Optimizing strategies for meningococcal C disease vaccination in Valencia (Spain)

BackgroundMeningococcal C (MenC) conjugate vaccines have controlled invasive diseases associated with this serogroup in countries where they are included in National Immunization Programs and also in an extensive catch-up program involving subjects up to 20 years of age. Catch-up was important, not only because it prevented disease in adolescents and young adults at risk, but also because it decreased transmission of the bacteria, since it was in this age group where the organism was circulating. Our objective is to develop a new vaccination schedule to achieve maximum seroprotection in these groups.MethodsA recent study has provided detailed age-structured information on the seroprotection levels against MenC in Valencia (Spain), where vaccination is routinely scheduled at 2 months and 6 months, with a booster dose at 18 months of age. A complementary catch-up campaign was also carried out in n for children from 12 months to 19 years of age. Statistical analyses of these data have provided an accurate picture on the evolution of seroprotection in the last few years.ResultsAn agent-based model has been developed to study the future evolution of the seroprotection histogram. We have shown that the optimum strategy for achieving high protection levels in all infants, toddlers and adolescents is a change to a 2 months, 12 months and 12 years of age vaccination pattern. If the new schedule were implemented in January 2014, high-risk subjects between 15-19 years of age would have very low seroprotection for the next 6 years, thereby threatening the program.ConclusionsHigh protection levels and a low incidence of meningococcal C disease can be achieved in the future by means of a cost-free change in vaccination program. However, we recommend a new catch-up program simultaneous to the change in regular vaccination program

Respiratory Syncytial Virus Disease Burden in Community-Dwelling and Long-Term Care Facility Older Adults in Europe and the United States: A Prospective Study

Background. Data on respiratory syncytial virus (RSV) disease burden in adults remain scarce. We assessed the burden of confirmed RSV-acute respiratory infections (cRSV-ARIs) in community-dwelling (CD) adults and those in long-term care facilities (LTCFs).Methods. In this prospective cohort study covering 2 RSV seasons (October 2019-March 2020 and October 2020-June 2021), RSVARIs were identified through active surveillance, in medically stable CD-adults =50 years (Europe) or adults =65 years in LTCFs (Europe and the United States). RSV infection was confirmed by polymerase chain reaction from combined nasal and throat swabs.Results. Of 1981 adults enrolled, 1251 adults in CD and 664 LTCFs (season 1) and 1223 adults in CD and 494 LTCFs (season 2) were included in the analyses. During season 1, overall incidence rates ([IRs] cases/1000 person-years) and attack rates (ARs) for cRSVARIs were 37.25 (95% confidence interval [CI], 22.62-61.35) and 1.84% in adults in CD and 47.85 (CI, 22.58-101.4) and 2.26% in adults in LTCFs. Complications occurred for 17.4% (CD) and 13.3% (LTCFs) of cRSV-ARIs. One cRSV-ARI occurred in season 2 (IR = 2.91 [CI, 0.40-20.97]; AR = 0.20%), without complications. No cRSV-ARIs led to hospitalization or death. Viral pathogens were codetected in =17.4% of cRSV-ARIs.Conclusions. RSV is an important cause of disease burden in adults in CD and LTCFs. Despite the observed low severity of cRSVARI, our results support the need for RSV prevention strategies among adults =50 years old

Thesis (Ph. D.)--Massachusetts Institute of Technology, Dept. of Electrical Engineering and Computer Science, 2007.This electronic version was submitted by the student author. The certified thesis is available in the Institute Archives and Special Collections.Includes bibliographical references (p. 195-204).This thesis develops a new scalable modeling framework at the interface of game theory and machine learning to recover economic structures from limited slices of data. Inference using economic models has broad applicability in machine learning. Economic structures underlie a surprisingly broad array of problems including signaling and molecular control in biology, drug development, neural structures, distributed control, recommender problems, social networking, as well as market dynamics. We demonstrate the framework with an application to genetic regulation. Genetic regulation determines how DNA is read and interpreted, is responsible for cell specialization, reaction to drugs, metabolism, etc. Improved understanding of regulation has potential to impact research on genetic diseases including cancer. Genetic regulation relies on coordinate binding of regulators along DNA. Understanding how binding arrangements are achieved and their effect on regulation is challenging since it is not always possible to study regulatory processes in isolation. Indeed, observing the action of regulators is an experimental and computational challenge. We need causal genome-wide models that can work with existing high-throughput observations. We abstract DNA binding as an economy and develop fast algorithms to predict average binding arrangements as competitive equilibria. The framework supports viewing regulation as a succession of regulatory states. We complete the framework with algorithms to infer causal structure from high-throughput observations. Learning here deviates from work in learning in games, it is closer to the economic theory of revealed preferences. Our algorithms predict the effect of experimental perturbations and can be used to refine experimental hypotheses. We show that the economic approach reproduces known behavior of a genetic switch (-phage), and that it can complete the map of coordinate binding in yeast.by Luis P茅rez-Breva.Ph.D

We develop and analyze game-theoretic algorithms for predicting coordinate binding of multiple DNA binding regulators. The allocation of proteins to local neighborhoods and to sites is carried out with resource constraints while explicating competing and coordinate binding relations among proteins with affinity to the site or region. The focus of this paper is on mathematical foundations of the approach. We also briefly demonstrate the approach in the context of the 位-phage switch.
