a comparison between young and old adults
Healthy aging is associated with a variety of functional and structural brain
alterations. These age-related brain alterations have been assumed to
negatively impact cognitive and motor performance. Especially important for
the execution of everyday activities in older adults (OA) is the ability to
perform movements that depend on both hands working together. However,
bimanual coordination is typically deteriorated with increasing age. Hence, a
deeper understanding of such age-related brain-behavior alterations might
offer the opportunity to design future interventional studies in order to
delay or even prevent the decline in cognitive and/or motor performance over
the lifespan. Here, we examined to what extent the capability to acquire and
maintain a novel bimanual motor skill is still preserved in healthy OA as
compared to their younger peers (YA). For this purpose, we investigated
performance of OA (n = 26) and YA (n = 26) in a bimanual serial reaction time
task (B-SRTT), on two experimental sessions, separated by 1 week. We found
that even though OA were generally slower in global response times, they
showed preserved learning capabilities in the B-SRTT. However, sequence
specific learning was more pronounced in YA as compared to OA. Furthermore, we
found that switching between hands during B-SRTT learning trials resulted in
increased response times (hand switch costs), a phenomenon that was more
pronounced in OA. These hand switch costs were reduced in both groups over the
time course of learning. More interestingly, there were no group differences
in hand switch costs on the second training session. These results provide
novel evidence that bimanual motor skill learning is capable of reducing age-
related deficits in hand switch costs, a finding that might have important
implications to prevent the age-related decline in sensorimotor function
Burden of disease and circulating serotypes of rotavirus infection in sub-Saharan Africa: systematic review and meta-analysis.
Two new rotavirus vaccines have recently been licensed in many countries. However, their efficacy has only been shown against certain serotypes commonly circulating in Europe, North America, and Latin America, but thought to be globally important. To assess the potential impact of these vaccines in sub-Saharan Africa, where rotavirus mortality is high, knowledge of prevalent types is essential because an effective rotavirus vaccine is needed to protect against prevailing serotypes in the community. We did two systematic reviews and two meta-analyses of the most recent published data on the burden of rotavirus disease in children aged under 5 years and rotavirus serotypes circulating in countries in sub-Saharan Africa. Eligible studies were selected from PubMed/Medline, Cochrane Library, EmBase, LILACS, Academic Search Premier, Biological Abstracts, ISI Web of Science, and the African Index Medicus. Depending on the heterogeneity, DerSimonian-Laird random-effects or fixed-effects models were used for meta-analyses. Geographical variability in rotavirus burden within countries in sub-Saharan Africa is substantial, and most countries lack information on rotavirus epidemiology. We estimated that annual mortality for this region was 243.3 (95% CI 187.6-301.7) deaths per 100,000 under 5 years (ie, a total of 300,000 children die of rotavirus infection in this region each year). The most common G type detected was G1 (34.9%), followed by G2 (9.1%), and G3 (8.6%). The most common P types detected were P[8] (35.5%) and P[6] (27.5%). Accurate information should be collected from surveillance based on standardised methods in these countries to obtain comparable data on the burden of disease and the circulating strains to assess the potential impact of vaccine introduction
Anodal Transcranial Direct Current Stimulation Does Not Facilitate Dynamic Balance Task Learning in Healthy Old Adults
Older adults frequently experience a decrease in balance control that leads to
increased numbers of falls, injuries and hospitalization. Therefore,
evaluating older adults’ ability to maintain balance and examining new
approaches to counteract age-related decline in balance control is of great
importance for fall prevention and healthy aging. Non-invasive brain
stimulation techniques such as transcranial direct current stimulation (tDCS)
have been shown to beneficially influence motor behavior and motor learning.
In the present study, we investigated the influence of tDCS applied over the
leg area of the primary motor cortex (M1) on balance task learning of healthy
elderly in a dynamic balance task (DBT). In total, 30 older adults were
enrolled in a cross-sectional, randomized design including two consecutive DBT
training sessions. Only during the first DBT session, either 20 min of anodal
tDCS (a-tDCS) or sham tDCS (s-tDCS) were applied and learning improvement was
compared between the two groups. Our data showed that both groups successfully
learned to perform the DBT on both training sessions. Interestingly, between-
group analyses revealed no difference between the a-tDCS and the s-tDCS group
regarding their level of task learning. These results indicate that the
concurrent application of tDCS over M1 leg area did not elicit DBT learning
enhancement in our study cohort. However, a regression analysis revealed that
DBT performance can be predicted by the kinematic profile of the movement, a
finding that may provide new insights for individualized approaches of
treating balance and gait disorders
A Resting-State fMRI Study
Mirror visual feedback (MVF) is a promising approach to enhance motor
performance without training in healthy adults as well as in patients with
focal brain lesions. There is preliminary evidence that a functional
modulation within and between primary motor cortices as assessed with
transcranial magnetic stimulation (TMS) might be one candidate mechanism
mediating the observed behavioral effects. Recently, studies using task-based
functional magnetic resonance imaging (fMRI) have indicated that MVF-induced
functional changes might not be restricted to the primary motor cortex (M1)
but also include higher order regions responsible for perceptual-motor
coordination and visual attention. However, aside from these instantaneous
task-induced brain changes, little is known about learning-related
neuroplasticity induced by MVF. Thus, in the present study, we assessed MVF-
induced functional network plasticity with resting-state fMRI (rs-fMRI). We
performed rs-fMRI of 35 right-handed, healthy adults before and after
performing a complex ball-rotation task. The primary outcome measure was the
performance improvement of the untrained left hand (LH) before and after right
hand (RH) training with MVF (mirror group [MG], n = 17) or without MVF
(control group [CG], n = 18). Behaviorally, the MG showed superior performance
improvements of the untrained LH. In resting-state functional connectivity
(rs-FC), an interaction analysis between groups showed changes in left visual
cortex (V1, V2) revealing an increase of centrality in the MG. Within group
comparisons showed further functional alterations in bilateral primary
sensorimotor cortex (SM1), left V4 and left anterior intraparietal sulcus
(aIP) in the MG, only. Importantly, a correlation analysis revealed a linear
positive relationship between MVF-induced improvements of the untrained LH and
functional alterations in left SM1. Our results suggest that MVF-induced
performance improvements are associated with functional learning-related brain
plasticity and have identified additional target regions for non-invasive
brain stimulation techniques, a finding of potential interest for
neurorehabilitation
Quality of Care in Contraceptive Services Provided to Young People in Two Ugandan Districts: A Simulated Client Study
BACKGROUND: Low and inconsistent use of contraceptives by young people contributes to unintended pregnancies. This study assessed quality of contraceptive services for young people aged 15-24 in two rural districts in Uganda. METHODS: Five female and two male simulated clients (SCs) interacted with 128 providers at public, private not-for-profit (PNFP), and private for profit (PFP) health facilities. After consultations, SCs were interviewed using a structured questionnaire. Six aspects of quality of care (client's needs, choice of contraceptive methods, information given to users, client-provider interpersonal relations, constellation of services, and continuity mechanisms) were assessed. Descriptive statistics and factor analysis were performed. RESULTS: Means and categorized quality scores for all aspects of quality were low in both public and private facilities. The lowest quality scores were observed in PFP, and medium scores in PNFP facilities. The choice of contraceptive methods and interpersonal relations quality scores were slightly higher in public facilities. Needs assessment scores were highest in PNFP facilities. All facilities were classified as having low scores for appropriate constellation of services. Information given to users was suboptimal and providers promoted specific contraceptive methods. Minority of providers offered preferred method of choice and showed respect for privacy. CONCLUSIONS: The quality of contraceptive services provided to young people was low. Muscle synergies in cycling after incomplete spinal cord injury: correlation with clinical measures of motor function and spasticity
Background: After incomplete spinal cord injury (iSCI), patients suffer important sensorimotor impairments, such as abnormal locomotion patterns and spasticity. Complementary to current clinical diagnostic procedures, the analysis of muscle synergies has emerged as a promising tool to study muscle coordination, which plays a major role in the control of multi-limb functional movements.Objective: Based on recent findings suggesting that walking and cycling share similar synergistic control, the analysis of muscle synergies during cycling might be explored as an early descriptor of gait-related impaired control. This idea was split into the following two hypotheses: (a) iSCI patients present a synergistic control of muscles during cycling; (b) muscle synergies outcomes extracted during cycling correlate with clinical measurements of gait performance and/or spasticity.Methods: Electromyographic (EMG) activity of 13 unilateral lower limb muscles was recorded in a group of 10 healthy individuals and 10 iSCI subjects during cycling at four different cadences. A non-negative matrix factorization (NNMF) algorithm was applied to identify synergistic components (i.e., activation coefficients and muscle synergy vectors). Reconstruction goodness scores (VAF and r(2)) were used to evaluate the ability of a given number of synergies to reconstruct the EMG signals. A set of metrics based on the similarity between pathologic and healthy synergies were correlated with clinical scales of gait performance and spasticity.Results: iSCI patients preserved a synergistic control of muscles during cycling. The similarity with the healthy reference was consistent with the degree of the impairment, i.e., less impaired patients showed higher similarities with the healthy reference. There was a strong correlation between reconstruction goodness scores at 42 rpm and motor performance scales (TUG, 10-m test and WISCI II). Light-intensity physical activity and cardiometabolic biomarkers in US adolescents
BackgroundThe minimal physical activity intensity that would confer health benefits among adolescents is unknown. The purpose of this study was to examine the associations of accelerometer-derived light-intensity (split into low and high) physical activity, and moderate- to vigorous-intensity physical activity with cardiometabolic biomarkers in a large population-based sample.MethodsThe study is based on 1,731 adolescents, aged 12–19 years from the 2003/04 and 2005/06 National Health and Nutrition Examination Survey. Low light-intensity activity (100–799 counts/min), high light-intensity activity (800 counts/min to <4 METs) and moderate- to vigorous-intensity activity (≥4 METs, Freedson age-specific equation) were accelerometer-derived. Cardiometabolic biomarkers, including waist circumference, systolic blood pressure, diastolic blood pressure, HDL-cholesterol, and C-reactive protein were measured. Triglycerides, LDL- cholesterol, insulin, glucose, and homeostatic model assessments of β-cell function (HOMA-%B) and insulin sensitivity (HOMA-%S) were also measured in a fasting sub-sample (n=807).ResultsAdjusted for confounders, each additional hour/day of low light-intensity activity was associated with 0.59 (95% CI: 1.18–0.01) mmHG lower diastolic blood pressure. Each additional hour/day of high light-intensity activity was associated with 1.67 (2.94–0.39) mmHG lower diastolic blood pressure and 0.04 (0.001–0.07) mmol/L higher HDL-cholesterol. Each additional hour/day of moderate- to vigorous-intensity activity was associated with 3.54 (5.73–1.35) mmHG lower systolic blood pressure, 5.49 (1.11–9.77)% lower waist circumference, 25.87 (6.08–49.34)% lower insulin, and 16.18 (4.92–28.53)% higher HOMA-%S.ConclusionsTime spent in low light-intensity physical activity and high light-intensity physical activity had some favorable associations with biomarkers. Consistent with current physical activity recommendations for adolescents, moderate- to vigorous-intensity activity had favorable associations with many cardiometabolic biomarkers. While increasing MVPA should still be a public health priority, further studies are needed to identify dose-response relationships for light-intensity activity thresholds to inform future recommendations and interventions for adolescents.</div
Cyberbullying Victimization in Context: The Role of Social Inequalities in Countries and Regions
The phenomenon of cyberbullying is gaining ever more attention by media and policy makers in many countries. Theoretical frameworks using a socio-ecological approach emphasise the importance of contextual explanatory factors located at the societal level. It has been suggested that in addition to cross-national differences, the analysis of smaller units of more adjacent cultural contexts (i.e., regions) might yield more explanatory power. Leaning on previous findings and theory, the current paper aims to identify and compare contextual explanatory factors associated with social inequality (i.e., crime rates, GDP, life expectancy and population density) for variation in cyber- and face-to-face bullying victimisation rates within one sample. Moreover, corresponding explanatory factors are investigated across national and regional levels. Cyber- and face-to-face bullying victimisation of 15,813 9-16 year olds (50% female) from the cross-national survey data of EU Kids Online were linked with contextual variables of 18 countries and 179 regions obtained from data of the European Social Survey (ESS). Hierarchical multilevel-modelling analyses, adding first regional and then country level contextual predictors for bullying victimisation, were performed. Against expectations, differences for cyber- and face-to-face victimisation between regions within countries were smaller than differences between countries. Regional level life expectancy showed a negative and crime rates showed a marginal positive relation with both cyber- and face-to-face victimisation. Population density showed a negative and GDP a positive relationship with cyber- but not face-to-face victimisation. Adding the same predictors on the country level did not improve model fit.
