    Change of direction in the biomechanics of atherosclerosis

    The non-uniform distribution of atherosclerosis within the arterial system has been attributed to pro-atherogenic influences of low, oscillatory haemodynamic wall shear stress (WSS) on endothelial cells (EC). This theory is challenged by the changes in lesion location that occur with age in human and rabbit aortas. Furthermore, a number of point-wise comparisons of lesion prevalence and WSS have failed to support it. Here we investigate the hypothesis that multidirectional flow-characterized as the average magnitude of WSS components acting transversely to the mean vector (transWSS)-plays a key role. Maps of lesion prevalence around aortic branch ostia in immature and mature rabbits were compared with equivalent maps of time average WSS, the OSI (an index characterizing oscillatory flow) and transWSS, obtained from computational simulations; Spearman's rank correlation coefficients were calculated for aggregated data and 95% confidence intervals were obtained by bootstrapping methods. Lesion prevalence correlated positively, strongly and significantly with transWSS at both ages. Correlations of lesion prevalence with the other shear metrics were not significant or were significantly lower than those obtained for transWSS. No correlation supported the low, oscillatory WSS theory. The data are consistent with the view that multidirectional near-wall flow is highly pro-atherogenic. Effects of multidirectional flow on EC, and methods for investigating them, are reviewed. The finding that oscillatory flow has pro-inflammatory effects when acting perpendicularly to the long axis of EC but anti-inflammatory effects when acting parallel to it may explain the stronger correlation of lesion prevalence with transWSS than with the OSI

    Dynamic landscapes and human dispersal patterns : tectonics, coastlines, and the reconstruction of human habitats

    Studies of the impact of physical environment on human evolution usually focus on climate as the main external forcing agent of evolutionary and cultural change. In this paper we focus on changes in the physical character of the landscape driven by geophysical processes as an equally potent factor. Most of the landscapes where finds of early human fossils and artefacts are concentrated are ones that have been subjected to high levels of geological instability, either because of especially active tectonic processes associated with faulting and volcanic activity or because of proximity to coastlines subject to dramatic changes of geographical position and physical character by changes of relative sea level. These processes can have both beneficial effects, creating ecologically attractive conditions for human settlement, and deleterious or disruptive ones, creating barriers to movement, disruption of ecological conditions, or hazards to survival. Both positive and negative factors can have powerful selective effects on human behaviour and patterns of settlement and dispersal. We consider both these aspects of the interaction, develop a framework for the reconstruction and comparison of landscapes and landscape change at a variety of scales, and illustrate this with selected examples drawn from Africa and Arabia

    Estimating arterial cyclic strain from the spacing of endothelial nuclei

    Background: The non-uniform distribution of atherosclerosis within the arterial system is widely attributed to variation in haemodynamic wall shear stress. It may also depend on variation in pressure-induced stresses and strains within the arterial wall; these have been less widely investigated, at least in part because of a lack of suitable techniques. Objectives: Here we show that local arterial strain can be determined from impressions left by endothelial cells on the surface of vascular corrosion casts made at different pressures, even though only one pressure can be examined in each vessel. The pattern of pits in the cast caused by protruding endothelial nuclei was subject to “retro-deformation” to identify the pattern that would have occurred in the absence of applied stresses. Methods: Retaining the nearest-neighbour pairs found under this condition, changes in nearest-neighbour vectors were calculated for the pattern seen in the cast, and the ratio of mean changes at different pressures determined. This approach removes errors in simple nearest-neighbour analyses caused by the nearest neighbour changing as deformation occurs. Results: The accuracy, precision and robustness of the approach were validated using simulations. The method was implemented using confocal microscopy of casts of the rabbit aorta made at systolic and diastolic pressures; results agreed well with the ratio of the macroscopic dimensions of the casts. Conclusions: Applying the new technique to areas around arterial branches could support or refute the hypothesis that the development of atherosclerosis is influenced by mural strain, and the method may be applicable to other tissues

    Assessing debris flows using LIDAR differencing: 18 May 2005 Matata event, New Zealand

    The town of Matata in the Eastern Bay of Plenty (New Zealand) experienced an extreme rainfall event on the 18 May 2005. This event triggered widespread landslips and large debris flows in the Awatarariki and Waitepuru catchments behind Matata. The Light Detection and Ranging technology (LIDAR) data sets flown prior to and following this event have been differenced and used in conjunction with a detailed field study to identify the distribution of debris and major sediment pathways which, from the Awatarariki catchment, transported at least 350,000 ± 50,000 m3 of debris. Debris flows were initially confined to stream valleys and controlled by the density and hydraulic thrust of the currents, before emerging onto the Awatarariki debris fan where a complex system of unconfined sediment pathways developed. Here, large boulders, clasts, logs and entire homes were deposited as the flows decelerated. Downstream from the debris fan, the pre-existing coastal foredune topography played a significant role in deflecting the more dilute currents that in filled lagoonal swale systems in both directions. The differenced LIDAR data have revealed several sectors characterised by significant variation in clast size, thickness and volume of debris as well as areas where post-debris flow cleanup and grading operations have resulted in man-made levees, sediment dumps, scoured channels and substantial graded areas. The application of differenced LIDAR data to a debris flow event demonstrates the techniques potential as a precise and powerful tool for hazard mapping and assessment

    Skeletonized internal thoracic artery harvesting reduces chest wall dysesthesia after coronary bypass surgery

    ObjectiveA pain syndrome related to intercostal nerve injury during internal thoracic artery harvesting causes significant morbidity after coronary bypass surgery. We hypothesized that its incidence and severity might be reduced by using skeletonized internal thoracic artery harvesting rather than pedicled harvesting.MethodsIn a prospective double-blind clinical trial, 41 patients undergoing coronary bypass were randomized to receive either unilateral pedicled or skeletonized internal thoracic artery harvesting. Patients were assessed 7 (early) and 21 (late) weeks postoperatively with reproducible sensory stimuli used to detect chest wall sensory deficits (dysesthesia) and with a pain questionnaire used to assess neuropathic pain.ResultsAt 7 weeks postoperatively, the area of harvest dysesthesia (percentage of the chest) in the skeletonized group (n = 21) was less (median, 0%; interquartile range, 0–0) than in the pedicled group (n = 20) (2.8% [0–13], P = .005). The incidence of harvest dysesthesia at 7 weeks was 14% in the skeletonized group versus 50% in the pedicled group (P = .02). These differences were not sustained at 21 weeks, as the median area of harvest dysesthesia in both groups was 0% (P = .89) and the incidence was 24% and 25% in the skeletonized and pedicled groups, respectively (P = 1.0). The incidence of neuropathic pain in the skeletonized group compared with the pedicled group was 5% versus 10% (P = .6) at 7 weeks and 0% versus 0% (P = 1.0) at 21 weeks.ConclusionsCompared with pedicled harvesting, skeletonized harvesting of the internal thoracic artery provides a short-term reduction in the extent and incidence of chest wall dysesthesia after coronary bypass, consistent with reduced intercostal nerve injury and therefore the reduced potential for neuropathic chest pain

    Anthropometric indices of Gambian children after one or three annual rounds of mass drug administration with azithromycin for trachoma control.

    BACKGROUND: Mass drug administration (MDA) with azithromycin, carried out for the control of blinding trachoma, has been linked to reduced mortality in children. While the mechanism behind this reduction is unclear, it may be due, in part, to improved nutritional status via a potential reduction in the community burden of infectious disease. To determine whether MDA with azithromycin improves anthropometric indices at the community level, we measured the heights and weights of children aged 1 to 4 years in communities where one (single MDA arm) or three annual rounds (annual MDA arm) of azithromycin had been distributed. METHODS: Data collection took place three years after treatment in the single MDA arm and one year after the final round of treatment in the annual MDA arm. Mean height-for-age, weight-for-age and weight-for-height z scores were compared between treatment arms. RESULTS: No significant differences in mean height-for-age, weight-for-age or weight-for-height z scores were found between the annual MDA and single MDA arms, nor was there a significant reduction in prevalence of stunting, wasting or underweight between arms. CONCLUSIONS: Our data do not provide evidence that community MDA with azithromycin improved anthropometric outcomes of children in The Gambia. This may suggest reductions in mortality associated with azithromycin MDA are due to a mechanism other than improved nutritional status
