Over 13,000 copies of the 4th edition were sold, and we conservatively estimate that it saves 210millioninresourcesand210 million in resources and 220 million in labor annually. Emerging issues and changes in the field, however, have precipitated the need for a new 5th edition of PCWD. Six editors and over 120 authors are working to create the next edition, which will include 36 new and 77 revised chapters. We anticipate that the 5th edition of PCWD and derivative products will be available as a two-volume book, CD, website, e-book, and mobile app, starting in 2015

    Early in-hospital exposure to statins and outcome after intracerebral haemorrhage – results from the Virtual International Stroke Trials Archive

    Introduction: Recent data suggest that statin use after intracerebral haemorrhage might be beneficial. However, data on the effects of early in-hospital statin exposure are lacking. Therefore, we sought to assess whether (1) early statin exposure during the acute phase after intracerebral haemorrhage and (2) early continuation of prevalent statin use are associated with favourable functional outcome. Patients and methods: Data were obtained from the Virtual International Stroke Trials Archive. Patients were categorised according to use patterns of statins during this early in-hospital phase (continuation, discontinuation or new initiation of statins). Univariate and multivariable analyses were conducted to explore the association between early statin exposure and functional outcome. Results: A total of 919 patients were included in the analysis. Early in-hospital statin exposure (n = 89, 9.7%) was associated with better functional outcome (modified Rankin Scale ≤ 3) compared with 790 patients without statin exposure before or early after the event (66% versus 47%, adjusted OR 2.1, 95% confidence interval 1.3–3.6).  Compared with patients without exposure to statins before and early after the event, early continuation of statin therapy (n = 57) was associated with favourable functional outcome (adjusted odds ratio 2.6, 95% confidence interval 1.3–5.2). The association between early continuation of statins and outcome remained robust in sensitivity analyses restricted to patients able to take oral medication within 72 h and one-week survivors. Discussion: It is possible that part of the observed associations are not due to a protective effect of statins but are confounded by indication bias. Conclusion: Statin exposure and continuation of prevalent statin therapy early after intracerebral haemorrhage are associated with favourable functional outcome after 90 days

    Analysing the opinions of UK veterinarians on practice-based research using corpus linguistic and mathematical methods

    The use of corpus linguistic techniques and other related mathematical analyses have rarely, if ever, been applied to qualitative data collected from the veterinary field. The aim of this study was to explore the use of a combination of corpus linguistic analyses and mathematical methods to investigate a free-text questionnaire dataset collected from 3796 UK veterinarians on evidence-based veterinary medicine, specifically, attitudes towards practice-based research (PBR) and improving the veterinary knowledge base. The corpus methods of key word, concordance and collocate analyses were used to identify patterns of meanings within the free text responses. Key words were determined by comparing the questionnaire data with a wordlist from the British National Corpus (representing general English text) using cross-tabs and log-likelihood comparisons to identify words that occur significantly more frequently in the questionnaire data. Concordance and collocation analyses were used to account for the contextual patterns in which such key words occurred, involving qualitative analysis and Mutual Information Analysis (MI3). Additionally, a mathematical topic modelling approach was used as a comparative analysis; words within the free text responses were grouped into topics based on their weight or importance within each response to find starting points for analysis of textual patterns. Results generated from using both qualitative and quantitative techniques identified that the perceived advantages of taking part in PBR centred on the themes of improving knowledge of both individuals and of the veterinary profession as a whole (illustrated by patterns around the words learning, improving, contributing). Time constraints (lack of time, time issues, time commitments) were the main concern of respondents in relation to taking part in PBR. Opinions of what vets could do to improve the veterinary knowledge base focussed on the collecting and sharing of information (record, report), particularly recording and discussing clinical cases (interesting cases), and undertaking relevant continuing professional development activities. The approach employed here demonstrated how corpus linguistics and mathematical methods can help to both identify and contextualise relevant linguistic patterns in the questionnaire responses. The results of the study inform those seeking to coordinate PBR initiatives about the motivators of veterinarians to participate in such initiatives and what concerns need to be addressed.     An Integrative Salt Marsh Conceptual Framework for Global Comparisons

    Salt marshes occur globally across climatic and coastal settings, providing key linkages between terrestrial and marine ecosystems. However, salt marsh science lacks a unifying conceptual framework; consequently, historically well-studied locations have been used as normative benchmarks. To allow for more effective comparisons across the diversity of salt marshes, we developed an integrative salt marsh conceptual framework. We review ecosystem-relevant drivers from global to local spatial scales, integrate these multi-scale settings into a framework, and provide guidance on applying the framework using specific variables on 11 global examples. Overall, this framework allows for appropriate comparison of study sites by accounting for global, coastal, inter-, and intra-system spatial settings unique to each salt marsh. We anticipate that incorporating this framework into salt marsh science will provide a mechanism to critically evaluate research questions and a foundation for effective quantitative studies that deepen our understanding of salt marsh function across spatial scales

    The Onboard Calibration (OBC) source incorporates a medical/scientific-grade halogen source with a precisely designed fiber coupling system, and a fiber-based intensity-monitoring feedback loop that results in radiometric and spectral stabilities to within less than 0.3 percent over a 15-hour period. The airborne imaging spectrometer systems developed at the Jet Propulsion Laboratory incorporate OBC sources to provide auxiliary in-use system calibration data. The use of the OBC source will provide a significant increase in the quantitative accuracy, reliability, and resulting utility of the spectral data collected from current and future imaging spectrometer instruments

    Recently, several interesting proposals were made modifying the law of gravity on large scales, within a sensible relativistic formulation. This allows a precise formulation of the idea that such a modification might account for galaxy rotation curves, instead of the usual interpretation of these curves as evidence for dark matter.     Remittances and land change: A systematic review

    Remittances—funds sent by migrants to family and friends back home—are an important source of global monetary flows, and they have implications for the maintenance and transformation of land systems. A number of published reviews have synthesized work on a variety of aspects of remittances (e.g., rural livelihoods, disasters, and economic development). To our knowledge, there are no reviews of work investigating the linkages between remittances and land change, broadly understood. This knowledge gap is important to address because researchers have recognized that remittances flows are a mechanism that helps to explain how migration can affect land change. Thus, understanding the specific roles remittances play in land system changes should help to clarify the multiple processes associated with migration and their independent and interactive effects. To address the state of knowledge about the connection between remittances and land systems, this paper conducts a systematic review. Our review of 51 journal articles finds that the linkages uncovered were commonly subtle and/or indirect. Very few studies looked at the direct connections between receipt of remittances and quantitative changes in land. Most commonly, the relationship between remittances and land change was found to occur through pathways from labor migration to household income to agricultural development and productivity. We find four non-exclusive pathways through which households spend remittances with consequent changes to land systems: (1) agricultural crops and livestock, (2) agricultural labor and technologies, (3) land purchases, and (4) non-agricultural purchases and consumables. In the papers reviewed, these expenditures are linked to various land system change outcomes, including land use change, soil degradation, pasture degradation, afforestation/deforestation/degradation, agricultural intensification/extensification/diversification, and no impact. These findings suggest four avenues for future research. One avenue is the use of the theoretical lens of telecoupling to understand how remittances may produce wider-scale changes in land systems. A second avenue is further examination of the impacts of shocks and disturbances to remittance flows on land change both in migrant sending and in remittance receiving areas. A third avenue is scholarship that examines the extent that household uses of remittances have a “ripple effect” on land uses in nearby interlinked systems. A fourth avenue for future work is the use of spatially explicit modeling that leverages land cover and land use data based on imagery and other geospatial information

    Multidisciplinary teams, and parents, negotiating common ground in shared-care of children with long-term conditions: A mixed methods study

    Background: Limited negotiation around care decisions is believed to undermine collaborative working between parents of children with long-term conditions and professionals, but there is little evidence of how they actually negotiate their respective roles. Using chronic kidney disease as an exemplar this paper reports on a multi-method study of social interaction between multidisciplinary teams and parents as they shared clinical care. Methods. Phases 1 and 2: a telephone survey mapping multidisciplinary teams' parent-educative activities, and qualitative interviews with 112 professionals (Clinical-psychologists, Dietitians, Doctors, Nurses, Play-specialists, Pharmacists, Therapists and Social-workers) exploring their accounts of parent-teaching in the 12 British children's kidney units. Phase 3: six ethnographic case studies in two units involving observations of professional/parent interactions during shared-care, and individual interviews. We used an analytical framework based on concepts drawn from Communities of Practice and Activity Theory. Results: Professionals spoke of the challenge of explaining to each other how they are aware of parents' understanding of clinical knowledge, and described three patterns of parent-educative activity that were common across MDTs: Engaging parents in shared practice; Knowledge exchange and role negotiation, and Promoting common ground. Over time, professionals had developed a shared repertoire of tools to support their negotiations with parents that helped them accomplish common ground during the practice of shared-care. We observed mutual engagement between professionals and parents where a common understanding of the joint enterprise of clinical caring was negotiated. Conclusions: For professionals, making implicit knowledge explicit is important as it can provide them with a language through which to articulate more clearly to each other what is the basis of their intuition-based hunches about parents' support needs, and may help them to negotiate with parents and accelerate parents' learning about shared caring.     Tox21Enricher-Shiny: an R Shiny application for toxicity functional annotation analysis

    Inference of toxicological and mechanistic properties of untested chemicals through structural or biological similarity is a commonly employed approach for initial chemical characterization and hypothesis generation. We previously developed a web-based application, Tox21Enricher-Grails, on the Grails framework that identifies enriched biological/toxicological properties of chemical sets for the purpose of inferring properties of untested chemicals within the set. It was able to detect significantly overrepresented biological (e.g., receptor binding), toxicological (e.g., carcinogenicity), and chemical (e.g., toxicologically relevant chemical substructures) annotations within sets of chemicals screened in the Tox21 platform. Here, we present an R Shiny application version of Tox21Enricher-Grails, Tox21Enricher-Shiny, with more robust features and updated annotations. Tox21Enricher-Shiny allows users to interact with the web application component (available at http://hurlab.med.und.edu/Tox21Enricher/) through a user-friendly graphical user interface or to directly access the application’s functions through an application programming interface. This version now supports InChI strings as input in addition to CASRN and SMILES identifiers. Input chemicals that contain certain reactive functional groups (nitrile, aldehyde, epoxide, and isocyanate groups) may react with proteins in cell-based Tox21 assays: this could cause Tox21Enricher-Shiny to produce spurious enrichment analysis results. Therefore, this version of the application can now automatically detect and ignore such problematic chemicals in a user’s input. The application also offers new data visualizations, and the architecture has been greatly simplified to allow for simple deployment, version control, and porting. The application may be deployed onto a Posit Connect or Shiny server, and it uses Postgres for database management. As other Tox21-related tools are being migrated to the R Shiny platform, the development of Tox21Enricher-Shiny is a logical transition to use R’s strong data analysis and visualization capacities and to provide aesthetic and developmental consistency with other Tox21 applications developed by the Division of Translational Toxicology (DTT) at the National Institute of Environmental Health Sciences (NIEHS)
