    Biomechanical Effects of Obesity on Balance

    International Journal of Exercise Science 5(4) : 301-320, 2012. The objective of this review was to analyze the current literature related to the effects of obesity on balance. These effects were observed during conditions of static balance, perturbed balance and dynamic balance during gait. The literature review included studies focused on the biomechanical effects of obesity, the relationship between weight loss and balance, and the relative efficacy of weight loss as a balance intervention. The older adult population, which faces increased risk of falls and related injuries, was highlighted where targeted research was available. The existing literature provides evidence for a strong link between obesity and balance impairments. This meta-analysis supports the efficacy of weight loss as an intervention to improve balance in the obese. Additional investigations are needed to confirm the reliability of relationships noted in this review paper, and to explore the potential of weight loss in simple and combined interventions. Future research should also determine whether efficacy differs among populations

    Effects of Foot Position during Squatting on the Quadriceps Femoris: An Electromyographic Study

    International Journal of Exercise Science 6(2) : 114-125, 2013. Weightlifters have commonly believed that changing joint position can alter specific muscle activation. The magnitude of force produced by a muscle is highly dependent upon the length of the muscle. The purpose of this study was to determine the effect of foot positioning on muscle activation of the superficial quadriceps as measured by surface electromyography (sEMG) during a functional squatting movement in healthy adults. Twenty physically active asymptomatic adults (7 females and 13 males) were included in the study while four different foot positions (Neutral, Internally Rotated, Externally Rotated, and Staggered) were assessed. Three quadriceps muscles (Rectus Femoris (RF), Vastus Medialis Oblique (VMO), and Vastus Lateralis Oblique(VLO) were measured. Raw EMG was transformed using a root mean square algorithm. Six one-way repeated measure ANOVAs were conducted to examine the peak and average RMS amplitude for each muscle across each condition, with an alpha level of 0.05 set a priori. Across all foot positions only the Staggered foot position reached statistical significance when compared to all other foot positions for each muscle group. Results suggest that altering the foot position during a partial weight squat has little to no effect on the EMG amplitudes of the quadriceps. However, more research is needed to examine the concentric and eccentric phases of the squatting motion separately with the addition of full weight bearing squats

    Physical and functional fitness changes in older adults in response to a traditional training program and a Wii enhanced training program

    The Wii Fit Plus Balance program has been recommended as an alternative training tool for older adults to improve physical and functional fitness. However, the effectiveness of the Wii Fit Plus in combination with traditional training approaches in older adults is unclear. PURPOSE: To compare changes in physical fitness measures in older adults (OA) engaging in two different training programs: 1) traditional exercises performed by the control group (CG) and 2) traditional exercises complemented by Wii balance games performed by the experimental group (EG). METHODS: 35 untrained OA (mean ± SD Age: 68.86 ± 6.13; BMI: 28.89 ± 5.32) with no exercise history were recruited and randomly assigned to either the CG or EG group. CG and EG programs both included cardio, strength, flexibility, and balance training exercises. However, subjects in the EG group performed additional balance exercises using Wii fit plus program. Both CG and EG subjects exercised twice per week for twelve weeks. Fitness assessments were based on standard procedures including the chair stand, gallon jug transfer, 8 foot up-and-go tests, as well as a long ramp walk and medicine ball throw tests. Data were collected at weeks 1, 6, and 12. Data analysis was conducted using the general linear mixed model with alpha level set at p\u3c0.05. RESULTS: A significant time effect was found for all fitness measures in both groups from week 1 to 6 (p\u3c0.001) and week 1 to 12 (p\u3c0.001). In general CG and EG subjects made 5.5% to 31.7% improvement from Week 1 to 6 and 8.1% to 52.5% improvement from Week 1 to 12. No significant group effects were observed between the CG and EG groups (p\u3e0.370). Also, no significant group by time interactions were observed for any of the fitness measures (p\u3e0.290). CONCLUSION: It appears that the use of the Wii Fit Plus Balance in conjunction with a traditional exercise program does not provide added significant fitness benefits for older adults. While additional fitness benefits are not evident, it appears that both a traditional exercise program alone and a traditional exercise program combined with the Wii Fit Plus Balance training may elicit significant fitness improvements

    Can Galvanic Vestibular Stimulation Reduce Simulator Adaptation Syndrome?

    Electrical stimulation of the vestibular sensory system during virtual environment simulations has been proposed as a method to reduce the incidence of simulator adaptation syndrome (SAS). However, there is limited empirical evidence to support this hypothesis. It is especially important to provide vestibular stimulation in driving simulators because an absence of vestibular cues may alter driver behaviour and reduce vehicle control. This study examined the application of galvanic vestibular stimulation (GVS) as a technique to reduce symptoms of SAS and improve vehicular control in a fixed-based driving simulator. Nineteen participants drove two visually distinct virtual environments (high and low visual cues). In addition, each of these worlds was experienced with and without GVS. Post-drive scores on the Simulator Sickness Questionnaire (SSQ) were used to evaluate the effect of GVS on SAS. In addition, three driving variables were measured to examine driving performance: steering variability, lane departures, and average vehicular speed. GVS application while driving resulted in significant decreases in total SSQ and disorientation symptoms. Greater vehicular control was also observed (as shown by reduced steering variability) when GVS was used in combination with visual cues along the simulated edge of the road. These results support that GVS may be used in fixed-base driving simulators to create vestibular motion cues and reduce SAS

    Relationship between the Romberg Test and the Wii Basic Balance Test and Cognition in Athletes with Concussion

    Background: Approximately 30% of individuals with a sport-related concussion present with postural instability. Multiple clinical balance tests exist to diagnose postural instability; yet little is known about the potential relationship between these type of postural assessments and cognition post-concussion. Aim: The purpose of the current study was to assess the relationship between the Romberg test, the Wii Fit basic balance test (WBBT), and the composite scores on the Immediate Post-Concussion Assessment and Cognitive Testing (ImPACT) test in a sample of athletes with concussions. Methods: 55 post-concussed athletes (40 male) completed the Romberg Test (RT) (−/+), the WBBT, and ImPACT test. Wii Fit basic balance test performance was operationalized as the number of successfully completed trials (of 5 trials of increasing difficulty) within 30 seconds. Pearson’s and point-biserial correlations examined univariate associations among the variables. Results: The RT and WBBT were not significantly related (r = −0.029, p = 0.832). The RT weakly correlated with ImPACT impairment scores (r= 0.26, p= 0.041), whereas WBBT the number of trials did not (r = − 0.20, p = 0.155). Romberg Test scores were significantly correlated with ImPACT Visual Processing Speed Score (r = 0.27, p = 0.036) and Reaction Time score (r = 0.34, p = 0.006). In contrast, WBBT trials were significantly correlated with the ImPACT Visual Memory Score (r = − 0.41, p = 0.003). Conclusions: These results suggest that the WBBT and RT assess unique aspects of postural control. The RT may relate directly to single sensory cognitive and motor processing, while the WBBT may relate to multi-sensory visually driven cognitive and motor processing. Relevance for patients: Clinical balance tests could point to different cognitive impairments post-concussion

    Peripheral Artery Disease and Activity-Induced Shifts in Quadriceps Median Frequency during Treadmill Walking: A Preliminary Study

    Peripheral artery disease (PAD) is associated with altered gait biomechanics. No previous research study has investigated the effect of activity on muscle activation in individuals with PAD. The purpose of this study was to investigate the effect of PAD on muscle activation in response to a ten-minute walking task. METHODS: Ten healthy young adults, ten healthy older adults and ten individuals with PAD performed a ten-minute treadmill walking trial at a self-selected velocity. Surface EMG was recorded from the vastus lateralis and medial gastrocnemius during five steps in the first and tenth minutes of the walking trial. EMG signals were rectified and smoothed using the root mean squared (RMS) with a 20 ms smoothing window. Peak RMS EMG and median frequencies (MdF) were calculated. Mixed-model ANOVAs with Tukey’s post-hoc was used to determine effects of group and activity on peak RMS EMG and MdF. RESULTS: PAD was associated with significantly greater reductions in MdF of the vastus lateralis compared to healthy young and healthy older adults. No significant differences were observed in peak RMS EMG. DISCUSSION: PAD is associated with exaggerated rates of fatigue in the quadriceps but not the gastrocnemius. Efficacy of evidence-based therapeutic interventions should be further investigated

    The genomes of two key bumblebee species with primitive eusocial organization

    Background: The shift from solitary to social behavior is one of the major evolutionary transitions. Primitively eusocial bumblebees are uniquely placed to illuminate the evolution of highly eusocial insect societies. Bumblebees are also invaluable natural and agricultural pollinators, and there is widespread concern over recent population declines in some species. High-quality genomic data will inform key aspects of bumblebee biology, including susceptibility to implicated population viability threats. Results: We report the high quality draft genome sequences of Bombus terrestris and Bombus impatiens, two ecologically dominant bumblebees and widely utilized study species. Comparing these new genomes to those of the highly eusocial honeybee Apis mellifera and other Hymenoptera, we identify deeply conserved similarities, as well as novelties key to the biology of these organisms. Some honeybee genome features thought to underpin advanced eusociality are also present in bumblebees, indicating an earlier evolution in the bee lineage. Xenobiotic detoxification and immune genes are similarly depauperate in bumblebees and honeybees, and multiple categories of genes linked to social organization, including development and behavior, show high conservation. Key differences identified include a bias in bumblebee chemoreception towards gustation from olfaction, and striking differences in microRNAs, potentially responsible for gene regulation underlying social and other traits. Conclusions: These two bumblebee genomes provide a foundation for post-genomic research on these key pollinators and insect societies. Overall, gene repertoires suggest that the route to advanced eusociality in bees was mediated by many small changes in many genes and processes, and not by notable expansion or depauperation

    Differential activation of inflammatory pathways in A549 type II pneumocytes by Streptococcus pneumoniae strains with different adherence properties

    BACKGROUND: Adherence of Streptococcus pneumoniae bacteria to lung cells is a first step in the progression from asymptomatic carriage to pneumonia. Adherence abilities vary widely among S. pneumoniae patient isolates. In this study, the binding properties of S. pneumoniae isolates and the effects of binding on activation of the Nuclear Factor-Kappa-B (NFκB) pathway and cytokine secretion by type II pneumocytes were measured. METHODS: Mechanisms of high- and low-binding S. pneumoniae adherence to A549 cells were investigated by blocking putative receptors on bacteria and host cells with antibody and by eluting choline-binding proteins off of bacterial surfaces. NFκB activation was measured by western blot and immunocytochemistry and cytokine secretion was detected by a protein array. RESULTS: This study shows that S. pneumoniae isolates from pneumonia patients (n = 298) can vary by as much as 1000-fold in their ability to bind to human lung epithelial cells. This difference resulted in differential activation of the NFκB pathway. High-, but not low-binding S. pneumoniae used Choline-binding protein A (CbpA) to bind to complement component C3 on epithelial cell surfaces. Interleukin-8 (IL-8) was the only cytokine secreted by cells treated with either low- or high-binding S. pneumoniae. CONCLUSION: These results indicate that S. pneumoniae clinical isolates are not homogeneous in their interaction with host epithelial cells. The differential activation of host cells by high- and low-binding S. pneumoniae strains could have implications for the treatment of pneumococcal pneumonia and for vaccine development

    The Efficacy of Exercise in Reducing Depressive Symptoms among Cancer Survivors: A Meta-Analysis

    INTRODUCTION: The purpose of this meta-analysis was to examine the efficacy of exercise to reduce depressive symptoms among cancer survivors. In addition, we examined the extent to which exercise dose and clinical characteristics of cancer survivors influence the relationship between exercise and reductions in depressive symptoms. METHODS: We conducted a systematic search identifying randomized controlled trials of exercise interventions among adult cancer survivors, examining depressive symptoms as an outcome. We calculated effect sizes for each study and performed weighted multiple regression moderator analysis. RESULTS: We identified 40 exercise interventions including 2,929 cancer survivors. Diverse groups of cancer survivors were examined in seven exercise interventions; breast cancer survivors were examined in 26; prostate cancer, leukemia, and lymphoma were examined in two; and colorectal cancer in one. Cancer survivors who completed an exercise intervention reduced depression more than controls, d(+) = -0.13 (95% CI: -0.26, -0.01). Increases in weekly volume of aerobic exercise reduced depressive symptoms in dose-response fashion (β = -0.24, p = 0.03), a pattern evident only in higher quality trials. Exercise reduced depressive symptoms most when exercise sessions were supervised (β = -0.26, p = 0.01) and when cancer survivors were between 47-62 yr (β = 0.27, p = 0.01). CONCLUSION: Exercise training provides a small overall reduction in depressive symptoms among cancer survivors but one that increased in dose-response fashion with weekly volume of aerobic exercise in high quality trials. Depressive symptoms were reduced to the greatest degree among breast cancer survivors, among cancer survivors aged between 47-62 yr, or when exercise sessions were supervised
