    PFAS levels and determinants of variability in exposure in European teenagers - Results from the HBM4EU aligned studies (2014-2021)

    Background: Perfluoroalkyl substances (PFAS) are man-made fluorinated chemicals, widely used in various types of consumer products, resulting in their omnipresence in human populations. The aim of this study was to describe current PFAS levels in European teenagers and to investigate the determinants of serum/plasma concentrations in this specific age group. Methods: PFAS concentrations were determined in serum or plasma samples from 1957 teenagers (12-18 years) from 9 European countries as part of the HBM4EU aligned studies (2014-2021). Questionnaire data were post-harmonized by each study and quality checked centrally. Only PFAS with an overall quantification frequency of at least 60% (PFOS, PFOA, PFHxS and PFNA) were included in the analyses. Sociodemographic and lifestyle factors were analysed together with food consumption frequencies to identify determinants of PFAS exposure. The variables study, sex and the highest educational level of household were included as fixed factors in the multivariable linear regression models for all PFAS and each dietary variable was added to the fixed model one by one and for each PFAS separately. Results: The European exposure values for PFAS were reported as geometric means with 95% confidence intervals (CI): PFOS [2.13 μg/L (1.63-2.78)], PFOA ([0.97 μg/L (0.75-1.26)]), PFNA [0.30 μg/L (0.19-0.45)] and PFHxS [0.41 μg/L (0.33-0.52)]. The estimated geometric mean exposure levels were significantly higher in the North and West versus the South and East of Europe. Boys had significantly higher concentrations of the four PFAS compared to girls and significantly higher PFASs concentrations were found in teenagers from households with a higher education level. Consumption of seafood and fish at least 2 times per week was significantly associated with 21% (95% CI: 12-31%) increase in PFOS concentrations and 20% (95% CI: 10-31%) increase in PFNA concentrations as compared to less frequent consumption of seafood and fish. The same trend was observed for PFOA and PFHxS but not statistically significant. Consumption of eggs at least 2 times per week was associated with 11% (95% CI: 2-22%) and 14% (95% CI: 2-27%) increase in PFOS and PFNA concentrations, respectively, as compared to less frequent consumption of eggs. Significantly higher PFOS concentrations were observed for participants consuming offal (14% (95% CI: 3-26%)), the same trend was observed for the other PFAS but not statistically significant. Local food consumption at least 2 times per week was associated with 40% (95% CI: 19-64%) increase in PFOS levels as compared to those consuming local food less frequently. Conclusion: This work provides information about current levels of PFAS in European teenagers and potential dietary sources of exposure to PFAS in European teenagers.     Approaches to mixture risk assessment of PFASs in the European population based on human hazard and biomonitoring data

    Per- and polyfluoroalkyl substances (PFASs) are a highly persistent, mobile, and bioaccumulative class of chemicals, of which emissions into the environment result in long-lasting contamination with high probability for causing adverse effects to human health and the environment. Within the European Biomonitoring Initiative HBM4EU, samples and data were collected in a harmonized way from human biomonitoring (HBM) studies in Europe to derive current exposure data across a geographic spread. We performed mixture risk assessments based on recent internal exposure data of PFASs in European teenagers generated in the HBM4EU Aligned Studies (dataset with N = 1957, sampling years 2014-2021). Mixture risk assessments were performed based on three hazard-based approaches: the Hazard Index (HI) approach, the sum value approach as used by the European Food Safety Authority (EFSA) and the Relative Potency Factor (RPF) approach. The HI approach resulted in the highest risk estimates, followed by the RPF approach and the sum value approach. The assessments indicate that PFAS exposure may result in a health risk in a considerable fraction of individuals in the HBM4EU teenager study sample, thereby confirming the conclusion drawn in the recent EFSA scientific opinion.     Clonal chromosomal mosaicism and loss of chromosome Y in elderly men increase vulnerability for SARS-CoV-2

    The pandemic caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2, COVID-19) had an estimated overall case fatality ratio of 1.38% (pre-vaccination), being 53% higher in males and increasing exponentially with age. Among 9578 individuals diagnosed with COVID-19 in the SCOURGE study, we found 133 cases (1.42%) with detectable clonal mosaicism for chromosome alterations (mCA) and 226 males (5.08%) with acquired loss of chromosome Y (LOY). Individuals with clonal mosaic events (mCA and/or LOY) showed a 54% increase in the risk of COVID-19 lethality. LOY is associated with transcriptomic biomarkers of immune dysfunction, pro-coagulation activity and cardiovascular risk. Interferon-induced genes involved in the initial immune response to SARS-CoV-2 are also down-regulated in LOY. Thus, mCA and LOY underlie at least part of the sex-biased severity and mortality of COVID-19 in aging patients. Given its potential therapeutic and prognostic relevance, evaluation of clonal mosaicism should be implemented as biomarker of COVID-19 severity in elderly people.     Reducing the environmental impact of surgery on a global scale: systematic review and co-prioritization with healthcare workers in 132 countries

    Abstract Background Healthcare cannot achieve net-zero carbon without addressing operating theatres. The aim of this study was to prioritize feasible interventions to reduce the environmental impact of operating theatres. Methods This study adopted a four-phase Delphi consensus co-prioritization methodology. In phase 1, a systematic review of published interventions and global consultation of perioperative healthcare professionals were used to longlist interventions. In phase 2, iterative thematic analysis consolidated comparable interventions into a shortlist. In phase 3, the shortlist was co-prioritized based on patient and clinician views on acceptability, feasibility, and safety. In phase 4, ranked lists of interventions were presented by their relevance to high-income countries and low–middle-income countries. Results In phase 1, 43 interventions were identified, which had low uptake in practice according to 3042 professionals globally. In phase 2, a shortlist of 15 intervention domains was generated. In phase 3, interventions were deemed acceptable for more than 90 per cent of patients except for reducing general anaesthesia (84 per cent) and re-sterilization of ‘single-use’ consumables (86 per cent). In phase 4, the top three shortlisted interventions for high-income countries were: introducing recycling; reducing use of anaesthetic gases; and appropriate clinical waste processing. In phase 4, the top three shortlisted interventions for low–middle-income countries were: introducing reusable surgical devices; reducing use of consumables; and reducing the use of general anaesthesia. Conclusion This is a step toward environmentally sustainable operating environments with actionable interventions applicable to both high– and low–middle–income countries

    Species biology of elasmobranch by-catch in bottom-trawl fishery in the northern Patagonian shelf, Argentina

    The present study analyzes the species composition of elasmobranch by-catch of bottom trawlers that targeted common hake Merluccius hubbsi on the northern and central Patagonian shelf during 2001, 2002 and 2003. Six surveys were conducted, and a total of 20 species of elasmobranchs were observed. Zearaja chilensis and Squalus acanthias were present in all surveys. The mean values of relative abundance ranged between 9.12 and 151 ind. km−2 for Z. chilensis and between 8.24 and 298 ind. km−2 for S. acanthias. Bathyraja brachyurops, B. macloviana, B. magellanica, Discopyge sp. and Psammobatis lentiginosa were registered only in 1 survey, and their relative abundance did not exceed 15 ind. km−2. There were no patterns detected between the mean size of the individuals and the depth of capture for any of the captured species. Length-to-weight relationships were estimated for 14 species, and size-at-maturity was estimated for 8 species.Fil: Crespi Abril, Augusto Cesar.     Food habits of the South American sea lion, Otaria flavescens, off Patagonia, Argentina

    Food habits of the South American sea lion (Otaria flavescens) off Patagonia were studied by means of stomach content analysis. The samples were collected during 1982–1987 and 1990–1998 in northern and central Patagonia. The samples (n=59) came from individuals found dead on beaches and from animals recovered in incidental catches of the fishery. Forty-one prey species (including fishes, cephalopods, crustaceans, gastropods, polychetes, sponges, and tunicates) were identified; most important were Argentine hake (Merluccius hubbsi), red octopus (Enteroctopus megalocyathus), Argentine shortfin squid (Illex argentinus), “raneya” (Raneya brasiliensis), Patagonian squid (Loligo gahi), and Argentine anchovy (Engraulis anchoita). Differences in diet were found between sexes but not between geographical area of sampling, period of sampling, or source of samples. Females fed mostly on benthic species, whereas males fed mostly on demersal-pelagic species. The difference in diet between sexes was associated with different feeding grounds or different home ranges and could be produced by different constraints in the feeding behavior of each sex. These different constraints and restrictions could lead females to feed in more coastal and shallower waters than those waters where males feed. Some of the important prey were commercial species (Argentine hake, Argentine shortfin squid, Patagonian squid) consumed at both commercial and noncommercial sizes by sea lions. The presence of gastroliths was independent of the presence of stomach parasites; however, gastrolith weight was positively correlated with individual sea lion’s length, indicating that gastroliths could be involved in buoyancy control. In summary, these stomach content analyses indicate that South American sea lions feed primarily on demersal and benthic species and, in general terms, use resources according to their environmental availability
