are presented. Comparisons are made of the performances of the 13 different types of solar cell/cover configurations, including new cover processes and materials, and the COMSAT violet cell. These performances are also compared to the performances of the LES 6 solar cell experiment, the ATS 6 main solar arrays, and the Hughes Aircraft Company solar arrays, and to laboratory spectrum electron irradiations. It was found that the cells of the ATS 6 experiment generally performed as expected through 6 to 9 months in orbit, but that at 2 years they were more severely degraded than expected

    A qualitative exploration of the meaning and experience of sexual desire among partnered women

    This qualitative study explored the meaning and experience of sexual desire for women. Data were gathered through semi-structured interviews with 40 partnered heterosexual women aged 20 to 61 years drawn from the general population. Thematic analysis of the interview transcripts Indicated that the participants understood and experienced their sexual desire primarily within the context of their partner relationships and most frequently reported responsive rather than autonomous experiences of sexual desire. The implications of the study findings are discussed in relation to the definition, classification, and treatment of sexual desire disorders in women.<br /
