    Acid-suppression medications and bacterial gastroenteritis:a population-based cohort study

    AIMS: To investigate whether acid suppression medicines (ASMs) increase the risk of bacterial gastroenteritis. METHODS: A population-based, propensity-score matched cohort study using a record-linkage database in Tayside, Scotland. The study consisted of 188,323 exposed to ASMs [proton pump inhibitors (PPIs) and H2 receptor antagonists (H2RA)] and 376,646 controls (a propensity-score matched cohort from the rest of population who were not exposed to ASMs) between 1999 and 2013. The main outcome measure was a positive stool test for C. difficile, Campylobacter, Salmonella, Shigella or Escherichia coli O157. The association between ASMs and risk of bacterial gastroenteritis was assessed by a Cox regression model. RESULTS: There were 22,705 positive test results (15,273 Clostridium difficile (toxin positive), 6,590 Campylobacter, 852 Salmonella, 129 Shigella and 193 Escherichia coli O157, not mutually exclusive) with a total of 5,729,743 person-years follow up time in Tayside, 1999-2013. The adjusted hazard ratios (HRs) for culture positive diarrhoea for the PPIs and H2RA exposed vs unexposed cohort were 2.72 [95% confidence interval (CI) 2.33, 3.17] during follow up time for samples submitted from the community and 1.28 (95% CI 1.08, 1.52) for samples submitted from hospitals. Compared with the unexposed cohort, patients in the exposed group had increased risks of C. difficile and Campylobacter [adjusted HRs of 1.70 (95% CI 1.28, 2.25), 3.71 (95% CI 3.04, 4.53) for community samples, and 1.42 (95% CI 1.17, 1.71), 4.53 (95% CI 1.75, 11.8) for hospital samples, respectively]. CONCLUSIONS: The results suggest that community prescribed ASMs were associated with increased rates of C. difficile and Campylobacter positive gastroenteritis in both the community and hospital settings

    The Critical Coronal Transition Region: A Physics-framed Strategy to Uncover the Genesis of the Solar Wind and Solar Eruptions

    Our current theoretical and observational understanding suggests that critical properties of the solar wind and Coronal Mass Ejections (CMEs) are imparted within 10 Rs, particularly below 4 Rs. This seemingly narrow spatial region encompasses the transition of coronal plasma processes through the entire range of physical regimes from fluid to kinetic, and from primarily closed to open magnetic field structures. From a physics perspective, therefore, it is more appropriate to refer to this region as the Critical Coronal Transition Region (CCTR) to emphasize its physical, rather than spatial, importance to key Heliophysics science. This white paper argues that the comprehensive exploration of the CCTR will answer two of the most central Heliophysics questions, "How and where does the solar wind form?" and "How do eruptions form?", by unifying hardware/software/modeling development and seemingly disparate research communities and frameworks.     Scoping review of community health participatory research projects in Ghana

    BACKGROUND: Community health participation is an essential tool in health research and management where community members, researchers and other relevant stakeholders contribute to the decision-making processes. Though community participation processes can be complex and challenging, evidence from previous studies have reported significant value of engaging with community in community health projects. OBJECTIVE: To identify the nature and extent of community involvement in community health participatory research (CHPR) projects in Ghana and draw lessons for participatory design of a new project on diabetes intervention in Accra called the Contextual Awareness Response and Evaluation (CARE) diabetes project. METHODS: A scoping review of relevant publications on CHPR projects in Ghana which had a participatory component was undertaken. PubMed, PsycINFO, African Journal Online, Health Source: Nursing/Academic Edition, Humanities International Complete and Google Scholar were searched for articles published between January 1950 and October 2021. Levac et al.'s (2010) methodological framework for scoping reviews was used to select, collate and characterise the data. RESULTS: Fifteen studies were included in this review of CHPR projects from multiple disciplines. Participants included community health workers, patients, caregivers, policymakers, community groups, service users and providers. Based on Pretty's participation typology, several themes were identified in relation to the involvement of participants in the identified studies. The highest levels of participation were found in two studies in the diagnosis, four in the development, five in the implementation and three in the evaluation phases of projects. Community participation across all studies was assessed as low overall. CONCLUSION: This review showed that community participation is essential in the acceptability and feasibility of research projects in Ghana and highlighted community participation's role in the diagnosis, development, implementation and evaluation stages of projects. Lessons from this review will be considered in the development, implementation, and future evaluation of the CARE diabetes project

    Phylotyping and Functional Analysis of Two Ancient Human Microbiomes

    Background: The Human Microbiome Project (HMP) is one of the U.S. National Institutes of Health Roadmap for Medical Research. Primary interests of the HMP include the distinctiveness of different gut microbiomes, the factors influencing microbiome diversity, and the functional redundancies of the members of human microbiotas. In this present work, we contribute to these interests by characterizing two extinct human microbiotas. Methodology/Principal Findings: We examine two paleofecal samples originating from cave deposits in Durango Mexico and dating to approximately 1300 years ago. Contamination control is a serious issue in ancient DNA research; we use a novel approach to control contamination. After we determined that each sample originated from a different human, we generated 45 thousand shotgun DNA sequencing reads. The phylotyping and functional analysis of these reads reveals a signature consistent with the modern gut ecology. Interestingly, inter-individual variability for phenotypes but not functional pathways was observed. The two ancient samples have more similar functional profiles to each other than to a recently published profile for modern humans. This similarity could not be explained by a chance sampling of the databases. Conclusions/Significance: We conduct a phylotyping and functional analysis of ancient human microbiomes, while providing novel methods to control for DNA contamination and novel hypotheses about past microbiome biogeography. We postulate that natural selection has more of an influence on microbiome functional profiles than it does on the species represented in the microbial ecology. We propose that human microbiomes were more geographically structured during pre-Columbian times than today

    Oil-Based Mud Cutting as an Additional Raw Material in Clinker Production

    Oil-Based Mud (OBM) cutting is a hazardous by-product generated during oil-well drilling. Its chemical composition suggests that it might be suitable as a raw material in cement manufacturing. It is rich in calcium oxide, silica, and aluminium oxide, which are the major oxides in raw materials for cement manufacturing. In this research, OBM cutting is used as a constituent of the raw meal for cement clinker production. Raw meal mixtures were prepared by mixing different ratios of raw materials increasing OBM content. The impact of the addition of OBM cutting on the resulting clinker has been investigated. The results demonstrate that OBM cutting could be recycled in the manufacturing of Portland cement clinker. Clinker prepared using OBM cutting had very similar properties to that prepared from limestone. This result could represent an opportunity for solving an environmental problem. The addition of OBM cutting lowers the calcination temperature, and increases the rate of carbonate dissociation. However, it also leads to a higher free lime in clinker, which is a result of the presence of trace elements, such as barium. Overall, its use as a raw material in cement production could provide a cost-effective, environment-friendly route for the management of OBM cutting

    Helminth immunomodulation in autoimmune disease

    Helminths have evolved to become experts at subverting immune surveillance. Through potent and persistent immune tempering, helminths can remain undetected in human tissues for decades. Redirecting the immunomodulating "talents" of helminths to treat inflammatory human diseases is receiving intensive interest. Here, we review therapies using live parasitic worms, worm secretions, and worm-derived synthetic molecules to treat autoimmune disease. We review helminth therapy in both mouse models and clinical trials and discuss what is known on mechanisms of action. We also highlight current progress in characterizing promising new immunomodulatory molecules found in excretory/secretory products of helminths and their potential use as immunotherapies for acute and chronic inflammatory diseases

    Genome-wide associations for birth weight and correlations with adult disease

    Birth weight (BW) has been shown to be influenced by both fetal and maternal factors and in observational studies is reproducibly associated with future risk of adult metabolic diseases including type 2 diabetes (T2D) and cardiovascular disease. These life-course associations have often been attributed to the impact of an adverse early life environment. Here, we performed a multi-ancestry genome-wide association study (GWAS) meta-analysis of BW in 153,781 individuals, identifying 60 loci where fetal genotype was associated with BW (P\textit{P}  < 5 × 108^{-8}). Overall, approximately 15% of variance in BW was captured by assays of fetal genetic variation. Using genetic association alone, we found strong inverse genetic correlations between BW and systolic blood pressure (R\textit{R}g_{g} = -0.22, P\textit{P}  = 5.5 × 1013^{-13}), T2D (R\textit{R}g_{g} = -0.27, P\textit{P}  = 1.1 × 106^{-6}) and coronary artery disease (R\textit{R}g_{g} = -0.30, P\textit{P}  = 6.5 × 109^{-9}). In addition, using large -cohort datasets, we demonstrated that genetic factors were the major contributor to the negative covariance between BW and future cardiometabolic risk. Pathway analyses indicated that the protein products of genes within BW-associated regions were enriched for diverse processes including insulin signalling, glucose homeostasis, glycogen biosynthesis and chromatin remodelling. There was also enrichment of associations with BW in known imprinted regions (P\textit{P} = 1.9 × 104^{-4}). We demonstrate that life-course associations between early growth phenotypes and adult cardiometabolic disease are in part the result of shared genetic effects and identify some of the pathways through which these causal genetic effects are mediated.For a full list of the funders pelase visit the publisher's website and look at the supplemetary material provided.
