    International audienceThe latest evolution of pilot controllers, referred to as ASSU (Active Side Sticks Units) provides static and dynamic tactile force (or haptic) feedback to the pilot at the grip. Combined with FBW (fly-by-wire), this promising technology has enhanced safety levels compared to the original mechanical linkage systems they have started to replace, while offering vast improved benefits in terms of carefree handling and pilot situational awareness. In the framework of a PhD thesis, the Information Processing and Systems Department (DTIS) of ONERA and SAFRAN Electronics & Defense have started a cooperation to evaluate the interest and the different possibilities offered by the ASSU technology to improve safety and handling qualities of rotary wing aircraft. Up to now, the design and tuning of these functions were essentially performed thanks to numerous simulator sessions or flight tests with pilots. More than just providing a set of values for the required parameters defining the cueing function (hopefully an optimal set of parameters), it is expected that the approach presented here would reduce the number of piloted simulation tests and associated difficulties of the availability of pilots, the significant amount of time and material resources. The main objective of this work is to develop a design methodology based on the simulation of the entire helicopter control loop (also including the pilot in some form) and enabling the definition and parameterization of cueing functions. Moreover, some objective criteria will be defined and used to design the force feedback laws, bringing additional means of evaluation and validation than the classical subjective rating scales

    International audienceSAFRAN Electronics & Defense and the Information and Systems Processing Department (DTIS) of ONERA have begun a cooperation to evaluate the interest and the methods of use of Active Side Stick Units (ASSU) to improve the safety and flight qualities of helicopters. This paper describes the work carried out to model an environment for simulation and evaluation of haptic feedback laws. An experiment, implemented in the simulator PycsHel at ONERA Salon de Provence, has brought some insight about the influence of ASSU's parameters on the detection of specific haptic feedbacks (Softstops).     Mulching treatments favour the recovery of ecosystem multifunctionality after a large wildfire in Northwest Spain

    [EN] Wildfires are a widespread phenomenon in forests across the Mediterranean Basin but have increased in risk and severity in recent decades. Post-fire treatments are measures that help recover burned vegetation and their functionality but to what extent they also help recover soil functionality is currently unknown. The main objective of this study was to assess the effect of post-fire treatments on ecosystem multifunctionality after a large wildfire in the Cabrera mountain range in 2017 (NW Spain) where close to 10000 Ha of forest were burnt. At the end of 2017 and during 2018, the administration applied different post-fire treatments in high fire severity affected areas: i) straw mulching, ii) woody debris and iii) subsoiling and iv) mechanical hole afforestation. In each treatment, we established ten 2 x 2 m plots and ten adjacent untreated burned plots and collected a composite soil sample from each plot four years after the fire (2021). We calculated regulating services as the standardized mean of total soil organic C (climate regulation), soil water repellence (water regulation) and soil aggregation (soil protection). Supporting services were measured as the standardized mean of mineral N-NH4+ and N-NO3- and available P (soil fertility), β-glucosidase, urease and acid phosphatase (nutrient cycling) and microbial biomass (soil quality). Ecosystem multifunctionality was measured as the standardized mean of all functions measured. Application of straw mulch and woody debris increased regulating ecosystem services in relation to burned control plots. Afforestation with holes had no impact but subsoiling decreased regulating ecosystem services in relation to burned control plots. Post-fire treatments did not have any effect on supporting services. Straw mulch, Woody debris and afforestation with holes improved ecosystem multifunctionality when compared with subsoiling methods.     Treatment with tocilizumab or corticosteroids for COVID-19 patients with hyperinflammatory state: a multicentre cohort study (SAM-COVID-19)

    Objectives: The objective of this study was to estimate the association between tocilizumab or corticosteroids and the risk of intubation or death in patients with coronavirus disease 19 (COVID-19) with a hyperinflammatory state according to clinical and laboratory parameters. Methods: A cohort study was performed in 60 Spanish hospitals including 778 patients with COVID-19 and clinical and laboratory data indicative of a hyperinflammatory state. Treatment was mainly with tocilizumab, an intermediate-high dose of corticosteroids (IHDC), a pulse dose of corticosteroids (PDC), combination therapy, or no treatment. Primary outcome was intubation or death; follow-up was 21 days. Propensity score-adjusted estimations using Cox regression (logistic regression if needed) were calculated. Propensity scores were used as confounders, matching variables and for the inverse probability of treatment weights (IPTWs). Results: In all, 88, 117, 78 and 151 patients treated with tocilizumab, IHDC, PDC, and combination therapy, respectively, were compared with 344 untreated patients. The primary endpoint occurred in 10 (11.4%), 27 (23.1%), 12 (15.4%), 40 (25.6%) and 69 (21.1%), respectively. The IPTW-based hazard ratios (odds ratio for combination therapy) for the primary endpoint were 0.32 (95%CI 0.22-0.47; p < 0.001) for tocilizumab, 0.82 (0.71-1.30; p 0.82) for IHDC, 0.61 (0.43-0.86; p 0.006) for PDC, and 1.17 (0.86-1.58; p 0.30) for combination therapy. Other applications of the propensity score provided similar results, but were not significant for PDC. Tocilizumab was also associated with lower hazard of death alone in IPTW analysis (0.07; 0.02-0.17; p < 0.001). Conclusions: Tocilizumab might be useful in COVID-19 patients with a hyperinflammatory state and should be prioritized for randomized trials in this situatio

    The evolution of the ventilatory ratio is a prognostic factor in mechanically ventilated COVID-19 ARDS patients

    Background: Mortality due to COVID-19 is high, especially in patients requiring mechanical ventilation. The purpose of the study is to investigate associations between mortality and variables measured during the first three days of mechanical ventilation in patients with COVID-19 intubated at ICU admission. Methods: Multicenter, observational, cohort study includes consecutive patients with COVID-19 admitted to 44 Spanish ICUs between February 25 and July 31, 2020, who required intubation at ICU admission and mechanical ventilation for more than three days. We collected demographic and clinical data prior to admission; information about clinical evolution at days 1 and 3 of mechanical ventilation; and outcomes. Results: Of the 2,095 patients with COVID-19 admitted to the ICU, 1,118 (53.3%) were intubated at day 1 and remained under mechanical ventilation at day three. From days 1 to 3, PaO2/FiO2 increased from 115.6 [80.0-171.2] to 180.0 [135.4-227.9] mmHg and the ventilatory ratio from 1.73 [1.33-2.25] to 1.96 [1.61-2.40]. In-hospital mortality was 38.7%. A higher increase between ICU admission and day 3 in the ventilatory ratio (OR 1.04 [CI 1.01-1.07], p = 0.030) and creatinine levels (OR 1.05 [CI 1.01-1.09], p = 0.005) and a lower increase in platelet counts (OR 0.96 [CI 0.93-1.00], p = 0.037) were independently associated with a higher risk of death. No association between mortality and the PaO2/FiO2 variation was observed (OR 0.99 [CI 0.95 to 1.02], p = 0.47). Conclusions: Higher ventilatory ratio and its increase at day 3 is associated with mortality in patients with COVID-19 receiving mechanical ventilation at ICU admission. No association was found in the PaO2/FiO2 variation

    Hyperoxemia and excess oxygen use in early acute respiratory distress syndrome : Insights from the LUNG SAFE study

We examined this issue in patients with ARDS enrolled in the Large observational study to UNderstand the Global impact of Severe Acute respiratory FailurE (LUNG SAFE) study. Methods: In this secondary analysis of the LUNG SAFE study, we wished to determine the prevalence and the outcomes associated with hyperoxemia on day 1, sustained hyperoxemia, and excessive oxygen use in patients with early ARDS. Patients who fulfilled criteria of ARDS on day 1 and day 2 of acute hypoxemic respiratory failure were categorized based on the presence of hyperoxemia (PaO2 > 100 mmHg) on day 1, sustained (i.e., present on day 1 and day 2) hyperoxemia, or excessive oxygen use (FIO2 ≥ 0.60 during hyperoxemia). Results: Of 2005 patients that met the inclusion criteria, 131 (6.5%) were hypoxemic (PaO2 < 55 mmHg), 607 (30%) had hyperoxemia on day 1, and 250 (12%) had sustained hyperoxemia. Excess FIO2 use occurred in 400 (66%) out of 607 patients with hyperoxemia. Excess FIO2 use decreased from day 1 to day 2 of ARDS, with most hyperoxemic patients on day 2 receiving relatively low FIO2. Multivariate analyses found no independent relationship between day 1 hyperoxemia, sustained hyperoxemia, or excess FIO2 use and adverse clinical outcomes. Mortality was 42% in patients with excess FIO2 use, compared to 39% in a propensity-matched sample of normoxemic (PaO2 55-100 mmHg) patients (P = 0.47). Conclusions: Hyperoxemia and excess oxygen use are both prevalent in early ARDS but are most often non-sustained. No relationship was found between hyperoxemia or excessive oxygen use and patient outcome in this cohort.     All-cause mortality in the cohorts of the Spanish AIDS Research Network (RIS) compared with the general population: 1997Ł2010

    Abstract Background: Combination antiretroviral therapy (cART) has produced significant changes in mortality of HIVinfected persons. Our objective was to estimate mortality rates, standardized mortality ratios and excess mortality rates of cohorts of the AIDS Research Network (RIS) (CoRIS-MD and CoRIS) compared to the general population. Methods: We analysed data of CoRIS-MD and CoRIS cohorts from 1997 to 2010. We calculated: (i) all-cause mortality rates, (ii) standardized mortality ratio (SMR) and (iii) excess mortality rates for both cohort for 100 personyears (py) of follow-up, comparing all-cause mortality with that of the general population of similar age and gender. Results: Between 1997 and 2010, 8,214 HIV positive subjects were included, 2,453 (29.9%) in CoRIS-MD and 5,761 (70.1%) in CoRIS and 294 deaths were registered. All-cause mortality rate was 1.02 (95% CI 0.91-1.15) per 100 py, SMR was 6.8 (95% CI 5.9-7.9) and excess mortality rate was 0.8 (95% CI 0.7-0.9) per 100 py. Mortality was higher in patients with AIDS, hepatitis C virus (HCV) co-infection, and those from CoRIS-MD cohort (1997. Conclusion: Mortality among HIV-positive persons remains higher than that of the general population of similar age and sex, with significant differences depending on the history of AIDS or HCV coinfection

Associée aux CDVE, cette technologie prometteuse offre une amélioration de la sécurité par rapport aux commandes mécaniques qu’ils ont commencé à remplacer, tout en offrant de nombreux avantages en termes de facilité de commande et connaissance de la situation au travers de différentes fonctions. Dans le cadre d’une thèse de doctorat, le département Traitement de l’Information et Systèmes (DTIS) de l’ONERA et SAFRAN E&D ont entamé une coopération visant à évaluer l’intérêt et les possibilités offertes par la technologie ASSU pour améliorer la sécurité et les qualités de vol des voilures tournantes. Jusqu’à présent, la conception et le réglage de ces fonctions étaient essentiellement réalisés grâce à de nombreux passages en simulateur ou des tests en vol avec des pilotes. En plus de fournir un ensemble de valeurs de paramètres définissant la fonction haptique (si possible optimaux), on s’attend à ce que l’approche présentée ici réduise le nombre des simulations et les difficultés associées à la disponibilité des pilotes, la quantité importante de temps et de ressources matérielles. L’objectif principal de ce travail est de développer une méthodologie de conception basée sur la simulation d’une boucle complète de pilotage hélicoptère (incluant également le pilote) et permettant la définition et le paramétrage des fonctions haptiques. En outre, certains critères objectifs seront définis et utilisés pour concevoir les lois de retour de force, apportant des moyens supplémentaires d’évaluation et de validation par rapport aux échelles classiques d’évaluation subjective
