    Setting a baseline for global urban virome surveillance in sewage

    The rapid development of megacities, and their growing connectedness across the world is becoming a distinct driver for emerging disease outbreaks. Early detection of unusual disease emergence and spread should therefore include such cities as part of risk-based surveillance. A catch-all metagenomic sequencing approach of urban sewage could potentially provide an unbiased insight into the dynamics of viral pathogens circulating in a community irrespective of access to care, a potential which already has been proven for the surveillance of poliovirus. Here, we present a detailed characterization of sewage viromes from a snapshot of 81 high density urban areas across the globe, including in-depth assessment of potential biases, as a proof of concept for catch-all viral pathogen surveillance. We show the ability to detect a wide range of viruses and geographical and seasonal differences for specific viral groups. Our findings offer a cross-sectional baseline for further research in viral surveillance from urban sewage samples and place previous studies in a global perspective

    Risk profiles and one-year outcomes of patients with newly diagnosed atrial fibrillation in India: Insights from the GARFIELD-AF Registry.

    BACKGROUND: The Global Anticoagulant Registry in the FIELD-Atrial Fibrillation (GARFIELD-AF) is an ongoing prospective noninterventional registry, which is providing important information on the baseline characteristics, treatment patterns, and 1-year outcomes in patients with newly diagnosed non-valvular atrial fibrillation (NVAF). This report describes data from Indian patients recruited in this registry. METHODS AND RESULTS: A total of 52,014 patients with newly diagnosed AF were enrolled globally; of these, 1388 patients were recruited from 26 sites within India (2012-2016). In India, the mean age was 65.8 years at diagnosis of NVAF. Hypertension was the most prevalent risk factor for AF, present in 68.5% of patients from India and in 76.3% of patients globally (P < 0.001). Diabetes and coronary artery disease (CAD) were prevalent in 36.2% and 28.1% of patients as compared with global prevalence of 22.2% and 21.6%, respectively (P < 0.001 for both). Antiplatelet therapy was the most common antithrombotic treatment in India. With increasing stroke risk, however, patients were more likely to receive oral anticoagulant therapy [mainly vitamin K antagonist (VKA)], but average international normalized ratio (INR) was lower among Indian patients [median INR value 1.6 (interquartile range {IQR}: 1.3-2.3) versus 2.3 (IQR 1.8-2.8) (P < 0.001)]. Compared with other countries, patients from India had markedly higher rates of all-cause mortality [7.68 per 100 person-years (95% confidence interval 6.32-9.35) vs 4.34 (4.16-4.53), P < 0.0001], while rates of stroke/systemic embolism and major bleeding were lower after 1 year of follow-up. CONCLUSION: Compared to previously published registries from India, the GARFIELD-AF registry describes clinical profiles and outcomes in Indian patients with AF of a different etiology. The registry data show that compared to the rest of the world, Indian AF patients are younger in age and have more diabetes and CAD. Patients with a higher stroke risk are more likely to receive anticoagulation therapy with VKA but are underdosed compared with the global average in the GARFIELD-AF.     An Evaluation of the COVID-19 Pandemic and Perceived Social Distancing Policies in Relation to Planning, Selecting, and Preparing Healthy Meals: An Observational Study in 38 Countries Worldwide

    Objectives: To examine changes in planning, selecting, and preparing healthy foods in relation to personal factors (time, money, stress) and social distancing policies during the COVID-19 crisis. Methods: Using cross-sectional online surveys collected in 38 countries worldwide in April-June 2020 (N = 37,207, Mage 36.7 SD 14.8, 77% women), we compared changes in food literacy behaviors to changes in personal factors and social distancing policies, using hierarchical multiple regression analyses controlling for sociodemographic variables. Results: Increases in planning (4.7 SD 1.3, 4.9 SD 1.3), selecting (3.6 SD 1.7, 3.7 SD 1.7), and preparing (4.6 SD 1.2, 4.7 SD 1.3) healthy foods were found for women and men, and positively related to perceived time availability and stay-at-home policies. Psychological distress was a barrier for women, and an enabler for men. Financial stress was a barrier and enabler depending on various sociodemographic variables (all p < 0.01). Conclusion: Stay-at-home policies and feelings of having more time during COVID-19 seem to have improved food literacy. Stress and other social distancing policies relate to food literacy in more complex ways, highlighting the necessity of a health equity lens.     Inferring vehicle spacing in urban traffic from trajectory data

    This study presents a new method to infer the average two-dimensional (2D) spacing between interacting vehicles in urban traffic from trajectory data. In this context, 2D spacing reflects the amount of road space consumed by pairs of interacting vehicles, and is related to 2D density at the macroscopic level. Due to complex interaction and conflicts in urban traffic, the inherent assumptions in traditional traffic flow models, such as unidirectional flow and homogeneity, are often violated. Such violation challenges direct measurement of urban vehicle spacing. The proposed method addresses this challenge by focusing on the relative movement between interacting vehicles and aggregating the accumulated presence of vehicles in similar scenarios. We apply the method to a large-scale urban trajectory dataset called pNEUMA, and validate the consistency of the method through bootstrapping. By applying the method we obtain a new empirical relation between the average 2D spacing and the relative speeds between interacting vehicles. There are similarities between this empirical relation with the classical Fundamental Diagram of traffic flow in terms of shape and interpretation, and so we term it the “interaction Fundamental Diagram” (iFD). However, there are also key differences. The iFD does not represent steady-state (homogeneous and stationary) longitudinal behaviour; it describes the average amount of road space needed for vehicle interactions at different relative speeds. We believe these iFD relations contribute to understanding vehicle interaction in urban traffic, and can offer new insights for designing safer and more efficient urban intersections.Transport and PlanningTransport and Logistic

    Beyond behavioural change: Investigating alternative explanations for shorter time headways when human drivers follow automated vehicles

    Integrating Automated Vehicles (AVs) into existing traffic systems holds the promise of enhanced road safety, reduced congestion, and more sustainable travel. Effective integration of AVs requires understanding the interactions between AVs and Human-driving Vehicles (HVs), especially during the transition period in which AVs and HVs coexist in a mixed traffic environment. Numerous recent empirical studies find reduced headways of human drivers following an AV compared to following an HV, and attribute this reduction to behavioural changes of drivers when they follow AVs. However, more factors may be at play due to the inherent inconsistencies between the comparison conditions of HV-following-AV and HV-following-HV. This study scrutinises three alternative explanations for the observed reduction in headways: (1) systematic differences in car-following states during data collection, (2) systematic differences in driving variability between leading AVs and HVs, and (3) systematic differences in driving characteristics of leading AVs versus HVs. We use a large-scale dataset extracted from Lyft AV motion data and examine each of these explanations through data stratification and simulation. Our results show that all three mechanisms contribute to the observed reduction in headways of human drivers following AVs. In addition, our findings highlight the importance of driving homogeneity and stability in achieving reliably shorter headways. Thereby, this study offers a more comprehensive understanding on the difference between HV–AV and HV–HV interactions in mixed traffic, and is expected to promote more effective integration of AVs into human traffic.Transport and PlanningTransport and Logistic

    Capecitabine maintenance therapy in patients with recurrent or metastatic breast cancer

    Our objective was to investigate the efficacy and safety of capecitabine maintenance therapy (CMT) after capecitabine-based combination chemotherapy in patients with metastatic breast cancer. The clinical data of 139 metastatic breast cancer patients treated from March 2008 to May 2012 with capecitabine-based combination chemotherapy were retrospectively analyzed. When initial disease control was achieved by the combination chemotherapy, we used CMT for 50 patients, while 37 patients were treated with a different (non-CMT) maintenance therapy. We compared time to progression (TTP), objective response rate, disease control rate, clinical benefit rate, and safety of the two groups, and a sub-group analysis was performed according to pathological characteristics. Sixty-four percent of the patients received a median of six cycles of a docetaxel+capecitabine combination chemotherapy regimen (range 1-45); the median TTP (MTTP) for the complete treatment was 9.43 months (95%CI=8.38-10.48 months) for the CMT group and 4.5 months (95%CI=4.22-4.78 months; P=0.004) for the non-CMT group. The MTTPs for the maintenance therapies administered after the initial capecitabine combined chemotherapy were 4.11 months (95%CI=3.34-4.87 months) for the CMT group and 2.0 months (95%CI=1.63-2.38 months) for the non-CMT group. Gastrointestinal side effects, decreased white blood cells and palmar-plantar erythrodysesthesia were the main adverse reactions experienced with the combination chemotherapies, CMT and non-CMT treatments. No significant differences in the incidence of adverse reactions were detected in the CMT and non-CMT patients. After initial disease control was achieved with the capecitabine-based combination chemotherapy, CMT can significantly prolong TTP rates with a favorable safety profile
