    Frailty in older people living with HIV: current status and clinical management

    This paper will update care providers on the clinical and scientific aspects of frailty which affects an increasing proportion of older people living with HIV (PLWH). The successful use of combination antiretroviral therapy has improved long-term survival in PLWH. This has increased the proportion of PLWH older than 50 to more than 50% of the HIV population. Concurrently, there has been an increase in the premature development of age-related comorbidities as well as geriatric syndromes, especially frailty, which affects an important minority of older PLWH. As the number of frail older PLWH increases, this will have an important impact on their health care delivery. Frailty negatively affects a PLWH's clinical status, and increases their risk of adverse outcomes, impacting quality of life and health-span. The biologic constructs underlying the development of frailty integrate interrelated pathways which are affected by the process of aging and those factors which accelerate aging. The negative impact of sarcopenia in maintaining musculoskeletal integrity and thereby functional status may represent a bidirectional interaction with frailty in PLWH. Furthermore, there is a growing body of literature that frailty states may be transitional. The recognition and management of related risk factors will help to mitigate the development of frailty. The application of interdisciplinary geriatric management principles to the care of older PLWH allows reliable screening and care practices for frailty. Insight into frailty, increasingly recognized as an important marker of biologic age, will help to understand the diversity of clinical status occurring in PLWH, which therefore represents a fundamentally new and important aspect to be evaluated in their health care

    Amamentação e depressão pós-parto: revisão do estado de arte

    Objective: To review the literature on the association between breastfeeding and postpartum depression. Sources: A review of literature found on MEDLINE/ PubMed database. Summary of findings: The literature consistently shows that breastfeeding provides a wide range of benefits for both the child and the mother. The psychological benefits for the mother are still in need of further research. Some studies point out that pregnancy depression is one of the factors that may contribute to breastfeeding failure. Others studies also suggest an association between breastfeeding and postpartum depression; the direction of this association is still unclear. Breastfeeding can promote hormonal processes that protect mothers against postpartum depression by attenuating cortisol response to stress. It can also reduce the risk of postpartum depression, by helping the regulation of sleep and wake patterns for mother and child, improving mother’s self efficacy and her emotional involvement with the child, reducing the child’s temperamental difficulties, and promoting a better interaction between mother and child. Conclusions: Studies demonstrate that breastfeeding can protect mothers from postpartum depression, and are starting to clarify which biological and psychological processes may explain this protection. However, there are still equivocal results in the literature that may be explained by the methodological limitations presented by some studies.Objetivo: Revisar a literatura sobre a associação entre a amamentação e a depressão pós-parto. Fontes: Uma revisão da literatura encontrada na base de dados MEDLINE/Pub-Med. Resumo dos achados: A literatura mostra, de forma consistente, que a amamentação fornece uma ampla quantidade de benefícios tanto para a criança quanto para a mãe. Ainda são necessárias mais pesquisas sobre os benefícios psicológicos para a mãe. Alguns estudos apontam que a depressão na gravidez é um dos fatores que pode contribuir para a não amamentação. Outros estudos sugerem, também, uma associação entre amamentação e depressão pós-parto, não estando clara ainda a direção dessa associação. A amamentação pode promover processos hormonais que protegem as mães contra a depressão pós-parto por atenuar a resposta do cortisol ao estresse. E isso também pode reduzir o seu risco, por auxiliar na regulação dos padrões do sono e vigília da mãe e do filho, melhorando a autoeficácia e o envolvimento emocional da mãe com a criança, reduzindo as dificuldades de temperamento e promovendo uma melhor interação entre eles. Conclusões: A pesquisa aponta que a amamentação pode proteger as mães da depressão pós-parto e começa a esclarecer que processos biológicos e psicológicos podem explicar essa proteção.     BACKGROUNDThe Fifth Evian Annual Reproduction (EVAR) Workshop Meeting discussed knowledge regarding contemporary genetics in female reproduction.METHODSSpecialist reproductive medicine clinicians and geneticists delivered presentations based on published literature and current research. The content of this report is based on the expert presentations and subsequent group discussions that took place during this Workshop.RESULTSNumerous ovarian genes with a role in infertility have been identified. Future challenges for genetic screening of patients, such as those with polycystic ovary syndrome, primary ovarian insufficiency or endometriosis, include the identification of high-throughput strategies and how to apply these findings to infertile patients. The identification of high-quality embryos in IVF using objective technologies remains a high priority in order to facilitate single-embryo transfer. Gene expression profiling of cumulus cells surrounding the oocyte, and proteomic and metabolomic approaches in embryo culture media may significantly improve non-invasive embryo quality assessment.CONCLUSIONSThe way forward in advancing the knowledge of genes involved in reproduction was considered to be through genome-wide association studies involving large numbers of patients. Establishing international collaboration is required to enable the application of such technologies in sufficient numbers of patients

    Need for recovery amongst emergency physicians in the UK and Ireland: A cross-sectional survey

    OBJECTIVES: To determine the need for recovery (NFR) among emergency physicians and to identify demographic and occupational characteristics associated with higher NFR scores. DESIGN: Cross-sectional electronic survey. SETTING: Emergency departments (EDs) (n=112) in the UK and Ireland. PARTICIPANTS: Emergency physicians, defined as any registered physician working principally within the ED, responding between June and July 2019. MAIN OUTCOME MEASURE: NFR Scale, an 11-item self-administered questionnaire that assesses how work demands affect intershift recovery. RESULTS: The median NFR Score for all 4247 eligible, consented participants with a valid NFR Score was 70.0 (95% CI: 65.5 to 74.5), with an IQR of 45.5-90.0. A linear regression model indicated statistically significant associations between gender, health conditions, type of ED, clinical grade, access to annual and study leave, and time spent working out-of-hours. Groups including male physicians, consultants, general practitioners (GPs) within the ED, those working in paediatric EDs and those with no long-term health condition or disability had a lower NFR Score. After adjusting for these characteristics, the NFR Score increased by 3.7 (95% CI: 0.3 to 7.1) and 6.43 (95% CI: 2.0 to 10.8) for those with difficulty accessing annual and study leave, respectively. Increased percentage of out-of-hours work increased NFR Score almost linearly: 26%-50% out-of-hours work=5.7 (95% CI: 3.1 to 8.4); 51%-75% out-of-hours work=10.3 (95% CI: 7.6 to 13.0); 76%-100% out-of-hours work=14.5 (95% CI: 11.0 to 17.9). CONCLUSION: Higher NFR scores were observed among emergency physicians than reported in any other profession or population to date. While out-of-hours working is unavoidable, the linear relationship observed suggests that any reduction may result in NFR improvement. Evidence-based strategies to improve well-being such as proportional out-of-hours working and improved access to annual and study leave should be carefully considered and implemented where feasible

    Near Real-time Map Building with Multi-class Image Set Labeling and Classification of Road Conditions Using Convolutional Neural Networks

    Road Weather Information Systems (RWIS) provide real-time weather information at point locations and are often used to produce road weather forecasts and provide input for pavement forecast models. Compared to the prevalant street cameras, however, RWIS are sometimes limited in availability. Thus, extraction of road conditions data by computer vision can provide a complementary observational data source if it can be done quickly and on large scales. In this paper, we leverage state-of-the-art convolutional neural networks (CNN) in labeling images taken by street and highway cameras located across North America. The final training set included 47,000 images labeled with five classes: dry, wet, snow/ice, poor, and offline. The experiments tested different configurations of six CNNs. The EfficientNet-B4 framework was found to be most suitable to this problem, achieving validation accuracy of 90.6%, although EfficientNet-B0 achieved an accuracy of 90.3% with half the execution time. The classified images were then used to construct a map showing real-time road conditions at various camera locations.     Machine Learning Approaches to Improve North American Precipitation Forecasts

    Numerical weather prediction (NWP) is a challenging task which involves working with micro and macro-scale spatio-temporal parameters susceptible to biases and accuracy problems. In recent years, machine learning has grown in popularity with the increasing demand in accurate weather predictions. In this study, we adopt a multimodel (ensemble) forecasting approach by collecting precipitation data from multiple NWP models of Canadian, American and European weather agencies in an effort to deploy an optimal machine learning-based weather model for real-time precipitation forecasting that will outperform the baseline. We considered 8 NWP models as inputs and combined them to create ensemble predictors using 5 different machine learning techniques along with a baseline model (mean of eight input NWP models). We demonstrate that machine learning approaches can improve upon the results of the individual NWP models. The best results were obtained by the neural-network variants with 17% improvement in the mean absolute error, 3% in the root mean squared error, 47% in the median absolute error, 5% in the maximum error, 70% in the relative bias, 41% in the false alarm ratio and 8% in the critical score index over the baseline. Neural networks also complied with the practicality constraints, with minutes of training time and near-real time prediction time
